    "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Tuberculous pleural effusion &#40;TPE&#41; is one of the most common forms of extrapulmonary tuberculosis&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Establishing a definitive diagnosis is difficult&#44; given that the percentage of cultures positive for <span class="elsevierStyleItalic">Mycobacterium tuberculosis</span> in both pleural fluid &#40;PF&#41; and sputum is low&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In a series of 320 patients with TPE&#44; the tuberculous bacillus was able to be isolated in just 18&#37; and 29&#37; of PF and sputum cultures &#40;solid and liquid media&#41;&#44; respectively&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In two other Spanish series&#44; PF and sputum cultures in solid media independently had an approximate sensitivity of 40&#37;&#46;<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3&#44;4</span></a> In addition to the low sensitivity&#44; the delay of several weeks in obtaining the results of conventional cultures is a limiting factor for making clinical decisions&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The combination of a histological analysis and culture of a pleural biopsy allowed for reaching a sensitivity of 86&#37; in 92 patients with TPE&#46;<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However&#44; pleural biopsy is an invasive&#44; operator-dependent procedure&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">Nucleic acid amplification or polymerase chain reaction &#40;PCR&#41; techniques are fast and specific&#46; However&#44; their sensitivity for TPE appears limited&#46; A study conducted in our center showed a sensitivity of 15&#37; for the Xpert&#174; MTB&#47;RIF test in PF&#46;<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In a meta-analysis of 24 studies&#44; this same automated PCR technique &#40;Xpert&#174; MTB&#47;RIF&#41; in PF had a sensitivity of 22&#46;7&#37; when it was used in patients with probable TPE&#44; that is&#44; those who had a clinical diagnosis of tuberculosis but who did not have microbiological confirmation&#46;<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In 2013&#44; results were published on a new PCR method known as FluoroType MTB&#174;&#46; It showed a sensitivity of 95&#46;1&#37; in 661 respiratory samples with positive mycobacterial cultures&#46;<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Later studies have shown different sensitivities in various biological specimens&#46; They range from 56&#37; in the saliva of 32 patients with a microbiological diagnosis of pulmonary tuberculosis&#44;<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> 71&#37; in the blood of 31 patients with a clinical suspicion of disseminated tuberculosis and coinfection with human immunodeficiency virus&#44;<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> 60&#37; in 17 paraffin-embedded tissue samples of granulomas of different etiologies&#44;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> and 63&#37; in 59 lymph node samples with positive mycobacterial cultures&#46;<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> However&#44; the sensitivity of this technique in the PF of patients with TPE has not yet been described to date&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aims of this study were&#58; 1&#41; evaluate the diagnostic accuracy of the FluoroType MTB&#174; technique in PF and sputum samples from patients with suspected TPE and 2&#41; analyze the increase in diagnostic accuracy of a second FluoroType MTB&#174; test in PF &#40;via a second thoracentesis&#41; when the first determination was negative&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">A prospective study was conducted on consecutive patients who underwent a diagnostic thoracentesis in the Arnau de Vilanova University Hospital of L&#233;rida&#44; Spain&#44; from January 2017 to September 2019&#46; A FluoroType MTB&#174; test in PF specimens and&#44; if possible&#44; sputum specimens&#44; was ordered for all patients suspected of having TPE&#46; As per the attending clinician&#8217;s judgment&#44; this PCR technique was also indicated in subjects with other causes of effusion and was generally avoided when the PF etiology was highly predictable &#40;for example&#44; in a significant proportion of those with heart failure&#41; &#40;<a class="elsevierStyleCrossRef" href="#fig0005">Fig&#46; 1</a>&#41;&#46; A second