Spain, 1996–2015.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">Where a patient dies depends on numerous factors of a clinical, healthcare, social and cultural nature. The location has important repercussions on the individual level, because the circumstances of an individual's death become part of their relatives’ history and that of their close friends and affects the organization of healthcare facilities for their care. It has been reported that most patients prefer to die at home, although in many cases death occurs in a hospital.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1–6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In Spain, the location of death (home, hospital, nursing home or other) is recorded in the statistical bulletins for deaths; however, the official statistics of the Spanish National Institute of Statistics (INE) do not include this datum. We were able to locate only one study based on this source of information, which was limited to one autonomous community. This study placed the hospital mortality rate (HMR) at 61%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> International studies based on surveys<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> and the statements of sentinel physicians,<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,9</span></a> which included Spanish population data, have placed the HMR for the general Spanish population at approximately 50%.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Internationally, the available studies show enormous variability in HMRs, from <20% in Albania and China to >75% in Brazil and Japan.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">6</span></a> Most countries in the European Union have HMRs of approximately 50–60%,<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">2,6,10,11</span></a> with a few exceptions, such as the Netherlands, Switzerland and Norway, with HMRs of approximately 35%.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6,12</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In terms of the variables that determine the location of death for elderly patients, it has been reported that hospital death is less frequent for patients older than 85<span class="elsevierStyleHsp" style=""></span>years with dementia and admitted to nursing homes<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> and in those areas with greater availability of healthcare resources.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> Information regarding the location of death is much more extensive for patients with cancer.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">3,9,14</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Our hypothesis was that the progressive medicalization of society is expressed in a progressive increase in the HMR over the 20-year study period and that the HMR would be higher in provinces with smaller elderly and rural populations, greater hospital bed infrastructure and fewer palliative care and healthcare resources. The study objectives were as follows: (a) to report the change in deaths in the hospital setting for the entire nation and (b) to report the distribution of this variable by province and examine its ecological correlation with demographic and social variables, healthcare and social care resources.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">The study used an ecological design. We obtained information on deaths for all of Spain between 1996 and 2015, using detailed data or <span class="elsevierStyleItalic">microdata</span> from the Natural Population Movement available online at INE<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> and those that occurred in hospitals during the same period through the Specialized Care Information System (SIAE), a survey that all public and private hospitals are required to participate in, whose detailed data are available online from the Ministry of Health, Social Services and Equality.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> The SIAE information by province for 2013 to 2015 was obtained by request from the Ministry of Health.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The INE provides data on deaths by province of residence. In contrast, hospital deaths recorded in the SIAE can include patients not included in the census. Two adjustments to the hospital death figures were therefore performed:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">1.</span><p id="par0040" class="elsevierStylePara elsevierViewall">By the flow of patients to undergo treatment: We crossed the provinces of residence and death of the patients who appear in the detailed data of the Natural Population Movement. In the provinces with more than 5% of deaths outside the province, the difference was assigned as hospital deaths to the bordering province in which they occurred.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">2.</span><p id="par0045" class="elsevierStylePara elsevierViewall">For the effect of deaths of patients not included in the census who, in areas of special tourist or occupational influx, could represent a significant number of hospital deaths: To estimate these deaths, we used the 2013–2015 data of the public hospital located in Costa del Sol, an area of high tourist influx, and we calculated the ratio between deaths of individuals not included in the census and hotel overnight stays of nationals and foreigners in the area.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> This ratio was applied to the overnight stays of Spaniards and foreigners in each province. For the national total, we applied the ratio only to overnight stays of foreigners.</p></li></ul></p><p id="par0050" class="elsevierStylePara elsevierViewall">From the quotient between the estimated number of deaths in hospitals and the total number of deaths, we obtained the HMR at the national and provincial level.