"apellidos" => "Aydogdu" ] ] "afiliaciones" => array:1 [ 0 => array:2 [ "entidad" => "Unidad de Cardiología, Docencia e Investigación Hospital Yuksek Ihtisas, Ankara, Turkey" "identificador" => "aff0005" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Evaluación de la adherencia a la dieta mediterránea en pacientes con antecedentes de revascularización coronaria" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Coronary artery disease is the leading cause of death in Western countries and prevalence of this disease will continue growing.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> Several risk factors have been associated development of atherosclerosis, among them high blood pressure, high- and low-density cholesterol (LDL), diabetes mellitus, smoking and physical inactivity are main risk factors.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a> Although lifestyle modification and diet are considered the main modifiable factors in prevention of cardiovascular disorders, <20% of cardiac patients with known coronary artery disease complete cardiac rehabilitation programs and adhere to their diet.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Mediterranean diet included relatively high proportion of fruit, cereals and vegetables; fish and poultry as the main sources of protein, and the use of virgin olive oil as the main source of fat.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> This dietary pattern is relatively high in total fat (35%), complex sugars, dietary fiber and antioxidants but low in saturated fats (<10%).<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> Protective effect of this diet in the development of cardiovascular disease previously reported by various studies and Mediterranean diet is associated with reduced cardiovascular mortality.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">8,9</span></a> Secondary prevention by diet modification is also important and adherence to the Mediterranean diet was associated with lower mortality risk and overall and cardiovascular event-free survival.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">10,11</span></a> Recent studies have emphasized that increased adherence to Mediterranean diet associated with reduced cardiovascular mortality and morbidity. In present study, we aimed to investigate secondary prevention status in patients compliant with Mediterranean diet after coronary revascularization and characteristics of the ones with good adherence.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population</span><p id="par0015" class="elsevierStylePara elsevierViewall">This is an observational study and a total of 226 consecutive outpatients who had a history of coronary revascularization at least 6 months prior to enrollment were included. Patients with an unstable condition, language problem, those were unwilling to participate, without coronary revascularization history or those with a history of coronary revascularization within 6 months prior to enrollment were excluded. The study was approved by the local ethics committee and patients were included after giving informed consent to participate.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study design and MedDiet score assessment</span><p id="par0020" class="elsevierStylePara elsevierViewall">After obtaining informed consents of patients for the study, demographic, social and clinical characteristics of patients and also the data of dietary habits, were collected with a questionnaire which was formed by the investigators of the study. Each participant filled out a questionnaire to collect the data of demographics and clinical characteristics. Alternate MedDiet score was calculated to evaluate the Mediterranean style dietary adherence.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12–14</span></a> Alternate MedDiet score was originally based on 14-item questionnaire; we adjusted it to for the relevant population (max 13 points) (<span class="elsevierStyleItalic">Question 14. How many times per week do you consume vegetables, pasta, rice, or other dishes seasoned with sofrito (sauce made with tomato and onion, leek, or garlic and simmered with olive oil)? (excluded from our study due to food habit).</span> The patients were divided into two group according to MedDiet score. Patients with MedDiet score<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>7 accepted as Mediterranean diet adherent group. The history of coronary artery disease and the revascularization type and details of the intervention were noted from medical records.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Definitions</span><p id="par0025" class="elsevierStylePara elsevierViewall">Regular physical activity accepted as at least 30<span class="elsevierStyleHsp" style=""></span>minutes intermediate intensity exercise three days in a week. Hypertension was defined as blood pressure >140/90<span class="elsevierStyleHsp" style=""></span>mmHg on two occasions during office measurements or use of anti-hypertensive treatment. Regulated hypertension defined as blood pressure <140/90<span class="elsevierStyleHsp" style=""></span>mm-Hg in office measurements for two times. Diabetes mellitus (DM) was defined as fasting blood glucose ≥126<span class="elsevierStyleHsp" style=""></span>mg/dl on two occasions or use of antidiabetic treatment. DM was accepted as regulated when HbA1c levels below 7.0%. Hyperlipidemia was defined as LDL<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>70<span class="elsevierStyleHsp" style=""></span>mg/dl or use of statins and when its level was under 70<span class="elsevierStyleHsp" style=""></span>mg/dl it was accepted as regulated. The family history for CAD was defined as a history of CAD or sudden cardiac death in a first degree relative before the age of 55 years for men and 65 years for women. Body mass index (BMI) was calculated by dividing weight in kilograms by height in squared meters. In this study, varying consumption levels were defined by converting the different amount of alcohol contained in various types of beverages (i.e. beer, wine, rice wine, whiskey and etc.) into a single unit of grams/day (g/day). Excessive alcohol consumption was defined as >40<span class="elsevierStyleHsp" style=""></span>g/day in men, >20<span class="elsevierStyleHsp" style=""></span>g/day in women.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> Household income was estimated in three categories: Low (<1000 Turkish Liras), Average (1000–4000 Turkish Liras) and High (>4000 Turkish Liras). These cut points corresponded to average family income in Turkey at the time of the study. The educational level was determined according to number of years at school and classified in three categories: primary or less (no formal education or up to first five years of school), secondary (6–11 years of education) or university or higher (4 years of college or additional years of graduate and post graduate school).