Gaucher disease screening.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Osteonecrosis, also known as avascular necrosis or aseptic necrosis, is bone cell death that follows impairment of blood flow to the bone of traumatic or non-traumatic origin<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Although the etiology and pathophysiology of avascular necrosis have been the subject of extensive study, there is no consensus about its exact origin. The most widely accepted hypothesis is that it is not a specific disease, but rather the end result of various pathological conditions, ultimately affecting microcirculation in the femoral or humeral head<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">From a pathophysiological standpoint, there are three mechanisms that can affect microcirculation in these bones and provoke avascular necrosis<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>: mechanical disruption of the bone vasculature, intravascular occlusion, and external compression.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Avascular necrosis of the humeral or femoral head is a devastating disease that usually affects patients aged 20–50 years. It normally progresses to total bone collapse and often leads to total hip or shoulder arthroplasty<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The real prevalence of this pathology is unknown. In the United States, there are estimated to be 20,000–30,000 new cases every year<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> and in the United Kingdom, a prevalence of 1.4–3 cases per 100,000 people has been estimated<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. Despite this prevalence, it is not a minor problem; it is estimated that approximately 10% of total hip replacements in the United States are due to avascular necrosis<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In Spain, avascular necrosis of the hip and shoulder is a little studied disease and, to our knowledge, there are no records of its incidence or prevalence. Furthermore, there is only one study about possible etiological factors for the development of this disease<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>.</p><p id="par0035" class="elsevierStylePara elsevierViewall">With regard to the etiology of this pathology, the following processes play a certain predisposing role<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>: (a) trauma which interrupts the vascular supply to the humeral or femoral head<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>; (b) corticosteroid use, considered the most common cause of avascular necrosis<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>; (c) alcohol consumption<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>; (d) blood dyscrasias, the most prominent example of which is sickle cell anemia<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>; although it can also appear in patients with polycythemia, myeloproliferative syndromes that produce increased erythrocytes, hemophilia, genetic factors such as Factor V Leiden mutation, protein C/S deficiency, increased von Willebrand factor, or increased lipoproteins; (e) Gaucher disease, this inherited metabolic disease is characterized by a deficiency in the acid-beta-glucosidase enzyme or glucocerebrosidase<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>; (f) human immunodeficiency virus (HIV)<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>; (g) autoimmune diseases such as systemic lupus erythematosus, due to the use of corticosteroids or the result of associated vasculitic processes<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>; (h) radiotherapy of the hip or shoulder<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>; (i) other processes including smoking, dyslipidemia, obesity, gout, pancreatitis, hyperparathyroidism, and inflammatory bowel disease<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">This study has two main aims: first, to analyze the prevalence of potential etiological factors for avascular necrosis of the hip and shoulder in patients from Almería (Spain) and second, to screen for Gaucher disease in patients whose medical records reveal no potential etiological factors for avascular necrosis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0045" class="elsevierStylePara elsevierViewall">This study comprised two parts.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Part one</span><p id="par0050" class="elsevierStylePara elsevierViewall">A retrospective, single-center, observational study was conducted on patients who had presented with at least one episode of avascular necrosis of the hip or shoulder and had been treated for this condition at the Hospital de Poniente (Almería) from January 2010 to December 2019. According to data from the end of 2021 from Spain’s National Institute of Statistics, the Hospital de Poniente serves a population of 262,486 people<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The patients were selected retrospectively, taking note of the diagnostic code for avascular necrosis of the hip or shoulder in hospital databases and the corresponding consultation and discharge reports.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The main inclusion criterion was that avascular necrosis must have been confirmed by nuclear magnetic resonance imaging tests.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The following variables were recorded based on epidemiological, clinical, and microbiological data in the patients’ medical records. Epidemiological variables included age, sex, country of origin, toxic habits (especially smoking and alcohol use disorder, defined respectively as any chronic tobacco use and drinking >4 standard drink units (SDU)/day or >21 SDU/week in men and >3 SDU/day or >14 SDU/week in women<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>. One SDU corresponds to 10<span class="elsevierStyleHsp" style=""></span>g of ethanol. Clinical variables included family history as well as the patient’s personal medical and surgical history, especially in regard to diabetes mellitus, obesity, hypercholesterolemia, pancreatitis, systemic autoimmune disease, previous bone trauma, and coagulation disorders. Data on previous treatments, especially the use of corticosteroids and/or bisphosphonates, were also recorded. Lastly, microbiological variables included HIV serology.