We live longer and survive numerous diseases that have transformed into chronic and progressive conditions, increasing the prevalence of comorbidities, especially in the population older than 65 years.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Polypathological patients (PPs) are a more complex group and have greater clinical vulnerability, frailty, mortality, functional impairment and polypharmacy and poorer health-related quality of life and state of dependence.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">1,2</span></a> They are also an emerging population in most clinical specialties.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">3</span></a> The prevalence of PPs in the general population is estimated at 1–1.5%, and its annual mortality is 6–10%. The prevalence of PPs in the hospital setting is 25–50%, depending on the area, and annual mortality reaches 37%.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">1,4,5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Although a common problem, there is no one universally accepted definition for polypathology or for the set of diseases that define a patient with comorbidities. Currently, the most accepted classification is the one established by Ollero-Baturone et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Individual management is needed to provide efficient care for these patients,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">6</span></a> as well as multidisciplinary and integrated healthcare according to the patients’ needs.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">7</span></a> Establishing a prognosis that is as accurate as possible is important for patients and their family. This prognosis should lead to the appropriate selection of the diagnostic and therapeutic approach, as well as to the correct planning of health care. We currently have numerous instruments that assess the vital prognosis of patients with comorbidity, the most widely used of which is currently the Charlson comorbidity index. This index measures the weight of comorbidity and relates it to survival at 10 years.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">8</span></a> However, most indices have been designed for specific populations, and all have limitations. The recently developed PROFUND index is a specific tool for determining the prognosis of PPs. This index classifies mortality at 12 months and is based on 9 clinical, laboratory and societal and family-related measures.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">9,10</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The aim of this study was to determine the prevalence of PPs in a hospital cardiology unit, their clinical characteristics and their prognosis. We also analyzed the usefulness of the PROFUND index in the prognostic stratification of PPs.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">An observational, retrospective cohort study was conducted that consecutively included all patients hospitalized in the Department of Cardiology during 2012. We excluded the scheduled hospitalizations and readmissions of previously included patients. The study was conducted in a single tertiary hospital, which had a hemodynamics and electrophysiology laboratory and a department of cardiac surgery.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The medical records and hospital databases were reviewed. The demographic characteristics recorded were sex and age. The clinical data included the cardiovascular risk factors, left ventricular ejection fraction (LVEF) measured by transthoracic echocardiography, the defining categories for PPs, the Charlson comorbidity index and the reason for admission. In terms of the characteristics of the admission, we recorded the length of stay and in-hospital mortality. For the PPs, we calculated the PROFUND index with the admission data. The necessary information for the functional, cognitive and social assessment of all patients was taken from the nursing registries and hospital medical records.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The follow-up data for the PPs was obtained in January 2014 through a review of the medical records and hospital databases, registering the overall mortality and major adverse events at 1 year of follow-up.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The study design and methodology were chosen in accordance with the 1975 Declaration of Helsinki and were approved by the hospital's ethics committee.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Definitions</span><p id="par0050" class="elsevierStylePara elsevierViewall">PPs were considered those who, during the hospitalization, had chronic diseases included in 2 or more of the categories predefined by Ollero-Baturone et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">4</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Left ventricular systolic dysfunction, assessed using LVEF, was classified according to the current echocardiography guidelines.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">11</span></a> Preserved LVEF was defined as values >52% in men and >54% in women; slightly depressed LVEF was 41–51% for men and 41–53% for women; moderately depressed LVEF was 30–40%; and intensely depressed LVEF were values <30% for both sexes.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Major adverse events were defined as the combination of all-cause mortality and/or unscheduled hospitals readmission.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The PROFUND index was calculated taking into account the demographic, clinical, laboratory, functional, social, family and healthcare aspects that determine its score (age ≥85 years, 3 points; main caregiver other than spouse, 2 points; active neoplasia, 6 points; dementia, 3 points; dyspnea class III–IV of the NYHA or the MRC scale, 3 points; delirium at admission, 3 points; hemoglobin <10<span class="elsevierStyleHsp" style=""></span>g/dL, 3 points; Barthel index <60, 4 points; ≥4 hospital admissions in the previous 12 months, 3 points), classifying patients in different risk groups depending on its value (low risk, 0–2 points; medium risk, 3–6 points; high risk, 7–10 points; very high risk, ≥11 points).