    "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">It is estimated that more than 240 million individuals worldwide are infected by the hepatitis B virus &#40;HBV&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">1</span></a> Of these&#44; between 15 and 20 million are coinfected with the hepatitis delta virus &#40;HDV&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">2</span></a> Between 130 and 150 million individuals have a chronic infection by the hepatitis C virus &#40;HCV&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a> In Western countries&#44; the number of infected individuals has declined significantly thanks to the implementation of HBV vaccination and to the surveillance of blood designated for transfusions&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">4</span></a> However&#44; the high immigration rates experienced by many of these countries in recent years have led to a substantial increase in these diseases&#44; causing a public health problem&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">5</span></a> In the province of Almeria&#44; specifically the area of Poniente&#44; there is a large group of African immigrants&#44; which can exceed 30&#37; of the population in many municipalities&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">The aim of this study is to determine the prevalence of the serological infection markers of HBV&#44; HCV and HDV in African patients treated at a specialized consultation&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">A descriptive study was conducted on the serological markers of HBV&#44; HCV and HDV in African patients treated in the Tropical Medicine Unit of Hospital of Poniente &#40;Almeria&#44; Spain&#41; between October 2004 and June 2015&#46; All patients underwent a medical history review and complete physical examination&#46; Laboratory tests were requested with hemogram&#44; renal and hepatic function and&#44; if not previously performed&#44; HBV&#44; HCV and HIV serology&#46; Patients with HIV infection were referred to a specialized medical office and were therefore excluded from the study&#46; For hepatitis B&#44; the hepatitis B surface antigen &#40;HBsAg&#41;&#44; core antibodies &#40;HBcAb&#41; and surface antibodies &#40;HBsAb&#41; were measured&#46; When the presence of HBsAg was detected&#44; the envelope antigens &#40;HBeAg&#41; and antigen antibodies &#40;HBeAb&#41; were measured&#46; In the event of suspected acute infection&#44; IgM HBcAb levels were measured&#46; The condition of chronic hepatitis B virus carrier was defined as the presence of HBsAg in 2 measurements separated by a minimum of 6 months&#46; For the patients who showed HBcAb as the only marker&#44; a past infection was considered if the patient had normal transaminase levels&#46; If these levels were high&#44; a polymerase chain reaction &#40;PCR&#41; on the HBV was performed to rule out viral replication as an indication of occult hepatitis B &#40;OHB&#41;&#46; The presence of HBsAb with no other serological markers was considered the result of HBV vaccination&#46; For the patients with HBsAg&#44; DNA quantification of HBV was performed using real-time PCR &#40;COBAS AmpliPrep&#47;COBAS TaqMan-Roche Diagnostics&#41;&#44; with a limit of detection of 20<span class="elsevierStyleHsp" style=""></span>IU&#47;mL&#46; The HBV genotype was determined using partial magnification and sequencing of the gene that encodes the HBsAg &#40;Abbott RealTime HBV&#41;&#46; Inactive chronic carriers were defined as those patients with HBsAg&#40;&#43;&#41; and HBeAb&#40;&#43;&#41;&#44; a viral load less than 2000<span class="elsevierStyleHsp" style=""></span>IU&#47;mL and persistently normal transaminase levels&#46; The rest of the patients with HBsAg&#40;&#43;&#41; were classified as chronic hepatitis B HBeAg&#40;&#43;&#41; &#40;HBeAb negative&#41; and chronic hepatitis B HBeAg&#40;&#8722;&#41; &#40;HBeAb positive&#41;&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">For the patients who had HBsAg&#44; the measurement of antibodies against HDV &#40;ETI-AB-DELTAK-2 &#91;total antibodies&#93; and ETI-DELTA-IGMK-2 &#91;class IgM antibodies&#93;&#41; was requested&#44; which were performed using enzyme immunoassay &#40;DiaSorin<span class="elsevierStyleSup">&#174;</span>&#41;&#46; For those patients whose results were positive&#44; the RNA of this virus was measured using molecular hybridization techniques&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The detection of anti-HCV antibodies was performed using chemiluminescence assays &#40;Beckman-Coulter&#44; DXI<span class="elsevierStyleSup">&#174;</span>&#41;&#46; A Western Blot &#40;BioRad<span class="elsevierStyleSup">&#174;</span>&#41; confirmation system was used for the anti-HCV results between 1 and 4<span class="elsevierStyleHsp" style=""></span>IU&#47;mL&#46; The quantification of the HCV viral load was performed using real-time PCR &#40;HCV COBAS 6800&#47;8800&#44; Roche<span class="elsevierStyleSup">&#174;</span>&#41;&#46; The HCV genotype was determined using direct automated sequencing of the NS5B&#44; E1 and E2 regions of the viral genome&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">SSPS software version 17 was employed for the statistical study&#46; The patients were divided into 2 groups &#40;Maghreb and sub-Saharan Africans&#41;&#46; A descriptive analysis of the quantitative variables was performed using mean&#44; standard deviation and range&#46; To detect the potential differences between the 2 groups&#44; we used Student&#39;s <span class="elsevierStyleItalic">t</span>-test for independent samples&#44; when the hypothesis of normality was met&#44; and the Mann&#8211;Whitney <span class="elsevierStyleItalic">U</span> test otherwise&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">A total of 2518 patients were included in the study&#46; Their sociodemographic characteristics are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46; Eighty-eight percent were men&#44; and more than 90&#37; came from sub-Saharan Africa&#44; especially from the western area &#40;690 from Senegal&#44; 530 from Mali and 355 from Guinea-Bissau&#41;&#46; Some 59&#46;8&#37; of the sub-Saharan patients were undocumented immigrants&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the distribution of the various serological patterns globally and by origin &#40;Maghreb and sub-Saharan&#41;&#46; A total of 1984 patients &#40;78&#46;8&#37;&#41; had some marker of HBV infection&#46; Fifty-one patients &#40;2&#37;&#41; with positive HBsAg were lost during follow-up&#44; and their clinical and virologic stage could not be established&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Of the 638 patients with chronic HBV infection who could be followed-up&#44; the distribution by age group was as follows &#40;patients with HBsAg&#47;total number of patients in that age group&#41;&#58; &#60;18 years &#40;17&#47;89&#59; 19&#46;1&#37;&#41;&#59; 19&#8211;29 years &#40;297&#47;1108&#59; 26&#46;8&#37;&#41;&#59; 30&#8211;39 years &#40;244&#47;917&#59; 26&#46;6&#37;&#41;&#59; and &#8805;40 years &#40;80&#47;404&#59; 19&#46;8&#37;&#41;&#46; In terms of sex&#44; 595 men &#40;26&#46;9&#37;&#41; and 43 women &#40;13&#46;9&#37;&#41; had chronic HBV infection &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>&#46;001&#41;&#46; Most of the women were of childbearing age&#44; and 2 cases of chronic HBV hepatitis were diagnosed at the time of the pregnancy&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">The HBV genotype was analyzed in 148 patients&#44; with