thoracentesis was performed when a definitive diagnosis was not able to be made after learning the results of the first procedure&#46; The study protocol was approved by the local ethics committee &#40;CREC-1674&#41; and all patients signed an informed consent form&#46;</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Diagnostic criteria</span><p id="par0030" class="elsevierStylePara elsevierViewall">The diagnosis of TPE was considered confirmed when the tubercle bacillus was isolated in sputum&#44; PF&#44; or a pleural biopsy or when granulomas were observed on a pleural biopsy&#46; A diagnosis of probable pleural tuberculosis was established when the following conditions were met&#58; a&#41; a clinical condition compatible with TPE&#44; b&#41; presence of pleural exudate with adenosine deaminase concentrations &#8805;35&#8239;U&#47;L&#44; c&#41; resolution of effusion with antitubercular treatment&#44; and d&#41; reasonable exclusion of other potential causes of effusion during at least one year of clinical follow-up&#46; A positive microbiological isolation was defined as observance of tubercle bacilli in a PF or sputum sample by means of a microscopic examination&#44; culture&#44; or PCR test&#46; Effusion was classified as idiopathic when it resolved without specific treatment and when its etiology was unable to be determined despite a comprehensive study that included analysis of the PF&#44; a computed tomography scan&#44; and a pleural biopsy&#46; Other diagnoses were established according to widely accepted clinical criteria&#46;<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Techniques</span><p id="par0035" class="elsevierStylePara elsevierViewall">PF samples were collected in 10-mL tubes that contained ethylenediaminetetraacetic acid&#46; The FluoroType MTB&#174; technique &#40;Hain Lifescience&#44; Nehren&#44; Germany&#41; was performed the same day as the extraction&#46; PF and sputum samples were decontaminated and concentrated using the sodium hydroxide-N-acetyl-l-cysteine method&#46;<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> They were centrifuged for 20&#8239;min at 2&#44;800&#8239;rpm&#46; The sediment obtained was suspended in 2&#8239;mL of phosphate buffer and 0&#46;5&#8239;mL of the mixture was used for the PCR&#46; DNA extraction was done manually using the FluoroLyse&#174; kit &#40;Hain Lifescience&#44; Nehren&#44; Germany&#41; according to the manufacturer&#8217;s instructions&#46; The FluoroCycler&#174; instrument with FluoroType&#174; MTB reactants was used for the final extension of the PCR with a melting curve&#46; The routine microbiological analyses included auramine staining of sputum samples and mycobacterial cultures in solid &#40;L&#246;wenstein-Jensen&#41; and liquid &#40;Middlebrook TH9&#41; media for both sputum and PF&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Measures of diagnostic efficiency &#40;sensitivity&#44; specificity&#44; likelihood ratios &#40;LR&#41;&#41; and their 95&#37; confidence intervals &#40;95&#37; CI&#41; were calculated based on a 2&#8239;&#215;&#8239;2 contingency table&#46; If one of the figures on the table was zero&#44; 0&#46;5 was added to each box to be able to obtain the LR value&#46; Calculations were done using the SPSS statistical package version 24&#46;0 &#40;Chicago&#44; IL&#44; USA&#41;&#46;</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patients</span><p id="par0045" class="elsevierStylePara elsevierViewall">Thirty-one patients with pleural tuberculosis &#40;22 men and nine women with a median age of 35 years&#41; and 176 with other diagnoses &#40;122 men and 54 women with a median age of 72 years&#41; were included&#46; In the first group&#44; 21 &#40;68&#37;&#41; of patients had a definitive histological or microbiological diagnosis of tuberculosis whereas ten &#40;32&#37;&#41; had a clinical diagnosis&#46; Only one patient was coinfected with human immunodeficiency virus&#46; The causes of nontuberculous pleural effusion are specified in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#46; The biochemical characteristics of PF in the study population are summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">FluoroType MTB&#174; in pleural fluid</span><p id="par0050" class="elsevierStylePara elsevierViewall">The index FluoroType MTB&#174; test in PF was positive in four of the 31 &#40;13&#37;&#41; patients with tuberculosis and in two of the 176 &#40;1&#37;&#41; of patients with effusion due to other causes&#46; This