Its incidence has been increasing considerably in recent years.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Macroscopic extrathyroidal extension has been shown to be very important factor of poor prognosis in papillary thyroid carcinoma.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–8</span></a> However, the role of microscopic or minimal extrathyroidal extension (mETE), which is extension that occurs towards the sternothyroid muscle or perithyroidal adipose tissue, remains unclear and there is controversy in the various studies that have been published.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–14</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">According to the European Thyroid Association,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> papillary thyroid carcinoma with mETE is considered to be high risk. According to the American Thyroid Association,<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> it is considered to be intermediate risk. The European Thyroid Association<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> recommends the use of radioiodine therapy (I<span class="elsevierStyleSup">131</span>) when there is mETE whereas the American Thyroid Association<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> recommends considering it, but that its use may not be justified in small tumors.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In recent years, there has been a growing trend against considering mETE to be an unfavorable prognostic factor, given that various studies have been published which have found that mETE does not affect disease-free survival.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–12</span></a> In addition, in the eighth edition of the American Joint Committee on Cancer's TNM classification,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> mETE is not taken into consideration; in fact, PTMC with mETE is classified as T1, not T3, as it was in the seventh edition.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Recently, an article by Liu et al.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> found that mETE may be associated with lower overall survival and lower disease-free survival. Therefore, they recommend more discussion and research in order to evaluate the omission of mETE in the new eighth edition of the American Joint Committee on Cancer’s TNM classification.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Due to the increase in incidence of PTMC, it is important to know the factors associated with mETE as well as its impact on disease prognosis in order to provide adequate treatment and reduce the recurrence and mortality associated with the disease. The aim of this work is to analyze and compare factors associated with PTMC according to presence of mETE as well as its long-term prognosis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population</span><p id="par0030" class="elsevierStylePara elsevierViewall">This work is a retrospective, observational, comparative study. For categorical variables, data were expressed in frequencies and percentages and were compared using Pearson’s chi-square test or Fisher’s exact test, as appropriate. For continuous quantitative variables, data were expressed as means ± standard deviation. The normal distribution of variables was verified using the Kolmogorov–Smirnov test. Quantitative variables from both groups were compared using Student’s <span class="elsevierStyleItalic">t</span>-test for independent data when they followed a normal distribution. In the event quantitative variables did not follow a normal distribution, the Mann-Whitney U nonparametric test was used to compare them.</p><p id="par0215" class="elsevierStylePara elsevierViewall">A multivariate analysis was conducted using a binary logistic regression model to evaluate the independent associations of all factors found to be statistically significant on the bivariate analysis. <span class="elsevierStyleItalic">p</span> = .001) and were more likely to have had ablation of thyroid remains with I<span class="elsevierStyleSup">131</span> (23.1% vs. 100%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleItalic"> </span>< .001) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Histopathological differences</span><p id="par0260" class="elsevierStylePara elsevierViewall">Patients with PTMC with mETE had a larger mean tumor size (5 ± 2.7 mm vs. 7.9 ± 2.3 mm; <span class="elsevierStyleItalic">p</span> < .001), greater frequency of tumor size ≥8 mm (20.3% vs. 61.1 %; <span class="elsevierStyleItalic">p</span> = .001), greater presence of bilaterality (11.9% vs. 44.4%; <span class="elsevierStyleItalic">p</span> = .002), and greater presence of metastatic lymph nodes (5.6% vs. 38.9%; <span class="elsevierStyleItalic">p</span> < .001) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Multivariate analysis</span><p id="par0265" class="elsevierStylePara elsevierViewall">In the multivariate analysis (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), the risk factors found to be associated with mETE were age ≥45 years (OR: 4.383; 95% CI: 1.051−18.283; <span class="elsevierStyleItalic">p</span> = .043), tumor size ≥8 mm (OR: 5.913; 95% CI: 1.795−19.481; <span class="elsevierStyleItalic">p</span> = .003), bilaterality (OR: 4.430; 95% CI: 1.294−15.173; <span class="elsevierStyleItalic">p</span> = .018), and presence of metastatic lymph nodes (OR: 12.588; 95% CI: 2.919−54.280; <span class="elsevierStyleItalic">p</span> = .001).