The incidence of VTE in Spain is approximately 120 cases per 100,000 inhabitants/year.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">1</span></a> A consistent increase has been documented in the number of cases due to the progressive increase in prevalence of a number of its risk factors, such as advanced age, obesity, hospitalization and surgery, which likely carry an increase in morbidity and mortality, in the number of hospitalizations and in healthcare costs.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">1–3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Hospitalization is a recognized risk and prognostic factor for VTE. There are studies that have shown that the risk attributed to hospitalization not only occurs during hospitalization but persists for up to 90 days after hospital discharge.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">4</span></a> Moreover, hospital VTE (i.e., that which develops during hospitalization due to another cause) is associated with a higher rate of fatal pulmonary embolism (PE), overall mortality and severe hemorrhage when compared with VTE that develops in an outpatient setting.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">5</span></a> There have also been reports that the rate of fatal PE is higher in patients hospitalized for medical disease compared with those hospitalized for surgical disease.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">6</span></a> This poorer prognosis of hospital VTE and of medical vs. surgical hospitalization could be explained by the greater degree of comorbidity and the poorer clinical situation of the hospitalized patients.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">In terms of outpatient VTE, it is not currently known whether patients who develop VTE after a recent hospitalization have a poorer associated short-term prognosis or whether there are differences depending on the type of hospitalization (medical [MH] or surgical [SH]). Therefore, the main objective of this study was to determine whether the presence and type of hospitalization, MH or SH, in the previous 3 months are factors associated with mortality at 30 days for patients hospitalized in the emergency department due to an acute symptomatic PE episode.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0020" class="elsevierStylePara elsevierViewall">An observational, retrospective cohort study was conducted that included all patients over a 6-year period who were admitted to the emergency department of Clinic Hospital San Carlos of Madrid with a diagnosis of acute symptomatic PE.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patient selection</span><p id="par0025" class="elsevierStylePara elsevierViewall">An online search was performed in an administrative electronic database of all patients aged 15 years or older with a hospitalization diagnosis of PE during the study period. The diagnosis of PE was based on the results of computed angiotomography or pulmonary ventilation/perfusion scintigraphy.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Study endpoints</span><p id="par0030" class="elsevierStylePara elsevierViewall">The data were collected in a structured form based on the patient's emergency department medical history and hospital discharge report. The independent variables were the demographic data (age and sex), medical history (arterial hypertension, diabetes mellitus, smoking habit, heart failure, ischemic heart disease, cerebrovascular disease, chronic obstructive pulmonary disease, chronic renal failure, dementia and active cancer), data regarding the hospitalization in the last 3 months (the time from the previous hospitalization to the PE episode, the type of hospitalization [medical or surgical] and the VTE risk factors), data on the acute PE episode (heart rate, respiratory rate, systolic blood pressure, oxygen saturation and score on the simplified pulmonary embolism severity index [PESIs] prognostic risk scale)<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">7</span></a> and the length of the hospital stay. The PESIs variable was calculated based on data recorded in the medical history, such that when a datum was not available, it was not scored on the risk scale.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The primary endpoint was mortality at 30 days from the date of care in the emergency department. Short-term patient follow-up was conducted using the hospital's computerized medical history and/or through telephone contact to record mortality at 30 days.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">The qualitative variables are shown with their frequency distributions. The quantitative variables are listed as mean and standard deviation, and the variables that did not still follow a normal distribution are expressed with median and interquartile range. For the analysis, the sample was divided into 3 groups, according to whether the patient had a history of a previous hospitalization and, if so, whether this was an MH or SH. For the univariate analysis, we employed the chi-squared or Fisher's exact test for the qualitative variables, in the event that more than 25% of the expected frequencies were less than 5. We employed an analysis of variance for the quantitative variables if the distribution was normal and the nonparametric test of the median for violations of the principle of normality according to the Kolmogorov–Smirnov test.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In order to determine whether there was a relationship between the type of prior hospitalization and mortality at 30 days, we conducted a logistic regression analysis that included all variables where <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.10 in the univariate model. For this model, we calculated the odds ratio (OR) with its 95% CI. The differences were accepted as statistically significant if <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 or when the 95% CI of the OR excluded the value of 1. Data processing and analysis were performed using the SPSS 19.0 statistical suite.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">A total of 870 cases of acute symptomatic outpatient PEs during the study period were identified. One hundred thirty-five (15.5%) patients had been hospitalized in the previous 3 months, 92 (10.6%) of whom were for an MH and 43 (4.9%) for a SH. The patients’ mean age was 72.7 (standard deviation, 15.5) years, 506 (58.2%) of whom were female. One hundred twelve (12.9%) patients died in the first 30 days after the index event.</p><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the patient profile and data on the PE episode according to the presence and type of previous hospitalization. The analysis found statistically significant differences between the 3 groups regarding comorbidity, severity of the acute episode and mortality at 30 days. In the prior MH group, we recorded a higher rate of high-risk PESIs scores (≥1) (90.2%, 65.1% and 64.2% for MH, SH and no prior hospitalization, respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) and mortality at 30 days (20.7%, 7% and 12.2% for MH, SH and no prior hospitalization, respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.038).