yields a sensitivity of 13&#37; &#40;95&#37; CI 5&#37;&#8211;29&#37;&#41;&#44; a specificity of 99&#37; &#40;95&#37; CI 96&#37;&#8211;100&#37;&#41;&#44; a positive LR of 11 &#40;95&#37; CI 2&#8211;59&#41;&#44; and a negative LR of 0&#46;9 &#40;95&#37; CI 0&#46;8&#8211;1&#41;&#46; The sensitivity of the FluoroType MTB&#174; was lower than that of other microbiological tests&#44; as shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#46; The aforementioned tests were used as references for defining the terms of confirmed and probable pleural tuberculosis&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The FluoroType MTB&#174; test was negative in nine patients &#40;33&#37;&#41; who had a PF culture positive for <span class="elsevierStyleItalic">M&#46; tuberculosis</span> &#40;<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>&#41;&#46; On the other hand&#44; two of the four &#40;50&#37;&#41; patients who had a positive FluoroType MTB&#174; test in PF did not have any other positive microbiological isolations&#46; That is&#44; in two of the 31 &#40;6&#46;5&#37;&#41; patients with TPE&#44; the confirming diagnosis was made solely through the PCR technique&#46;</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">FluoroType MTB&#174; in sputum</span><p id="par0060" class="elsevierStylePara elsevierViewall">The sputum of 25 patients was analyzed using FluoroType MTB&#174;&#46; It was positive in three of the 14 &#40;21&#37;&#41; patients with TPE and in one of the 11 &#40;9&#37;&#41; with other diagnoses &#40;<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#41;&#46; The three patients with TPE and a positive result on the PCR technique in sputum also had positive sputum cultures&#59; one also had a positive PF culture&#46;</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">FluoroType MTB&#174; in patients with an initial negative determination</span><p id="par0065" class="elsevierStylePara elsevierViewall">In 57 patients &#40;24 with tuberculosis and 33 with other diagnoses&#41;&#44; a second FluoroType MTB&#174; test in PF was carried out with a median time between the two thoracentesis procedures of three days &#40;25th and 75th quartiles&#58; 1&#8211;7&#41;&#46; The second analysis was negative in all cases&#46;</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">FluoroType MTB&#174; false positives</span><p id="par0070" class="elsevierStylePara elsevierViewall">There were two patients with a FluoroType MTB&#174; test in PF that were interpreted as false positives&#46; One patient had transudate secondary to heart failure and another had parapneumonic effusion&#46; A pleural biopsy was performed on the latter and showed acute inflammation without granulomas&#46; In both cases&#44; a second FluoroType MTB&#174; sample two days after the first was negative&#46; There was also a false positive on a FluoroType MTB&#174; test on a sputum sample&#46; It corresponded to a patient with malignant effusion secondary to lymphoma&#46;</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">In this study&#44; the FluoroType MTB&#174; technique in PF demonstrated limited utility in the diagnosis of TPE due to its low sensitivity &#40;13&#37;&#41;&#46; Nevertheless&#44; in 6&#46;5&#37; of patients with TPE&#44; diagnostic confirmation was made exclusively using this PCR technique&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall">PCR techniques for diagnosing tuberculosis can be done using commercial methods or in-house methods developed in the laboratory&#46;<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> Among the former&#44; the Xpert&#174; MTB&#47;RIF test has been endorsed by the World Health Organization&#46;<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> In a Cochrane review&#44; the overall sensitivity of the Xpert&#174; MTB&#47;RIF test in PF was 51&#37; &#40;95&#37; CI 40&#37;&#8211;63&#37;&#41;&#44; but decreased to 18&#37; &#40;95&#37; CI 10&#37;&#8211;31&#37;&#41; when the gold standard applied was a clinical diagnosis of PTE without microbiological confirmation&#46; The test had a specificity of 99&#37;&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> In a recent study&#44; the new Xpert&#174; MTB&#47;RIF Ultra technique in PF showed an overall sensitivity that was superior to a culture &#40;44&#37; vs&#46; 26&#37;&#41; in 208 patients with TPE&#44; though the percentage of positive results decreased to 20&#37; in cases of probable pleural