Ocho años de hospitalización en las Unidades de Medicina Interna (2007-2014). ¿Qué ha cambiado?" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">The European Federation of Internal Medicine defined internal medicine as the main medical discipline for the care of adult patients with one or more complex, acute or chronic illnesses.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">1</span></a> The dimensions related to “comprehensiveness” and “continuity” and the care of patients with complex diseases characterize the most often recognized scope of action of internal medicine, even more so in the context of populational aging, which entails a significant increase in the number of complex chronic patients.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">2</span></a> The presence of older age groups in internal medicine units (IMUs) is a reported fact.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">3</span></a> This population group has specific requirements (e.g., care for their nutritional state),<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">4</span></a> as well as frailty and vulnerability that involve risks during hospitalization, which can worsen and lead to new readmissions if not properly treated.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">This situation is faced by the IMUs of the Spanish National Health System (SNHS), with considerable variability in their indicators of structure, activity and management and with notable margins for improvement in the clinical management of hospitalized patients and in caring for complex chronic patients.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> In 2009, the Clinical Management Group of the Spanish Society of Internal Medicine analyzed hospital discharges from IMUs of the SNHS during the biennium 2005–2006 and found a high prevalence of cardiopulmonary disease as the cause of hospitalization. The average age of the hospitalized patients was 70.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.3 years, and a high percentage of the patients had comorbidity.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> The burden of care in SNHS IMUs has grown in parallel with the increase in the age of the treated population. However, there are still unanswered questions: Can the increase in age and the burden of care be quantified? Has the profile of care for patients treated in IMUs changed? Have the results worsened (e.g., mortality and readmissions) or improved?</p><p id="par0015" class="elsevierStylePara elsevierViewall">The present study attempts to answer these questions by analyzing the evolution of care provided in SNHS IMUs, using the data on discharges from these units in 2007 and 2014.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">We analyzed all discharges from SNHS IMUs in 2007 (539,249 discharges) and 2014 (648,750 discharges), encoded according to the International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM),<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> using the SNHS Minimum Basic Data Set. To compare the comorbidities in patients treated in the IMUs, we used the condition categories of Pope et al.,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> updated annually by the Agency for Healthcare Research and Quality. These comorbidities were the clusters that the Centers for Medicare and Medicaid Services<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a> use as risk factors for adjusting the readmissions rates for heart failure, because this disease is the most commonly treated in IMUs. For mortality risk adjustments and readmissions for heart failure, pneumonia and chronic obstructive pulmonary disease (COPD), we followed the methodology of the Centers for Medicare and Medicaid Services.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11,12</span></a> The risk adjustment for the IMUs’ entire activity was conducted using the Charlson index.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">13,14</span></a> The aggregate mortality rate by specific conditions was calculated according to the criteria of the Agency for Healthcare Research and Quality,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> without including hip fractures, given the low number of episodes for this process in patients discharged by IMUs.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Risk adjustment is necessary due to the centers’ and patients’ characteristics, which influence the healthcare results regardless of the care provided.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> Hospital mortality and readmissions are adjusted by risk, according to the referenced models.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11–14</span></a> The adjustment coefficients and factors that were ultimately entered into the models were derived from our own data and were used to estimate the expected results (mortality or readmission) and calculate the ratio for the risk-adjusted hospital mortality or readmission, as well as the ratio between the observed result and the expected result. The probability of a patient dying or being readmitted is considered a combination of their individual risk factors (characteristics) and the quality of the care provided (function).<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11,12,17</span></a> The multilevel risk adjustment models consider not only the patients’ demographic and clinical variables but also a specific “hospital” effect.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">18–20</span></a> Based on the multilevel models, we calculated the hospital mortality and readmission rates as the ratio between the predicted result (which individually considered the functioning of the hospital that cared for the patient) and the expected result (which considered a standard function according to the mean of all hospitals) multiplied by the raw mortality rate and readmissions rate.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">17,19</span></a> Thus, if a hospital's ratio is greater than the crude mortality or readmission rate, the probability of lethality or readmission in this hospital is greater than the mean of the hospitals considered.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The quantitative variables are listed as mean and standard deviation, and the qualitative variables are listed as numbers and percentages. Based on the individual episodes, the data were grouped by center. For the comparisons between 2007 and 2014, we used Student's <span class="elsevierStyleItalic">t</span>-test after studying the normality with the Kolmogorov–Smirnov test and the homogeneity of variances with the Bartlett test. For the patient profile variables, we applied the chi-squared test. For all comparisons, we rejected the null hypothesis, with an alpha error <0.05. The statistical calculations were performed with the STATA program version 13.0.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">Between 2007 and 2014, there was a marked increase (14.3%) in patients discharged by the IMUs, with a slight increase (1.9%) in the crude mortality rate and a reduction in the mean stay by somewhat more than 1 day (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Among the most common diagnoses at discharge, sepsis (231%) and aspiration pneumonitis (80.9%) grew the most. In contrast, there was a notable decrease (25.4%) in discharges for chronic bronchitis and other respiratory diseases (atelectasis, emphysema, etc.).