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The change in HMR at the national level between 1996 and 2015 was analyzed using the nonparametric runs test. At the provincial level, we studied the relationship between HMR and the following explanatory variables using simple and multiple linear regression: (a) aging: percentage of the population older than 65<span class="elsevierStyleHsp" style=""></span>years<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a>; (b) rural: percentage of the population residing in municipalities of fewer than 10<span class="elsevierStyleHsp" style=""></span>000 inhabitants<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a>; (c) hospital beds installed for 10<span class="elsevierStyleHsp" style=""></span>000 inhabitants<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a>; (d) ratio of nursing home places for 100 individuals 65 years of age or older<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a> and (e) availability of palliative care resources (physicians and nurses for 100<span class="elsevierStyleHsp" style=""></span>000 inhabitants) according to the 2015 Directory of Resources prepared by the Spanish Society of Palliative Care in Spain.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">The descriptive analysis was performed using medians and interquartile ranges. The relationship between the possible explanatory variables and HMR was studied using simple and multiple linear regression by the stepwise forward method. The level of significance was established at <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. The data were treated with Microsoft Excel and Microsoft Access, and the statistical analysis was performed with SPSS for Windows version 19.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">During the 20-year study period, there were 7.5 million deaths in Spain, 4 million of which occurred in hospital centers. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> show the change in HMR at the national level, with an increase from 49.2% in 1996 to 55.9% in 2007 (runs test: <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006). In contrast, the HMR remained fairly stable in the following years, up to 55.9% in 2015 (runs test: <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.70).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">For 2013–2015, 82.6% of the hospital deaths occurred in acute care centers, and 17.4% occurred in other hospital modalities (geriatric, medium-to-long stay and palliative care), 90.3% occurred in hospital wards and 9.7% occurred in emergency departments. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> summarizes the HMR situation by province. The general pattern is that the provinces of the Canary Islands, Barcelona, Madrid and Leon have an HMR of over 60%. At the other extreme, we have the provinces of Segovia, Lugo, Basin and Toledo, with approximately 41%.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">For the HMR adjustment according to the flow of patients, the provinces of Cuenca, Avila, Zamora and Jaen showed high death rates outside the province (17%, 7%, 6% and 6%, respectively). In all cases, there was a bordering province with a significant number of deaths originating from the above-mentioned provinces (Albacete, Madrid, Salamanca and Granada, respectively).</p><p id="par0080" class="elsevierStylePara elsevierViewall">For the HMR adjustment according to the effect of the deaths of individuals not included in the census, we applied the ratio of one death of nonresidents for every 120<span class="elsevierStyleHsp" style=""></span>647 overnight hotel stays to the approximately 200 million overnight stays of foreigners annually, which represents 1641 hospital deaths that were subtracted for the HMR calculation. Furthermore, the 110 million overnight stays annually of Spaniards provided an estimate of 914 deaths, which were redistributed among the provinces of origin and destination. The adjustment by the 2 factors resulted in a small variation in the national total (−0.4%). The same occurred in most provinces, except for 4 with an adjusted HMR clearly higher than the gross: Cuenca, Avila, Zamora and Jaen (+15.5%,<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>6.2%,<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>5.5% and<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>5.2%, respectively) and 2 with a lower HMR: Albacete and the Balearic Islands (−10.1% and −5.4%, respectively).</p><p id="par0085" class="elsevierStylePara elsevierViewall">The simple linear regression analysis showed that the HMR was higher in provinces with smaller elderly populations (correlation coefficient [<span class="elsevierStyleItalic">r</span>], −0.30; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03), smaller rural populations (<span class="elsevierStyleItalic">r</span>, −0.58; <span class="elsevierStyleItalic">P</span><<span class="elsevierStyleHsp" style=""></span>0.0001), greater provisioning of hospital beds (<span class="elsevierStyleItalic">r</span>, 0.36; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.009) and fewer nursing home places (<span class="elsevierStyleItalic">r</span>, −0.31; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace">=</span><span class="elsevierStyleHsp" style=""></span>0.03). As expected, several of these variables showed a high correlation with each other; e.g., aging and rurality (<span class="elsevierStyleItalic">r</span>, 0.62; <span class="elsevierStyleItalic">P</span><<span class="elsevierStyleHsp" style=""></span>0.0001) and aging and the provisioning of nursing home places (<span class="elsevierStyleItalic">r</span>, 0.51; <span class="elsevierStyleItalic">P</span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><p id="par0090" class="elsevierStylePara elsevierViewall">The HMR showed no significant correlation with the provisioning of palliative care human resources. The provinces with greater provisioning (the three provinces of the Basque country and Valencia) were close to the average HMR, and the provinces with lesser provisioning varied significantly: Cuenca and Toledo were among those with the lowest HMR, and Murcia and Tenerife were among those with the highest HMR.