    "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 57-year-man&#44; smoker of a pack a day&#44; with a history of arterial hypertension and moderate-effort dyspnea&#44; consulted due to symptoms that started 3 days ago with fever and chills&#44; with a certain increase in the typical dyspnea&#44; associated with yellowish and occasionally hemoptoic expectorate&#46; The patient also reported general syndrome of 3&#8211;4 weeks of progression&#44; with a weight loss of approximately 4<span class="elsevierStyleHsp" style=""></span>kg&#46; An anteroposterior and lateral radiography of the chest revealed a round consolidation at the base of the right hemithorax&#44; signs of generalized hyperinflation with diaphragmatic flattening and horizontalization of the ribs suggestive of chronic bronchial disease and blunting of the costophrenic angle &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>A and B&#41;&#46; Thoracic computed tomography revealed signs of paraseptal and centriacinar emphysema&#44; with large bullous lesions&#44; especially in the right hemithorax&#44; one of which presented an air-fluid level&#44; surrounded by an area of consolidation&#44; all of which suggested superinfection &#40;<a class="elsevierStyleCrossRef" href="#fig0010">Fig&#46; 2</a>A and B&#41;&#46; Bronchoscopy revealed redness of the bronchial tree&#46; Samples were obtained for bacteriological and histological processing&#44; with negative results including the cultures for mycobacteria&#46; The alpha 1-antitrypsin reading was normal&#44; and spirometry confirmed obstruction with forced expiratory volume in 1 second of 40&#37;&#46; Empiric antibiotic therapy was started with amoxicillin&#8211;clavulanate at high doses&#44; which the patient maintained for 2 weeks&#44; with good progression&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span>"