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the univariate and multivariate analysis of the factors associated with mortality at 30 days. In terms of the presence and type of previous hospitalization and taking as reference having not undergone a hospitalization, a history of MH was associated in a statistically significant manner with mortality at 30 days (OR, 1.87; 95% CI 1.08–3.24; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.018), which was not the case for SH (OR, 0.54; 95% CI 0.16–1.77; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.236). We conducted a logistic regression analysis that included the variables that differed among the groups in the univariate analysis with a <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.10 and eliminated those that were included in the PESIs (age older than 80 years, heart failure, chronic obstructive pulmonary disease, active cancer, heart rate ≥110<span class="elsevierStyleHsp" style=""></span>bpm, systolic blood pressure 100<span class="elsevierStyleHsp" style=""></span>mm Hg and oxygen saturation <90%). We found that a score ≥1 on the PESIs scale (OR, 3.62; 95% CI 2.06–6.36; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) was an independent factor related to mortality at 30 days.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">The present study shows that patients hospitalized for acute symptomatic PE in outpatient care with a history of an MH in the previous 3 months have increased mortality at 30 days when compared with those with no prior hospitalization or with SH. The study also shows that this effect is explained exclusively by the greater severity of the VTE episode in this patient group.</p><p id="par0070" class="elsevierStylePara elsevierViewall">These data add greater evidence to the close association, not only as a risk factor but also as a prognostic factor, between hospitalization and VTE.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">4,8,9</span></a> Hospital VTE has been known to have a poorer prognosis than outpatient VTE and an even poorer prognosis in medical patients than in surgical patients.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">5,6</span></a> The results of this study reveal that this increased risk extends up to 3 months after hospital discharge when compared with surgical patients or those who have not been hospitalized, which is explained by the greater severity of the episode presented by these patients. This conclusion has also been reached in studies performed on hospital VTE. Hospitalization by itself does not therefore appear to represent a poorer prognosis for the VTE but could encompass those patients with greater comorbidity and episode severity, as was observed in our sample, and therefore includes a greater risk of mortality in the short-term. This severity was defined by the PESIs scale, which considers not only the hemodynamic instability triggered by the PE episode itself but also the patient's comorbidity (age and cancer) and cardiopulmonary reserve (heart failure and COPD) for withstanding this episode, all of which are an actual determinant of the risk of all-cause mortality at 30 days. Other variables that can cause increased mortality in patients with prior medical hospitalization, although not assessed in this study, could include diagnostic delays (due to a poorer baseline condition that masks new symptoms) and factors that are more related to the factors of the cognitive, functional and social spheres of elderly patients.</p><p id="par0075" class="elsevierStylePara elsevierViewall">We also observed that 15% of the patients treated for a VTE episode had been previously hospitalized and that the MH to SH ratio was almost double, with more than 75% occurring in the first 2 months after discharge. These results are consistent with previous registries that have documented the inadequate thromboprophylaxis of hospitalized patients, which is even poorer for medical patients than for surgical patients. However, recently published conflicting results have shown a tendency toward an overuse of thromboprophylaxis.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">10–14</span></a> Previous studies conducted on patients with VTE have indicated that the risk is present during the 90 days following hospitalization<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">4</span></a> and that the frequency was higher in the first 20 days after hospital discharge for abdominal and major orthopedic surgery.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">8</span></a> Our data, although not strictly comparable given that it only includes patients with PE, support the contention that the risk of VTE attributed to hospitalization persists at least for the first 90 days after discharge, for both medical and surgical patients. It is therefore essential to perform proper VTE prophylaxis during hospitalization, especially for medical patients<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">10–14</span></a> and to determine, as with patients with SH, the profile of patients hospitalized for acute medical disease who would benefit from extended prophylaxis.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">15</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">This study has a number of limitations, the first of which is the inherent limitations of this type of retrospective, single-center study with a small sample size. Secondly, the endpoint was overall mortality, and we could not perform an analysis by subgroup based on whether death was related or not to the acute episode or was a consequence of the adverse effects of the treatment or underlying disease. Thirdly, a specific analysis was not conducted in terms of the prophylaxis received during the hospitalization in the 3 previous months. Lastly, there were no data on plasma biomarkers, such as troponin and B-type natriuretic peptide, or echocardiographic data, which could have had an effect when determining mortality in the short-term.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Despite these limitations, this study allows us to conclude that the increased risk of mortality at 30 days associated with a history of an MH in the 3 previous months for patients with an acute symptomatic PE treated in the outpatient is a consequence of the greater severity, assessed by the PESIs scale, which considers both the hemodynamic variables of the acute episode and the patient's age and comorbidity. Additionally, a history of a previous medical hospitalization is more common than a previous surgical hospitalization for patients with a PE. This hospitalization can occur in the first 3 months after the discharge. These conclusions have certain clinical importance given that they could be of help to clinicians during the initial care and risk stratification of patients with acute symptomatic PE.
