The most frequent problems<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> include conflicts related to performing techniques considered inappropriate; conflicts about healthcare costs<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2,4–6</span></a>; disagreements among care team members<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,7,8</span></a>; problems with the informed consent process<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,6,8–10</span></a>; difficult situations at the end of life (limitation of therapeutic effort, therapeutic obstinacy, palliative care, among others)<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,5,6,8</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,11,12</span></a>; excessive workload<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>; difficulties in the doctor-family<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6,8</span></a> or doctor-patient relationship; issues related to confidentiality; patient discrimination; conflicts of interest<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>; pressure to issue false reports; cultural confrontations<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,11</span></a>; requests for a change in professional; situations that threaten the professional’s life<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>; or domestic violence.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> All of these studies have been conducted in Anglo-Saxon and European settings.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">However, given the differences in bioethical traditions, there are reasons to believe that ethical problems may be different in the Iberoamerican context. Therefore, it is doubtful that results obtained in Anglo-Saxon or Northern European studies are able to be extrapolated to our clinical context.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Indeed, the existence of Mediterranean or Latin bioethics, which would be more predominant in the countries of Southern Europe and Latin America, has been proposed given their different cultural framework. In this type of ethics, the Catholic tradition may have a greater influence, more value may be placed on beliefs and principles, and there may be a more community-based approach to problems (coming from the family and the community).</p><p id="par0020" class="elsevierStylePara elsevierViewall">In turn, Anglo-Saxon utilitarian pragmatism places more weight on practical issues and a consequentialist assessment of actions, with an approach to problems that is more individualistic and more grounded in respect for autonomy.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a> It is unknown if these cultural differences, defined in the theoretical sphere, have a real impact on healthcare professionals’ perception of ethical conflicts.</p><p id="par0025" class="elsevierStylePara elsevierViewall">To date, there have been no studies on the types of ethical conflicts that are most frequently faced and which cause the most concern to clinicians in Spain. Without this knowledge, it is not possible to realistically approach professionals’ sources of moral concern, help them effectively, or propose specific training programs that have a positive effect on daily practice.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Therefore, the aim of this work is to describe the ethical conflicts most frequently identified by Spanish internists as well as the importance professionals ascribe to each issue.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Study design</span><p id="par0035" class="elsevierStylePara elsevierViewall">This work is an observational, cross-sectional study conducted by means of an anonymous, voluntary survey aimed at Spanish internists and distributed through an <span class="elsevierStyleItalic">ad hoc</span> platform created by the Spanish Society of Internal Medicine (SEMI, for its initials in Spanish). There are 5866 internists registered on this platform. This system guarantees that each user can only answer the survey once, that his or her responses remain anonymous, and that no physicians from other specialties can complete it.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Creation of the questionnaire</span><p id="par0040" class="elsevierStylePara elsevierViewall">The questionnaire was created by a multidisciplinary team of internists, experts in bioethics, and experts in research methods. To develop it, two literature searches were conducted. The first was to determine what the main ethical conflicts described by internists were.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–14</span></a> The second was on questionnaires used to explore the presence of these conflicts.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5,6</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The form was built taking into account the results of the searches. The adaptation of the questions to our setting is described in the Variables section. A pre-test was administered to ten internists and ten internal medicine residents. This allowed us to evaluate understanding of the questionnaire and improve the wording of the questions.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Variables</span><p id="par0050" class="elsevierStylePara elsevierViewall">The survey allowed for respondents to score on a scale of 0–5 the frequency with which they identified ethical conflicts and the significance in their clinical practice attributed to each of them.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Nineteen types of ethical conflict were asked about. When adapting other questionnaires to the Spanish context, questions regarding assisted suicide and euthanasia were deleted, as they were illegal in Spain at the time the survey was conducted. Other significant ethical conflicts that were eliminated because they were considered uncommon in the general practice of internists in our setting were issues regarding abortion, reproductive or genetic counseling problems, and issues concerning healthcare in transplantation programs. Questions related to payment issues or access to health insurance, which are frequent in the United States of America, were also not included, given the universal coverage offered by the Spanish National Health System.</p><p id="par0060" class="elsevierStylePara elsevierViewall">As an additional adaptation, all questions regarding patients’ caregivers were rewritten to refer to “family members,” as this is the most common group of caregivers in our setting.