</p><p id="par0030" class="elsevierStylePara elsevierViewall">A history of acute coronary syndrome was defined according to the detection of rise of cardiac troponins together with symptoms of ischemia or ischemic changes on electrocardiogram (ECG) from past medical records. Presence of atrial fibrillation was also searched from ECG. Medical records were detected for history of cerebrovascular events. Renal failure was defined as the value of e-GFR according to Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) formula <15<span class="elsevierStyleHsp" style=""></span>ml/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span> or being on routine dialysis program. Cardiac function was noted from the echocardiographic evaluation of ejection fraction (EF) at least 3 months after coronary revascularization and heart failure with reduced ejection fraction was defined as having left ventricular EF <%40. Heart failure with mid-range ejection fraction was not noted in the study. Severity of angina pectoris was defined according to Canadian Cardiovascular Society grading of angina pectoris.</p><p id="par0035" class="elsevierStylePara elsevierViewall">All patients underwent laboratory tests following an overnight fasting to determine serum creatinine, fasting blood glucose, and lipid levels including total cholesterol, low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and triglyceride. HbA1c levels were measured in the patients who were previously diagnosed with diabetes mellitus.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Continuous variables were expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD or median with interquartile range, and categorical variables were expressed as percentages and numbers. The normality of distributions of the parameters was assessed by using the Kolmogorov–Smirnov test. Comparison between continuous variables was made by use of independent samples t test for normally distributed variables, and Mann–Whitney <span class="elsevierStyleItalic">U</span> test when the distribution was skewed.</p><p id="par0045" class="elsevierStylePara elsevierViewall">A <span class="elsevierStyleItalic">χ</span><span class="elsevierStyleSup">2</span> test or Fisher's exact test was used to compare the categorical variables. Univariate logistic regression analysis was used to evaluate the relationship between variables and good MedDiet scores (≥7). Variables found to be <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.10 in the univariate analysis were further used in a multivariate logistic regression analysis with forward stepwise method in order to determine the independent predictors of good Mediterranean diet adherence. The results of the regression analyses were presented as odd ratios (OR) and 95% confidence intervals (CI). All statistical procedures were performed using SPSS software version 20.0 (SPSS Inc., Chicago, IL). A <span class="elsevierStyleItalic">p</span> value <0.05 was considered statistically significant.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">Mean age of the 226 patients were who 61.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.9 years, with males comprising 72% of the study group. Of all, 138 (61.3%) patients had hypertension, 69 (30.5%) had diabetes mellitus, 98 (43.8%) had previous infraction and 28 (12.5%) had heart failure. Among the study, 112 (49.6%) patients had previous percutaneous coronary intervention, 77 (34.1%) patients had coronary by-pass graft surgery, and 36 (15.9%) patients had both of the revascularization procedures. The median duration after coronary artery revascularization was 60 (24–108) months.</p><p id="par0055" class="elsevierStylePara elsevierViewall">A total of 61 (26%) patients were adherent to Mediterranean Diet that their score was above six (≥7). Demographic and anthropometric characteristics of the study patients according to MedDiet score are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The patients with adherent to Mediterranean diet were older than non-adherent patients (65<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>12.0 vs. 60.0<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.0 years, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006). Education level as university or higher is significantly higher in MedDiet score<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>7 group (19.7% vs. 3.6%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and both of the revascularization procedure (PCI<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>CABG) (23.0% vs. 13.3%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.042) is higher in MedDiet<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>7 group. Marital status, occupation status, household income and comorbidities did not differ between the groups.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">The clinical characteristics, laboratory findings and medications of the study subjects are summarized in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Annual influenza vaccination (32.8% vs. 13.9%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005) regular follow-up (75.0% vs 58.2%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.021), regular exercise (36.1% vs. 15.2%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.001) and duration after last revascularization [94 (36–147) vs 48 (24–96] months, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002) higher in MedDiet adherent group than non-adherent group.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The results of the univariate and multivariate logistic regression analyses are listed in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. In the multivariate regression analysis, high education level (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002, OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7.688 95% CI: 2.068–28.589), longer time after last revascularization (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.034, OR: 1.007 95% CI: 1.001–1.013) and lower waist circumference (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.046, OR: 0.978 95% CI: 0.956–1.000) were found to be independent predictors of high MedDiet scores in the patients with previous revascularization.