It remains one of the leading causes of direct maternal death in Developed Regions<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> according to the WHO. There are several limitations to the existing epidemiological data for PE during pregnancy and postpartum. Frequently cited incidence figures reported in the PE literature combine with deep venous thromboembolism (DVT), use different denominators such as pregnancies, deliveries, or births, and often report pregnancy and postpartum cases together.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> Based on retrospective cohort studies, the most accurate estimated incidence of pregnancy-related PE ranges from 5 to 12 cases per 10,000 pregnancies -during pregnancy- and 3–7 events per 10,000 births- during the postpartum.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–7</span></a> A recent study reporting data from 20 years in the USA found a lower incidence, with 1.6 cases per 10,000 pregnancies.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Nonetheless, they also reported a rising trend during the studied period, which the authors attributed to increased comorbidity, maternal age, as well as increased use of computed tomography pulmonary angiography and D-dimer testing.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> When comparing these figures with the incidence of PE in non-pregnant women, the daily incidence is found to be ten times higher during pregnancy and up to 35 times higher during the postpartum period.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a> The pregnancy-induced hypercoagulability that persists more than four weeks after delivery explains this increased risk. Accurate diagnosis of PE relies on understanding the prevalence of the condition and the a priori probability of PE based on specific findings in individual patients. Precise knowledge of pregnancy-related PE frequency is essential for adequate counselling to patients for appropriate shared decision-making, for example, in deciding whether to perform computed tomography angiography.</p><p id="par0010" class="elsevierStylePara elsevierViewall">No published data regarding PE incidence during pregnancy and postpartum in Spain exists. As PE in high-risk patients, such as pregnant women, is routinely managed and admitted to hospitals, the use of a National Hospital discharge database can be an appropriate source of information to analyse the frequency of this disease. This source has been widely used and validated for epidemiological research.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">This study aims to estimate the overall and annual age-standardized incidence of pregnancy-related PE in Spain between 2016 and 2021, describe the distribution of events during pregnancy and postpartum, identify possible risk factors and estimate the mortality rate during hospital admission.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design and data source</span><p id="par0020" class="elsevierStylePara elsevierViewall">This is an observational, population-based, retrospective study using the Spanish National Hospital Discharge Database (Registro de Actividad de Atención Especializada-Conjunto Mínimo Básico de Datos: RAE-CMBD). The RAE-CMBD collects individual patient information (sex, age, admission, and discharge dates), diagnoses (up to 20), procedures (up to 20), and discharge destination (home, voluntary discharge, social institution, and deceased). Diagnoses include conditions present on admission or diagnosed during hospitalisation. Procedures include therapeutic or diagnostic procedures performed during admission. The RAE-CMBD is a mandatory registry annually submitted by all private and public hospitals in Spain to regional health authorities for assessment purposes and subsequently to the Ministry of Health. The International Classification of Diseases ICD is used to code diagnoses and procedures, with the 10th revision available in Spain since 2016.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study population</span><p id="par0025" class="elsevierStylePara elsevierViewall">The study population included all women aged 15–49 with hospital episodes of pregnancy-related PE between 2016 and 2021 identified by specific ICD-10 codes (O88.21, O88.22, O88.23) or a combination of pulmonary embolism (I26) with a diagnosis of pregnancy or pregnancy-related pathology (Z33.1 or any ICD-10 codes from chapter XV). A PE was considered a postpartum PE in women who had ICD-10 O85-O92 recorded. Postpartum or puerperium period begins immediately after childbirth and continues for the following six weeks. A postpartum or puerperal complication is a complication that occurs within six weeks after childbirth. (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>)</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Outcomes and covariates</span><p id="par0030" class="elsevierStylePara elsevierViewall">The primary outcome of interest was the overall and annual age-standardized incidence of pregnancy-related PE. Estimates for each year were calculated by dividing the number of events by the number of births in that year. As a secondary aim of the study, we describe the distribution of events during pregnancy and puerperium and calculate the age-standardized mortality rate during admission, which is defined as the number of patients who died while admitted and were diagnosed with PE during pregnancy or postpartum.</p><p id="par0035" class="elsevierStylePara elsevierViewall">For each identified case, we calculated the Obstetric Comorbidity Index<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> a validated index<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> that provides a simple measure for summarizing the burden of maternal illness. Furthermore, we individually identified a series of specific conditions, particularly relevant during pregnancy, including hypertension, diabetes mellitus, asthma, autoimmune diseases, neoplastic diseases, thrombophilia, obesity, SARS-CoV2 infection, bleeding, pre-eclampsia, previous caesarean delivery and personal history of other venous thrombosis and embolism.