However, achieving the dual objective of promoting knowledge and earning profits can occasionally generate conflicts of interest (CI) among healthcare professionals. These CI occur when the doctor-patient relationship is conditioned by a third party, such as a pharmaceutical company, which influences—sometimes indirectly, surreptitiously, or unconsciously—diagnostic or treatment decisions. This influence is especially serious in cases that put medicine’s main objective at risk: the care of patients’ health.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The pharmaceutical industry is not the only, or perhaps even the main, source of CI in routine medical practice. Nevertheless, it is evident that company profits depend in large part on medical prescribing. The pharmaceutical industry’s influence on prescribing through incentives has been known for decades.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> This influence increases the more professionals interact with sales representatives, if they receive gifts or fees for research, or if they receive subsidies for attending conferences or training courses.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The pharmaceutical industry’s involvement in continuing medical education (CME), either through the sponsorship of activities or subsidizing registration fees, generates CI that is often difficult to identify or address.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> On the one hand, the collaboration of pharmaceutical companies in CME is widely accepted by professionals, in large part due to the high costs of CME. On the other hand, there is the risk of wanting to influence prescribing through financial support for CME. If medical prescribing ceases to be independent, pharmaceutical costs will increase and the quality of prescribing will decrease.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In July 2020, a draft bill was introduced in Spain’s Congress on measures for social and economic rebuilding in the context of the crisis caused by COVID-19. This document proposed that CME be funded exclusively by the government, barring involvement from private entities.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This draft bill was received with varying degrees of support and many medical groups have come out against it.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> The aims of this work are to gather the opinions of physicians and hospital pharmacists about the draft bill and learn what they think about how CME should be funded and about CI in CME.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">This work is a descriptive, cross-sectional study conducted via voluntary anonymous surveys aimed at physicians and hospital pharmacists. Personnel in training (residents), specialist physicians, and retired professionals were included.</p><p id="par0030" class="elsevierStylePara elsevierViewall">To create the survey, a literature search on “CI between healthcare professionals and the pharmaceutical industry” was conducted. In the review, no studies were found that evaluated the 2020 draft bill. The research team adapted questions from surveys included in other publications on CI in CME.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a> The survey included 17 questions: five on sociodemographics, 11 closed-ended multiple choice questions (yes/no or inverted Likert scale), and one free text field for additional comments. The survey was distributed through the Google Forms application on the Twitter, Facebook, and WhatsApp social networks. It remained open for two weeks, from August 19, 2020 to September 1, 2020.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Variables</span><p id="par0035" class="elsevierStylePara elsevierViewall">To address the study aims, the questions were divided into four categories: (1) questions related to the draft bill, (2) questions to evaluate the professional’s perception of CI, (3) a set of questions aimed at evaluating other possible training alternatives, and (4) questions about the professional’s relationship to the industry (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). In addition, data on sociodemographic variables (sex, age, and residence), professional category, and specialty were gathered.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">For the analysis, the population was divided into age groups and specialties were grouped as medical specialties, surgical specialties, central services, and other. For qualitative variables, percentages were calculated as measures of central tendency in the descriptive analysis. For the quantitative variables, means and standard deviation were calculated.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Student’s <span class="elsevierStyleItalic">t</span>-test was used to compare responses among independent variables. All tests were two-tailed. Statistical significance was established as <span class="elsevierStyleItalic">p</span> < .05.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The data were analyzed using the SPSS computer program version 19.0 (IBM, Armonk, NY, United States of America).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Ethical considerations</span><p id="par0055" class="elsevierStylePara elsevierViewall">The study was conducted in accordance with the principles of the Declaration of Helsinki (1996) and current good clinical practice guidelines. Prior to starting the survey, all participants were informed that their answers were voluntary in nature and anonymous. All data were processed with maximum confidentiality, in accordance with current legislation.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Sample characteristics</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of 491 responses were received. After excluding duplicate surveys, those with too many empty values, and those from subjects who did not belong to the study population, a total of 435 surveys were analyzed, 88.6% of the initial sample. Of the respondents, 59.8% (260) were women and 98.2% (427) resided in Spain. In regard to professional category, 87.6% (381) were specialist physicians, 9.9% (43) were residents, and 2.5% (11) were retired.