Due to this aging, the prevalence of chronic diseases has also increased. Individuals are increasingly likely to have several diseases at a time and to take numerous drugs.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Polypathological patients have 2 or more chronic, progressive, inter-related diseases that result in progressive incapacitation and cause frequent hospitalizations.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">1</span></a> These patients have high mortality and consume significant healthcare resources.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">2</span></a> Up to 38% of patients hospitalized in internal medicine departments have multiple diseases.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">If we follow the clinical practice guidelines, the recommendations for treating polypathological patients include numerous drug and nondrug measures.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">5,6</span></a> On average, polypathological patients take more than 8 drugs a day, and a third of them take more than 10.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">7</span></a> This high number of drugs increases the complexity of the drug regimen, and increased complexity has been associated with poorer adherence.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">8</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Typically, therapeutic complexity is usually assessed by counting the number of drugs the patient is taking. However, this approach is incomplete. It is known that the adherence rate is affected not only by the number of drugs and daily doses but also the additional administration instructions and restrictions and the galenical form of the drug.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">9–12</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The therapeutic complexity has been evaluated in patients older than 65 years<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">13</span></a> but not specifically in polypathological patients. The aim of this study was to assess the complexity of the therapeutic regimen in polypathological patients hospitalized in internal medicine departments at hospital discharge and the relationship between the complexity and the various categories of disease.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0030" class="elsevierStylePara elsevierViewall">This was an observational, prospective and multicenter study that included patients recruited in the internal medicine departments of 5 hospitals of Aragon (a regional hospital, 2 secondary hospitals and 2 tertiary hospitals). The study included patients hospitalized between March and June in internal medicine departments who met the criteria of polypathological patient according to the definition of the government of Andalusia.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">1</span></a> Readmissions and patients who died during the hospitalization were excluded.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data</span><p id="par0035" class="elsevierStylePara elsevierViewall">The following variables were collected: age, sex, living at home or in assisted living, diagnostic category of polypathology, comorbidity, capacity for performing the basic and instrumental activities of daily life, cognitive state, social-family situation, number of previous hospitalizations, onset of delirium in the previous hospitalization, need for caregiver, availability of caregiver and PROFUND index score. Comorbidity was measured with the Charlson index.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">14</span></a> The capacity to perform basic activities of daily life was measured with the Barthel index, which measures 10 activities and scores from 0 to 100, with lower scores indicating greater disability.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">15</span></a> The capacity to perform instrumental activities of daily life was measured with the Lawton–Brody scale, which includes 8 activities and scores from 0 to 8, with lower scores indicating greater disability.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">16</span></a> An individual was considered dependent if the score was <8 for women or <5 for men. The cognitive state was assessed with the Pfeiffer brief mental questionnaire, which measures errors on 10 questions; the more errors there are, the greater the cognitive impairment.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">17</span></a> The social-family situation was measured with the Gijón scale, which scores from 5 to 25; the higher the score, the greater the risk of a social problem.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">18</span></a> The PROFUND index assesses the probability of survival at the end of a year in polypathological patients and includes demographic, clinical, analytical, psychometric, functional, social-family and healthcare measures.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">19</span></a> The index scores between 0 and 30 points and establishes 4 levels of mortality risk: low (0–2 points), intermediate (3–6 points), high (7–10 points) and very high (11–30 points).</p><p id="par0040" class="elsevierStylePara elsevierViewall">A patient is considered to need a caregiver if the Barthel score is <60 and/or the Pfeiffer questionnaire score is >3 errors. Polypharmacy was considered the consumption of 5 or more drugs daily, and extreme polypharmacy was considered 10 or more.</p><p id="par0045" class="elsevierStylePara elsevierViewall">To assess the regimen complexity of the drug treatment, we used the Medication Regimen Complexity Index (MRCI).<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">20</span></a> This is a 65-item tool that assesses and quantifies the characteristics that make a therapeutic regimen complex, beyond just considering the number of drugs taken by a patient. The MRCI is divided into 3 sections. Section A refers to the galenic form, scoring according to the difficulty of use or taking of the drug, scoring once for each galenic form. Section B refers to the posology, with scores increasing as the number of times the drug needs to be taken each day increases and the stricter the schedule. Section C refers to the additional instructions and scores according to the instructions associated with taking the drug. The sum of the 3 sections is the final value; the higher the final score, the more complex the therapeutic regimen. The MRCI was calculated based on the data in the discharge report. Patients with multiple diseases are known to take numerous drugs. We therefore considered that the drug regimen was complex when the MRCI score was in the fourth quartile.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The study was approved by the Clinical Research Ethics Committee of Aragon. All patients gave their written informed consent. For the incapacitated patients, their direct relatives provided this consent.