Palliative care plan.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Chronic heart failure (CHF) is a major challenge for health services worldwide, with a focus on maintaining patient quality of life. Its prevalence is very high due to the aging of the population and the increased survival of cardiovascular patients.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This explains that most of the cases correspond to older patients with multiple comorbidities, with frequent decompensations, and who are admitted due to heart failure (HF) in Spanish Departments of Internal Medicine. These frequent hospital admissions not only imply a worsened prognosis and high mortality, but also generate very high costs.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2,3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">This situation has created a pressing need to develop efficient management strategies throughout the course of the disease,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> including the management of CHF on an outpatient basis.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> The primary objective is to stabilize the patient and prevent decompensation, with the consequent improvement of quality of life, fewer hospital admissions and visits to the emergency room and, consequently, a decrease in healthcare costs.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Within this framework, the protocol for the ambulatory management of CHF, developed by the HF and atrial fibrillation (HF<span class="elsevierStyleGlyphsbnd"></span>AF) working group of the Spanish Society of Internal Medicine (Sociedad Española de Medicina Interna [SEMI]), is a valuable resource for operationally unifying outpatient care for patients with CHF.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This protocol focuses on the profile of patients seen by Internal Medicine.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,8</span></a> It thus provides guidelines for assessing and treating the most common comorbidities in patients with CHF, underscoring the important role that nursing professionals must play in the management of these patients.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Methodology</span><p id="par0020" class="elsevierStylePara elsevierViewall">The protocol for the outpatient follow-up of patients with CHF is the result of an in-depth review of the clinical practice guidelines and other documents by a group of experts belonging to the HF<span class="elsevierStyleGlyphsbnd"></span>AF working group of the SEMI. It was developed indifferent phases. In a first phase the protocol was designed, identifying sections that were considered essential, based on the existing guidelines for the comprehensive care of patients with HF in Internal Medicine. In a second phase, these sections were distributed for development among groups of professionals who are experts in the corresponding areas. In a final phase, the different sections were unified by a coordinating team, establishing the full protocol. It was subsequently distributed to the authors for review, and once agreed by all contributors, the final version was prepared and made available on the website of the SEMI.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Based on the contents of this protocol, the present document was prepared (slightly modified due to updates), and was likewise distributed to all the authors for review and final approval.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Care management of CHF</span><p id="par0030" class="elsevierStylePara elsevierViewall">This protocol<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> is structured according to the different stages which CHF patients may go through during their outpatient follow-up. It includes the “first visit”, the periodic “scheduled review visit”, the “post-discharge visit” to reduce risks in the critical period corresponding to the first month after hospital discharge, and “unscheduled visits” designed to resolve decompensations.</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">First visit</span><p id="par0035" class="elsevierStylePara elsevierViewall">The first visit involves the patient's first contact with the healthcare system in charge of follow-up. Thus, it is necessary to collect complete and accurate information allowing an adequate diagnosis and treatment and follow-up plan, adapted to the individual needs of each patient.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In this context, the first section begins with the patient’s social history. Information is collected on the origin of the patient, whether from a hospital, emergency room, or primary care (PC). It should also be documented whether the patient is being monitored by other specialties (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). All these data provide a comprehensive perspective of the care received by the patient and facilitate communication among the different professionals involved in his/her follow-up. This section also assesses the level of dependency and the availability of social support that will serve to establish a future care plan (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The second section analyzes the cardiovascular risk factors and comorbidities that contribute to the development and course of HF. Emphasis is placed on the importance of recording the presence of different conditions, including diabetes mellitus (DM), arterial hypertension (AHT), dyslipidemia and/or smoking, all of which are associated to an increased risk of complications.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> In addition, mention is made of the need to identify from the start of follow-up the presence of relevant cardiac comorbidities (valve disease, cardiomyopathy, ischemic heart disease, atrial fibrillation (AF), etc.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>) and non-cardiac comorbidities (stroke, chronic obstructive pulmonary disease [COPD], obesity-hypoventilation syndrome [OHS], chronic kidney disease [CKD], iron deficiency, anemia, thyroid disorders, neuropsychiatric disorders, peripheral arterial disease, obesity, etc.).<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> The frequency of emergency room visits and hospital admissions not related to HF should also be recorded (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The third section describes the previous medications taken by the patient for both HF and for comorbidities, with adoption of the appropriate adjustments (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0055" class="elsevierStylePara elsevierViewall">The fourth section focuses on the history of HF and provides an idea of the evolution of the disease. In this respect the number of emergency room visits and hospital admissions related to HF are recorded, and the measurement of natriuretic peptides, different echocardiographic parameters, and other procedures, if any, is also documented. In the event of a prior diagnosis of HF, recording is made of the type of HF according to the left ventricular ejection fraction (LVEF) and <span class="elsevierStyleItalic">New York Heart Association</span> (NYHA) functional class, which offer prognostic and therapeutic guidance <a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0060" class="elsevierStylePara elsevierViewall">The fifth and sixth sections address the medical history, physical examination and complementary trests, which are essential elements in any Internal Medicine clinic. In particular, in a patient with CHF it is essential to collect hemodynamic parameters and perform a comprehensive assessment of congestion. In this respect, the protocol underscores three specific pillars: 1. The medical history and physical examination focused on HF (symptoms and signs of HF); 2. Clinical ultrasound (study of the inferior vena cava, B lines, third space, etc.); 3. Interstitial and intravascular congestion parameters (CA125 and natriuretic peptides, respectively).