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The distribution by age was as follows: younger than 30 years 12.2% (53); 31–40 years 46% (200); 41–50 years 18.9% (82); 51–65 years 20.2% (88), and older than 65 years 2.8% (12). Of the total, 77.2% (336) were medical specialists, 11.7% (51) were surgical specialists, and 9.2% (40) were central services specialists. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the distribution of participants according to specialty.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Survey results</span><p id="par0070" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> summarizes the results of the responses. A total of 125 free-text responses were received, grouped in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Men showed less confidence that the government would fund CME (81.7% vs. 65%) and more considered CME funded by the industry as low quality (11.4% vs. 2.3%). Fewer men thought that collaborating with the industry does not influence their prescribing (68% vs. 78.8%).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Participants 31–65 years old valued industry-subsidized CME more: 71% considered it to be of quality versus 58% of those younger than 30 years old. Subjects 31–40 years old invested more in their training than those over 50 years old and 90.5% had funded at least one activity in the last five years. Participants 31–50 years old received more funding in the previous five years than those over 50 years old (91.4% vs. 73%). Those younger than 40 years old had received more free meals in restaurants compared to other age groups (52.8% <30 years; 38% 31–40 years old; 25% 51–65 years old).</p><p id="par0085" class="elsevierStylePara elsevierViewall">More retired subjects supported the draft bill (63.6%) than residents and specialists (32.6% and 32%, respectively), although they showed less confidence about whether the law would be enacted (90.9% retirees vs. 72.1% residents and 71.1% specialists).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Central services specialists mostly supported the government ensuring their training (97.5%), more so than medical (91.7%) and surgical (92.2%) specialists. A higher percentage of medical specialists surveyed considered that each professional should fund their training (15.8%; surgical 5.9%; central services 10%). More surgical specialists thought that there are no educational alternatives that are not funded by pharmaceutical companies (84.3%; 63.7% medical specialists; 37.5% central services) and that training from the industry is of sufficient quality (80.4% vs. 67.3% medical specialists). No surgical specialists surveyed considered that they had received poor quality training from the industry.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Medical and surgical specialists had received free meals at restaurants more often (39.3% and 39.2%, respectively) than those from central services (22.5%).</p><p id="par0100" class="elsevierStylePara elsevierViewall">More medical specialists believed that funding influences their prescribing (11.3% vs. 2% of surgical specialists) and more felt uncomfortable accepting funding (22.9% vs. 11.8% of surgical specialists).</p><p id="par0105" class="elsevierStylePara elsevierViewall">Regarding the free-text comments, 51 considered that the price of training was too high and impossible to pay without the support of the pharmaceutical industry, 29 indicated that the workplace should train professionals and fund that training, and 23 stated that the government did not have the money needed to fund training.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">More than half of those surveyed did not agree with a draft bill that intends to forbid the pharmaceutical industry from directly or indirectly funding CME. In addition, most did not believe the government would enact the draft bill. However, nearly all participants (92.4%) agreed that the government and private institutions should guarantee and subsidize their workers’ training. Therefore, most of those surveyed were in favor of the industry being involved in CME and that training should also be paid for by their institutions.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The pharmaceutical industry’s involvement in CME is currently guided by ethical<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a> and legal<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> guidelines focused on transparency. However, transparency, which includes the declaration of possible CI, may not be enough to limit pharmaceutical companies’ influence on prescribing.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,17</span></a> Is a specific regulation, like the 2020 draft bill, necessary? It would be needed so long as it were effective, and for it to be effective, it must be accepted by the parties involved, including healthcare professionals.</p><p id="par0120" class="elsevierStylePara elsevierViewall">For physicians’ training not to depend on the pharmaceutical industry, it must be financially affordable and there must also be institutional support for CME. If not, we will remain in the hands of companies whose main objective is for physicians to prescribe a certain medical product.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Those surveyed received a high degree of support from pharmaceutical companies in their training, more than in other studies:<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,18,19</span></a> 86% received financial support in the last five years, especially those 30–50 years old. The specialists who most considered that their workplace should fund CME were those from central services, possibly because they received fewer training opportunities from pharmaceutical companies. Other studies have described inequalities in support offered by the industry to physicians; indeed, hospital physicians received more offers than those who worked outside of a hospital.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> This divergence in training opportunities may generate inequality in access to CME.</p><p id="par0130" class="elsevierStylePara elsevierViewall">On the other hand, there is little perception of CI. Only one in five professionals recognized feeling uncomfortable with funding received from the pharmaceutical industry and three out of four believed this funding does not impact their prescribing. This lack of perception of CI has been described in other works conducted on both physicians<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> and residents.