Congresses provide for the generally less demanding preliminary presentation of studies; however, these congresses should not exempt studies from the ultimate goal of publishing their researches. The aim of this study was to determine the percentage of publications derived from the abstracts presented at a congress of the Spanish Society of Internal Medicine (SEMI) and the factors that favored these publications.</p><p id="par0010" class="elsevierStylePara elsevierViewall">We reviewed all posters and oral presentations submitted to the XXXVI Congress of SEMI held in Seville (Spain) from the 11th to the 13th of November 2015. We then analyzed the publications that resulted from these posters/presentations during the 4 subsequent years (to the 31st of December 2019). To this end, we performed a search on PubMed, using as the search strategy the name of the signatories and, when necessary, the keywords of the poster/presentation title or the institution where the study was conducted. We collected the following data on the posters/presentations: classification within the 14 subject areas established by the organizing committee, prospective or retrospective design, explicit reference to the statistical analysis, single-center or multicenter character, sample size, positive or negative results referencing the established hypothesis or the experimental treatment, whether the study was awarded by the organization running the congress and, in the event the study was ultimately published, the elapsed time to publication, as well as the name, national or international setting and range (quartiles [Q] 1 to 4 of the Journal Citation Reports) of the journal. The results are expressed as numbers and percentages, with their 95% confidence intervals (CI). Chi-squared and Mann–Whitney <span class="elsevierStyleItalic">U</span> tests were employed for the intergroup comparisons, as appropriate. We performed a multivariate logistic regression analysis to determine which of the previously mentioned variables were independently associated with the subsequent publication of the poster/presentation.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Of the 1448 studies reviewed, 1311 (90.5%) were presented as posters, and 137 (9.5%) were oral presentations. Overall, 73 publications resulted from these posters/presentations (5%, 95% CI 4–6%), with a higher publication rate for the oral presentations than for the posters (15% vs. 4%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). Of the awarded studies in the congress (15 posters and 5 oral presentations), only 1 poster was published; this factor therefore did not affect the publication rate (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.993). The same year as the congress (2015), 18 studies were published (24.7%), while the rest were distributed among successive years (28.8% in 2016, 24.7% in 2017 and 22% during 2018 and 2019). The journals chosen were in the national (35, 48%) or international (38, 52%) setting and were stratified as Q1 (33%), Q2 (12%), Q3 (38%), Q4 (4%) or were not included in Journal Citation Reports (13%). The journals’ median impact factor was 1.98 (Q25–75, 1.04–3.65). The articles were published in 40 separate journals; the 4 most common were <span class="elsevierStyleSmallCaps">Revista Clínica Española</span> (11, 15%), <span class="elsevierStyleItalic">Medicina Clínica</span> (10, 14%), <span class="elsevierStyleItalic">European Journal of Internal Medicine</span> (6, 8%) and <span class="elsevierStyleItalic">Revista Española de Cardiología</span> (4, 5.5%). All publications were accepted in the Originals section (including 2 meta-analysis). In the multivariate analysis, the factors independently associated with subsequent publication were an oral presentation format (odds ratio [OR], 3.74; 95% CI 1.97–7.1), a prospective study design (OR, 3.57; 95% CI 2.09–6.1), participation by more than 1 center (OR, 2.45; 95% CI 1.46–4.12), diabetes-related subject matter (OR, 5.64; 95% CI 1.12–28.53), participants with multiple diseases (OR, 13.37; 95% CI 3.03–59.04), infection by the human immunodeficiency virus (OR, 9.36; 95% CI 1.57–55.8) and participants with heart failure (OR, 5.94; 95% CI 1.21–29.15).</p><p id="par0020" class="elsevierStylePara elsevierViewall">In this study, we found that only 5% of the posters and presentations accepted in a Spanish internal medicine congress were published during the 4 years after their presentation. The publications were original articles of moderate quality, given that two-thirds of the journals were not encompassed in the first quartile (Q1). Similarly, the journals chosen belonged, in almost a third of the cases, to the general Spanish internal medicine setting (<span class="elsevierStyleSmallCaps">Revista Clínica Española</span> and <span class="elsevierStyleItalic">Medicina Clínica</span>). This low publication rate after a congress is far from that reported in the literature. A meta-analysis using a random effects model, which included 425 studies and a total of 307,028 abstracts from medical congresses, showed a derived publication rate of 37.3% after a follow-up of at least 4 years for most of the studies.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">1</span></a> Similar rates have been reported in the context of Spanish congresses on clinical pharmacology (26%),<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">2</span></a> cardiology (38.3%)<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">3</span></a> and hepatology (52.2%).<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">4</span></a> A number of the factors that predict future publication (multicenter prospective studies that are presented as an oral presentation) were foreseeable and consistent with that described in the medical literature.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">1</span></a> The revealed data require reflection on the causes of this insufficient publication rate. The congress committees should search for a proper balance between rigor in the scientific selection criteria for posters/presentations and the organizational demands. The directors of research groups and of medical services should persuade new residents and researchers of the curricular and scientific importance of publishing versus the merely submitting of oral presentations and posters to congresses.