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">The quantitative variables are presented as means (standard deviation), and the qualitative variables are presented as absolute frequencies (percentage). The qualitative variables were compared with the chi-squared test, and the quantitative variables were compared with Student's <span class="elsevierStyleItalic">t</span>-test.</p><p id="par0060" class="elsevierStylePara elsevierViewall">To study the association between the variables and the therapeutic complexity, we constructed a logistic regression model that included in the multivariate analysis those variables that were associated with a <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.1 in the univariate analysis. The goodness of fit of the logistic regression model was verified with the Hosmer–Lemeshow test.</p><p id="par0065" class="elsevierStylePara elsevierViewall">In all cases, the level of statistical significance was <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patient characteristics</span><p id="par0070" class="elsevierStylePara elsevierViewall">We included a total of 223 patients. The flow diagram of patients is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. The patient characteristics are presented in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The number of polypathology categories was greater in the patients with complex drug regimens. Heart failure and respiratory diseases were the most common in the patients with complex drug regimens, and nervous system diseases and peripheral arterial disease were less common. There were no differences in the cognitive state, social-family situation and capacity for performing daily life activities.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Drug regimen</span><p id="par0075" class="elsevierStylePara elsevierViewall">The mean number of drugs prescribed was 8.4 (3.3). A total of 198 (88.8%) patients presented polypharmacy, and 75 presented extreme polypharmacy (33.6%).</p><p id="par0080" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> lists the characteristics by patient of the therapeutic regimen. The mean MRCI score was 32.0 (15.2), and the score by quartiles was 0–20, 20.5–30, 30.5–42 and 42.5–80. The most common galenic forms were capsules, inhalers, preloaded injections and patches. Although the fixed administration rates of once, twice or thrice daily were the most common, 28.7% of the patients had to take the medication on alternating days or less frequently. Additionally, 222 (99.6%) patients received further instruction.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The means scores on sections A, B and C and the total score on the MRCI were 6.1 (4.1), 13.2 (5.9), 12.7 (6.7) and 32.0 (15.2), respectively. The patients with respiratory diseases (category C) had the highest scores (8.4 [4.1], 15.2 [6.4], 15.4 [7.5] and 39.0 [16.5]), and the patients with neurological diseases (category E) had the lowest (5.3 [3.7], 11.7 [5.3], 10.7 [5.6] and 27.7 [13.0]) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Factors associated with therapeutic complexity</span><p id="par0090" class="elsevierStylePara elsevierViewall">The factors related to a more complex therapeutic regimen are listed in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. In the multivariate analysis, respiratory diseases (OR, 4.185; 95% CI 2.015–8.693; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) were associated with greater complexity, and nervous system diseases with permanent mental deficiency (OR, 0.265; 95% CI 0.085–0.828; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.022) were associated with lesser complexity in the drug regimen. The model showed a good fit (Hosmer–Lemeshow test, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.534).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">Our study's main findings were that the drug regimens prescribed to polypathological patients after a hospitalization were of considerable complexity and were more complex in patients with respiratory diseases and less complex in the patients with cognitive impairment.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Polypathology is the coexistence of 2 or more chronic diseases and is frequently associated with polypharmacy. In the PROFUND project, a Spanish multicenter study, the polypathological patients consumed a mean of 8 drugs a day.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">19</span></a> The PLUPAR study observed that the prevalence of extreme polypharmacy in polypathological patients was 33% and that polypharmacy was not associated with increased survival.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">4,7</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Our study observed the relationship between the number of drugs and therapeutic complexity. However, the complexity of a drug regimen covers more aspects than just the simple number of drugs consumed. Aspects such as the galenic form, the dosage rate and the method for preparing the drug can complicate the understanding and following of prescriptions. In this study, the mean complexity of the drug regimen measured with the MRCI was 32, a value that can be considered very high. Several aspects illustrate this complexity in our polypathological patients. Sixty-nine percent of our patients had to take some drug twice daily, 33% had to do so thrice daily, and 39% had alternating regimens.</p><p id="par0110" class="elsevierStylePara elsevierViewall">A recent Swedish study on elderly patients showed a mean MRCI score of 9, more than 3 times lower than our mean score.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">21</span></a> In a multicenter study of elderly institutionalized patients in Portugal, the mean score was 18, much lower than in our study.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">13</span></a> However, in a study with a similar population in Australia, the median score was 43, much higher than for our patients.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">22</span></a> Ferreira et al., in a study in primary care to determine high and low complexity levels, found a median MRCI of 8.5 and established high complexity levels in 15.5–25.4.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">23</span></a> In our study, the median MRCI was almost 4-fold greater. Applying these levels, 60.5–83.9% of the polypathological patients had highly complex drug regimens. If we compare our results with those of the study by Mansur et al.,<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">24</span></a> performed on acutely ill patients hospitalized in Israel, there are barely any differences in complexity and in the mean MRCI score (30.27), although the number of drugs was higher in our study (8.4 vs. 5.9).