The relationship between the CRP measurement and potential confounding factors was studied for the categorical variables using the chi-squared test or Fisher's exact test. We employed the Kolmogorov–Smirnov test to determine the possible normal distribution of the quantitative variables. The comparison of means/medians was performed with an analysis of variance test for the normal distributions and with the Wilcoxon rank-sum test for the continuous variables with non-normal distribution. Using a logistic regression analysis, we determined the initial factors that (a) were correlated with the CRP values at admission and (b) were independently associated with higher all-cause mortality at 3 months, using the CRP value in the latter case as a continuous variable. By using this procedure, we calculated the unadjusted and adjusted odds ratios, with a 95% CI. Although this finding might seem contradictory, it can be explained if we assume that an increase in the inflammatory response could be more obvious in the most severe condition (that of hypotensive AHF), as has been reported in cardiogenic shock that complicates acute coronary syndrome.31

Although hospital admissions for AHF are increasing, the mean hospital stay has progressively decreased. The VERITAS13 and ASCEND-HF14 trials were conducted in younger patient cohorts that were relatively homogeneous and had a predominance of HFrEF. Both studies revealed the lack of association between us-CRP (ASCEND) or CRP (VERITAS) and adverse events, except for a longer hospital stay in the ASCEND trial and worsening heart failure at 30 days and 180-day mortality in the VERITAS trail (adjusted univariate analysis). Regarding cohort studies, the data from the BASEL study,18 conducted with patients similar to ours (although, once again, most of them with HFrEF), showed that higher plasma us-CRP values at admission predicted, after the multivariate adjustment, a higher risk of mortality at 2 years, as well as the combined event of mortality and readmission. The Korean Heart Failure Registry,22 which includes a large heterogeneous patient group of AHF similar to our cohort although younger, also showed that us-CRP values at admission help predict the risk of death at 12 months of the index hospitalization, especially when used in combination with natriuretic peptide values. Lastly, the Japanese ATTEND cohort,24 which is very similar to the RICA registry, also showed the value of increased CRP levels for predicting the risk of cardiovascular and noncardiovascular death at 6 months of the discharge. Our study's negative result clearly contradicts those of the ATTEND study. A possible explanation for this discrepancy is that, since the readmission and death rates in the short-term for patients in the low and medium CRP terciles are comparable (including all patients with “normal” CRP values defined by the clinical cut-off), other unevaluated variables common to elderly populations (treatment adherence, comorbidities, functional impairment, greater risk of readmission and admission to nursing homes) and unrelated to heart failure could play a relevant role in the incidence of adverse effects that cancel or compensate for those exerted by the inflammatory condition related to the AHF episode.

Our study's main strength lies in the fact that it is derived from a multicenter heart failure registry, which allows us to include a large number of representative patients “from the real world” with heart failure. However, we should mention a number of limitations. The first limitation is that we did not have CRP values from almost half of the registered patients, which could have created a certain selection bias. However, there were no significant differences in the baseline characteristics between the populations with and without available CRP values. Secondly, CRP was measured only at admission. We cannot therefore provide information regarding the prognostic value resulting from the change in plasma CRP values during the stabilization, discharge or follow-up, as was possible in the ASCEND trial17 or, in combination with other biomarkers, as in the RELAX-AHF trial.34 The third limitation was related to the lack of more specific information regarding the factors that contribute to the development of AHF and that probably influenced the plasma CRP, such as the causes, intensity and treatment of the respiratory and nonrespiratory infections. The fourth limitation was that the follow-up data were restricted to the registration of the death or readmission and its causes; no data was recovered regarding the events that occurred after the discharge and before these events. The last and possibly most significant limitation was that we lacked data on the patients who died during the index hospitalization. These patients were possibly those who experienced the most severe or complicated AHF episodes and surely had the highest associated increase in CRP levels.
Conclusion
Our data do not support the routine use of CRP measurements in elderly patients with AHF to assess the short-term risk of death or readmission after hospital discharge, regardless of their indication for other purposes.
