The estimated annual incidence is between 0.5 and 1.6 cases per 1000 people per year,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> with a sex-standardized prevalence of around 4.40 cases per 1000 people.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Depending on the anatomical location of the occlusion, in the central vein or one of the branches, it is classified as a central retinal vein occlusion (CRVO) or branch retinal vein occlusion (BRVO). The prevalence of the latter is higher (3.77 versus 0.65 per 1000 people).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The incidence of RVO increases considerably with age, as age is one of the main risk factors, with a global prevalence of 0.27 per 1000 people in individuals younger than 50 years and 5.44 in those older than 80 years.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> This represents an incidence that is twenty times higher in the elderly than in younger adults.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Unlike venous occlusions of other vascular beds, RVO is more likely associated with classic cardiovascular risk factors (arterial hypertension, diabetes mellitus, dyslipidaemia, smoking, chronic kidney disease, sleep apnoea-hypopnea syndrome, etc.). Despite being an occlusion of a venous bed, this association with risk factors typically related to arterial manifestations must be taken into consideration as it has important clinical and therapeutic implications. There are also local risk factors, such as glaucoma.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–10</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Among the thrombophilia classically associated with venous occlusion, antiphospholipid syndrome is particularly significant in RVO.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Treatment is based on controlling said risk factors. Antithrombotic therapy is reserved for very specific situations, such as patients with persistent positive antiphospholipid antibodies.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Rey Juan Carlos University Hospital (Móstoles, Madrid, Spain) is a secondary hospital in the outskirts of Madrid and works according to three strategic lines: health, efficiency, and patient experience. It is characterized by its dynamic ability to adapt its strategies to achieve goals.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Within the context of the first core strategy, the Internal Medicine and Ophthalmology services launched a specific care and assistance program for patients with RVO to improve risk factor detection and treatment.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The aim of this study is to audit the results of said program.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Intervention</span><p id="par0035" class="elsevierStylePara elsevierViewall">The RVO patient care improvement program is a multidisciplinary program coordinated by the Retina Unit of the Ophthalmology Service and the Venous Thromboembolic Disease (VTE) Unit of the Internal Medicine Service. The program started in April 2021.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The care pathway (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) starts with an ophthalmologic evaluation in which the patient is diagnosed with RVO. All patients newly diagnosed with RVO were referred for a specialized consultation with the VTE unit. The first assessment was performed over the phone to avoid unnecessary hospital visits for patients who, according to the internists, would not benefit from additional testing, such as: highly dependent patients, patients with close follow-up by other services due to their risk factors, patients older than 90 years (or older than 85 with severe comorbidity) with close follow-up by primary care (at least one biannual visit, and patient-perceived ease of access to consultation when necessary), etc.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Prior to this evaluation, all patients were required to undergo standard laboratory testing. No appointment was required, and the request was automated at the same time as the clinical assessment request to prevent variability. Said testing included: hemogram, fasting glucose, glomerular filtration, liver function testing, total and LDL cholesterol, glycated haemoglobin, lupus anticoagulant, anti-cardiolipin antibodies and anti-beta-2 glycoprotein 1 antibodies.</p><p id="par0050" class="elsevierStylePara elsevierViewall">An initial assessment of classic risk factors and their control was conducted at the first consultation:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">□</span><p id="par0055" class="elsevierStylePara elsevierViewall">Smoking (based on patient-reported use).</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">□</span><p id="par0060" class="elsevierStylePara elsevierViewall">Overweight or obesity.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">□</span><p id="par0065" class="elsevierStylePara elsevierViewall">Arterial hypertension (HTN) (according to self-monitoring or measurements recorded by primary care).</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">□</span><p id="par0070" class="elsevierStylePara elsevierViewall">Diabetes mellitus (DM) and dyslipidaemia (according to lab results).</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">□</span><p id="par0075" class="elsevierStylePara elsevierViewall">Presence of antiphospholipid antibodies.</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">□</span><p id="par0080" class="elsevierStylePara elsevierViewall">Other lab alterations suggestive of hypercoagulability, such as polyglobulia, and any suspected risk factors based on patient reporting.</p></li></ul></p><p id="par0085" class="elsevierStylePara elsevierViewall">For those patients without any identified risk factors or whose risks were under control, the following testing was requested: factor V Leiden mutation, G20210A prothrombin gene mutation and protein C, S and antithrombin levels.