</p><p id="par0115" class="elsevierStylePara elsevierViewall">Wimmer et al.,<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">25</span></a> in a study on elderly patients, reported an association between the drug regimen complexity and age, comorbidity, pain, better cognitive state and functional impairment. As with other studies, we found no association with age.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">26</span></a> However, the age of our study's patients was 5–7 years older than that of the patients included in other studies.<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">25,26</span></a> Logically, the greater the comorbidity the higher the number of prescribed drugs and complexity of the therapeutic regimen. A study with 4 patient cohorts with only one disease (geriatric depression, human immunodeficiency virus infection, diabetes or hypertension) showed lower complexity values.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">27</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">In our study, therapeutic complexity was associated with some diseases, such as heart failure, chronic respiratory disease, peripheral arterial disease and cognitive impairment, which are frequently associated with each other. A number of studies have evaluated the drug regimen complexity in heart and respiratory diseases, and all have found high complexity.<a class="elsevierStyleCrossRefs" href="#bib0360"><span class="elsevierStyleSup">28–30</span></a> The clinical practice guidelines for treating heart failure and chronic obstructive pulmonary disease recommend using multiple drugs. It is therefore not surprising that, in our study, these diseases were associated with high therapeutic complexity. We have found no previous studies on treatment regimen complexity in peripheral arterial disease. There are few studies that have evaluated the relationship between drug regimen complexity and cognitive function, but in all of these studies, a negative association was observed,<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">25,31</span></a> as was the case in our study.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In studies conducted in adults and elderly individuals, the main causal factor for drug regimen complexity is the dosage rate, which corresponds to section B of the MRCI.<a class="elsevierStyleCrossRefs" href="#bib0325"><span class="elsevierStyleSup">21,27</span></a> In our study, however, the weight of the additional instructions (12.7 points) in the final MRCI score was equivalent to the weight of the dosage rate (13.2 points). This result tells us that, for polypathological patients, following the specific instructions for taking the drug is an issue of considerable importance. It is therefore important to assess whether the patients understand the medication plan. To this end, tools have been developed such as the “Evaluation tool to test the handling of the medication plan”.<a class="elsevierStyleCrossRef" href="#bib0380"><span class="elsevierStyleSup">32</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The high scores on the MRCI have been associated with greater hospital mortality, more nosocomial infections and more pressure ulcers,<a class="elsevierStyleCrossRef" href="#bib0385"><span class="elsevierStyleSup">33</span></a> as well as hospitalization for the adverse drug effects, with higher mortality and institutionalization after the hospital discharge.<a class="elsevierStyleCrossRefs" href="#bib0335"><span class="elsevierStyleSup">23,34–37</span></a> The association between drug regimen complexity and treatment adherence has also been studied.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">8</span></a> Although a study of patients on hemodialysis observed greater adherence in the complex regimens, other studies on patients with diabetes, high blood pressure or human immunodeficiency virus/hepatitis C virus coinfection have observed the opposite.<a class="elsevierStyleCrossRefs" href="#bib0410"><span class="elsevierStyleSup">38–41</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Presumably, the simplification of the drug regimen is associated with benefits for the patient and healthcare system. Thus, reviews of the medication, its indications and its regimens are needed, beyond just reducing the number of drugs. Interventions have been conducted by taking advantage of the hospitalization and have yielded good results.<a class="elsevierStyleCrossRefs" href="#bib0430"><span class="elsevierStyleSup">42,43</span></a> These interventions should be directed at the hospital physicians and primary care physicians.<a class="elsevierStyleCrossRef" href="#bib0440"><span class="elsevierStyleSup">44</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">This study has a number of strengths. To our knowledge, this is the first study to analyze the complexity of the therapeutic regimen in polypathological patients. Although it is expected that polypathological patients, due to their high morbidity burden, consume numerous drugs and have complex regimens, they are patients who frequently have cognitive and functional impairment and depend on others for their care. For this reason, the study of the therapeutic complexity is important to be able to design future educational strategies that help patients and caregivers to comply with the treatment specifications. Another strength is the multicenter nature of the study, which reinforces the validity of its findings.</p><p id="par0145" class="elsevierStylePara elsevierViewall">However, the study also has limitations. Firstly, patient enrollment occurred during spring. It is possible that enrolling patients at other times of the year could modify the complexity of the therapeutic regimens. However, polypathological patients by definition present several chronic diseases and exacerbations of those diseases, which results in hospitalization having a less seasonal nature, occurring throughout the year. Secondly, the patients were recruited exclusively after hospitalization in internal medicine departments. Our results cannot therefore be extrapolated to other settings and circumstances. The treatment complexity could be different for primary care patients, those in other hospital departments and those recruited from outpatient clinics.</p><p id="par0150" class="elsevierStylePara elsevierViewall">In summary, therapeutic regimens in polypathological patients are of considerable complexity. Strategies that simplify those regimes are recommended, along with educational and reinforcement measures that help patients and their caregivers comply with the therapy's specifications. Data were collected on age, sex, residence, disease, Charlson, Barthel and Lawton–Brody indices, Pfeiffer questionnaire, Gijón scale, number of hospitalisations in the previous year, delirium, need for and availability of caregivers and the PROFUND index score. We calculated the therapeutic complexity with the Medication Regimen Complexity Index (MRCI). We considered a therapeutic regimen complex when the MRCI score was in the fourth quartile.