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">A Likert scale was used (one to four points according to level of agreement) to explore whether ethical problems were frequent in clinical practice and the degree of difficulty they generated.</p><p id="par0070" class="elsevierStylePara elsevierViewall">This work also collected demographic variables (age, sex), number of years worked, position within the institution, area of professional activity, training on bioethics, and hospital type.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis and sample size calculation</span><p id="par0075" class="elsevierStylePara elsevierViewall">The sample size was calculated based on the total population of SEMI members who had logged on to receive information online (5866 professionals). To achieve a 90% confidence level, a limit of 5%, and an estimated frequency of occurrence of ethical conflicts of 50%, the calculated sample size was 259.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Data were initially recorded in an Excel® spreadsheet (Microsoft Co., Redmond, WA, USA) and were later exported and analyzed using SPSS Statistics 22® (IBM, Armonk, NY, USA).</p><p id="par0085" class="elsevierStylePara elsevierViewall">Qualitative variables were described using frequency tables and quantitative variables using means and standard deviations. To explore the correlation between how often the various issues occurred and the importance attributed to each, McNemar’s test and the Kappa coefficient were calculated.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Ethical considerations</span><p id="par0090" class="elsevierStylePara elsevierViewall">The study meets the ethical regulations and standards included in the World Medical Association’s Declaration of Helsinki and the Oviedo Convention on Human Rights and Biomedicine.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Given its nature (online survey willingly completed), it was not evaluated by a clinical research ethics committee beforehand.</p><p id="par0100" class="elsevierStylePara elsevierViewall">All data were processed with the maximum confidentiality, in accordance with current legislation.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0105" class="elsevierStylePara elsevierViewall">The survey was sent three times (until the calculated sample size was reached): it was first sent on June 2, 2017 and then two reminders were sent, one every two weeks. The response period ended on July 10, 2017.</p><p id="par0110" class="elsevierStylePara elsevierViewall">In total, 261 surveys were analyzed, 4.4% of the sample. The demographic characteristics are summarized in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. In total, 70.1% (183) of those surveyed frequently or almost always faced ethical conflicts in their clinical practice whereas 29.9% (78) rarely or almost never faced ethical conflicts. For 60.1% (157), the conflicts rarely or almost never hindered their healthcare activity whereas for 39.9% (104), it often or almost always did. Of those who identified ethical conflicts often or almost always, 50.3% (92) reported that they often or almost always hindered their healthcare activity. Among those who not often or almost never detected ethical conflicts, 84% (66) reported that ethical conflicts rarely or almost never hindered their healthcare activity (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">The ethical conflicts internists in Spain faced most frequently and those considered most significant are summarized in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. McNemar’s test identified ethical conflicts related to the end of life and clinical relationships as being the most frequent and significant. By order of frequency, these include those related to DNR orders, limitation of therapeutic effort (LTE), use of palliative treatments, those related to doctor-patient communication, and conflicts with family members (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). Upon calculating the Kappa coefficient on concordance between frequency and significance (conflicts that are both frequent and significant), the maximum value was found for LTE.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">The results of this work indicate that the most frequent and significant ethical problems for Spanish internists are those related to the end of life (decisions on limitation of therapeutic effort, use of palliative treatments, or issues arising from do-not-resuscitate orders); conflicts that occur in the clinical relationship, whether that is with family members or with the patient; and decision making regarding noncompetent patients.</p><p id="par0125" class="elsevierStylePara elsevierViewall">These results are very similar to those observed in previous series in both the Anglo-Saxon and continental European settings. Along these lines, in studies from the United States of America<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> and Croatia,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> ethical problems related to the end of life were also the most frequent (in 51% and 68% of cases, respectively) whereas in a European multicenter study, they were the third most frequent (79%).<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">In an observational study conducted exclusively on US internists/hospitalists, McCarthy et al.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> identified conflicts arising from goals-of-care setting—that is, discrepancies in the appropriateness/limitation of therapeutic effort—as being particularly problematic. This group of ethical conflicts were also the most frequent source of consultations made to healthcare ethics committees in the aforementioned series.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5,6</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">In second place are ethical conflicts arising from dealings with the patient and family members, a common cause of worry among Spanish internists. This category of moral conflicts has likewise been identified in previously published series, with a prevalence that ranged from 35% to 77%.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5,6</span></a> In addition, these moral conflicts often occur as a consequence of the difficulty of shared decision-making among professionals and family members on behalf of noncompetent patients.