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">In a study conducted on family physicians in Catalonia, few considered that accepting a free meal had an influence on their prescribing or represented a CI.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> However, there are studies that show how the industry negatively influences medical prescribing, worsening its quality<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,24</span></a> and increasing healthcare costs.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In addition, it has been observed that gifts, free meals, and other incentives influences prescribing.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> For example, free meals at restaurants, which were commonly received among those surveyed, were associated with more prescribing of the promoted drug.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Luckily, the payment of expenses not related to CME (restaurants, transportation, lodging, tourism visits) is becoming less and less common, possibly because there is a more demanding regulatory framework.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Despite the negative effects the industry may have on prescribing, the relationship with pharmaceutical company, as seen in our study, is viewed as normal. It is assumed to be something legitimate and natural.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Professionals’ desire to receive support for funding their training may possibly be due to its cost. Indeed, the most numerous group of free-text comments (40.8% of them) refer to this problem, which is not exclusive to our setting. One study from the United States of America showed that in spite of professionals recognizing that financial support from the pharmaceutical industry may entail CI, they prefer to have it in order to reduce the price of training.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> In our study, though most of those surveyed wanted to receive support for funding their training, a large part had funded training on their own on some occasion, especially those 30–40 years old. Although no significant differences were observed among responses from specialist physicians and residents, other works have described that residents justify the funding of CME because of their lower salaries<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> and the sacrifices they make at work.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Additional studies also show that residents who worked with physicians who accept gifts are more likely to accept them as well.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,22</span></a> This highlights the significance of education on CI for healthcare professionals, especially during the educational period (residents and students). The education of new cohorts of students, in addition to courses and other formal education, should begin with the example of physicians.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Industry-funded CME is widely accepted and considered trustworthy. More than two out of three believe it is of sufficient quality, especially those 31–65 years old and surgical specialists. These data are in agreement with the study by Makowska,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> which describes a high degree of physician participation in training events sponsored by the industry and that physicians considered them in large part trustworthy. The majority of those surveyed believe there are no valid alternatives to training offered by the industry, especially surgical specialists who, in addition, believe that training from pharmaceutical companies is of greater quality.</p><p id="par0155" class="elsevierStylePara elsevierViewall">It cannot be ignored that the training of surgeons involves a practical component that increases training costs; this may make them more favorable to CME funding from the private sector.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Our study has several limitations. As it was a closed-answer survey, the reasons underlying these opinions were not explored in depth. Therefore, the interpretation of the results may differ from the hypotheses proposed. As the survey was disseminated via social media, we cannot know the degree of participation. It is also possible that there was selection bias, such that more people interested in this issue may have participated. Lastly, it is an unvalidated survey. Despite these limitations, the size of the sample and the fact that the survey provides relevant data on an issue that has not been studied in our setting should be highlighted.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Conclusions</span><p id="par0165" class="elsevierStylePara elsevierViewall">More than half of those surveyed did not agree with a draft bill that intends to forbid the pharmaceutical industry from directly or indirectly funding CME. In addition, most did not believe the government would enact the draft bill. However, nearly all participants (92.4%) agreed that the government and private institutions should guarantee and subsidize their workers’ training. In general, those surveyed did not detect CI in their relationship with the industry.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Lastly, it should be noted that if there is no awareness of a problem, it is not possible to change attitudes. Therefore, it is important to educate professionals so that they are aware of CI and learn to manage them. This education should provide professionals with tools that allow for differentiating between and separating out training from marketing.</p></span><span id="sec0055" class="elsevierStyleSection opinions on this draft bill, learn what they think about the funding of continuing medical education and possible conflicts of interest.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">This work is a descriptive cross-sectional study conducted via voluntary anonymous surveys aimed at physicians and hospital pharmacists.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Four hundred thirty-five surveys were analyzed, 59.8% of which were from women. Of the respondents, 87.6% were specialist physicians and 77.2% were part of a specialist department. Furthermore, 52.4% disagreed that each professional should pay for his or her own training while 14% agreed they should.