<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,13–15</span></a> This multiparametric assessment of congestion will be repeated at the different protocol visits, since knowing the degree of congestion at all times during the course of the disorder is essential for optimizing diuretics, which are the mainstay of the symptomatic treatment of patients with HF <a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0065" class="elsevierStylePara elsevierViewall">This section also includes general biochemical profiles (blood count, renal and liver profile, urine, etc.), thyroid hormone values, lipid profile, iron profile, calcium and phosphorus levels, uric acid, glycosylated hemoglobin, and microalbuminuria. Arterial blood gas testing is also included if OHS or some other respiratory disease requiring it is suspected (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The information obtained from these more specific tests may be crucial for anticipating potential complications and optimizing treatment.</p><p id="par0070" class="elsevierStylePara elsevierViewall">As regards complementary cardiac studies, a baseline electrocardiogram and echocardiogram (the latter being required for the diagnosis of HF) should be obtained.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> From this point on, and depending on the comorbidities, results should be obtained from other selected studies (stress tests, spirometry, chest radiographs, etc.) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0075" class="elsevierStylePara elsevierViewall">The final section refers to "diagnosis and treatment planning". It emphasizes the importance of characterizing the type of HF (according to the LVEF and NYHA functional class), as well as the diagnosis of structural heart disease and comorbidities. The degree of dependence and cognitive status of the patient is also considered.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Based on the above, a therapeutic strategy will be proposed (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), highlighting disease-modifying drugs in both HF with reduced or slightly reduced LVEF, and in HF with preserved LVEF, where there is currently a grade AI recommendation for the use of SGLT2i<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">In addition to the pharmacological approach, the importance of patient and/or caregiver education in self-care and the recognition of warning signs is acknowledged. This section proposes the start of an adapted educational program<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), evidencing the need to empower the patient to actively participate in his/her care and to be able to recognize and respond adequately to signs that may indicate HF decompensation.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Finally, the next review visits are scheduled, with the relevant studies, during which drug therapy will be optimized and continued education will be provided to reinforce some attitudes and correct others.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Scheduled review visit</span><p id="par0090" class="elsevierStylePara elsevierViewall">The scheduled or follow-up visit, either face-to-face or by telephone, is presented as an essential component of outpatient follow-up. It provides an opportunity to know the course of the patient, determine the hemodynamic and congestive condition, and make appropriate therapeutic adjustments <a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). Key elements of this visit are described below:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0095" class="elsevierStylePara elsevierViewall">Evaluation of patient progress: the incidents and care required by the patient since the last visit are analyzed, with special attention to events related to HF: admissions, emergency room visits, diuretic rescue in day hospital, or adjustments at home.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0100" class="elsevierStylePara elsevierViewall">Medical history, physical examination and complementary tests: performed in line with that previously established, placing emphasis on completing the assessment of congestion with clinical ultrasound and the congestion markers (CA125 and natriuretic peptides). Likewise, evaluation must be made of the course of the specific comorbidities of each patient (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0105" class="elsevierStylePara elsevierViewall">Update of the diagnosis and therapeutic optimization: with the information collected, the diagnosis should be updated and the treatment of HF and the comorbidities should be optimized. It should also be noted that the greatest number and the highest tolerated dosage of disease-modifying drugs should be used, as well as the minimum necessary doses of diuretics. This visit is also used to reinforce the educational plan (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0110" class="elsevierStylePara elsevierViewall">Organization of upcoming appointments and pertinent studies: this guarantees continuity of care adapted to the needs of the patient (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p></li></ul></p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Unscheduled visits</span><p id="par0115" class="elsevierStylePara elsevierViewall">Unscheduled visits are designed to respond early when the patient experiences symptoms or signs of HF decompensation, so that a protocolized and rapid intervention is made to improve the clinical condition and prevent progression leading to hospital admission.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The process starts when the patient contacts the HF unit or clinic because of alarm symptoms in which he/she has previously been educated. An initial assessment of severity is made, taking into account the intensity of dyspnea, hemodynamic constants and the response to previous therapeutic measures. If the patient contacts by telephone, this first approach may be performed on a non-face-to-face basis. If there are no signs of severity or low cardiac output, the medication should be adjusted, usually increasing the dose of oral diuretics, and the clinical response should be assessed by a new follow-up visit (possibly by telephone) within 3–5 days. In the event of a beneficial clinical response, diuretics should be reduced. If no improvement is observed or if there are signs of seriousness from the start, an in-person visit will be scheduled for the same day or the next (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). The ability to attend within 24 h is necessary to prevent the patient from having to go to the emergency room.