"textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">The idea that some habitual clinical practices may be inappropriate has been progressively gaining ground among healthcare professionals. These are actions with little value, meaning that their efficacy has not been demonstrated, their effectiveness is doubtful, or they are not cost-effective<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. In addition to reducing efficiency, they may compromise patient safety and entail an excessive financial burden<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Since 2012, various initiatives have been developed to Improve and Adapt Clinical Care (MAPAC, for the phrase’s initials in Spanish). Of note among them are Choosing Wisely, Do not do, and <span class="elsevierStyleItalic">Compromiso por la calidad</span> [Commitment to Quality]. They include recommendations that can be of great help to clinicians as well as healthcare managers, in addition to representing a component of clinical safety<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">However, in spite of the involvement of various scientific societies and the extensive work on disseminating them<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>, at present there is no evidence that they have been sufficiently implemented<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,4</span></a>. This lack of response could be explained by an absence of true knowledge of the initiatives as well as by difficulties in modifying clinical practice. The aim of our work was to evaluate the first of these two factors, analyzing knowledge of these initiatives among professionals.</p><p id="par0020" class="elsevierStylePara elsevierViewall">This is a descriptive, cross-sectional study on the knowledge that professionals have of the initiatives in our center, a reference teaching hospital, in 2020. The professional categories included the various healthcare teams.</p><p id="par0025" class="elsevierStylePara elsevierViewall">For the study, a survey was administered through a form that was sent in a link via email. The following variables were gathered: age, sex, professional category, and healthcare department as well as seven questions on knowledge of appropriateness and initiatives. The degree of concern for inappropriateness was estimated on a scale of 0–10 and was evaluated dichotomously as less than or greater than seven. The distribution of responses based on age and healthcare department were compared.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The data were analyzed using the R statistical package version 4.0.4 for Linux®. Frequencies were calculated and for comparing categorical variables, the χ<span class="elsevierStyleSup">2</span> test was performed and the odds ratio calculated. Values of <span class="elsevierStyleItalic">p</span> < .05 were considered significant.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A total of 192 participants responded to the survey (31.1%). In terms of age, 30.7% were between 25–35 years of age; 34.3% between 36–45; 16.6% between 46–55, and 18.2% were older than 55. A total of 62.5% were women. In terms of professional categories, 91.1% were physicians, 7.2% were nursing department staff, and 1.5% were healthcare technicians. According to healthcare departments, 51.2% were from medical departments, 16.6% from surgical departments, 16.6% from central services, and 15.6% from the maternity department.</p><p id="par0040" class="elsevierStylePara elsevierViewall">A total of 64.4% respondents reported having no knowledge of the initiatives. The responses are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. In addition, 51.1% of those interviewed considered that in their field, more than 10% of prescriptions or procedures were inappropriate and, in regard to diagnostic tests, 49.1% thought that more than 10% may be inappropriate. The degree of concern for inappropriate practices was equal to or greater than seven for 77.4% of respondents.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">When comparing the distribution of responses among the different age groups and departments, significant differences were only observed in the oldest group, which considered that defensive medicine, pressure from the patient or family members, and insufficient time were less frequent causes of inappropriateness, and in professionals in the maternity departments, who considered that unclear indications more frequently influenced inappropriateness.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The lack of knowledge of MAPAC initiatives among healthcare professionals is cause for study and reflection. A study by Colla and Mainor<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>, estimated that around 40% of professionals are aware of them, a very low figure that did not significantly change between 2014 and 2017. These figures are just modestly higher than our results, which estimate that around 36% of professionals know of them. In our setting, we only have a similar estimate from the hospital setting<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>, which found that around 50% of professionals were unaware of the initiatives. Another study in the scope of primary care<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> concluded that just 43.5% knew about the initiatives.</p><p id="par0055" class="elsevierStylePara elsevierViewall">On the contrary, another study in the emergency department setting<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> showed a slightly higher percentage of professionals (52.5%) who knew about Choosing Wisely. Nonetheless, this study mentioned the difficulties in complying with the recommendations and indeed, this observation has been cited in various works. Despite considering inappropriateness to be an important problem, the study by Bishop et al.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> justified its persistence as being due to a lack of time for implementing changes as well as the pressure of patients’ expectations. Other studies also describe worry of a possible lawsuit as well as a lack of time for shared decision-making with patients as barriers<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Therefore, beyond knowledge of these initiatives, it is also necessary to reflect on the capacity to put these recommendations into practice. Various authors believe that strategies should be developed that involve both the physician and the patient, that feedback mechanisms should be established to learn the results, and that time with the patient must be available<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a>. All of this must be conducted within the framework of a general plan which includes the participation of leaders, a culture of trust and innovation, and a commitment to making organizational resources available<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Nevertheless, there is criticism of these initiatives; some consider that they do not take into account the underlying reasons for inappropriateness and therefore they will never have a significant influence<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>.</p><p id="par0070" class="elsevierStylePara elsevierViewall">We believe the limitations of our study are the low number of responses as well as the fact that it was conducted in a single center. In addition, given the study’s design, this work does not allow for evaluating whether professionals may have already incorporated some of the recommendations into their practice even if they did not know their source and thus, if they may have underestimated their true knowledge.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In conclusion, this work is in accordance with other author’s findings that dissemination is very difficult in the healthcare field<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a>. Therefore, we believe that it is worth redesigning information strategies and specifically outlining a wide variety of measures for supporting clinicians in order to advance on improving appropriateness.</p></span>"
