However, their sensitivity for TPE appears limited. A study conducted in our center showed a sensitivity of 15% for the Xpert® MTB/RIF test in PF.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In a meta-analysis of 24 studies, this same automated PCR technique (Xpert® MTB/RIF) in PF had a sensitivity of 22.7% when it was used in patients with probable TPE, that is, those who had a clinical diagnosis of tuberculosis but who did not have microbiological confirmation.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In 2013, results were published on a new PCR method known as FluoroType MTB®. It showed a sensitivity of 95.1% in 661 respiratory samples with positive mycobacterial cultures.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Later studies have shown different sensitivities in various biological specimens. They range from 56% in the saliva of 32 patients with a microbiological diagnosis of pulmonary tuberculosis,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> 71% in the blood of 31 patients with a clinical suspicion of disseminated tuberculosis and coinfection with human immunodeficiency virus,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> 60% in 17 paraffin-embedded tissue samples of granulomas of different etiologies,<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and 63% in 59 lymph node samples with positive mycobacterial cultures.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> However, the sensitivity of this technique in the PF of patients with TPE has not yet been described to date.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aims of this study were: 1) evaluate the diagnostic accuracy of the FluoroType MTB® technique in PF and sputum samples from patients with suspected TPE and 2) analyze the increase in diagnostic accuracy of a second FluoroType MTB® test in PF (via a second thoracentesis) when the first determination was negative.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A prospective study was conducted on consecutive patients who underwent a diagnostic thoracentesis in the Arnau de Vilanova University Hospital of Lérida, Spain, from January 2017 to September 2019. A FluoroType MTB® test in PF specimens and, if possible, sputum specimens, was ordered for all patients suspected of having TPE. As per the attending clinician’s judgment, this PCR technique was also indicated in subjects with other causes of effusion and was generally avoided when the PF etiology was highly predictable (for example, in a significant proportion of those with heart failure) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). A second thoracentesis was performed when a definitive diagnosis was not able to be made after learning the results of the first procedure. The study protocol was approved by the local ethics committee (CREC-1674) and all patients signed an informed consent form.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Diagnostic criteria</span><p id="par0030" class="elsevierStylePara elsevierViewall">The diagnosis of TPE was considered confirmed when the tubercle bacillus was isolated in sputum, PF, or a pleural biopsy or when granulomas were observed on a pleural biopsy. A diagnosis of probable pleural tuberculosis was established when the following conditions were met: a) a clinical condition compatible with TPE, b) presence of pleural exudate with adenosine deaminase concentrations ≥35 U/L, c) resolution of effusion with antitubercular treatment, and d) reasonable exclusion of other potential causes of effusion during at least one year of clinical follow-up. A positive microbiological isolation was defined as observance of tubercle bacilli in a PF or sputum sample by means of a microscopic examination, culture, or PCR test. Effusion was classified as idiopathic when it resolved without specific treatment and when its etiology was unable to be determined despite a comprehensive study that included analysis of the PF, a computed tomography scan, and a pleural biopsy. Other diagnoses were established according to widely accepted clinical criteria.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Techniques</span><p id="par0035" class="elsevierStylePara elsevierViewall">PF samples were collected in 10-mL tubes that contained ethylenediaminetetraacetic acid. The FluoroType MTB® technique (Hain Lifescience, Nehren, Germany) was performed the same day as the extraction. PF and sputum samples were decontaminated and concentrated using the sodium hydroxide-N-acetyl-l-cysteine method.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> They were centrifuged for 20 min at 2,800 rpm. The sediment obtained was suspended in 2 mL of phosphate buffer and 0.5 mL of the mixture was used for the PCR. DNA extraction was done manually using the FluoroLyse® kit (Hain Lifescience, Nehren, Germany) according to the manufacturer’s instructions. The FluoroCycler® instrument with FluoroType® MTB reactants was used for the final extension of the PCR with a melting curve. The routine microbiological analyses included auramine staining of sputum samples and mycobacterial cultures in solid (Löwenstein-Jensen) and liquid (Middlebrook TH9) media for both sputum and PF.