Abstract
Introduction

Anti-NOR90 antibodies were initially described in patients with autoimmune diseases based on staining of a nucleolar region known as the nucleolar organizer region (NOR). This study aims to explore the clinical aspects of anti-NOR90 antibodies in patients with systemic autoimmune diseases.

Methods

Observational study of patients with positive anti-NOR90 antibodies using the EUROLINE Systemic Sclerosis profile (IgG) kit (Euroimmun, Germany). Data on demographics, comorbidities, autoimmune diseases, treatment, and clinical manifestations were collected.

Results

Fifteen Patients with positive anti-NOR90 antibodies were included. Majority were female (86.7%), with median age of 54 years. Most common clinical manifestations were Raynaud's phenomenon, dryness, and interstitial lung disease (ILD). Some patients had hematological or solid organ neoplasms. EPID was prevalent, with one case showing rapid progression requiring aggressive treatment.

Discussion and conclusion

This study highlights the association between anti-NOR90 antibodies and systemic autoimmune diseases, particularly SS and ES. EPID was a notable feature, suggesting its consideration in IPAF diagnosis for anti-NOR90 positive patients. Further multicenter studies are needed for better understanding and detection methods optimization.

Introducción

Los anticuerpos anti-NOR90 fueron descritos inicialmente en pacientes con enfermedades autoinmunes basándose en la tinción de una región nucleolar conocida como región organizadora nucleolar (NOR). Este estudio tiene como objetivo explorar los aspectos clínicos de los anticuerpos anti-NOR90 en pacientes con enfermedades autoinmunes sistémicas.

Métodos

Estudio observacional de pacientes con anticuerpos anti-NOR90 positivos mediante el kit EUROLINE Systemic Sclerosis profile (IgG) (Euroimmun, Alemania). Se recopilaron datos demográficos, comorbilidades, enfermedades autoinmunes, tratamiento y manifestaciones clínicas.

Resultados

Se incluyeron quince pacientes con anticuerpos anti-NOR90 positivos. La mayoría eran mujeres (86.7%) con una mediana de edad de 54 años. Las manifestaciones clínicas más comunes fueron el fenómeno de Raynaud, la sequedad y la enfermedad pulmonar intersticial (EPI). Algunos pacientes presentaban neoplasias hematológicas u orgánicas sólidas. La EPI fue prevalente, con un caso mostrando progresión rápida que requirió tratamiento agresivo.

Discusión y conclusiones

Este estudio destaca la asociación entre los anticuerpos anti-NOR90 y las enfermedades autoinmunes sistémicas, particularmente el SS y la ES. La EPI fue una característica notable, sugiriendo su consideración en el diagnóstico de la neumopatía intersticial autoinmune (IPAF) en pacientes con anticuerpos anti-NOR90 positivos. Se necesitan estudios multicéntricos adicionales para una mejor comprensión y optimización de los métodos de detección.
