"textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">Atrial fibrillation (AF) is the most prevalent type of arrhythmia in our setting. It has been estimated that up to 25% of individuals older than 40 years develop AF.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">1,2</span></a> In the study by Barrios et al.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">3</span></a> on more than 120,000 patients treated in primary care (PC) showed that 6.1% had AF.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">AF is responsible for at least 35% of strokes,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> for which the best therapeutic prevention option, except where contraindicated, is long-term oral anticoagulation, classically with vitamin K antagonists (VKAs)<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">4</span></a> or, more recently, with direct oral anticoagulants (DOAs). DOAs have been shown to be as effective as VKAs in nonvalvular AF (NVAF) but with a better safety profile, especially because of the significantly lower risk of intracranial hemorrhage,<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">5–7</span></a> which has resulted in DOAs becoming the drugs of choice for some patients.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">A significant number of patients with AF do not undergo appropriate preventive treatment.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">8</span></a> Several recent Spanish studies have revealed that a significant proportion of patients with NVAF treated with VKA have inadequate anticoagulation control, which increases the risk of stroke and severe hemorrhage.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">9–12</span></a> It is essential that we ensure that the treatment and follow-up of anticoagulated patients complies with the best quality care criteria.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">13</span></a> However, if the necessary and available resources are not known, we will never determine the starting point for implementing improvements, and we will be left acting without goals and, as a result, without the ability to assess the results.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The main objective of this study was to therefore understand how the healthcare process was conducted for anticoagulated patients with NVAF. The secondary objective was to determine the healthcare resources and needs currently involved in caring for these patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design and participants</span><p id="par0025" class="elsevierStylePara elsevierViewall">The Healthcare Resources and Needs in the Anticoagulant Therapy of Patients with Atrial Fibrillation (SAMOA) study was a cross-sectional multicenter study conducted in the public healthcare setting throughout Spain, between May and September 2014. The study was approved by the Clinical Research Ethics Committee of the Jimenez Diaz Foundation and was endorsed by the Spanish Society of Quality Care (SECA), the Spanish Society of Neurology (SEN), the Spanish Society of Cardiology (SEC), the Spanish Society of Thrombosis and Hemostasis (SET), the Spanish Society of Internal Medicine (SEMI), the Spanish Society of Primary Care Physicians (SEMERGEN), the Spanish Society of General and Family Practitioners (SEMG), the Spanish Society of Family and Community Medicine (SEMFYC) and the Spanish Society of Urgent and Emergency Medicine (SEMES). The recruitment of participants was first conducted through an invitation using a leaflet (which provided the study's URL address) and then by email, inviting the physicians registered in the “<span class="elsevierStyleItalic">El Médico Interactivo</span>” (“The Interactive Physician”) database (<a href="http://www.elmedicointeractivo.com/">http://www.elmedicointeractivo.com</a>) who had the specialties of interest for the study (PC, 30,428; Cardiology, 3427; Hematology, 1814; Internal Medicine, 4537; Neurology, 2848; Emergency care, 3169; total, 46,664 physicians). The specialists were assigned to public hospitals in Spain. At least one representative for each of these hospitals was requested, as well as one or more PC physicians of the area of influence of each hospital.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study tools</span><p id="par0030" class="elsevierStylePara elsevierViewall">A multidisciplinary scientific committee developed 2 questionnaires to cover the study objectives on the healthcare resources and needs for patients with NVAF undergoing anticoagulant therapy: one targeted to health professionals who work in health centers and another for those who work in hospital settings. The participating physicians voluntarily completed the online questionnaires, without any type of incentive, on a specific microsite.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">A descriptive analysis was conducted using mean and standard deviation for the quantitative variables and frequencies and percentages for the qualitative variables. We employed Fisher's exact test to compare the qualitative variables. The statistical analysis was performed with SAS version 9.1.3 Service Pack 3 (SAS Institute Inc., Cary, NC, USA).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">A total of 893 physicians involved in caring for patients with NVAF undergoing anticoagulant therapy participated in the study. Forty-two percent of the physicians worked in PC centers, and the remaining 58% worked in specialized care (SC) centers. Of these physicians, 253 were cardiologists (29%), 46 were internists (6%), 24 were hematologists (3%), 14 were neurologists (2%) and 25 were emergency care physicians (3%). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the distribution of participants by autonomous community.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Healthcare data in primary care</span><p id="par0045" class="elsevierStylePara elsevierViewall">The surveyed physicians had an average professional experience in PC of 23.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.4 years. A total of 263 (60%) physicians stated that the follow-up of patients undergoing anticoagulant therapy was shared with SC, and 146 (34%) physicians stated that the follow-up was exclusively performed in PC. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the percentages of indication, start and follow-up of anticoagulant therapy according to the PC physicians. Two hundred and seventy-three (62%) of these physicians stated that DOAs may be indicated and prescribed from PC.