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Measures of diagnostic efficiency (sensitivity, specificity, likelihood ratios (LR)) and their 95% confidence intervals (95% CI) were calculated based on a 2 × 2 contingency table. If one of the figures on the table was zero, 0.5 was added to each box to be able to obtain the LR value. Calculations were done using the SPSS statistical package version 24.0 (Chicago, IL, USA).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patients</span><p id="par0045" class="elsevierStylePara elsevierViewall">Thirty-one patients with pleural tuberculosis (22 men and nine women with a median age of 35 years) and 176 with other diagnoses (122 men and 54 women with a median age of 72 years) were included. In the first group, 21 (68%) of patients had a definitive histological or microbiological diagnosis of tuberculosis whereas ten (32%) had a clinical diagnosis. Only one patient was coinfected with human immunodeficiency virus. The causes of nontuberculous pleural effusion are specified in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The biochemical characteristics of PF in the study population are summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">FluoroType MTB® in pleural fluid</span><p id="par0050" class="elsevierStylePara elsevierViewall">The index FluoroType MTB® test in PF was positive in four of the 31 (13%) patients with tuberculosis and in two of the 176 (1%) of patients with effusion due to other causes. This yields a sensitivity of 13% (95% CI 5%–29%), a specificity of 99% (95% CI 96%–100%), a positive LR of 11 (95% CI 2–59), and a negative LR of 0.9 (95% CI 0.8–1). The sensitivity of the FluoroType MTB® was lower than that of other microbiological tests, as shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The aforementioned tests were used as references for defining the terms of confirmed and probable pleural tuberculosis.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The FluoroType MTB® test was negative in nine patients (33%) who had a PF culture positive for <span class="elsevierStyleItalic">M. tuberculosis</span> (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). On the other hand, two of the four (50%) patients who had a positive FluoroType MTB® test in PF did not have any other positive microbiological isolations. That is, in two of the 31 (6.5%) patients with TPE, the confirming diagnosis was made solely through the PCR technique.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">FluoroType MTB® in sputum</span><p id="par0060" class="elsevierStylePara elsevierViewall">The sputum of 25 patients was analyzed using FluoroType MTB®. It was positive in three of the 14 (21%) patients with TPE and in one of the 11 (9%) with other diagnoses (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The three patients with TPE and a positive result on the PCR technique in sputum also had positive sputum cultures; one also had a positive PF culture.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">FluoroType MTB® in patients with an initial negative determination</span><p id="par0065" class="elsevierStylePara elsevierViewall">In 57 patients (24 with tuberculosis and 33 with other diagnoses), a second FluoroType MTB® test in PF was carried out with a median time between the two thoracentesis procedures of three days (25th and 75th quartiles: 1–7). The second analysis was negative in all cases.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">FluoroType MTB® false positives</span><p id="par0070" class="elsevierStylePara elsevierViewall">There were two patients with a FluoroType MTB® test in PF that were interpreted as false positives. One patient had transudate secondary to heart failure and another had parapneumonic effusion. A pleural biopsy was performed on the latter and showed acute inflammation without granulomas. In both cases, a second FluoroType MTB® sample two days after the first was negative. There was also a false positive on a FluoroType MTB® test on a sputum sample. It corresponded to a patient with malignant effusion secondary to lymphoma.</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">In this study, the FluoroType MTB® technique in PF demonstrated limited utility in the diagnosis of TPE due to its low sensitivity (13%). Nevertheless, in 6.5% of patients with TPE, diagnostic confirmation was made exclusively using this PCR technique.</p><p id="par0080" class="elsevierStylePara elsevierViewall">PCR techniques for diagnosing tuberculosis can be done using commercial methods or in-house methods developed in the laboratory.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Among the former, the Xpert® MTB/RIF test has been endorsed by the World Health Organization.