To this end, we assessed their behavior in various types of PEs to understand whether they provided some type of relevant diagnostic clinical information.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">We conducted an observational, cross-sectional retrospective study analyzing the demographic, clinical and analytical characteristics of all adult patients who, between January 2004 and December 2014, underwent a diagnostic thoracentesis at our center due to presenting PE. All data were collected according to the ethical principles of human research, and the study was approved by our hospital's ethics committee (registration 2016/293).</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Pleural fluid analysis</span><p id="par0020" class="elsevierStylePara elsevierViewall">The PEs were studied according to our center's standard protocol. The samples for the PF cell counts were collected using DB Vacutainer<span class="elsevierStyleSup">®</span> vacuum systems with plastic tubes that contained ethylene diamine tetra-acetic acid (EDTA) as an anticoagulant. The total nucleated cell count in the PF was performed within the first 4<span class="elsevierStyleHsp" style=""></span>h after the thoracentesis, using a Hematology ADV 2120 analyzer (Siemens Healthcare Diagnostic S.L.). For the differential nucleated cell count, we employed conventional optical microscopy after cytocentrifugation (450<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic">g</span> for 10<span class="elsevierStyleHsp" style=""></span>min) and May-Grünwald-Giemsa staining of the PF. The count percentage was obtained from 200 cells.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The PEs with 50% or more neutrophils or lymphocytes were considered predominantly neutrophilic or lymphocytic, respectively, and those with an eosinophil count ≥10% were considered eosinophilic. We also assessed whether a PE had >5% of mesothelial cells. A PE might not have cellular predominance if none of the populations achieved the established cutoffs or, in contrast, if several populations achieved the cutoffs simultaneously. If a patient underwent more than one thoracentesis, only the results of the first thoracentesis were considered.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Diagnostic criteria</span><p id="par0030" class="elsevierStylePara elsevierViewall">The diagnosis of malignant PE (MPE) was established when the cytology or pleural biopsy were positive for malignancy. The PE was diagnosed as parapneumonic (PPE) if there was an associated bacterial pneumonia, lung abscess or infected bronchiectasis.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a> We considered whether the PPE was simple or uncomplicated, complicated (pH <7.20 or loculated PE in an ultrasound or chest CT scan), or empyema (pus in the PF or positive PF bacterial cultures). The PF was considered tuberculous if demonstrated by a Ziehl-Neelsen staining or a culture positive for <span class="elsevierStyleItalic">Mycobacterium tuberculosis</span> in sputum, PF or pleural biopsy or if granulomas were observed in the biopsy. The diagnosis of heart failure was based on the Framingham clinical criteria, the response to diuretic treatment and, if available, the echocardiographic or diagnostic evidence of systolic dysfunction.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> In cases of suspected heart failure, only those patients with asymmetric PE or those with chest pain or fever were punctured.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> The other diagnoses were established according to previously defined criteria.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">9</span></a> According to the etiology, the PEs were categorized into 6 groups: heart failure, other transudates, parapneumonic, tuberculous, neoplastic and other exudates.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">After confirmation using the D’Agostino-Pearson test for the lack of a normal distribution, the cell data were expressed as medians and 5–95% percentiles. For the comparison between the groups, we employed Pearson's chi-squared test if the endpoints were qualitative and the nonparametric Mann–Whitney <span class="elsevierStyleItalic">U</span> test if they were quantitative.</p><p id="par0040" class="elsevierStylePara elsevierViewall">To assess the effectiveness of the cell populations in the diagnosis of the various PE groups, we employed the analysis of the ROC curves and their area under the curve (AUC).<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> The cutoff for each parameter was identified through Youden's index. The statistical analysis was conducted with MedCalc<span class="elsevierStyleSup">®</span> software (version 16.8).</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">During the study period, 2102 thoracenteses were performed. We excluded 201 duplicates, 257 for incomplete data, 130 for lacking a definitive diagnosis and 47 for a possible double diagnosis. A total of 1467 patients were ultimately included. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the etiology of the study PEs with their cellular predominance.