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">In a face-to-face unscheduled visit, the patient is again assessed using the three pillars mentioned in the previous consultation models (physical examination, clinical ultrasound and biomarkers), and with the applicable urgent tests (laboratory tests, electrocardiogram and/or chest radiograph). If the patient is found to be congestive, but with no evidence of seriousness, oral loop diuretic treatment or intravenous treatment on a day hospital basis can be intensified,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> associated or not to oral coadjuvants (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). If improvement occurs, the therapeutic measures will be gradually optimized, reducing the diuretic dose to the minimum necessary dose. In contrast, if there is evidence of seriousness, or no response, direct hospital admission should be considered, avoiding the emergency room (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><p id="par0130" class="elsevierStylePara elsevierViewall">At discharge from each episode, clinical resolution and hemodynamic stabilization, as well as residual congestion, should be checked. In this regard, weight reduction, as well as normalization of the natriuretic peptide levels, or their reduction to patient baseline values, are of great help in the short term.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The concentration of CA125 also provides valuable information on patient congestion status, but because its half-life is considerably longer (7–10 days), the information it provides on congestive improvement is obtained later.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> At this visit it is also important to know what factor influenced decompensation (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>), and the status of the rest of the comorbidities should be established.</p><p id="par0135" class="elsevierStylePara elsevierViewall">As always, the last section of the visit focuses on reviewing the treatment and scheduling the next visit with the pertinent studies, ensuring the continuum of patient care (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Post-discharge visit</span><p id="par0140" class="elsevierStylePara elsevierViewall">The post-discharge visit plays a key role in transitioning patients from the hospital to the outpatient setting.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,17</span></a> The month after hospital discharge due to HF represents a period of high risk of readmission and mortality. To mitigate this risk, early follow-up visits have been implemented by both primary care and specialized units.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">The post-discharge visit, conducted no more than 15 days after hospital discharge, allows the person in charge to check the clinical stability of the patient and review the medication prescribed at discharge, verifying adequate administration and tolerability. In addition, it is an opportunity to titrate prognosis-modifying drugs or reduce the dose of diuretics, if the conditions allow (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0150" class="elsevierStylePara elsevierViewall">During this visit, the educational process started during hospital stay should be continued, insisting on self-care and warning signs. As on all visits, it is important to schedule follow-up, with the studies and appointments for the upcoming visits (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Coordination with advanced HF unit and palliative care</span><p id="par0155" class="elsevierStylePara elsevierViewall">This last section addresses the management of patients with advanced and symptomatic HF. Two scenarios are identified. On one hand, there are patients with severe and persistent symptoms, for whom alternative resources are still available in specialized advanced HF units. On the other hand, there are symptomatic patients in whom the treatment resources have been exhausted and palliative care should be started<span class="elsevierStyleSup">10</span> (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0160" class="elsevierStylePara elsevierViewall">Palliative care is provided in parallel to the measures already received by the patient, seeking symptoms control (using opioids, benzodiazepines, antidepressants, etc.) and taking into account the cognitive, functional, emotional and social status of the patient (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>).</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Diagnostic and treatment annexes</span><p id="par0165" class="elsevierStylePara elsevierViewall">This protocol focuses on the profile of the CHF patients usually managed by internists, characterized by being older individuals with a high comorbidity burden that make such cases clinically complex and with a high risk of decompensation.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,8</span></a> In this regard, the protocol includes a set of annexes contemplating this integrative view of HF and its comorbidities.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Annex I refers to the outpatient diagnosis of CHF, both the syndromic diagnosis (Annex Ia) and the etiological diagnosis (Annex Ib) (<a class="elsevierStyleCrossRef" href="#fig0025">Fig. 5</a>). In turn, Annex II refers to the management of the most common comorbidities associated with these patients. This includes anemia and iron deficiency (Annex IIa), cardiorenal syndrome (Annex IIb), type 2 diabetes mellitus (Annex IIc), AF (Annex IId) and a table of precipitating factors that often trigger decompensation (Annex IIe) (<a class="elsevierStyleCrossRefs" href="#fig0015">Figs. 3 and 6</a>).</p><elsevierMultimedia ident="fig0025"></elsevierMultimedia><elsevierMultimedia ident="fig0030"></elsevierMultimedia><p id="par0175" class="elsevierStylePara elsevierViewall">Finally, the important role of nursing professionals as a pivotal management element in the follow-up of these patients is highlighted under Annex IV, which was developed by nursing professionals from HF units. Of note is smoking, where the patients are educated in the applicable self-care measures and the recognition of alarm signs (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), along with the protocolized specific nursing visit, including the discharge visit (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Good practice recommendations in patients with CHF and conclusions</span><p id="par0180" class="elsevierStylePara elsevierViewall">The protocol ends with a series of good practice recommendations comprising 10 key points to be complied with at all times during follow-up, thereby ensuring optimum patient monitoring (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0185" class="elsevierStylePara elsevierViewall">To summarize, the document underscores the importance of comprehensive, coordinated, protocolized and patient-centered care to address CHF and its comorbidities effectively. It includes a focus on patient education and care by the nursing professionals as an element of key importance.