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> In a Cochrane review, the overall sensitivity of the Xpert® MTB/RIF test in PF was 51% (95% CI 40%–63%), but decreased to 18% (95% CI 10%–31%) when the gold standard applied was a clinical diagnosis of PTE without microbiological confirmation. The test had a specificity of 99%.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> In a recent study, the new Xpert® MTB/RIF Ultra technique in PF showed an overall sensitivity that was superior to a culture (44% vs. 26%) in 208 patients with TPE, though the percentage of positive results decreased to 20% in cases of probable pleural tuberculosis.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">This is the first study to evaluate diagnostic accuracy of the FluoroType MTB® in PF. Another study compared the FluoroType MTB® and Xpert® MTB/RIF tests in 70 samples of sputum which were positive for <span class="elsevierStyleItalic">M. tuberculosis</span> and 226 which had negative cultures.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> The sensitivity of the FluoroType MTB® was superior to the Xpert® MTB/RIF (88.6% vs. 78.6%), but its specificity was lower (59.7% vs. 93.8%). That is, it had a greater percentage of potential false positives.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Though the FluoroType MTB® test allows for receiving results quickly and is cheaper than the Xpert® MTB/RIF Ultra test (approximately Є39 vs. Є76 per sample in our center, respectively), it does not allow for detecting drug resistance, as the latter does. Some reasons that may underlie the low sensitivity of PCR techniques in PF are the paucibacillary nature<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> of pleural tuberculosis and the presence of PCR inhibitors in this medium.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,20</span></a> The utility of the FluoroType MTB® is also limited by false positives which, although they were not frequent in our study, have already been noted in previous works.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,19</span></a> They have been associated with cross-reactions or an unsatisfactory reading of the curve shown by the FluoroCycler®.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Repeating the PCR technique when the first test is negative has previously been evaluated. One study showed that out of 645 cases of pulmonary tuberculosis which had a positive Xpert® MTB/RIF test, 615 (95%) were positive on the first determination and 30 (5%) on the second.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In a work on 168 cases of extrapulmonary tuberculosis, the percentages were 89% and 11%, respectively.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In our study, a second sample did not increase diagnostic accuracy in any patient.</p><p id="par0100" class="elsevierStylePara elsevierViewall">This study has several limitations. First, the main limitation is that it included a small number of patients with TPE, of which one-third did not have microbiological confirmation. Nevertheless, the inclusion of patients with probable TPE according to the established criteria is widely accepted in the medical literature. Our work only tested a sputum sample using the PCR technique in half of the cases. We were also unable to obtain a second PF sample in 11% of patients with TPE who had a first FluoroType MTB® determination which was negative. Likewise, a microscopic examination and auramine staining of PF specimens (the only test that can be compared to the PCR due to the quick obtaining of results) were not performed due to the low sensitivity this technique showed in a previous study in our center (3% of positive results in a series of 274 PF samples).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Lastly, unlike other PCR techniques on the market, an inherent limitation of the FluoroType MTB® test is that it does not detect resistance to antituberculosis drugs.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions</span><p id="par0105" class="elsevierStylePara elsevierViewall">The FluoroType MTB® technique applied to PF has a limited capacity for diagnosis tuberculosis and its specificity is not absolute. The fact that PTE was confirmed exclusively by means of this technique in 6.5% of cases does not seem to justify its implementation in routine clinical practice outside of the scope of research. Of the 31 cases of TPE, 21 (68%) were confirmed histologically or microbiologically; the other cases were considered probable.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">The operational characteristics of FluoroType MTB® in PF for identifying tuberculosis were a sensitivity of 13%, a specificity of 99%, a positive likelihood ratio of 11, and a negative likelihood ratio of 0.9. The diagnostic efficacy data for sputum samples were 21%, 91%, 2.4, and 0.9, respectively. PF and sputum cultures in solid and liquid media had greater sensitivity (36% and 31%, respectively). A second FluoroType MTB® test in PF was negative for 24 patients who had TPE and for whom the first FluoroType MTB® test was also negative.