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Action protocols and quality of the follow-up</span><p id="par0050" class="elsevierStylePara elsevierViewall">According to 323 of the participants (74%), their work center had a protocol for managing patients who are anticoagulated with VKA, and 269 (62%) declared that the protocol was shared between PC and SC. For patients who were anticoagulated with DOAs, these figures were 229 (52%) with PC-exclusive protocols and 128 (29%) with shared protocols (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). A total of 400 (91%) PC physicians stated that they had sufficient knowledge for the appropriate use of DOAs, but 218 (50%) acknowledged that there were difficulties when dealing with special situations.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> lists the rates for applying safety and quality control systems in the follow-up of anticoagulated patients in PC. Sixty-nine (16%) of those surveyed indicated that they did not have digital puncture coagulometers for measuring the patients’ international normalized ratio (INR). Three hundred and thirty (76%) participants indicated that more than 75% of the patients undergoing anticoagulant therapy had good therapeutic adherence, and 252 (58%) of the participants estimated that more than 75% of the patients received health education. Among the PC physicians, 40 (9%) and 19 (4%) indicated that more than 25% of their patients had experienced a severe thromboembolic or hemorrhagic event, respectively.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Barriers in the use of anticoagulant therapy in primary care</span><p id="par0060" class="elsevierStylePara elsevierViewall">In terms of the barriers to using oral anticoagulants, 288 (66%) of those surveyed stated that there were administrative barriers to free prescription, and 175 (40%) detected other barriers related to the prescription circuit and subsequent monitoring. Some 148 (34%) of the physicians identified shortcomings in training, and 76 (17%) identified lack of technical media, such as digital puncture coagulometers and access to time in therapeutic range (TTR) data (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Healthcare data in specialized care (hospital centers)</span><p id="par0065" class="elsevierStylePara elsevierViewall">The participants had an average experience in SC of 21.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.4 years. A total of 371 (81%) of those surveyed stated that there was no anticoagulation unit in their center. Of the 131 (47%) centers in which more than one specialist participated, 49 (37%) did not provide matching information on the presence or not of anticoagulation units. Of the 157 general or university hospitals, 130 (83%) lacked anticoagulation units. This situation also occurred in 36 (77%) of the 47 regional or district hospitals. Among those that had anticoagulation units, 76 (89%) were dependent on the hematology department and 8 (9%) on the cardiology department. Two hundred and thirteen (47%) specialists stated that unstable coagulation checks, with regard to the prescription of a DOA in place of VKAs, were performed by the cardiologist, followed by the hematologist (162, 36%), family doctor (52, 11%) and internist (29, 6%).</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Action protocols and quality of the follow-up</span><p id="par0070" class="elsevierStylePara elsevierViewall">A total of 203 (63%) participants indicated that their hospital had clinical protocols or guidelines for the care of anticoagulated patients, while 259 (57%) indicated that there was some protocol for the care of complications associated with anticoagulant therapy. According to 203 (45%) specialists, their hospital center had specific protocols or circuits for the initial indication and follow-up of patients with DOAs, and 261 (57%) expressed that the follow-up protocols were common between PC and SC. One hundred and seventy-one (37%) physicians stated that more than 75% of the patients received specific healthcare education on their condition. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the application of safety and quality control systems in the follow-up of anticoagulated patients.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Among the consulted specialists, 244 (53%) and 265 (58%) declared that the approximate annual incidence rate of severe thromboembolic and hemorrhagic complications, respectively, of the patients monitored in their center was less than 3%. The remaining physicians estimated the annual incidence rate of both types of complications at 4% to 10%.</p><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Barriers to the care of patients undergoing anticoagulant therapy in specialized care</span><p id="par0080" class="elsevierStylePara elsevierViewall">Two hundred and eighty-four (62%) surveyed specialists stated that there was no restriction in their work center for prescribing DOAs, while the remaining 172 (38%) stated otherwise. Regarding the main barriers to the care of anticoagulated patients, 297 (65%) specialists mentioned the presence of administrative obstacles to free prescription, and 284 (62%) found them in the prescription and control circuit, 191 (42%) found obstacles in aspects of physician training, and 103 (23%) found obstacles in the technical media (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p></span></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">For decades, the importance of maintaining adequate anticoagulation in patients with NVAF to prevent strokes and hemorrhagic complications has been known.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">4,14</span></a> This study described, through a survey of physicians involved in the care of anticoagulated patients with NVAF, the relevant aspects of standard clinical practice. In this survey, 42% and 58% of the participants were PC and SC physicians, respectively, with representation from all autonomous communities.</p><p id="par0090" class="elsevierStylePara elsevierViewall">According to the information obtained in PC, the percentage of autonomous family physicians for the indication and start of anticoagulant therapy is low; only 42%. However, this figure is much higher than in previous studies, such as the Current Situation of Atrial Fibrillation in Primary Care in Spain study, in which it was only 12.4%.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">9</span></a> This probably reflects a marked increase in the involvement of PC in anticoagulant therapy. In fact, most of those surveyed stated that follow-up is performed in the health center itself, which represents a change in the health care model and could result in reduced healthcare costs.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">15</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Three quarters of the family physicians stated that they had guidelines or protocols for the follow-up of patients anticoagulated with VKAs, while only half of the physicians had these protocols/guidelines for DOAs, as shown by the Analysis of the Regional Management and Observation of Anticoagulation (AGORA) report.