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">The PEs of neutrophilic predominance were more common in systemic lupus erythematosus (57.1%), pneumonia (57%) and hemothorax (54.5%). In the PPEs, the neutrophilic predominance was 35.1% in the uncomplicated PPEs but greater in the complicated PPEs (77.7%) and empyemas (87.1%). Eighty-eight percent of the tuberculous PEs had lymphocytic predominance, as with almost 50% of the MPEs (62.5% of the mesotheliomas and 88% of the hematological malignancies), 44% of the heart failures and 29.2% of the uncomplicated PPEs. Some 32.5% (25/77) of the eosinophilic PEs were of neoplastic origin, which represented 6.3% of all the MPEs. Some 27.3% (6/22) of the post-trauma PEs also had eosinophilic predominance. In contrast, none of tuberculous PEs had eosinophilic predominance. The etiologies that most frequently presented with a PE that contained more than 5% of mesothelial cells were heart failure (69, 19.5% of mesothelial cells), pleural metastases (56, 16.9%), uncomplicated PPE (34, 18.4%) and tuberculous PE (16, 12%). Only one of the 164 complicated/empyema PPEs showed a mesothelial cell count greater than 5%. Some 8.1% (119) of the PEs had several simultaneous cellular predominances, while 24.3% (356) had none.</p><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the patient characteristics according to the established etiological group, as well as the median and 5–95% percentiles of the total count and percentage of nucleated cells. The patients with a tuberculous PE were significantly younger than the others, and those with heart failure were significantly older. The time elapsed from symptom onset to the performance of the thoracentesis was significantly greater in the neoplastic PEs. A small but significant correlation was found between this period and the percentage of neutrophils (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.1521; 95% CI −0.2047 to −0.09855; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.0001) and lymphocytes (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.1613, 95% CI 0.1080–0.2138; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.0001). The largest and smallest number of nucleated cells was observed in the PPEs and transudates, respectively. Some 86.9% (80/92) of the PEs with more than 10,000<span class="elsevierStyleHsp" style=""></span>cells/μL were PPEs. The percentage of neutrophils was also significantly greater in the PPEs, as was the percentage of lymphocytes in the tuberculous PEs and the percentage of macrophages in the transudates (cardiac or noncardiac). <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the percentages of neutrophils and lymphocytes for each patient, as well as the median of these cell populations, according to the etiology of the PE. We found significant differences between the percentages of neutrophils and lymphocytes in the heart failure, parapneumonic, tuberculosis, malignancies (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001 in all of them) and other transudate (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0036) groups but not in the other exudates group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0582).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the etiologies of the 202 PEs with a percentage lymphocyte count ≥80%. The most common causes were tuberculosis (71, 35.1%) and malignancies (47, 23.3%). Some 53.4% of the tuberculous PEs exceeded this cutoff compared with 11.8% of the malignant PEs.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the discriminating power of the cell parameter that showed greater effectiveness in the diagnosis of each of the etiology groups, according to the areas under the curve for each of the groups compared with the others. The percentage of neutrophils showed a high discriminating value for the diagnosis of complicated parapneumonic pleural effusions and empyemas, while a lymphocyte percentage ≥80% was highly specific for tuberculous PE.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Although this study shows that the total and percentage counts of nucleated cells in PF are rarely diagnostic, the cell patterns observed in the various diseases can provide useful information for differentiating the various causes of PE (AUC that could mostly be interpreted as having moderate [0.70–0.80] to good [0.80–0.90] discriminative value, although with a non-negligible overlapping of values). The most relevant aspect of this study, however, is that the results do not support some of the assertions supported in the literature in recent years: the absence of mesothelial cells in tuberculous PEs,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a> predominantly neutrophilic in all PPEs<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a> and the ability to rule out neoplasia (except lymphoma) if the percentage lymphocyte count is ≥80%.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">12,13</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The PF samples for determining the cell populations can be collected in tubes of different compositions and with different anticoagulants. The samples were collected in plastic tubes containing EDTA because EDTA is the anticoagulant that best preserves PF cell integrity for its counting.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">The extensive overlap of the total number of nucleated cells between the various etiologies means that, except for PPEs, the cell counts are not useful for differentiating one PE from another. The predominant cell population in a PE is determined not only by the type of pleural injury but also by the time between the onset of the acute pleural damage and the thoracentesis. The acute response to a pleural aggression (generally due to infectious, inflammatory or immunologic diseases) attracts neutrophils to the pleural space. In these circumstances, the predominant cell should be neutrophils. Most of the 327 neutrophilic PEs of this series corresponded to PPEs (199 patients) and to acute diseases such as pancreatitis, pulmonary embolism, abdominal infection, pleuropericarditis and hemothorax, as well as to the postoperative and post-trauma periods. The neutrophil percentage is the best at discriminating the PPEs from the others; however, it can be more useful for the diagnosis of empyema (cutoff >64%; AUC, 0.906) and complicated PPEs/empyemas (cutoff >41%; AUC, 0.907). Nevertheless, 38 (9.6%) MPEs also had this cellular predominance, a finding already observed in a previous study.