In truth, we face significant challenges every day due to the numerous gaps in science and medical practice that need resolving to help improve day-to-day practice. Without a doubt, those gaps highlight the importance of continuing to research and delve deeper into our knowledge, answer questions that arise, and improve patient care.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">However, despite its difficulties and challenges, the attraction that clinical practice has for younger colleagues is entirely understandable, as it allows knowledge acquired on a daily basis to be immediately applied to patient care, as well as the opportunity to directly help vulnerable individuals and those in need of care.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Simultaneously, while regularly treating complex medical conditions, colleagues may feel drawn by the curiosity to research the underlying biological or physiological mechanisms of diseases. While clinical practice is an irrefutably essential aspect of medicine, it is important to recognise that research, which is inherent to clinical care, plays an equally important role, providing new resources for better patient care and treatment.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> It cannot be denied that research is demanding and requires time, generosity, dedication, and perseverance, but it also provides great satisfaction in addition to the pleasure derived from intellectual gratification. We agree that, just as solving a difficult clinical case is highly gratifying for doctors, solving a complex scientific problem, either in the laboratory or via a clinical trial, can be equally gratifying.</p><p id="par0020" class="elsevierStylePara elsevierViewall">As clinical doctors, we believe it is essential to focus on translational research, as it is the channel for filling in the persistent gaps in scientific knowledge and accelerating progress. Translational research is the nexus between basic scientific discoveries and their clinical application, allowing laboratory findings to be translated into better care and patient outcomes.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> On the other hand, it is essential to continuing advances in understanding the mechanisms of diseases in order to address understanding of the diseases, the foundations of their clinical courses, and response to treatment.</p><p id="par0025" class="elsevierStylePara elsevierViewall">As mentioned above, the best model for getting involved in the generation of knowledge is that which is followed by professionals who supplement healthcare activity with clinical research.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> This duality of using knowledge learned by others while also driving new advancements is not easy and requires certain qualities and extra effort to achieve it. This type of doctor makes it possible for new knowledge to reach their patients sooner. Along these lines, the Carlos III Health Institute (ISCIII), leading public research agency for the funding and management of biomedical research in Spain, via its annual Strategic Health Action open call, allows clinical researchers to obtain public funding, via public competition, to carry out their projects within the National Health System (SNS), with a special emphasis placed on the incorporation of budding researchers.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">While research skills are not sufficiently recognised as one of the most important assets of a good doctor, in Spain, fortunately, we have structured programmes that help boost this talent. One success case is the initiative launched by the ISCIII some years ago, which has been progressively establishing itself. Its main objective, focused on human resources, is the maintenance, refinement, and growth of the scientific network in National Healthcare System centres, and takes the form of regular open calls. The aim is to subsidise the hiring of doctors who have finished specialised training and want to embark on a research path in combination with their care responsibilities. These types of contracts, known as Río Hortega contracts, allow candidates to receive research training for 2 years with the aim of defending a doctoral thesis in the second year, thus completing a study track that would have started previously during their residency period. Candidates then move on to the consolidation phase via a Juan Rodés contract, which lasts 4 years, after which they will have reached a high level of training as a physician-scientist. These contracts are interesting in that they allow doctors to combine healthcare and research activities and the commitment of the receiving institution to training said professionals. The call for these subsidies occurs on a yearly basis and are increasingly more competitive.</p><p id="par0035" class="elsevierStylePara elsevierViewall">An interesting question that arises is whether Artificial Intelligence (AI) can be a tool to make research more attractive to younger doctors. AI offers new opportunities for clinical research across a wide range of fields, from improving the capacity to analyse mass amounts of data, to speeding up drug discovery processes, identifying promising molecules, and predicting their efficacy and side effects.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Add to that thousands of other opportunities such as improving the design of clinical trials or the ability to identify patient subgroups that could benefit from specific treatments. Young doctors are not unaware of this potential and are often interested in adopting new technologies in addition to being at the forefront of medical breakthroughs. AI can be seen as a powerful tool for exploring new questions and obtaining new perspectives of complex medical problems.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Doctors share the duty of working to advance medicine, so it is particularly important that the most experienced among them shoulder the responsibility of acting as mentors, tutoring younger colleagues and encouraging them to start down their own path in the world of research. By offering them guidance, support, and constructive feedback, mentors can help younger colleagues face and conquer both the challenges and opportunities presented to them, thereby starting down their own path in the world of research. Similarly, institutions dedicated to science are also uniquely responsible for making an effort to speed up advances. In fact, it is worth noting the effort made by the Spanish Society of Internal Medicine to promote educational support for research among its youngest professionals. A good example of this is the Clinical Research School, whose aim is to present to all internists, especially the youngest, the means and tools needed to produce patient-centred clinical research and high-quality innovation.