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Our study demonstrated that the patients who obey the Mediterranean diet after coronary revascularization were older and their body mass index and diastolic blood pressure was lower. They make much exercise and annual influenza vaccination rate was high in these group. Active smoking was lower in the patients with high MedDiet scores. Symptomatic coronary artery disease was low in MedDiet adherent group and blood pressure of these groups was almost on target compared to low MedDiet scores. High educational level and duration after last revascularization were the independent predictors of good MedDiet adherence.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Lifestyle plays essential role both primary and secondary prevention of cardiovascular disease which causes mortality and morbidity. Dyslipidemia, diabetes and hypertension were directly related to diet.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> Traditional Mediterranean diet composed of olive oil, plant foods (fresh fruit, vegetables, legumes and nuts) fish and red wine, but low in red/processed meats, whole-fat dairy and sweets.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> Previous studies have shown that Mediterranean diet was associated lower cardiovascular disease,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> cancer<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> and all-cause mortality.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> Several observational trials demonstrated that increased adherence to MedDiet showed inverse association with total mortality.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">21,22</span></a> Recent cohort study has showed strong association between adherence to the MedDiet score and all-cause mortality including cardiovascular mortality.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">According to the American Heart Association (AHA) and European dietary guidelines for secondary prevention recommend a healthy dietary pattern that can be largely achieved by adhering to a Mediterranean diet consisting primarily of fruits, vegetables, fish, fiber, and low amounts of saturated fats.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> PREDIMED trial reported that adherence to a Mediterranean diet among individuals without history of coronary artery disease to be associated with a 29% and 11% lower risk for CV disease and all-cause mortality, respectively, over 4.8 years of follow-up.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12,25,26</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">The total effect of foods found in Mediterranean diet (nutrients and food components) has been shown to confer beneficial effects on the individual components of the metabolic syndrome, including a better lipid profile<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> and lower systolic and diastolic blood pressure.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> In contrast to these studies, present study showed no difference lipid profile and systolic blood pressure between the MedDiet groups. But diastolic blood pressure was different between groups. We also found that regulated hypertension ratio was high in MedDiet. Blood pressure might be positively modulated by intakes of fruit and vegetables that are rich in K, fiber and anti- oxidants while intakes of plant protein and foods from plant sources are inversely associated with blood pressure.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">In the present study, we have used alternate MedDiet score which were previously validated by.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">12,13</span></a> This test composed of various question to assess food and drinks which should include in Mediterranean diet. In our study, the patients who adherent to Mediterranean diet were older. Steffen et al. also showed that participants in their study with higher MedDiet scores were older, and were more physically active than those with lower scores.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> In parallel to our study, they also found that patients with higher scores had more years of education. It seems to education is important to make appropriate diet.</p><p id="par0095" class="elsevierStylePara elsevierViewall">As expected, body mass index and waist circumference lower in high MedDiet score group than lower ones. Interestingly, annual influenza vaccination rate higher and active smoking was lower in patients with high MedDiet score than lower scores. It may be related with older age and higher educational level. Symptomatic coronary artery disease was lower in the high MedDiet score group. It might be due to anti-inflammatory and anti-atherosclerotic effect of Mediterranean diet content. In MedDiet score high group time after last revascularization was longer than lower ones. It may be related with living longer time with coronary artery disease, the patients tried to make right things for their health.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusion</span><p id="par0100" class="elsevierStylePara elsevierViewall">The adherence rate to a healthy diet was low in patients with previous coronary revascularization. MedDiet score seems to be practical and useful item to evaluate the dietary habits in outpatient setting. Mediterranean diet adherence rates were associated with high education level, duration after revascularization and waist circumference.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Study limitations</span><p id="par0105" class="elsevierStylePara elsevierViewall">Limitations of the present study are reliance on self- reported dietary intake, which may cause considerable measurement error. We aimed to investigate the dietary habits of the patients with history of coronary revascularization, characteristics of the ones with good adherence by using alternate MedDiet questionnaire.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">We included outpatients who had a history of coronary revascularization at least 6 months prior to enrollment. Each participant filled out a questionnaire to collect the data of demographics and clinical characteristics. Alternate MedDiet score was calculated to evaluate the Mediterranean style dietary adherence. Alternate MedDiet was originally based on 14-item questionnaire; we adjusted it to our population (max 13 points).</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">We enrolled 226 consecutive outpatients (age 61.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.9 years, 72% males). The median duration after revascularization was 60 months. A total of 112 (49.6%) patients had previous percutaneous coronary intervention (PCI), 77 (34.1%) had coronary by-pass graft surgery (CABG), and 36 (15.9%) had both revascularization procedures.
