Specifically, spironolactone and eplerenone have shown a clear benefit in terms of the morbidity and mortality of these patients.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">1–3</span></a> European and American guidelines have therefore recommended its use for all patients with symptomatic HF (New York Heart Association [NYHA] functional class II or higher).<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Several studies have shown that treatment with an aldosterone inhibitor can reduce myocardial fibrosis and oxidative stress and improve the echocardiographic parameters related to diastolic myocardial function.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">6–9</span></a> Similarly, spironolactone is the first drug that has been shown to improve diastolic function and left ventricular remodeling in patients with <span class="elsevierStyleItalic">heart failure with preserved ejection fraction (HFPEF)</span>.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> However, evidence on the benefit of this therapy in HFPEF in terms of morbidity and mortality is scarce and controversial.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">The aim of this study was to analyze the prescription of spironolactone and its prognostic implication over the course of a year of follow-up in a cohort of patients with HFPEF from departments of internal medicine.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patients and methods</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Patient selection</span><p id="par0045" class="elsevierStylePara elsevierViewall">Patient data were collected between March 9, 2008 and January 31, 2013 from the National Heart Failure Registry (<span class="elsevierStyleItalic">Registro Nacional de Insuficiencia Cardiaca</span>, RICA)<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">12–14</span></a> coordinated by the Heart Failure Workgroup of the Spanish Society of Internal Medicine (<span class="elsevierStyleItalic">Grupo de Trabajo de Insuficiencia Cardiaca de la Sociedad Española de Medicina Interna</span>). This is a multicenter prospective cohort study that includes data from 52 public and private hospitals in Spain that voluntarily joined the study. The study protocol was approved by the Ethics Committee of University Hospital Reina Sofia of Cordoba, and informed consent was obtained from all patients before their enrollment in the study, in accordance with the requirements of each hospital's ethics committee. The study included all patients hospitalized in internal medicine units with decompensated HFPEF (considered to be an ejection fraction >50%) and who were treated by physicians who participated in the registry. In addition to providing their informed consent, patients had to meet the following criteria: age over 50 years and hospitalization for decompensated HF according to the criteria of the European Society of Cardiology.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Exclusion criteria were HF due to pulmonary hypertension, refusal to participate in the study and death during the hospitalization in question.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The patients were included in the registry upon admission for HF. The follow-up consisted of a minimum of 2 scheduled visits at 3 and at 12 months. Additional visits were allowed, provided they were necessary due to the patient's clinical condition. The assessment variables were hospitalizations and all-cause mortality, as well as the combination of both events, which were recorded in the outpatient visits during the year of follow-up. The data were recorded in the registry website (<a href="http://www.registrorica.org/">http://www.registrorica.org</a>) that contains the database, which required a password to access it. Confidentiality was ensured given that no personnel data was stored, except the date of birth and the patient's initials to prevent data duplication. The current study included data from those patients who completed a year of follow-up and from those who died or were readmitted during that period. Central monitoring of the data was conducted to ensure the quality of the data.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Study variables</span><p id="par0060" class="elsevierStylePara elsevierViewall">The registry included sociodemographic data, medical history, comorbidities (Charlson index),<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a> the status of basic activities of daily life (Barthel index)<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> prior to the decompensation that prompted the hospitalization (range, 0–100), clinical data (blood pressure, heart rate, weight and height), complications during hospitalization and prescribed treatment at discharge. In terms of HF severity, the functional class prior to the index hospitalization (New York Heart Association scale [NYHA]) was assessed. The left ventricle ejection fraction (LVEF) was determined using echocardiography and was considered preserved when it was ≥50%. The cardiac index was determined by chest radiography.