Within the framework of globalization and in the context of a pronounced financial crisis, skilled European workers are increasingly traveling to low-income countries with emerging economies in search of new work opportunities (e.g., the economic growth of sub-Saharan Africa went from 4.7% in 2013 to a predicted 5.2% for 2014).<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> In short, these individuals are traveling to ecosystems that have a high incidence of infectious diseases.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A study performed in 2012 at the North Metropolitan International Health Unit revealed an increasing trend in international business travelers (IBTs) in recent years. The study analyzed 1227 travelers (487 in 2008 and 740 in 2011), who as a whole showed a reduction in tourist travel (from 70% to 64%) and a significant increase in business travel (3.7% to 8%) (data not published).</p><p id="par0015" class="elsevierStylePara elsevierViewall">So far, the traveling population considered to be at the greatest risk has been immigrants established in Europe who travel to their countries of origin; the so-called “travelers visiting friends and family” (TVFF).<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">3–6</span></a> Their greater risk is due to the longer duration of their travel and their close contact with the local population. Nevertheless, a number of authors have warned of the increase in severe imported diseases acquired by the IBT group.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> This increase has been attributed mainly to stays that can be as long as (or longer) than those of TVFFs<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> or to the accumulation of multiple shorter trips, which are not always preceded by preventive measures.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">9</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">To date, there have been no studies similar to this one in PubMed, EMBASE or the summaries of the Spanish Biomedical Index (IME-biomedicine). The present study as a whole should be considered an approach to the problem of traveler infections, an approach that attempts to provide data that can help generate new hypotheses, spark debate and elicit the opinion of experts.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">An observational retrospective study was conducted that included all IBTs who visited the International Health reference unit of the study area (North Metropolitan PROSICS, Santa Coloma de Gramenet, Catalonia, Spain). This unit covers 1,400,000 individuals in the north metropolitan area of Barcelona and belongs to the main public provider of healthcare services in Catalonia (Catalan Institute of Health). Travelers must, however, make a nominal copayment that includes Travelers Advice and the international vaccination. Data were obtained through an analysis of questionnaires from travelers treated in pretravel visits in 2007, given that this is the year prior to the date considered as the start of the crisis (2008, the bankruptcy of Lehman Brothers). Data was once again acquired 5 years later in 2012, when the crisis was considered established. At the same time, the data collected from 2007 to 2012 was reviewed to determine the evolution of the percentage of IBTs compared with the total number of travelers who consulted prior to travel in our unit.</p><p id="par0030" class="elsevierStylePara elsevierViewall">IBTs were defined as those travelers whose declared reason for traveling abroad was to conduct business-related activities, technical services or labor.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The following variables were analyzed: age, sex, trip destination divided into geographical areas (Sub-Saharan Africa, Indian subcontinent, Southeast Asia, Central America and South America), trip duration (<1 month, 1–3 months, 3–6 months, ≥6 months), type of IBT based on native country of origin (born in Spain) or immigrant (born outside Spain), area in the destination (rural/urban), use of malaria prevention treatment (yes/no/self-treatment), presence of comorbidities (yes/no) and comorbidity characteristics.</p><p id="par0040" class="elsevierStylePara elsevierViewall">To better characterize the IBT series, we used a randomly selected sample of TVFFs as a control group (1:1) from individuals who would have traveled during the same periods of 2007 and 2012 and compared the study variables. The data were analyzed using the Stata 10 program, version 10.0 (Stata Corporation, TX). We applied the chi-squared test to compare qualitative variables and Student's <span class="elsevierStyleItalic">t</span>-test to compare quantitative variables. We performed bivariate logistic regression to determine the association of variables and a tendency test (incidence ratio test [IRR], relative risk) by means of a Poisson regression. <span class="elsevierStyleItalic">p</span>-Values <.05 were considered statistically significant.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">During 2007 and 2012, a total of 9197 travelers were treated at the unit, 344 (3.74%) of whom stated that they were traveling for business reasons. In 2007 and 2012, there were 101 (2.8%) and 24 (4.5%) IBTs, respectively, which represents an increase of 142 travelers (58.7%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). The evolution in the number of IBTs during this period is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>, and the travel-related sociodemographic data is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The most frequent destination was Sub-Saharan Africa (47.6%). Some 88.9% of the travelers had been born in Spain, and 63.95% of the travelers visited rural areas. Some 23.2% of the travelers stated that they had comorbidities, with depression, heart disease, psoriasis, asthma and diabetes the most common, in order of frequency. Five (6.2%) travelers were treated with long-term immunosuppressive medication, and 1 (1.2%) of the travelers was pregnant (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">The increase in the number of IBTs during the study period translates into the significance of the trend test (IRR: 1.62; CI: 1.59–1.66; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). There was also significant increases in the proportion of men (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.006), age (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.05), the proportion of IBTs with extended stays (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001), travel to rural areas (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.009) and the presence of comorbidities (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.018) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The characteristics of the IBTs and TVFFs are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>, and the comparative analysis between the TVFFs of 2007 and those of 2012 are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">The present study attempts to reflect the effects of a period of economic crisis in IBTs, at least those that are treated in an international health unit. Our results show an increase in the number of IBTs. It is worth asking whether their profile is high risk, as is that of TVFFs, or whether it resembles that of tourist travelers. The answer to this question has significant health implications.