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a> Therefore, a neutrophilic PE does not rule out a neoplasm. It is possible to consider that the mean time between symptom onset and the thoracentesis was shorter in the patients who presented an MPE of neutrophilic predominance. Although this was the case (29 vs. 42.5 days), the difference was not statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.08). It is possible that the small number of cases in the sample (38) was decisive in not reaching this significance.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Once the acute pleural damage has ceased, the mononuclear cells enter the pleural space from the blood and become the predominant population.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a> Thus, the predominance will be lymphocytic in diseases of insidious onset and with a greater evolution time (heart failure,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a> tuberculosis [except in early phases],<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> malignancies, sarcoidosis,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> etc.). However, 29.2% of the uncomplicated PPEs also had a lymphocytic PE. Two factors could explain these results: the time to the thoracentesis and the prior use of antibiotics. With the first factor, we found no significant differences (9.6 days in the PPEs of neutrophil predominance and 11.1 in those of lymphocytic predominance; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.43). As to the second factor, a small study was conducted in which 25 (43.9%) of the patients who underwent a differential cell count had a lymphocytic PPE. The authors showed through a multivariate analysis that prior antibiotic treatment was the only independent predictor of this cellular predominance.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">19</span></a> We were not able to verify this finding because our study was not designed with this objective.</p><p id="par0090" class="elsevierStylePara elsevierViewall">It has been reported that when the percentage lymphocyte count is ≥80%, the differential diagnosis is reduced to a few diseases. The most common diseases were tuberculosis, lymphomas and chylothorax; however, metastatic PEs are not included,<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8,12</span></a> whose typical lymphocyte range would be 50%–75%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a> In our series, 17.6% of the PEs with a lymphocyte count ≥80% were metastatic or mesothelial (35/199; 10% of all the metastatic/mesothelial PEs), which seems to indicate that reaching this percentage does not rule out an MPE. Regardless, when faced with this high lymphocyte count, the most probable diagnosis is tuberculosis (90.6% specificity).</p><p id="par0095" class="elsevierStylePara elsevierViewall">The number of eosinophilic PEs (77/1467, 5.2%) was slightly lower than that published in other series (8.3%–12%).<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">20–22</span></a> However, the percentage of patients with an MPE of eosinophilic predominance (25/77, 32.5%) was similar to those same studies (range, 24.4–34.8%).<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">20–22</span></a> In contrast to the classically stated position,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a> these results confirm that an eosinophilic PE cannot be considered an indicator of benignity.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Mesothelial cells can be seen in the PF, but it is rare to find them in diseases in which a marked inflammatory response covers the entire pleura, because it prevents its exfoliation to the pleural space. For this reason, a mesothelial cell count >5% has always been considered an indication of the improbability of a diagnosis of complicated PPE/empyema or tuberculosis.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a> Our results seem to confirm the first statement (only 1 patient in 164 had a higher count) but not the second because 16 (12%) of 133 patients with a tuberculous PE had >5% of mesothelial cells. This finding has been seldom documented.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">24</span></a> Therefore, an increase in mesothelial cells in the PF appears not to rule out a diagnosis of tuberculosis.</p><p id="par0105" class="elsevierStylePara elsevierViewall">No diagnostic value is attributed to the PF macrophage count, which originate from blood monocytes.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">15</span></a> In our series, the higher values were found in the transudates, and the lower values were found in the infectious PEs. However, these results are difficult to interpret because there are no studies with which to establish a comparison.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The main study limitation is its retrospective nature, which meant that certain data could not be obtained, such as the number of patients with a PPE previously treated with antibiotics. The study was also conducted in a single center, and therefore the results cannot be generalized. Lastly, due to the study's long inclusion period, the cell count was not always performed by the same individual (although most cases were performed by the same person [E.S.J.]), which could have caused certain variability in the results.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In summary, the total and percentage nucleated cell counts in the PF can help reduce the differential diagnosis in the study of a PE, because each etiology usually has a predominant cell characteristic.