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0095" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0155">Follow-up</span><p id="par0270" class="elsevierStylePara elsevierViewall">Over a mean follow-up period of 119.8 ± 65 months, there were no differences between groups in terms of recurrence (group 1 (0%) vs. group 2 (5.6%); <span class="elsevierStyleItalic">p</span> = .112) or overall survival (group 1 (124.9 ± 5.6 month) vs. group 2 (103.1 ± 11.7 months); <span class="elsevierStyleItalic">p</span> = .088), but there were differences in terms of disease-free survival, which was lower in group 2 (124.9 ± 5.6 vs. 97.4 ± 10.3 months; <span class="elsevierStyleItalic">p</span> = .034) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0100" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0160">Discussion</span><p id="par0275" class="elsevierStylePara elsevierViewall">The frequency and importance of extrathyroidal extension of papillary thyroid carcinoma depend on the criteria established to define it.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In the seventh edition of the American Joint Committee on Cancer’s TNM classification,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> extrathyroidal extension is classified as minimal or microscopic (for example, to the sternothyroid muscle or the perithyroidal fatty tissue, assuming that the definition includes the rest of the infrahyoid muscles) and significant or macroscopic (to the subcutaneous tissue, larynx, trachea, esophagus, or recurrent laryngeal nerve). According to a 2014 definition from the College of American Pathologists, mETE includes the presence of carcinoma which extends to the perithyroidal tissues, including infiltration of the skeletal muscle as well as the areas around important vascular structures and nerves.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In the eighth edition of the American Joint Committee on Cancer’s TNM classification,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> mETE is not taken into consideration and only macroscopic extrathyroidal extension to the infrahyoid muscles is evaluated (T3b). Therefore, in this most recent classification, PTMC with mETE is classified as T1 and not T3, as in the seventh edition.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0280" class="elsevierStylePara elsevierViewall">Due to this lack of consensus on its definition, mETE is a subjective histopathological variable in endocrine disease with a large degree of interobserver variability. The degree of concordance is highest when mETE occurs in the infrahyoid muscles and lowest when it occurs in perithyroidal adipose tissue or comes close to the thick wall vessels or nerves.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0285" class="elsevierStylePara elsevierViewall">According to several published series, mETE occurs in 9.6%–30.9% of patients with PTMC.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,21,22</span></a> In this study, it was present in more than 11% of all cases. Although there are a growing number of studies which analyze the prognostic value of mETE in papillary thyroid carcinoma or in differentiated thyroid cancer,<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–14</span></a> there are few published studies that analyze the associated factors and prognostic value of mETE in the PTMC subgroup.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,21,22</span></a></p><p id="par0290" class="elsevierStylePara elsevierViewall">Upon analyzing both study groups, it was observed that PTMC with mETE has risk factors for a worse prognosis. In this work, it was associated with older patients (age ≥45 years), though this is in contrast to what has been found in other studies, which report mETE to be associated with age <45 years<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> or not associated with any age group.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a></p><p id="par0295" class="elsevierStylePara elsevierViewall">In regard to histopathological factors, larger PTMC were associated with the presence of mETE, a finding that is in line with other published studies.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,22,23</span></a> On the contrary, some studies only associate mETE with tumor size >1 cm.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a> This can be explained by the fact that when PTMC is located in the peripheral part of the thyroid gland, if it is larger, it has a greater likelihood of invading the thyroid capsule, surpassing it, and infiltrating the perithyroidal fat or infrahyoid musculature.</p><p id="par0300" class="elsevierStylePara elsevierViewall">Although multifocality of PTMC was not associated with the presence of mETE, bilaterality was, as all the multifocal PTMC with mETE were bilateral. However, in the scientific literature, the multifocality of PTMC, whether unilateral or bilateral, has indeed been associated with presence of mETE.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Therefore, in patients with PTMC with mETE who initially undergo a lobectomy, systematically completing the thyroidectomy could be indicated in order not to leave residual disease in the contralateral thyroid lobe.</p><p id="par0305" class="elsevierStylePara elsevierViewall">The presence of metastatic lymph nodes has also been associated with mETE of PTMC in different published studies as well as in this one, which demonstrates their greater aggressiveness.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,21,22</span></a> For this reason, more intensive treatments beyond a total thyroidectomy, such as a cervical lymphadenectomy and ablation of thyroid remains with I<span class="elsevierStyleSup">131</span>, should be considered.</p><p id="par0310" class="elsevierStylePara elsevierViewall">PTMC with mETE has a good long-term prognosis with few cases of recurrence, no associated mortality, and no differences with respect to overall survival. This could be due to more aggressive treatment, with a greater frequency of cervical lymphadenectomy and ablation of thyroid remains with I<span class="elsevierStyleSup">131</span>. The only patient who had recurrence had a PTMC with mETE and metastatic lymph nodes in the central compartment. Although there were no differences between the groups in this work in terms of the number of cases of recurrence or overall survival, mETE has been associated with PTMC recurrence<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26–30</span></a> and with lower overall survival in other studies.