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Based on this initial assessment and the patient’s characteristics, additional necessary testing could be requested (glucose tolerance test, assessment to detect SAHS if suspected, Holter monitoring of blood pressure or self-monitoring, new antiphospholipid antibody testing if the first measurement was positive, etc.) and any necessary treatment changes were made.</p><p id="par0095" class="elsevierStylePara elsevierViewall">At the second and subsequent assessments (where applicable), the efficacy of the measures was evaluated, and any necessary modifications were made. Lastly, once acceptable control had been achieved, the patient was referred for follow-up with their primary care physician.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Intervention assessments</span><p id="par0100" class="elsevierStylePara elsevierViewall">Since the program is aimed at improving the detection of undiagnosed risk factors or poor control of known risk factors, the proportion of patients with the following were evaluated:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">a</span><p id="par0105" class="elsevierStylePara elsevierViewall">Patients who received a new diagnosis according to clinical practice guidelines<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–17</span></a> for HTN, diabetes mellitus, dyslipidaemia, SAHS, active smokers, and antiphospholipid syndrome.</p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">b</span><p id="par0110" class="elsevierStylePara elsevierViewall">Patients with poor control of one or more of the following risk factors according to the current European clinical practice guidelines<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>: HTN, diabetes mellitus, dyslipidaemia.</p></li><li class="elsevierStyleListItem" id="lsti0045"><span class="elsevierStyleLabel">c</span><p id="par0115" class="elsevierStylePara elsevierViewall">Patients who received a new incidental diagnosis of any pathology that increased their risk of RVO.</p></li></ul></p><p id="par0120" class="elsevierStylePara elsevierViewall">At the criteria of the researchers, the goal of the program was defined as detecting new risk factors or poor control of pre-existing ones in at least one third of patients, and for this to translate into some kind of therapeutic action in at least two thirds of the latter. All the analyses were specified according to the location of the occlusion via its distribution of frequencies.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Clinical data for all the patients seen in the program between April 2021 and April 2022 were collected via an automated download of the dissociated data (without identifying data). Given the retrospective nature of the data collection and collection method, no informed consent was required.</p><p id="par0130" class="elsevierStylePara elsevierViewall">This study was approved by the Research Ethics Committee of the Jiménez Díaz Foundation Healthcare Research Institute (protocol EO-061-22). The researchers did not receive any financial support from public or private institutions for the execution of this study. The study complies with the World Medical Association code of ethics.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0135" class="elsevierStylePara elsevierViewall">A total of 64 patients were seen in the care pathway between April 2021 and April 2022. Of those, 4 were incorrect referrals (all retinal artery occlusion), 2 were duplicate patients and another 2 were patients with suspected RVO without definitive confirmation. The remaining 56 cases were analysed. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the baseline characteristics of the patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">Following consultation assessments, the risk factors described in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> were detected.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall">Twelve patients did not present any new or poorly controlled risk factors. So, according to protocol, thrombophilia testing was extended as reported in the methods. Only one heterozygous factor V Leiden mutation was detected (central vein occlusion).</p><p id="par0150" class="elsevierStylePara elsevierViewall">The autoimmunity patterns of patients with a positive result, at some point, for antiphospholipid antibodies are described in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. Of those patients, positive results were only confirmed after 12 weeks in 5 patients, or 8.9% of the total sample (2 with central vein occlusion of the retina and positive for anti-cardiolipin IgM), and 3 with branch occlusion (2 positive for anti-cardiolipin IgM and one with double positivity: for cardiolipin IgM and lupus anticoagulant). None of the cases had clinical or laboratory data suggestive of associated systemic autoimmune disease.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0155" class="elsevierStylePara elsevierViewall">The therapeutic actions taken are described in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. The correlation between the number of new diagnoses and the actions taken was moderate and statistically significant (Spearman’s ρ 0.61; <span class="elsevierStyleItalic">p</span> = 0.000).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0160" class="elsevierStylePara elsevierViewall">The implementation of a specific care program for patients with retinal vein occlusion was effective at detecting undiagnosed or poorly controlled risk factors.