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Part two</span><p id="par0070" class="elsevierStylePara elsevierViewall">Patients in whom no potential etiological factors for avascular necrosis was observed were notified by telephone and offered the possibility of screening for Gaucher disease, provided they signed an informed consent form.</p><p id="par0075" class="elsevierStylePara elsevierViewall">This screening was performed via enzymatic tests. Enzymatic activity for beta-glucocerebrosidase, chitotriosidase, and acid sphingomyelinase on dried blood spots was determined for a differential diagnosis with acid sphingomyelinase deficiency. Beta-galactosidase was also measured as a quality control of the sample.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Data analysis was performed using SPSS, version 15.0 statistical software. A descriptive analysis was made of the study variables. Measures of central tendency and dispersion were calculated for quantitative variables and absolute and relative frequencies for qualitative variables.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The study was approved by the Almería Research Ethics Committee under code Pl_20_01.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0090" class="elsevierStylePara elsevierViewall">Of the 81 patients included in the study sample, 58 (71.6%) were male. The mean age at presentation of osteonecrosis was 45.9 years<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>12.7 (range 15–67). Sixty patients (74.1%) were Spanish, thirteen (16%) were African, seven (8.6%) were from Eastern European countries, and one (1.2%) was from South America. Of these patients, 80 (98.8%) presented with avascular necrosis of the hip, 37 of whom (46.3%) had bilateral disease. In addition, four presented with avascular necrosis of the shoulder and three of these patients had hip joint involvement.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The most frequent potential etiological factors for osteonecrosis were smoking (38 patients, 46.9%), obesity and dyslipidemia (14 patients for each, 17.3%), and chronic alcohol use disorder (12 patients, 14.8%). Twenty-four patients (29.6%) had two or more potential etiological factors. The frequency of these etiological factors is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">A review of the patients’ medical records revealed no potential etiological factors in 35 individuals (43.2%). Of them, 25 (71.4%) were male. The mean age of presentation of osteonecrosis in this group was 46.8 years<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.5 (range 32–66). All had avascular necrosis of the hip and one also had it in the shoulder. These patients were included in the second part of the study, the screening for Gaucher disease. However, thirteen could not be contacted, either due to no contact information in their medical records or because they did not answer the telephone call. Of the remaining 22, 12 refused to participate. Therefore, ten patients were screened in the end (12.3% of the total and 28.6% of those with no apparent risk factors). All tested negative for Gaucher disease. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the diagnostic algorithm followed.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0105" class="elsevierStylePara elsevierViewall">Worldwide, few rigorous epidemiological studies have been undertaken to investigate the risk factors for developing avascular necrosis of the hip or shoulder. This study is, to our knowledge, the second of its kind to be carried out in Spain and the first in the world in which Gaucher disease screening is performed in patients with no known potential etiological factors.</p><p id="par0110" class="elsevierStylePara elsevierViewall">In the study population, the main potential etiological factor was active smoking, which was recorded in 46.9% of the patients. This coincides with the other study conducted in Spain, by De Sautu et al., which found a prevalence of smokers of 44.2%<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>. In the other prevalence studies, most of which were conducted in Asia, no information is given on the percentage of smokers<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–22</span></a>.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The next most frequent potential etiological factors in the study population after smoking were obesity and dyslipidemia, which were each recorded in 17.3% of the patients. In De Sautu et al.’s work, 35.7% of the patients had dyslipidemia<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>. The difference in these figures is probably due to two main reasons. First, the mean age in our population (45.9 years) was lower than that of the study by De Sautu et al. (58.4 years)<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>. Second, there was a significant number of missing values. Both studies were retrospective, a fact that can significantly influence the composition of the study group. For example, some of our patients did not have a blood test to determine their lipid profile on dates close to when the episode of osteonecrosis occurred. Moreover, data on the lipid profile were not collected in the studies conducted in Asia.</p><p id="par0120" class="elsevierStylePara elsevierViewall">With respect to the prevalence of patients with chronic alcohol use disorder, there is a striking difference between the 14.8% recorded in our study group and the 20% reported by De Sautu et al.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>, and even more so with regard to the studies carried out in Asian countries, where the corresponding figure ranges from 11% in Thailand<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> to 32.4% in Korea<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> to 45.2% in Taiwan<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. These differences might be explained by the high proportion (16%) of individuals of African descent among our study population, who were mostly Muslims and did not consume alcohol.