Estudio REINA-SEMI: Registro de Interconsultas y Asistencia Compartida de la Sociedad Española de Medicina Interna" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">In recent years, there has been a significant change in hospital medicine toward polypathological patients, elderly patients and those with a high burden of comorbidity, both in the medical and surgical areas.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a> The complexity of these patients’ care, the progressive superspecialization and the organizational problems that hinder daily patient follow-up by the same physician have contributed to a progressive increase in the participation of internal medicine (IM) departments in the care of surgical patients.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">1–3</span></a> Similar issues are occurring in some medical departments for the same reasons.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The classical interconsultation is the most typical organizational model and consists of requesting another physician to provide an opinion on the diagnosis and/or treatment or to assume direct responsibility for a specific patient problem.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a> In recent years, the experiences of several Spanish IM departments regarding this activity have been published. There are data on the clinical characteristics of the treated patients,<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">6</span></a> clinical management,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">7</span></a> protocolized care for specific healthcare processes,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">8</span></a> satisfaction of medical personnel<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">9</span></a> and, more recently, the experiences of shared care, in which the clinical care provided by the internist in other departments is performed in a regulated manner within the framework of a multidisciplinary team, without needing an interconsultation.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">10,11</span></a> However, the data are from retrospective studies that analyzed the information from individual hospital centers. Moreover, the workload entailed by this task has not been quantified.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aims of this study were to analyze the characteristics of the interconsultation activity conducted by IM departments, establish the profile of clinical care required by the various requesting departments and to quantify the workload entailed by this activity.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Method</span><p id="par0020" class="elsevierStylePara elsevierViewall">A multicenter, observational prospective study was conducted by the Shared Care and Interconsultations Workgroup of the Spanish Society of Internal Medicine. The study consecutively recruited hospitalized patients treated by IM departments through on-demand interconsultations, according to previously established definitions,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a> requested between May 15 and June 15, 2016, with clinical follow-up until the end of the hospitalization. The study was classified as an observational non-postauthorization study by the Spanish Agency for Medicines and Medical Devices and approved by the clinical research ethics committee of Galicia (code 2016-055).</p><p id="par0025" class="elsevierStylePara elsevierViewall">We recorded the clinical, progression and formal characteristics of the interconsultations.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a> The workload was recorded by measuring the time dedicated to the patient's clinical assessment by the internist in multiples of 10<span class="elsevierStyleHsp" style=""></span>min (1<span class="elsevierStyleHsp" style=""></span>work unit<span class="elsevierStyleHsp" style=""></span>[WU]<span class="elsevierStyleHsp" style=""></span>=10<span class="elsevierStyleHsp" style=""></span>min), which included the time needed to request/manage tests, inform relatives, <span class="elsevierStyleItalic">etc.</span></p><p id="par0030" class="elsevierStylePara elsevierViewall">The quantitative variables are expressed as mean and standard deviation, and the qualitative variables are expressed as absolute values and percentages, with their respective 95% confidence intervals. We employed a level of statistical significance of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05. In the univariate analysis, we compared the 2 means with Student's <span class="elsevierStyleItalic">t</span>-test or the Mann–Whitney <span class="elsevierStyleItalic">U</span> test. For the qualitative variables, we employed the chi-squared test; when the cells contained expected values below 5, we used Fisher's exact test.</p><p id="par0035" class="elsevierStylePara elsevierViewall">For the statistical analysis, we employed the statistical package SPSS<span class="elsevierStyleSup">®</span> version 15.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">We recorded 1141 on-demand interconsultations from 39 hospitals, representing the autonomous communities of Asturias, Andalusia, Catalonia, Castilla-La Mancha, Castilla y León, Valencia, Extremadura, Galicia, the Canary Islands, La Rioja, Madrid and the Basque Country, as well as a hospital in the city of Buenos Aires (Argentina). Of these, 488 interconsultations were from hospitals with fewer than 500 beds, and 653 were from hospitals with more than 500. The care organization of the participating hospitals was heterogeneous. The allocation of interconsultations was consecutive for all IM department physicians in 51.1% of the cases, for all physicians with allocation depending on the requesting department in 5.9%, for a single responsible consultant in 9.6% and as a team of consultants in 33.2%. The consultants’ dedication to the interconsultation activity was part-time, compared with other healthcare tasks, in 90.9% of cases. Twenty-two (56.4%) of the participating centers performed additional shared care activity with other departments, in addition to the interconsultations.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The requesting departments are listed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Overall, 927 (81.2%) interconsultations came from surgery departments, and 214 (18.8%) came from medical departments. Some 75.5% of the hospitalizations were urgent, and 24.5% were elective. Of the patients hospitalized in the surgery departments, 702 (75.5%) underwent an operation. In this group, 161 (22.9%) interconsultations were presurgical, and 541 (77%) were postsurgical.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">The mean age was 69.4 years (SD, 16.2), and 584 (51.2%) patients were men. The Charlson index (not adjusted for age) was 2.3 (SD, 2.2); 436 patients (38%) had a score ≥3 points. The most common reasons for the interconsultations were general clinical assessments (313 cases, 27.4%), fever (206, 18.1%), dyspnea (155, 13.6%), electrolyte-metabolic disorder (109, 9.6%), arterial hypertension (72, 6.3%), delirium (60, 5.3%), arrhythmia (28, 2.5%) and others (198, 17.3%). The diagnoses performed by the consultant are listed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The diagnoses of heart failure, ileus, venous thromboembolism disease, anemia, delirium and electrolyte-metabolic disorder were significantly more common in the surgery departments, while stroke and urinary tract infection were more common in the medical departments (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">The duration of the first visit was 4 WUs (SD, 5.9), and the sum of all subsequent visits was 7.3 WUs (SD, 21.5). In 328 (28.7%) interconsultations, more than 30<span class="elsevierStyleHsp" style=""></span>min was required for the initial patient assessment. The mean follow-up was 4.7 days (SD, 6.8); 325 (28.4%) cases were resolved in a single medical act. At the closure of the interconsultations, 52 (4.5%) patients had died, 90 (7.8%) when the end of the hospitalization was considered. Eighty-five patients were transferred to the IM department (7.4%), and 85 (7.4%) were transferred to other departments. An independent report was provided to the originating department in 142 cases (12.4%), and a joint report was performed in 177 cases (15.5%). Outpatient follow-up was scheduled by the IM department in 178 interconsultations (15.6%). The surgical patients were older and required longer follow-up than the medical patients. There were no differences in terms of comorbidity, the time needed for the patient care or mortality between the two groups (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">The formal characteristics of the interconsultations are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>. The medical services more often included clinical data of interest in the interconsultation requests. Of the total, 2 of every 3 interconsultations demanded attention within 24<span class="elsevierStyleHsp" style=""></span>h. The surgery departments requested urgent or preferential interconsultations more often than the medical departments. Considering the patients’ clinical condition and available data, the internists considered the interconsultation time inadequate in 21% of the cases and that the priority established by the applicant was inadequate in 16% of the cases. In both aspects, the consultant's agreement was greater with the interconsultations from the medical departments.