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In fact, this type of conflict also generates numerous consultations to ethics committees (DuVal et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> 38%) and in the European studies, they are among the most difficult to resolve (Hurst et al.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> 8%–28%).</p><p id="par0140" class="elsevierStylePara elsevierViewall">Given the demographics of the population cared for in large part in internal medicine departments, it is not surprising that ethical conflicts related to the appropriateness/limitation of end-of-life measures and shared decision-making with respect to noncompetent patients are considered to be a priority and significant. Nevertheless, a recent study by SEMI’s Bioethics Working Group highlights that, despite the fact that the majority of internists consider conflicts surrounding LTE to be frequent and significant, only a minority really know what LTE is; this term is often used generally and imprecisely.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">A correlation has been detected between the frequency with which professionals identify ethical problems in their regular practice and the difficulty they pose to their care; for clinicians who find fewer ethical problems in their daily practice, these problems hinder their healthcare activity less. It is possible that this is because there are internists who do not identify all ethical problems: either they do not notice them or they consider them to be a different type of problem (clinical, legal, social, etc.). And if they identify fewer ethical problems, it is logical that they would have less difficulty.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The inability to identify ethical problems in daily practice has been named “axiological blindness” and is one of the main obstacles that must be overcome in the ethical training of professionals. The results of this study lead us to reflect on whether clinical ethics training should first and foremost be focused not so much on conflict resolution but rather on training to recognizes conflicts. In fact, a study from the United States of America showed that a specific ethics “awareness-building” program was able to increase professionals’ ability to identify ethical problems by 13%.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">After conducting this study, the SARS-CoV-2 occurred without warning and internists had to care for many patients as front-line medical workers. This crisis has caused numerous ethical conflicts and some of them are included in the survey. Of note are the following: distribution of scarce healthcare resources (as occurred with intensive care unit beds) and caring for patients when it poses a threat to the physician’s life. Although these conflicts are among the least frequent (such as a threat to the physician’s life, for example, which ranked last), a certain degree of significance is placed on them. If the survey were done today, the data would possibly change. This supports the notion that ethical conflicts are evaluated as significant when they are frequent.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The crisis caused by the epidemic has affected our professional duties and even our legal duties, leading us to contemplate whether we should care for patients without an appropriate level of safety for the physician. In light of this dilemma, the Spanish Medical Colleges Organization has indicated that, “The physician is not and must not be a hero or a martyr. What he or she should be is a good professional. When faced with situations of such extreme risk as the current one, the physician may too feel fear without being intimidated or taking undue risks.”<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">In light of the similarities between our results and those published previously, it does not appear that the different Mediterranean ethical tradition has an influence on the manner of perceiving ethical problems in clinical practice. This may be explained by the “globalization” of medicine in recent decades, both in the manner of providing healthcare and in clinical ethics.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Even coming from different philosophical traditions, the second half of the 20th century has seen a change in focus of the doctor-patient relationship, which is now oriented toward greater respect for the patient and toward empowering their autonomy, which may explain the similarity of ethical conflicts in different traditions. Nevertheless, though it seems that the ethical concerns of healthcare professionals are similar in different settings, we cannot answer the question about whether the Mediterranean bioethics tradition has an influence on how ethical conflicts are approached.</p><p id="par0175" class="elsevierStylePara elsevierViewall">This study has some limitations. As it was an anonymous survey, survey respondents’ quality of understanding of the questions cannot be ensured. In regard to this study’s strengths, it is possible and perhaps inevitable for there to be a certain degree of selection bias, as it is reasonable to think that professionals with greater awareness of the issue the survey dealt with may have been more likely to respond. Likewise, the sample size of this series is of note, as it is larger than previous series, as well as how comprehensive the set of conflict types was, which limits memory bias.</p><p id="par0180" class="elsevierStylePara elsevierViewall">In conclusion, it is important to highlight that the most frequent and significant ethical conflicts among Spanish internists are those related to end-of-life care followed by those arising from the clinical relationship and from managing noncompetent patients. In light of the similarities between our results and those published previously, it does not seem that the Mediterranean ethical tradition has an impact the manner of perceiving ethical problems in clinical practice. In addition, we would like to highlight that axiological blindness, a barrier to identifying ethical problems in internists’ healthcare activity, makes the design of training programs aimed at training individuals to recognise ethical programs a priority.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conflicts of interest</span><p id="par0185" class="elsevierStylePara elsevierViewall">The authors declare no conflicts of interest related to the conduct or publication of this work. These results are similar to those of other English and European series.