<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">9</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">The qualitative variables are expressed as percentages, and the association contrasts were analyzed using the chi-squared test or, in lieu thereof, Fisher's test. The quantitative variables are expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation. Their distribution was analyzed with the Kolmogorov–Smirnov test, and the differences were analyzed using Student's <span class="elsevierStyleItalic">t</span>-test for the variables that followed a normal distribution and with the Mann–Whitney <span class="elsevierStyleItalic">U</span> test for the variables that did not. The survival analysis was performed with the Kaplan–Meier method, and its multivariate analysis was conducted using Cox regression.</p><p id="par0075" class="elsevierStylePara elsevierViewall">For the analysis of events during the follow-up and to calculate the prognostic value of the PROFUND index, those PPs who died during the first hospitalization were excluded.</p><p id="par0080" class="elsevierStylePara elsevierViewall">To perform the multivariate analysis of the variables associated with overall mortality and major adverse events in the follow-up, we included all variables that had a <span class="elsevierStyleItalic">p</span> value <.1 in the bivariate analysis or had clinical significance.</p><p id="par0085" class="elsevierStylePara elsevierViewall">All results were analyzed using the SPSS statistical program (Statistical Package for the Social Sciences, version 20.0; IBM Corp.; Armonk, New York, USA). The level of statistical significance was established at a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0090" class="elsevierStylePara elsevierViewall">The study included a total of 1033 patients, with a mean age of 67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.1 years, 363 of whom (35.1%) were women. Of the defining categories of PPs, the most prevalent was category A with 921 patients (89.2%), followed by category B with 179 patients (17.3%) and category C with 148 patients (14.3%). The most common causes for admission were ischemic heart disease (628 patients, 60.8%) and heart failure (264 patients, 25.6%). The mean Charlson index during the hospitalization was 4.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.2. The mean hospital stay was 6.8<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.7 days; 18 patients died during hospitalization (1.7%). The set of baseline characteristics is presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Analysis of the subgroup of polypathological patients</span><p id="par0095" class="elsevierStylePara elsevierViewall">Of the total number of patients, 381 (36.9%) met the criteria of PPs. The mean PROFUND index at admission was 2.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.5. One hundred and forty-one patients (37%) were classified as low risk; 215 (56.4%) as intermediate risk; 21 (5.5%) as high risk; and 4 (1%) as very high risk.</p><p id="par0100" class="elsevierStylePara elsevierViewall">When we analyzed the variables for the calculation of the PROFUND index of the PPs, 35 patients (9.2%) were older than 85 years; 6 (1.6%) had a caregiver other than the spouse; 12 (3.1%) had an active neoplasm, 21 (5.5%) had dementia; 130 (34.1%) had functional class III–IV dyspnea; 14 (3.7%) had delirium during hospitalization; 65 (17.1%) had hemoglobin levels <10<span class="elsevierStyleHsp" style=""></span>g/dL during hospitalization; 18 (4.7%) had a Barthel index <60; and 18 (4.7%) had 4 or more hospital admissions over the last year.</p><p id="par0105" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> lists the differential characteristics between the group of PPs and the other patients. The PPs had a significantly older mean age and a greater proportion of men. In terms of the cardiovascular risk factors, these patients had a higher prevalence of hypertension, diabetes mellitus and dyslipidemia. There were more exsmokers and fewer active smokers in this group. The PPs also had a greater prevalence of depressed LVEF and intense systolic dysfunction. In the group of PPs, there was a greater prevalence of chronic diseases recorded in the defining clinical categories of PPs. Within category A, heart failure was significantly more prevalent in the group of PPs, while ischemic heart disease showed no differences. The Charlson score calculated at admission was also significantly higher.</p><p id="par0110" class="elsevierStylePara elsevierViewall">In terms of the reason for admission for the PPs, heart failure predominated over ischemic heart disease, but there were no significant differences for the other cardiovascular diseases (arrhythmias, pericardial diseases, etc.). Mortality and length of stay were significantly greater among the PPs.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Events during the follow-up and prognostic value of the PROFUND index</span><p id="par0115" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the flow diagram for the patients who underwent follow-up. The median follow-up times was 14 months (interquartile range, 11–18). During the 1-year follow-up, 60 patients died (16.2%), and major adverse events occurred in 192 (51.9%) patients, 167 of whom were readmitted to the hospital. The reasons of readmission were heart failure in 85 (23%) patients, ischemic heart disease in 42 (11.4%) and arrhythmias or syncope in 23 (6.2%). Ninety-one patients (24.6%) had more than 1 readmission during the 1-year follow-up; 54 patients (14.6%) had 2 readmissions, 20 (5.4%) had 3 readmissions, and 17 (4.6%) had more than 3.