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0315" class="elsevierStylePara elsevierViewall">In the scientific literature, contradictory results have been found in terms of the prognostic significance of mETE: it either did not affect<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–12</span></a> or did affect<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14,19</span></a> disease-free survival. In addition, depending on tumor size (>1.5 cm), mETE has been described as an unfavorable prognostic factor.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Although there were no differences in terms of overall survival in this work, patients with PTMC with mETE had a significantly lower rate of disease-free survival.</p><p id="par0320" class="elsevierStylePara elsevierViewall">The controversy on the influence of mETE on the disease’s prognosis may be related to interobserver variability in the histopathological diagnosis of mETE. For this reason, there should be broader consensus on the definition of mETE in order to decrease interobserver variability, better stratify the entity, and more precisely evaluate its impact on disease prognosis.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0325" class="elsevierStylePara elsevierViewall">In terms of treatment, there are studies which do not recommend the administration of I<span class="elsevierStyleSup">131</span> in papillary thyroid carcinoma ≤4 cm with mETE which do not have an aggressive histology or metastatic lymph nodes, given that they have a recurrence rate of just 2.2%.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> It is also not recommended in patients older than 45 years of age and in tumors ≤4 cm, as it does not affect survival.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> On the contrary, in this work, all patients with mETE received metabolic therapy with I<span class="elsevierStyleSup">131</span>, which may be the reason why there were so few cases of recurrence. Even still, disease-free survival was lower in patients with PTMC and mETE.</p><p id="par0330" class="elsevierStylePara elsevierViewall">In conclusion, PTMC with mETE was found to be associated with older age, larger tumor size, bilateral thyroid involvement, and presence of metastatic lymph nodes. Likewise, although there were few cases of recurrence, no associated mortality, and good overall survival, it had a lower disease-free survival rate. For all of these reasons, mETE should be considered a factor of a worse prognosis and more aggressive treatments beyond a total thyroidectomy, such as a cervical lymphadenectomy and ablation of thyroid remains with I<span class="elsevierStyleSup">131</span>, should be considered. Further studies that analyze the impact of mETE not just on PTMC prognosis, but also on papillary thyroid carcinoma >1 cm must be conducted, given that the scientific evidence is limited and it could be a factor to take closely into account in the treatment of this type of tumor.</p></span><span id="sec0105" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Background</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Although the incidence of papillary thyroid microcarcinoma (PTMC) has increased in recent decades, the role played by minimal extrathyroidal extension (mETE) in the prognosis of PTMC is still unclear. The aim of this study is to analyze the factors associated with PTMC with mETE and its long-term prognosis.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">We conducted a retrospective study on patients with a histological diagnosis of PTMC. We excluded patients who had previously undergone thyroid surgery, those who had other synchronous malignancies, those with an ectopic location of the PTMC, and those lost to follow-up within two years. We compared group 1 (PTMC without extrathyroidal extension) to group 2 (PTMC with mETE) and performed a multivariate analysis.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">We observed PTMC with mETE in 11.2% (n = 18) of patients. On the multivariate analysis, mETE was associated with an age ≥45 years (OR: 4.383; 95% CI: 1.051−18.283, <span class="elsevierStyleItalic">p</span> = .043), tumor size ≥8 mm (OR: 5.913; 95% CI: 1.795−19.481; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleItalic"> </span>= .003), bilaterality (OR: 4.430; 95% CI: 1.294−15.173; <span class="elsevierStyleItalic">p</span> = .018) and metastatic lymph nodes (OR: 12.588; 95% CI: 2.919−54.280; <span class="elsevierStyleItalic">p</span> = .001). Over the mean follow-up period of 119.8 ± 65 months, one case of recurrence was detected in group 2 (0% vs. 5.6%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleItalic"> </span>= .112). No patients died of the disease. El objetivo de este estudio es analizar los factores asociados al MCPT con EETm y su pronóstico a largo plazo.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Material y métodos</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">Estudio retrospectivo cuya población a estudio la constituyen los pacientes con diagnóstico histológico de MCPT. Se excluye a los pacientes con: cirugía tiroidea previa, otras enfermedades malignas sincrónicas, localización ectópica del MCPT o pérdida durante el seguimiento antes de los 2 años. Se comparan 2 grupos: grupo 1 (MCPT sin extensión extratiroidea) y grupo 2 (MCPT con EETm). Se realiza un análisis multivariante.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">El 11,2% (n = 18) de los pacientes presentaron MCPT con EETm.