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Nevertheless, the SIAE has recorded the deaths in palliative care hospital units since 2010, which have increased from 4.3% of total deaths in 2010 to 6.0% in 2015, varying by more than 10% of the deaths in Catalonia and the Canary Islands and less than 1% in Navarra and Ceuta-Melilla.</p><p id="par0100" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the multiple linear regression analysis between the five explanatory study variables and HMR. The model showed a coefficient of determination (<span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span>) of 0.50 (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001), with a statistically significant positive relationship between HMR and the provisioning of hospital beds (beta coefficient, 0.40; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001) and a negative relationship with the province's rurality (beta, −0.58; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.0001).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0105" class="elsevierStylePara elsevierViewall">Our results show that the HMR in Spain increased from 49% in 1996 to 56% in 2007 and then remained stable to 2015. The variation in HMR among the provinces was from 40% to 70%, with a higher HMR in less rural provinces and those with a greater provisioning of hospital beds.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The place of death is recorded in the statistical bulletins of death, although it is not included among the data that INE displays on its website for access by researchers. In answer to our query, the INE provided this data after a request and agreement, although they specified that in 10% of the death certificates the location of death was not recorded. In the only study we found based on death bulletins, corresponding to Andalusia in 2009, only 49% of the bulletins recorded where the death occurred.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> This fact could represent a bias and confer less validity to the HMR based on death bulletins than that which we were able to estimate in our study.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Compared with other countries in Europe, the HMR observed in Spain is below that reported in the United Kingdom (58%)<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a> and France (60%)<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a> and above that reported in the Netherlands, Switzerland and Norway of approximately 35%.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">6,12</span></a> In terms of the trend in HMRs, the data from the available years showed an increase in the HMR in Spain up to 2007, remaining stable thereafter. A stable trend has been reported in Germany for 1990 to 2002,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> a descending trend in Norway for 1987 to 2011<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> and a descending trend in England and Wales for 2004 to 2014.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">We cannot make any conclusions regarding the causes for the change in HMRs observed since 2007. One possible explanation, due to its coinciding with the start of the financial crisis, is the reduction in hospital beds, which hindered the hospitalization of terminal patients who in previous years would have been hospitalized. Hospital beds in operation decreased at the national level by a total of 7853 (5.4%) in 2015 compared with 2006, the year before the start of the financial crisis.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> We also cannot rule out a development in home palliative care, with an increase in deaths at this level, as has occurred with deaths in hospital palliative care.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Another finding in the study was the wide variability in the location of death among provinces. This variability has already been reported in the literature for patients with similar conditions, which has been considered a reflection of the considerable influence of sociocultural and healthcare factors on the location of death.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> A wide variability has been reported in the United Kingdom among the various primary care areas (between 46% and 77%).<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Interesting conclusions can be extracted from the analysis of the situation in specific provinces. For example, the differential behavior of the province of Leon, which has a fairly rural and aged population but also a high HMR, could be explained by the high availability of beds in medium-to-long stay centers,<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a> in contrast to Zamora, with a 14% lower HMR. Moreover, the three island provinces are among the nine provinces with the highest HMR after adjusting for the effect of tourism. Within this group, the province of Tenerife has a 9% higher HMR than Gran Canaria, which could be attributed to the fact that the former does not have home palliative care.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> Other unique situations include Badajoz, with a 12% greater HMR than Caceres, which has a much higher number of places in nursing homes. In addition, we have the province of Cuenca, where almost a third of hospital deaths of its residents occur in bordering provinces.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In 2006, our group published a similar study; however, it was limited to the national level and autonomous communities.