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">16</span></a> Although the weight of anticoagulant therapy falls mainly on PC, we observed that the recommendations for periodic patient reassessment are poorly applied, as is INR and TTR monitoring, parameters that are closely associated with relevant clinical results such as stroke, hemorrhage, myocardial infarction and mortality.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">17</span></a> In fact, 16% of these physicians indicated that they did not have digital puncture coagulometers for measuring the INR. Due to their importance, it is essential that these tools be provided to all health centers to achieve greater control and faster results.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">18</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">We also observed various shortcomings in SC. Thirty-seven percent of the specialists indicated that they did not have protocols for the use of oral anticoagulation, and 43% had no protocols for addressing the potential complications of anticoagulant therapy. Furthermore, there was an absence of shared work protocols between PC and SC, revealing the poor coordination between healthcare levels for anticoagulation in general and DOAs in particular. Additionally, inconsistencies were detected in the hospitals regarding the presence of an anticoagulation unit. There were cases in which surveyed specialists from the same hospital responded differently to the question of whether there was an anticoagulation unit in their center, which is worrying given the benefits of these specialized departments for achieving optimal anticoagulation.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">There was a noteworthy high perception of embolism and hemorrhage events among PC physicians, which could reflect a certain lack of confidence in the safety and efficacy of anticoagulant therapy. Several studies have suggested that previous experience regarding thromboembolic or hemorrhagic complications can affect the prescription of anticoagulant therapy,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">21</span></a> such as a reduction in the prescription of VKA after a severe hemorrhage.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">22</span></a> However, there is evidence that the rate of hemorrhagic complications is similar in patients treated in the PC and SC setting.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">23</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">A key component for optimal anticoagulant therapy is patient education.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">24,25</span></a> This study showed an insufficient level of healthcare education, in particular for DOAs, at both healthcare levels. An effective patient education program can increase treatment adherence<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">26</span></a> and, in our setting, is an aspect that needs improving.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Lastly, the main barriers identified for the care of anticoagulated patients with NVAF were the administrative obstacles to free prescription of anticoagulant therapy and the prescription circuit and expenditure control, which are based on economic criteria, which are not scientifically supported and create significant autonomic differences.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">27</span></a> These appraisals were consistent among the PC and SC physicians, indicating potential areas for improvement in these patients’ healthcare process.</p><p id="par0120" class="elsevierStylePara elsevierViewall">This study has a number of limitations. As with all qualitative studies, this study was exploratory and does not seek to generalize its conclusions. Given that it is a survey, the results are based on opinion and not on objective data. Additionally, the included physicians mainly represented centers in Andalusia, the Community of Madrid, Catalonia and the Community of Valencia, and others to a lesser extent. Therefore, differences in operation and funding could have influenced the participants’ responses. In SC, most of the specialists (29%) were cardiologists, which could create a bias in the assessment of the healthcare process. The study is therefore a first approach to the reality and shortcomings associated with oral anticoagulant therapy and not a systematic investigation.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In summary, the SAMOA study provides relevant information on the healthcare process of anticoagulated patients in Spain. The role of PC in identifying new cases and the therapeutic control has become preeminent. However, the monitoring of anticoagulated patients is heterogeneous in both the PC and SC settings. There is a clear lack of standardization of protocols and quality in the TTR follow-up. We need to promote the relationship between healthcare levels, establish strategies for standardizing the follow-up and improve training in the use of DOAs. It is worth implementing and announcing anticoagulation units in the various healthcare areas to improve the quality and results of anticoagulant therapy. The improvement strategies should probably be directed more toward PC than to SC. We can conclude that a number of administrative barriers affect the prescription of DOAs, which negatively affects the possibility of an optimal use.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Funding</span><p id="par0130" class="elsevierStylePara elsevierViewall">This study was funded by Boehringer Ingelheim. The questionnaires referred to the healthcare process, the indication and prescription of anticoagulant therapy and the barriers and deficiencies present for these patients.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 893 physicians participated in the study, 437 of whom worked in primary care and 456 of whom were specialists (mostly cardiologists). Forty-two percent of the family doctors indicated that they assessed and prescribed anticoagulant therapy, and 66% performed the regular follow-up of these patients. In both healthcare settings, the physicians noted the lack of standardized protocols. There was also a lack of quality control in the treatment.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">The role of primary care in managing anticoagulated patients has grown compared with previous reports. The responses of the participating physicians suggest marked gaps in the standardization of the healthcare process and several areas for improvement in these patients’ follow-up. The promotion of training in direct-acting anticoagulant drugs remains pivotal.</p></span>"