tuberculosis&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">This is the first study to evaluate diagnostic accuracy of the FluoroType MTB&#174; in PF&#46; Another study compared the FluoroType MTB&#174; and Xpert&#174; MTB&#47;RIF tests in 70 samples of sputum which were positive for <span class="elsevierStyleItalic">M&#46; tuberculosis</span> and 226 which had negative cultures&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> The sensitivity of the FluoroType MTB&#174; was superior to the Xpert&#174; MTB&#47;RIF &#40;88&#46;6&#37; vs&#46; 78&#46;6&#37;&#41;&#44; but its specificity was lower &#40;59&#46;7&#37; vs&#46; 93&#46;8&#37;&#41;&#46; That is&#44; it had a greater percentage of potential false positives&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Though the FluoroType MTB&#174; test allows for receiving results quickly and is cheaper than the Xpert&#174; MTB&#47;RIF Ultra test &#40;approximately &#1028;39 vs&#46; &#1028;76 per sample in our center&#44; respectively&#41;&#44; it does not allow for detecting drug resistance&#44; as the latter does&#46; Some reasons that may underlie the low sensitivity of PCR techniques in PF are the paucibacillary nature<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> of pleural tuberculosis and the presence of PCR inhibitors in this medium&#46;<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17&#44;20</span></a> The utility of the FluoroType MTB&#174; is also limited by false positives which&#44; although they were not frequent in our study&#44; have already been noted in previous works&#46;<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11&#44;19</span></a> They have been associated with cross-reactions or an unsatisfactory reading of the curve shown by the FluoroCycler&#174;&#46;<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Repeating the PCR technique when the first test is negative has previously been evaluated&#46; One study showed that out of 645 cases of pulmonary tuberculosis which had a positive Xpert&#174; MTB&#47;RIF test&#44; 615 &#40;95&#37;&#41; were positive on the first determination and 30 &#40;5&#37;&#41; on the second&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In a work on 168 cases of extrapulmonary tuberculosis&#44; the percentages were 89&#37; and 11&#37;&#44; respectively&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In our study&#44; a second sample did not increase diagnostic accuracy in any patient&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">This study has several limitations&#46; First&#44; the main limitation is that it included a small number of patients with TPE&#44; of which one-third did not have microbiological confirmation&#46; Nevertheless&#44; the inclusion of patients with probable TPE according to the established criteria is widely accepted in the medical literature&#46; Our work only tested a sputum sample using the PCR technique in half of the cases&#46; We were also unable to obtain a second PF sample in 11&#37; of patients with TPE who had a first FluoroType MTB&#174; determination which was negative&#46; Likewise&#44; a microscopic examination and auramine staining of PF specimens &#40;the only test that can be compared to the PCR due to the quick obtaining of results&#41; were not performed due to the low sensitivity this technique showed in a previous study in our center &#40;3&#37; of positive results in a series of 274 PF samples&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Lastly&#44; unlike other PCR techniques on the market&#44; an inherent limitation of the FluoroType MTB&#174; test is that it does not detect resistance to antituberculosis drugs&#46;</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Conclusions</span><p id="par0105" class="elsevierStylePara elsevierViewall">The FluoroType MTB&#174; technique applied to PF has a limited capacity for diagnosis tuberculosis and its specificity is not absolute&#46; The fact that PTE was confirmed exclusively by means of this technique in 6&#46;5&#37; of cases does not seem to justify its implementation in routine clinical practice outside of the scope of research&#46; It also is not cost-effective to repeat the test when a first determination is negative&#46;</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Funding</span><p id="par0110" class="elsevierStylePara elsevierViewall">This study has been funded by the <span class="elsevierStyleGrantSponsor" id="gs0005">Spanish Society of Internal Medicine</span> &#40;SEMI&#44;