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">We conducted a descriptive analysis of demographic and clinical characteristics, including age, CCI, comorbidities of interest, length and department of hospital stay, and admission type (urgent or planned). Frequency distributions were utilised for categorical variables, while mean and standard deviation (or median and interquartile range, as appropriate) were used for quantitative variables. We used the chi-square test for categorical variables with the Yates correction, and the Fisher's exact test for dichotomic variables when the expected value of a cell was less than 5, and Student T for quantitative variables.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Age-standardized incidence and mortality estimates were estimated based on the provided definitions, and their 95% confidence intervals were calculated accordingly. Age-standardized incidence were calculated using the European Standard Population 2013<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> as the reference population. The number of births per year of analysis was obtained from the Spanish National Statistics Institute. The annual trend in incidence was calculated using the Mann-Kendall test. The Stata/BE 17.0 program (StataCorp LLC) was used for analysis.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Ethics approval and consent to participate</span><p id="par0050" class="elsevierStylePara elsevierViewall">The data were obtained under the standard operating procedures of the Ministry of Health of Spain. The study has been approved by the Jimenez Diaz Foundation Ethics Committee (EO285-23_HRJC). According to current Spanish legislation and given the nature of the study using pseudonymised data, obtaining informed consent was not required.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">During 2016–2021, in Spain, there were 2,178,805<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> births and 522 women diagnosed with pregnancy-related PE, 343 (66%) during pregnancy and 179 (34%) during the puerperium. The characteristics of the study population are presented in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The mean age was 33 years (SD 0.28), with 35% being over 35 years old and an additional 10% over 40. In 81.8% of cases, the diagnosis was recorded as the primary diagnosis. The age-specific incidence of pregnancy-related PE increased with maternal age (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), but the mean age of the women did not change significantly over the study period (p = 0.638). The prevalence of comorbidities was 24.52% of the women having an Obstetric Comorbidity Index of 2 or more, only 12 women (2.3%) had a record of having a previous venous thrombosis or embolism episode. During the final two years of the period, 38 (18%) women were concurrently diagnosed with SARS-CoV-2 infection during hospitalisation; 19% of patients with PE in 2020 and 24% of those in 2021 had a SARS-CoVv2 infection (13 and 25, respectively). Out of the total 38 cases with SARS-CoV2 infection, 19 (14%) women were admitted with PE during pregnancy, and 19 (26%) experienced PE during the postpartum period.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">The overall age-standardized incidence of pregnancy-related PE during the period was 2.83 (95% CI 2.23–3.43) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The incidence increased from 2.43 (95%CI 0.41–4.44) cases per 10,000 births in 2016 to 4.18 (95%CI 2.50–5.86) cases per 10,000 births in 2021. This upward trend showed an increasing tendency (Mann-Kendall test, p = 0.06) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The average length of hospital stay was higher among those with a PE during the puerperium (11.4 days) than those with a PE during pregnancy (7.2 days) (p < 0.001). Fourteen patients died during admission, accounting for 2.9% of cases, resulting in an age-standardized fatality rate of 0.07 (95% CI 0.01−0.13) deaths per 10,000 births. Women who died had a similar age to those who did not die (mean 33.28 vs. 32.0; p = 0.563), and most of them, 10 (66.6%), had experienced a PE during the postpartum. Among women who died, there were more cases of cancer than in the patients who did not die (13% vs. 1%; p < 0.001).</p><p id="par0070" class="elsevierStylePara elsevierViewall">Most cases were discharged from the Obstetrics department, 40.4%, followed by Internal Medicine (31.5%), and Pneumology (18.5%). Among the total identified pregnancy-related PE cases, 355 (68%) were diagnosed with a specific ICD-10 code for pregnancy-related PE (O88.21, O88.22, or O88.23) and 167 cases (32%) were recorded with the ICD-10 code I26 and an ICD-10 code from Chapter XV.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0075" class="elsevierStylePara elsevierViewall">Over the six-year period, the age-standardized incidence of pregnancy-related PE in Spain was 2.83 out of every 10.000 births. We also observed a non-significant upward trend, slightly increasing from 2.43 cases in 2016 to 4.18 in 2021. It is known that the risk of PE is higher during pregnancy and puerperium compared to non-pregnant women of the same age.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Our study is the first to report on the incidence of pregnancy-related PE from Spain using population-based data. While its incidence is low, the consequences of PE during pregnancy and postpartum can be dramatic for both women and their offspring and is a major cause for concern among clinicians who provide care for pregnant women. Previously reported figures for pregnancy-related PE vary across different studies.