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">When dealing with surgical patients, the profile of interconsultations requested of IM departments is that of a patient with moderate to high comorbidity, requiring medical action within 24<span class="elsevierStyleHsp" style=""></span>h during the postoperative period. The reasons for the interconsultation and the clinical diagnoses performed are the internist's responsibility and are frequently not related to the disease that motivated the hospitalization. The workload that these interconsultations represent for the consultant is high.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The patients’ mean age in this study was somewhat lower than that currently observed in IM departments.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">12</span></a> Regarding other similar studies on interconsultations, the data vary depending on the origin of the analyzed series, whether they involve general departments (lower)<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">7,11</span></a> or whether the patients have specific diseases such as hip fractures in which the age increases.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">8</span></a> The reasons for the interconsultations and diagnoses performed by the consultants are very similar to those indicated in other Spanish series<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">6,7</span></a> and do not differ much from those that characterize an IM hospital ward. There are logical differences in terms of the diagnoses depending on whether medical or surgical departments are analyzed. However, in the distribution of other diseases (heart failure, electrolyte disorder, delirium), clinical areas are mentioned in which surgeons can have difficulties managing the diseases and are therefore candidates for attention by an internist.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The work of interconsultations within hospitals has been underestimated for some time. However, it is estimated that 45% of Spanish IM departments have a specialized interconsultation unit.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">13</span></a> These patients’ care represents a workload that has so far not been quantified. Our data indicate that interconsultations require a mean dedication time of 40<span class="elsevierStyleHsp" style=""></span>min for the first visit and a bit more than 70<span class="elsevierStyleHsp" style=""></span>min for the sum of the other visits. These times are very close to those assigned to patients hospitalized in an IM department. Less than a third of the cases are solved in a single medical act, and almost 5<span class="elsevierStyleHsp" style=""></span>days of clinical follow-up are required; the latter finding is consistent with that of another study.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">6</span></a> It is interesting that there were no significant differences between the attention time required for the medical patients and that for the surgical patients, although the latter required more follow-up time.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The percentage of repeated and duplicated interconsultations was less than 8% and 4%, respectively, lower than in previous studies.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">14,15</span></a> The fact that all participating departments had a specialized interconsultation unit that ensured patient follow-up could explain these results. The information provided by the requesting specialist in the interconsultations was insufficient in many cases. This situation has already been noted in other studies<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">15,16</span></a> and could result in a deficient response to the interconsultations, especially if effective verbal contact between the specialists is not achieved. This effective communication was only achieved in little more than half of the interconsultations and is essential for determining the reason for the interconsultation, the degree of autonomy required and the resolution of potential conflicts. Proper communication is one of the most important aspects to consider in resolving an interconsultation.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Approximately a fifth of the interconsultations were not requested at the appropriate time, with a predominance of delayed interconsultations, whose repercussions on the patient's prognosis could be decisive. This problem is especially pronounced in the surgery departments, which could indicate greater difficulty in recognizing various severe clinical conditions<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">14</span></a> and could explain the trend of more frequently requesting urgent or preferential interconsultations.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">15</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">A report was created that recorded the internist's activity in 1 of each 4 interconsultations. The patient's home medication is frequently changed during hospitalization, and these changes are often not communicated properly to the patient at discharge.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">19,20</span></a> On many occasions, the medical consultant's actions are responsible for these changes; however, these actions are often not included in the discharge report,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">15</span></a> a situation that should be considered when resolving the interconsultation.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">18</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">This study had a number of limitations. The study only included on-demand interconsultations, and several of the participating IM departments conduct additional shared-care activities with specific surgery departments, which were not included in this study. Therefore, the data only partially reflect the burden of care that IM departments support outside their department. The study has a selection bias in terms of the characteristics of the sample, with a predominance of interconsultations from trauma departments (among the surgical departments) and psychiatry (among the medical departments). Nevertheless, this distribution reflects the reality of care in the interconsultation activity among Spanish internists.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">15</span></a> Moreover, the assessment of the workload was quantified by the researchers, which introduced a component of subjectivity into the analysis. Similarly, in terms of the formal considerations of the interconsultations, the study provides an overview of the internist consultant and not that of the applicant on issues such as the appropriateness of the interconsultation time and the quality of the response.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In conclusion, the work of hospital interconsultations conducted by internists represents a high workload, focused on tasks that are the specialty's responsibility, which should be considered by the management teams when planning the activity.