E the most common &#40;121&#59; 81&#46;7&#37;&#41;&#44; followed by D &#40;13&#59; 8&#46;8&#37;&#41;&#44; A &#40;12&#59; 8&#46;1&#37;&#41; and B &#40;2&#59; 1&#46;4&#37;&#41;&#46; Seventy patients &#40;11&#37;&#41; required antiviral treatment&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">Detection of HDV antibodies was conducted in 400 patients&#46; The results were positive in 19 patients &#40;4&#46;75&#37;&#41;&#46; Viremia due to the HDV was observed 4 of them through PCR&#46; The measurement of HCVAb was performed in 2505 patients&#44; and the results were positive in 68 &#40;2&#46;7&#37;&#41; of them&#46; The genotype was analyzed in the 26 cases with positive RNA&#44; with 2 the most common &#40;<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>19&#41;&#46; HBV and HCV coinfection was confirmed in 3 patients&#46; Of the 26 patients with chronic HCV infection&#44; the distribution by age group was as follows &#40;patients with positive HCV RNA&#47;total number of patients in that age group&#41;&#58; &#60;18 years &#40;0&#47;89&#41;&#59; 19&#8211;29 years &#40;4&#47;1108&#59; 0&#46;4&#37;&#41;&#59; 30&#8211;39 years &#40;15&#47;917&#59; 1&#46;6&#37;&#41;&#59; and &#8805;40 years &#40;7&#47;404&#59; 1&#46;7&#37;&#41;&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">At the time of the diagnosis&#44; 4 patients had a hepatocarcinoma&#44; and 11 had hepatic cirrhosis&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">With the data from this study&#44; we can confirm that viral hepatitis&#44; especially chronic HBV infection&#44; represents a major healthcare problem in areas with groups of African immigrants&#46; This was not a study on the prevalence of the disease in the general immigrant population&#44; but rather in a select population referred to a specialized medical office&#46; However&#44; the study was conducted in Spain&#44; which has the largest number of African patients&#44; and the data are sufficiently demonstrative of the importance of viral hepatitis in this group and its repercussion on the healthcare system and public health in Spain&#46; It should be noted that the 3 main reasons for consultations included abnormal hepatic test results&#44; which in most cases were due to HBV infection&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">For HBV&#44; more than 78&#37; of the patients showed serum markers of active or past infection&#46; This proportion was very high in the sub-Saharan patients&#46; The overall prevalence of markers is higher than in other series&#44;<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">4&#44;6&#8211;9</span></a> which is probably due to the patients&#8217; country of origin&#44; the profiles of the diseases treated in the units where the study was conducted and&#44; very likely&#44; to the implementation of a prospective screening protocol for these diseases in the primary care centers of the area&#44; which detected a high number of cases&#46; In terms of the patients&#8217; place of origin&#44; a recent study showed that the countries with the highest representation in our study &#40;Mali&#44; Senegal or Mauritania&#41; are indeed those with the highest prevalence&#44; which varies between 11&#37; and 16&#37;&#46;<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">10</span></a> This situation&#44; coupled with the fact that this was a study on a selected population and not the general population&#44; would explain the high rates of chronic infection in immigrants&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">The prevalence of HBV infection was significantly higher in the men than in the women &#40;26&#46;9&#37; vs&#46; 13&#46;9&#37;&#44; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>&#46;001&#41;&#44; data that agree with other studies on the sub-Saharan population&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">11</span></a> This fact could be explained by the earlier start of sexual relations&#44; the higher number of sexual partners&#44; and the greater proportion of alcohol and parenteral drug abuse among the men&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">11</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">The standard follow-up of patients with HBV infection requires performing several annual visits with a specialist&#44; with laboratory and imaging tests&#44; whose objective is to assess the need for treatment and to prevent or diagnose the presence of complications&#46; When there is an indication for treatment&#44; the number of visits increases substantially&#46; For example&#44; in our center&#44; 68&#37; of the patients undergoing HBV treatment are immigrants&#46; All of this has a direct impact on the increased healthcare burden and pharmaceutical expenditure&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">From the public health point of view&#44; there is an obvious risk of transmission in the host countries&#46; For some years now&#44; there has been an infant HBV vaccination program in Spain&#59; however&#44; a large number of middle-aged individuals have not be immunized and are therefore susceptible to contracting the disease&#44; mainly through sexual relations&#46; Disease screening in the immigrant population and vaccination of unimmunized individuals and cohabitants have been essential for preventing transmission in our community&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">It is worth noting the high number of cases with positive HBcAb as the only marker&#46; Although this might be due to false positives or patients in the window period of acute hepatitis&#44; this is typically due to the loss of immunity against HBV with the passing of time or&#44; less likely&#44; OHB&#46; Implementing OHB screening for individuals with HBcAb with no other data indicating liver disease is not cost-effective&#46;<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">12</span></a> However&#44; this possibility should be taken into account for immunosuppressed patients in whom hepatitis reactivation can occur with fatal results&#46;<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">13</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">With regard to HBV&#8211;HDV and HBV&#8211;HCV coinfection&#44; our study shows results similar to other published series&#46;<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">7&#44;8</span></a> It is also worth noting the low prevalence of chronic HCV infection in African patients&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">In conclusion&#44; given the high prevalence of viral hepatitis among the African immigrant population&#44; especially of sub-Saharan origin&#44; it is imperative that they be screened for this disease&#46; This screening should preferably be conducted in primary care&#44; given that this is the entry into the healthcare system&#46; Moreover&#44; it is essential that universal vaccination be recommended to those patients with no immunity markers&#44; as well as to conduct healthcare campaigns in the immigrant communities to prevent HBV transmission in these communities and among the native population&#46; Lastly&#44; system the healthcare should provide healthcare centers with the necessary resources&#44; both human and material&#44; to address this type of patient and the potential complications resulting from these diseases&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conflicts of interest</span><p id="par0105" class="elsevierStylePara elsevierViewall">The authors declare that they have no conflicts of interest&#46;</p></span></span>"