</p><p id="par0060" class="elsevierStylePara elsevierViewall">A nonsignificant increase was observed in the absolute number and proportion of travelers to Sub-Saharan Africa (44.5% in 2007 vs. 50.2% in 2012), along with an increase in the mean stay. Therefore, taking into account that approximately 90% of the imported cases of malaria in Europe come from Sub-Saharan Africa,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> it is very likely that there is an increased risk of malaria, including severe malaria based on the greater age of IBTs. Consequently, we should reconsider who should be treated and how long the preventive antimalarial treatment should be maintained. The role of self-treatment in the context of long-term travel should also be reconsidered.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Although the IBT group's travel duration and length of stay in rural areas might be similar to those of the TVFF, we do not know whether the perception of risk is different between the two groups, which could influence these travelers’ approach to adopting preventive measures. In our study, the TVFFs also showed an increased number of trips, although with no other essential changes that would represent a modification of their risk profile compared with previous periods. There is evidence of the lower perception of disease risk among TVFFs during their stay abroad, as well as lower compliance with antimalarial chemoprophylaxis among tourists.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">12</span></a> International health practitioners tend to view IBTs as a population that is well informed on diseases that are mosquito-transmitted, sexually transmitted, waterborne or foodborne and on the risks of noninfectious diseases. This might not be the case, however. In fact, a number of experts agree with our results and include IBTs in a risk profile similar to that of TVFFs,<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a> although as stated earlier, there are no extensive studies that support these results. There is isolated evidence on the poor compliance, both for mosquito prevention and malaria chemoprophylaxis, of IBTs when compared with tourists.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">14–16</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">In this study, we also found an increase in the age of IBTs and the presence of comorbidities, which makes them more susceptible to certain diseases and travel-related risks.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> The most frequently identified disease was depression. Depression could be related to adaptation and, therefore, could worsen when traveling to other countries, far from family and in an unknown social context and environment. In a study that compared 2962 international travelers, IBTs had significantly increased alcohol consumption, problems with insomnia, reduced confidence in keeping up-to-date at work, anxiety, feelings of cultural shock and problems adapting.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">18,19</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">A multicenter study of the GeoSentinel organization conducted for 5 years (2007–2011) analyzed data from 42,173 travelers with imported diseases. Among them, 7 of the 28 (25%) registered deaths corresponded to IBTs, 3 of which were for severe malaria.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">20</span></a> Consequently, it seems reasonable to focus on the knowledge, approaches and preventive practices of IBTs, with special emphasis on malaria prevention.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">21</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Spain has felt the effects of an economic crisis that has raised unemployment rates above 20%. More than 82,000 Spaniards emigrated in search of work opportunities in 2012, which represented an increase of 5.5% compared with 2011.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">22</span></a> For the first time in more than 20 years, the migratory trend in 2013 was negative. If this flow remains constant in the medium term, Spain could resemble countries such as the United States or other European countries in which the number of IBTs is similar to that of TVFFs.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">In summary, the risk profile of IBTs in Spain could be considered high, closer to that of TVFFs, among whom the incidence of imported diseases is higher than that among tourists. Ours is a pioneering study in Spain and in Europe. New studies are therefore needed to compare the data resulting from this study and determine whether the increased risk in travel is truly accompanied by an increase in imported diseases. Healthcare practitioners should be conscious of this reality and identify risk factors for IBTs, especially for older individuals and those with underlying medical problems. Preventive measures should be focused on these individuals to provide understandable, realistic and applicable information. The new risk profiles for travelers should also be taken into account for making decisions on vaccines and chemoprophylaxis against malaria.</p><p id="par0090" class="elsevierStylePara elsevierViewall">IBTs represent a new high-risk traveler profile, which is important for international health units to consider. These units can minimize some of the risks through counseling prior to trips and by providing recommendations on vaccines and antimalarial chemoprophylaxis. The objective of this study was to assess whether this context has produced an increase in the risks to international travelers for work reasons (TWR).</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Observational, retrospective study. The study population included TWR who were attended before traveling at an International Health Unit in the year 2007 (the year before the initiation of the European crisis) and in the year 2012 (when the structural crisis was established). A comparative socioeconomic analysis was performed as well as an analysis of the risk factors present in both groups.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">In 2007 and 2012 a total of 9197 travelers were attended. Of these, there were 344 TWR (3.4%); 101 TWR (2.8%) in 2007 and 243 TWR (4.5%) in 2012 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). The average age of the travelers was 38.1 years (SD: 10.57). The most common destination was Sub-Saharan Africa, in 164 (47.6%) of the cases. Malaria chemoprophylaxis was prescribed to 152 travelers (44%) and 80 presented comorbidity (23.25%).