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5,8,10</span></a> In a recent study based on databases of women hospitalised for pregnancy or childbirth in the USA,<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> aimed at characterising risk profiles and cardiovascular complications over the last 20 years, an average of 1.6 PEs per 10,000 births is reported, ranging from 0.9 in 2002 to 4.0 in 2023, mirroring our results. Some authors have suggested that the increase in pregnancy-related PE could be attributed to women's age<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>; however, we did not observe significant variations in women's age over the study period to support this hypothesis. Other aspects, such as the use of fertility treatments, which predispose to a higher risk of thromboembolic disease, could also justify the increase.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> SARS-CoV-2 infection during the same hospital admission appears as an associated comorbidity<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> during the last two years of our study in a relevant number of women; up to 24%% of women with pregnancy-related PE had coronavirus infection in 2021. We also observed an increase in the incidence of pregnancy-related PE during these two years, which is coherent with previous literature showing an increase in acute pulmonary embolism hospitalisations during the COVID-19 pandemic in the general population in patients with COVID and without COVID-19 infection.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">According to the Spanish Ministry of Health,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> the mortality rate for PE during hospital admission in non-pregnant women aged 15–49 is 6%, and the majority of these cases are related to cancer. Our study found that 3% of patients with pregnancy-related PE died during their hospital stay, although maternal death in Spain is less than 0.01%, which suggests that pregnancy-related PE significantly increases the risk of death. Although mortality rates vary depending on the severity of PE, this figure is consistent with global statistics provided by the World Health Organization.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Our study has limitations, some common to all research that uses administrative datasets<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>; it is subject to the availability and quality of data entry and confounding. We may be missing patients who do not get admitted or who die from PE before or after discharge since we do not have outpatient data. However, we are confident we have captured all recorded cases using population-based data. Although inaccurate coding is possible, the diagnosis of venous thromboembolic disease using this dataset has been previously validated against medical records, suggesting a positive predictive value of more than 90%.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Moreover, previous studies suggest that pregnancy-related PE cases might not always be admitted to the obstetric department and identified solely by obstetric ICD codes for pregnancy-related PE. Therefore, we opted not to restrict the search to any specific admission department to increase the sensitivity of our search. We included cases with an ICD-10 code for pulmonary embolism (I26) combined with an ICD-10 code for pregnancy or puerperium. We identified a significant number of cases, 167 (30%), that did not have a specific ICD-10 code for pregnancy-related PE. This finding confirms that exclusively selecting obstetric ICD-10 codes may underestimate the number of cases. It could also explain why we found a lower incidence of PE during the postpartum period than reported in other series.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Moreover, we found mortality was higher during the postpartum (5.6%) compared to the pregnancy (1.5%) (p = 0.007), which might reflect more severity in those cases or that after delivery, clinicians may be less aware of the association of PE with pregnancy, and do not include a related ICD10 code, except in most severe cases with poor outcome where the recent pregnancy might be highlighted.</p><p id="par0090" class="elsevierStylePara elsevierViewall">We were unable to identify some of the known risk factors for pregnancy-related PE,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> such as immobilisation during pregnancy, smoking, or body mass index, due to the limitations of our data. The pharmacological prophylaxis protocols during pregnancy and postpartum have remained unchanged during the study period. Hence, we do not consider this could explain the increase in cases over the period. However, our data could not be linked to prescription records, and thus, we cannot determine which women received thromboprophylaxis.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Our results have important implications as it is the first to report data on the incidence of pregnancy PE in Spain, and we have confirmed that although uncommon, it implies a very high relative risk for pregnant women in the context of very low maternal-related death. Accurately estimating the frequency of the condition should help clinicians have a high index of suspicion, especially relevant during the puerperium, and provide adequate counselling to patients in the diagnosis process. An effort should be made to improve the pregnancy-related PE codification during the puerperium.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Ethical statement</span><p id="par0100" class="elsevierStylePara elsevierViewall">The data were obtained under the standard operating procedures of the Ministry of Health of Spain.