</p><p id="par0165" class="elsevierStylePara elsevierViewall">The patients treated in this care pathway presented characteristics similar to those previously reported in the literature<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,4,5,8–11</span></a> and notably to those of other Spanish patients<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>: they are mostly patients in their seventies, with classic risk factors (mainly HTN and diabetes mellitus), with a similar prevalence between the sexes, and the majority with more than one risk factor. It is worth mentioning that, at diagnosis, there were no patients with known antiphospholipid syndrome and 5 patients had no known risk factors.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Bearing in mind the standards of care proposed by the clinical practice guidelines,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> at least one new risk factor, or poor control of one known risk factor, was identified in 69% of patients, thereby surpassing the predefined criteria of one third of new diagnoses. These results are in line with other published results suggesting there is a benefit to opportunistic screening of cardiovascular risk factors by taking advantage of patient contact with the healthcare system for other reasons such as surgery<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> or new diagnosis of erectile dysfunction.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">The prevalence of antiphospholipid syndrome in our sample (8.9%) was similar to that of other hospitals in our environment.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In terms of autoimmunity patterns, we deemed that, with only 5 cases detected, generalisations or comparisons with other studies cannot be made, though positivity of a single antibody is more common than double or triple positivity, similar to that found in our setting.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">Only one patient without any apparent risk factors presented a low-risk thrombophilia mutation (heterozygous factor V Leiden), which in our opinion supports the etiopathogenetic hypothesis of a more significant role of atherosclerosis in the development of RVO. Even with the limitations presented by a very small number of patients without cardiovascular risk factors, our data could suggest greater efficiency in studying said factors than the search for hereditary thrombophilia.</p><p id="par0185" class="elsevierStylePara elsevierViewall">The number of risk factors detected in our sample moderately and positively correlates with the number of actions taken. More than three quarters of the patients received some therapeutic action following inclusion in the program, a number which well exceeds the researchers’ predefined threshold. This could indicate that the program is effective at improving the quality of risk factor detection and management in patients with RVO. Nevertheless, we believe that the program could be improved by performing an electrocardiogram and Doppler ultrasound of the supra-aortic trunks in the study, which would help identify arrhythmias and carotid atheromatosis.</p><p id="par0190" class="elsevierStylePara elsevierViewall">A notable limitation of this study is the absence of a counterfactual analysis of the intervention, meaning it is hard to accurately attribute the contribution of the program to the results. Nevertheless, RVO is related to atherosclerosis, and this requires that the factors leading to it be active over a considerable period of time. Despite this, many of the risk factors or their poor control went unnoticed and were not detected until inclusion in the program, which supports the hypothesis of its efficacy.</p><p id="par0195" class="elsevierStylePara elsevierViewall">The study strengths include the systematic approach to intervention in all patients as part of regular clinical practice, reducing interindividual variability and, with it, potential classification and selection bias.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0200" class="elsevierStylePara elsevierViewall">An RVO diagnosis represents an opportunity to evaluate patients’ cardiovascular risk. A multidisciplinary care program improves the quality of risk factor detection and treatment.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Ethical considerations</span><p id="par0205" class="elsevierStylePara elsevierViewall">This work was conducted in accordance with the World Medical Association Code of Ethics. A specific program was designed to detect and treat risk factors in patients with RVO. All patients with RVO were screened, at minimum, for hypertension, diabetes, dyslipidemia, smoking, overweight, and antiphospholipid syndrome. New risk factors or poor control of known risk factors were expected to be found in at least one-third of the patients. Among them, therapeutic measures were expected to be taken in at least two-thirds. A dissociated automated search of the data of all patients who entered the program between April 2021 and April 2022 was performed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Fifty-six patients were included for analysis. Of these, 39 (69.6%) had at least one new or poorly controlled risk factor and 43 (76.8%) had their treatment modified in some way. Antiphospholipid syndrome was detected in five (8.9%). Only one patient had low-risk hereditary thrombophilia. Se esperó encontrar nuevos factores de riesgo o pobre control de los ya conocidos en, al menos, un tercio de los pacientes. Entre ellos, se esperó tomar alguna medida terapéutica en, al menos, dos tercios. Se llevó a cabo una búsqueda automatizada disociada de los datos de todos los pacientes que entraron en el programa entre abril de 2021 y abril de 2022.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0090" class="elsevierStyleSimplePara elsevierViewall">Cincuenta y seis pacientes se incluyeron para el análisis. De ellos, 39 (69,6%) tenían al menos un factor de riesgo nuevo o mal controlado, y 43 (76,8%) vieron modificado en algún modo su tratamiento. Se detectó síndrome antifosfolípido en 5 (8,9%).