</p><p id="par0065" class="elsevierStylePara elsevierViewall">In the first laboratory readings on admission, biochemical variables were collected that included lipid and glycemic profiles, renal function and levels of uric acid, troponin, ions, hemoglobin and natriuretic peptides, when available. The estimated glomerular filtration rate was calculated using the modification of diet in renal disease equation, defining renal dysfunction as an estimated glomerular filtration rate <60<span class="elsevierStyleHsp" style=""></span>mL/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span>. The patients’ discharge medication was recorded, which included angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers (ARB), beta-blockers, diuretics, digoxin, statins and antivitamin K drugs. The patients were divided into 2 groups: patients for whom spironolactone was included in the discharge treatment and patients for whom this drug was not included in the discharge treatment. Mortality and readmission rates at 1 year of follow-up from the moment of discharge were recorded.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">A descriptive analysis of the sample was conducted using mean and standard deviation for the quantitative variables and percentages for the categorical variables. The patients were divided in 2 groups: patients on treatment with spironolactone and patients with no spironolactone treatment at hospital discharge. The baseline characteristics of the 2 groups were compared using the ANOVA and chi-squared tests for the quantitative and categorical variables, respectively. We analyzed mortality, readmissions and the combination of mortality/readmissions at 1 year of follow-up in the 2 patient groups. Curves for cumulative events were created using the Kaplan–Meier method to observe the prognostic differences between the 2 groups. A Cox multivariate regression analysis of the proportional risks was performed using the conditional backward method for the variables that, in the univariate analysis, demonstrated a statistically significant relationship with the probability of readmission or death.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Statistical significance was established at <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05. The data analysis was performed using the statistical software IBM SPSS version 21.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Sample of patients analyzed</span><p id="par0080" class="elsevierStylePara elsevierViewall">The study cohort included 1212 patients with HFPEF from a total of 2051 patients (59%) who were discharged after being hospitalized for decompensation due to HF and who, after the discharge, either died or had a follow-up of at least 1 year. Our patients’ mean age was 79 years (standard deviation, 7.9), there was a predominance of women (64.1%) and the predominant HF etiology was hypertensive heart disease (50.7%). The baseline characteristics and differences according to having undergone treatment (26.7%) or not (73.3%) with spironolactone at discharge are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Patient characteristics at the time of hospital discharge</span><p id="par0085" class="elsevierStylePara elsevierViewall">There were no differences between the 2 groups in terms of the mean age and sex. The prevalence of atrial fibrillation was significantly higher in the spironolactone group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). Other more common parameters in this group were diabetes, arterial hypertension and dyslipidemia, although they did not reach statistical significance. Likewise, there was a nonsignificant tendency among the patients who took spironolactone to present lower hemoglobin readings and higher glomerular filtration rates (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">With regard to the HF symptoms, the patients treated with spironolactone had a clearly more advanced functional class (50% in NYHA functional class III–IV vs. 33% of the patients who did not take spironolactone; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p><p id="par0095" class="elsevierStylePara elsevierViewall">There were also no differences between the 2 groups in terms of concomitant cardiovascular medication, except in the prescription of loop diuretics, digoxin and antivitamin K, which was higher in the spironolactone group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Prescription of spironolactone and rate of events during the follow-up</span><p id="par0100" class="elsevierStylePara elsevierViewall">The patients who underwent treatment with aldosterone antagonists had a higher readmission rate at 1 year (44.3% vs. 29.1%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) and a higher combined objective of readmissions/death (39.0% vs. 29.0%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001) (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>). A poorer result for the 1-year mortality variable was achieved by treated patients (19.5% vs. 17.5%), without achieving statistically significant differences (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>.05). Moreover, the serum potassium concentrations were statistically higher at the 3, 6 and 12-month follow-ups of the patients treated with spironolactone (4.56 vs. 4.43; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.002; 4.61 vs. 4.44; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.019; and 4.60 vs. 4.50; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.021).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Multivariate analysis of mortality predictors</span><p id="par0105" class="elsevierStylePara elsevierViewall">The results of the multivariate analysis show a nonsignificant tendency between the administration of spironolactone and an increase in the combined variable of mortality and hospital readmissions (RR, 1.9; 95% CI 0.9–1.4; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.07). In addition to the use of spironolactone, other factors that were associated with an increase in the combined event were the male sex (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.015), a body mass index <30 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), reduced glomerular filtration rate (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), a more advanced NYHA functional class (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), hyperlipidemia (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.036), left ventricular hypertrophy (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.027) and treatment with antivitamin K drugs (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.047).