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the differential characteristics between the patients who died and those who survived and between those who had major adverse events and those who did not. The mortality according to the score on the PROFUND index was 8.1%, 20.3% and 45.5% for the low-risk (0–2 points), intermediate-risk (3–6) and high-risk (≥7) subgroups, respectively (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). The major adverse event-free survival in the 12 months of follow-up was different in these groups (63.2%, 37.4% and 15.8%, respectively, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall">The PROFUND index was an independent predictor of overall mortality (hazard ratio [HR], 1.13; 95% CI 1.01–1.27; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.034) and major adverse events in the 1-year follow-up (HR, 1.13; 95% CI 1.03–1.23; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.008). The variables associated with overall mortality and major adverse events at 1 year of follow-up are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">This study shows that the high prevalence of PPs hospitalized in a hospital cardiology unit (36.9%) is similar to that of other hospital units of internal medicine and geriatric medicine.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">2,3</span></a> The progressive aging of the population, the increasingly high prevalence of chronic diseases and the developments in diagnostic and therapeutic techniques have contributed to the increasingly common finding of this type of patient in hospital cardiology wards. These types of patients result in a greater healthcare overload and the need for more complex care, for which medical personnel are not always sufficiently prepared.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In our setting, comorbidities are very common and directly affect the prognosis and quality of life, especially for patients with heart failure, in which the comorbidities are closely related to acute decompensation.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">12–14</span></a> It is therefore essential that we identify and treat these comorbidities before starting the healthcare process.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">15</span></a> The typical polypharmacy of PPs and heart disease increases the risk of interactions and adverse effects, which can sometimes be more disabling than the heart failure itself.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">16,17</span></a> According to recent studies, however, polypharmacy does not affect survival.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">18</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">In our experience, the PROFUND index predicted greater mortality and a higher rate of adverse events during the follow-up, behaving as an independent predictor in the multivariate analysis. The mortality at 1 year in this series was very similar to that calculated according to the risk groups derived from the PROFUND index score. Nevertheless, we grouped the high-risk and very high-risk patients into a single group due to their low prevalence. This prognostic index helps classify patients with comorbidities into groups according to the estimated risk of mortality. The index can therefore be especially useful from a clinical, epidemiological and healthcare planning standpoint.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">9</span></a> The identification of low-risk patients can help speed and intensify the healthcare actions that improve their prognosis. Identifying those at high risk can be useful for rapidly establishing the indication for palliative care. In any case, the index can be a useful tool for making difficult decisions that might diminish hope or quality of life, such as disconnecting an implanted defibrillator and performing invasive diagnostic or therapeutic procedures such as cardiac catheterization and heart surgery.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The PALIAR index can be useful for patients classified by the PROFUND index at very high risk. The PALIAR index is a tool that can complement the PROFUND index in complex clinical decision-making processes in highly advanced stages of the disease.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">19</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">We are faced with a population group that is poorly studied in clinical trials performed in the setting of cardiovascular diseases. We therefore believe that it is important to include these patients in research projects to minimize the significant selection bias present in current clinical trials. Since this population has considerable frailty, functional dependence and associated comorbidities, an individual and multidisciplinary approach that intensifies strategies for controlling cardiovascular risk and chronic diseases will have a positive impact on health outcomes.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">20,21</span></a> Future studies in this field can help design a policy of investments and resources for managing this population that is increasingly prevalent in the various hospital units.</p><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Study limitations</span><p id="par0155" class="elsevierStylePara elsevierViewall">Given that this is a retrospective study, biases could be present due to the difficulty in collecting data and to the quality of the information. Moreover, we only included patients hospitalized in the department of cardiology. There is therefore no data on those patients who were admitted for cardiovascular disorders and who died in the emergency department or intensive care units. Lastly, the self-sufficiency and self-care scales and the assessment of the psychosocial situation were not reflected in the medical history of all patients, although data could be obtained on the social, cognitive and functional assessment (Barthel index) of all patients through the registries conducted by nursing during the hospitalization.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusions</span><p id="par0160" class="elsevierStylePara elsevierViewall">A high percentage of patients who are admitted to a hospital cardiology unit have comorbidities. These patients have a higher prevalence of cardiovascular risk factors, longer stays and greater hospital mortality.