Abstract
Background

Viral hepatitis is a significant health problem in African countries. The increase in the immigrant population from this continent represents a challenge for the Spanish healthcare system.

Material and methods

A descriptive study was conducted on the prevalence of the serological markers of hepatitis B (HBV), C (HCV) and D (HDV) in African immigrants treated in a specialised doctor's office.

Results

The study included 2518 patients (87.7% Sub-Saharan natives), with a mean age of 31.3 years. Some 78.8% of the patients had a positive infection marker for HBV, and 638 patients (25.3%) were diagnosed with active hepatitis B (HBsAg+). In 19 cases, antibodies against HDV were detected (4 cases with detection of the viral genome). Sixty-eight patients had antibodies against HCV, 26 of whom had a positive viral load.

Conclusions

The high prevalence of viral hepatitis in immigrants, especially HBV infection, represents a significant change in the profile of patients treated in Spain and requires measures aimed at early diagnosis and transmission prevention.

Introducción

Las hepatitis virales representan un importante problema de salud en los países africanos. El incremento de la población inmigrante procedente de este continente supone un reto para el sistema sanitario.

Material y métodos

Estudio descriptivo sobre la prevalencia de marcadores serológicos de hepatitis viral B (VHB), C (VHC) y delta (VHD) en inmigrantes africanos atendidos en una consulta especializada.

Resultados

Se incluyó a 2.518 pacientes (87,7% subsaharianos) con una edad media de 31,3 años. El 78,8% presentó algún marcador positivo de infección por el VHB y en 638 pacientes (25,3%) se diagnosticó una hepatitis B activa (AgHBs+). En 19 casos se detectaron anticuerpos frente al VHD (4 con detección del genoma viral). Sesenta y ocho pacientes presentaron anticuerpos contra el VHC, de los que 26 tenían carga viral positiva.

Conclusiones

La elevada prevalencia de hepatitis virales en inmigrantes, en especial la infección por VHB, ha supuesto un cambio significativo en el perfil de pacientes atendidos en nuestro país, y precisa de medidas encaminadas a un diagnóstico precoz y prevención de la transmisión.