</p><p id="par0110" class="elsevierStylePara elsevierViewall">With regard to readmissions, the multivariate analysis showed that, after correcting for age, body mass index, renal function, use of diuretics, NYHA functional class and hyperlipidemia, the relationship between spironolactone and readmissions continued to show statistical significance. Thus, spironolactone was significantly associated with a 40% greater risk of readmissions, with a 95% confidence (RR, 1.4; 95% CI 1.16–1.78; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">The results reported in other patient cohorts with similar characteristics have indicated that the prescription of aldosterone inhibitors was not associated with increased mortality or an increased rate of readmissions.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> In contrast, the results of the present analysis of elderly patients with HFPEF showed that the use of spironolactone is associated with an increase in all-cause readmissions and a significant increase in the combined objective of readmissions and mortality. However, the use of spironolactone did not influence total mortality.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Aldosterone, a mineralocorticoid agonist, has fibrotic and hypertrophic activity associated with a poorer cardiovascular prognosis.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">19</span></a> Aldosterone inhibitors have been shown to improve mortality and readmissions of patients with HFPEF.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">1–3</span></a> However, in this study, the drugs showed no improvement in the prognosis of patients with HFPEF. These conflicting effects are similar to those that occurred with other drugs such as ACEIs and ARBs, which also showed no benefits for patients with HFPEF.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">20–22</span></a> These differences could be due to the higher comorbidity of patients with HFPEF and to differences in the causes of mortality between the 2 groups, with cardiovascular disease and heart failure the most common among patients with HFPEF.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">The results of the TOPCAT (Treatment of Preserved Cardiac Function Heart Failure with an Aldosterone Antagonist) clinical trial have recently been published. The trial had a mean follow-up of 3.3 years and found no statistically significant differences in the main combined outcome of cardiovascular death, cardiopulmonary arrest (recovered) and hospitalizations for HF (18.6% vs. 20.4%; HR, 0.89; 95% CI 0.77–1.04; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.138). However, there was a significant reduction in hospitalization for HF in the patient group with spironolactone (12% vs. 14.2%; HR, 0.83; 95% CI 0.69–0.99; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.04).<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">24</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Our cohort is a representative sample of patients with HFPEF in real life (elderly, predominant hypertensive etiology, pronounced comorbidity and a higher percentage of women) and has obvious differences when compared to the TOPCAT study. Specifically, patients in the RICA registry are older (10 year older) and have greater comorbidity than those included in the TOPCAT study.</p><p id="par0135" class="elsevierStylePara elsevierViewall">A possible explanation for the increase in readmissions of patients treated with spironolactone in the RICA registry could be the onset of hyperpotassemia during the follow-up, as has been confirmed in other studies.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">1–3</span></a> Unfortunately, the RICA registry did not specifically record hyperpotassemia as a cause of readmission. Moreover, it should be noted that a significant worsening of renal function was not observed in the patients treated with spironolactone.</p><p id="par0140" class="elsevierStylePara elsevierViewall">We should also note that the present study was conducted with a large group of patients who represent the reality of hospitalized patients, with none of the selection or exclusion biases that can occur in clinical trials. Among the study limitations, we should mention that we selected patients on treatment with spironolactone and were unaware of patients on treatment with eplerenone. The present study cannot therefore be generalized to all aldosterone inhibitors. Additionally, we did differentiate dosages and only included patients who at discharge were taking spironolactone without specifying whether the patients were taking this drug before or during hospitalization or whether the drug's discontinuation, initiation or change in dosage occurred during the follow-up. We did not differentiate the causes for mortality or readmissions. We can therefore not select among causes of origin cardiovascular, other causes or by hyperpotassemia secondary to the use of spironolactone. Ultimately, it should be noted that this was an observational study in which the use of spironolactone was not controlled.