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The fifth non-traumatic potential etiological factor for avascular necrosis observed was chronic corticosteroid use, recorded in 11.2% of patients. This finding also contrasts with those from other studies, especially those carried out in Asia, where the prevalence of patients with chronic corticosteroid use and avascular necrosis ranges from 14.6% in Korea<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>, 35% in Thailand<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>, 46% in China<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>, and 51% in Japan<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>. These differences are possibly due to the higher incidence of some autoimmune diseases, such as systemic lupus erythematosus<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>, Behçet's disease<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a>, or autoimmune uveitis<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> in Asian countries compared to Spain. All these diseases may require long-term corticosteroid treatment.</p><p id="par0130" class="elsevierStylePara elsevierViewall">The prevalence of potential etiological factors for avascular necrosis, as reported in the above-mentioned studies, are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">An important issue that concerns the potential etiological factors for this disease is the percentage of patients diagnosed with idiopathic avascular necrosis. In our study, 43.3% of patients had no apparent risk factors. In the other studies referenced, this value ranges from 13.2% to 33.1%<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,16–22,26</span></a>. The considerable differences in rates may be due to one or both of the following reasons. First, the type of study performed, i.e. whether it was retrospective or prospective. Second, the nature of the potential etiological factors considered. Most Asian studies only included chronic alcohol use disorder and/or corticosteroid use and did not consider smoking, obesity, dyslipidemia, and coagulation disorders, among other possible factors.</p><p id="par0140" class="elsevierStylePara elsevierViewall">In this study, the high proportion of patients with no apparent potential etiological factors may be due to the retrospective design and missing data in patients’ medical records. However, in the literature on patients diagnosed with idiopathic avascular necrosis, it is striking how little emphasis is placed on the search for risk factors for the development of this disease. Only Liu et al.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> analyzed patients with idiopathic avascular necrosis, studying possible clinical and analytical risk factors. A univariate analysis of these factors revealed a statistically significant relationship between idiopathic avascular necrosis and body mass index, triglyceride levels, LDL and HDL cholesterol, and alkaline phosphatase. However, the multivariate logistic regression analysis only detected a statistically significant association with LDL and HDL cholesterol.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Today, the management of patients with idiopathic avascular necrosis remains challenging, and little or no research is being done to consider the less common potential etiological factors that could produce this pathology. For this reason, the second part of the study sought to advance the diagnostic process by screening for Gaucher disease in patients whose medical record described no known risk factors. To our knowledge, no such study has been undertaken previously.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Ten patients in our study group agreed to this screening. Although all test results were negative, the importance of screening for Gaucher disease in these patients lies in the fact that there is an effective treatment that drastically reduces complications such as avascular necrosis. It is estimated that an untreated patient has a lifetime risk of developing osteonecrosis of 22.8 per 1000 person-years<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a>, but this value falls to 13.8 per 1000 person-years in patients given appropriate enzyme therapy<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>. For this reason, it is essential to screen for Gaucher disease in patients with idiopathic avascular necrosis. Bearing in mind that the prevalence of this disease is one case per 60,000 people, the fact that no cases were found in this study may be due to the small sample size.</p><p id="par0155" class="elsevierStylePara elsevierViewall">In Spain, patients with avascular necrosis of the femur or hip are normally treated by specialists in traumatology and orthopedics. However, traumatological causes of osteonecrosis represent only 1.6%–16.3% of all potential etiological factors<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,16,18,19</span></a>. In our study group, the figure was 16%. For this reason, a multidisciplinary approach should be taken that also involves other specialties, such as internal medicine, hematology, and/or endocrinology.</p><p id="par0160" class="elsevierStylePara elsevierViewall">This study has two important limitations. On the one hand, the retrospective study design means that a significant amount of information could have been missing. Furthermore, the small sample size means that definitive conclusions cannot be drawn, especially concerning the indication of screening for Gaucher disease.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0165" class="elsevierStylePara elsevierViewall">Based on the study findings, in our hospital, the main potential etiological factors for a patient to develop avascular necrosis of the shoulder or hip are smoking, obesity, dyslipidemia, and chronic alcohol use disorder. Both in this work and in previous studies in this field, a high percentage of patients are diagnosed with idiopathic avascular necrosis.</p><p id="par0170" class="elsevierStylePara elsevierViewall">We firmly believe that a more detailed study of possible risk factors should be carried out in these patients, including endocrine factors (obesity, dyslipidemia), smoking, coagulation disorders, and screening for rare diseases such as Gaucher disease or sickle cell disease.