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> In our opinion, the provincial level or even smaller population levels are more appropriate for this type of ecological study, because a single autonomous community can contain heterogeneous populations and healthcare resources within its territory.</p><p id="par0140" class="elsevierStylePara elsevierViewall">In our study, the lowest HMRs were in eminently rural provinces, such as Segovia, Lugo and Cuenca, the 3 provinces where more than 50% of the population lived in municipalities of fewer than 10<span class="elsevierStyleHsp" style=""></span>000 inhabitants.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> The rural nature of an autonomous community could be related to a more traditional mindset and lifestyle that views death at home as more normal<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> or to poorer physical or geographical access to healthcare services.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Other countries have reported significant death rates in nursing homes and care homes (equivalent in Spain to hospitals for the chronically ill and intermediate care centers), including Norway (45%),<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> Switzerland (35%),<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a> England and Wales (21%)<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> and the Netherlands (17%).<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> Significant rates have also been reported for hospices (equivalent to palliative care centers in Spain), including Great Britain (6%)<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a> and the Netherlands (3%).<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> In our study, the collection of these centers represents less than 20% of hospital deaths and less than 10% of total deaths.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Most studies on the variables related to the location of death have focused on patients with cancer<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">3,4,9,14,27</span></a> or on elderly patients.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,13,28</span></a> It has been observed that the probability of dying in a hospital increases the lower the socioeconomic level, the greater the distance from the home to hospital and by the lack of healthcare centers.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> In Spain, there are reports indicating that death at home for patients with cancer was more common among residents in rural areas and among those with higher socioeconomic levels.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">It has also been reported that elderly patients in Spain living in a nursing home have a lower chance of dying in a hospital than those who live at home, which is also true for dementia and more advanced ages.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">7,8,13</span></a> The likelihood of dying in a hospital is also related to the low availability of healthcare resources<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,8</span></a> and to certain diagnoses.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">1,7</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">Intrahospital mortality rates, both raw and risk-adjusted, are often used as an indicator of quality care to assess hospitals and clinical services. Our study showed that Spain, at the provincial level, has considerable heterogeneity in terms of the likelihood of dying in a hospital, which is partly explained by demographic reasons (e.g., rural population) and by the healthcare structure (e.g., hospital bed provisioning), and therefore do not influence the quality of healthcare.</p><p id="par0165" class="elsevierStylePara elsevierViewall">The limitations of this study include those inherent in ecological studies as a result of their methodology,<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> among them the so-called ecological fallacy. It is therefore not possible to ensure that the relationships between the variables observed in our study are reproduced in terms of causality at the individual level.</p><p id="par0170" class="elsevierStylePara elsevierViewall">We know that there is much to learn about the circumstances of a patient's death. Given that this issue is affected by healthcare, societal and cultural factors, the findings of studies conducted in other societies do not have to be extrapolatable to Spain, where research on the matter is still scarce. The aim of this study was to describe the evolution in the hospital mortality rate (HMR) in Spain over the course of 20<span class="elsevierStyleHsp" style=""></span>years and its distribution by province during a more recent period and to explore its relationship with potential explanatory variables.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">This was an ecological study. The population mortality rates were obtained from the Natural Population Movement (<span class="elsevierStyleItalic">Movimiento Natural de la Población</span>), and the hospital mortality rates were obtained from the Specialized Care Information System (<span class="elsevierStyleItalic">Sistema de Información en Atención Especializada</span>), which includes information from all hospitals in Spain. We calculated the mortality rates for patients who were not surveyed and the HMR at the national level between 1996 and 2015 and for provinces between 2013 and 2015. The relationship between the provincial distribution of HMR and various demographic, socioeconomic and healthcare variables were analyzed through simple and multiple linear regressions.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The HMR in Spain increased from 49% in 1996 to 56% in 2007, having remained stable from 1996 to 2015.