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The comparison of the populations treated by IMUs between 2007 and 2014 (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) showed an increase in the patients’ average age (71.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.1 years vs. 74<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>16.2 years, respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), which was accompanied by a proportional increase in women. The average Charlson index of the patients treated in the IMUs increased dramatically (4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.7 in 2007 vs. 4.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>3.9 in 2014; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), as did the number of comorbidities per episode, which increased by 24% (2.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.8 vs. 3.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.9; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). We observed a significant increase in comorbidities (cancer, malnutrition, dementia and other mental problems, heart failure, renal failure and decubitus ulcers).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">There was marked interhospital variability (calculated by the mean odds ratio) for most of the adjusted indicators of mortality and readmissions. The multilevel adjustment therefore dramatically improved the predictive value, which resulted in an area under the ROC curve of approximately 0.8 for mortality by pneumonia and COPD, between 0.6 and 0.75 for the other indicators and only 0.58 for the readmissions rate adjusted by the Charlson index (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). When the indicators of the IMU results for 2007 and 2014 were compared, there were no significant differences in the average number of discharges per IMU, in the mean stay (crude) or in the adjusted mean stay index (AMSI). There were also no differences between the 2 periods in the crude mortality and readmissions rates (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). However, when the adjusted rates were compared, there was a slight but significant reduction in the mortality rates in 2014 compared with 2007. The exceptions were COPD, which worsened slightly, and HF, which experienced no significant changes. In contrast, there was a tendency toward a slight but statistically significant increase in the risk-adjusted readmission rate, with the exception of COPD and bacterial pneumonia, which decreased.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0050" class="elsevierStylePara elsevierViewall">During 2007–2014, the SNHS has confronted a 2-fold increase in the burden of care, an increase in demand (increased number of discharges) and an increase in the mean age and comorbidities of treated patients. In response to this growth in demand, the SNHS increased the number of general acute care hospitals in the public network during the analyzed period, thereby increasing the number of IMUs, while not changing the average number of discharges from IMUs. The latter do not show the probable increase in demand resulting from other departments, especially surgical, which also treat patients who are increasingly older and have more comorbidities, which has led to the creation by the IMUs of structured interconsultation units.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">7,21</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">IMUs reduced their mean stay by somewhat more than 1 day during the analyzed period. Although this reduction has no statistical meaning, it still has clinical relevance, if we consider that IMUs treat older patients with greater comorbidity. The reduction in mean stay has occurred in virtually all main healthcare processes treated in the IMU (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The slight reduction in hospital mortality might indicate an improvement in the quality of care. However, the increase in readmissions probably identifies gaps in the quality of care identified in other studies.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">7,22</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">A notable aspect of this study is the quantification of the increase in the burden of care resulting from the aging of the population. Both the increase in the average Charlson index of patients hospitalized in IMUs and the increase in the proportion of processes such as septicemia and aspiration pneumonia are associated with the increased complexity, comorbidity and severity of these patients. HF and pneumonia are the most common processes treated by IMUs, and the number of discharges from these processes increased dramatically (more than 25%) in the analyzed period. The greater proportion of comorbidities in the patients treated in IMUs (24%) is a notable indicator of the increase in complexity, which entails greater workloads and should translate into a lower ratio of patients to nurses than the current number in the average IMU.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> The results also suggest that involvement by other practitioners (multidisciplinary teams) is needed in the care of patients in the IMU.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The comorbidities that increased the most in terms of percentages were chronic renal failure, severe hematologic disorders, malnutrition, psychiatric disorders and pressure ulcers. The percentage increase in chronic renal failure is especially striking and could be due to factors associated with the increase in the patients’ age and comorbidities and to an earlier and more sensitive diagnosis, based on calculating the glomerular filtration rate.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The increase in malnutrition in more than 70% of patients and other psychiatric disorders are examples that indicate the need for practitioner profiles that should be integrated into the management of patients hospitalized in IMUs. Faced with the need to develop multidisciplinary teams, it is surprising that only a third of IMUs conduct joint physician-nurse rounds.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The increase in readmissions could be revealing the insufficient adaptation of the SNHS to the increase in comorbidity, dependence and frailty. In 2013, only 41% of the IMUs had implemented a systematic care program for complex chronic patients, and these programs are estimated to have an average population coverage of 56%.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> In contrast, systematic care programs for specific processes, such as HF, have shown a reduction in mortality and readmissions in our setting,<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">24–26</span></a> with a better cost/benefit ratio than “standard” care.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Limitations</span><p id="par0080" class="elsevierStylePara elsevierViewall">Although this was a retrospective analysis based on administrative data, the use of administrative registries for investigating the results of health services<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a> has been validated by comparing the data with data from medical records.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> The reliability of this type of analysis enables the comparison of results between hospitals.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">In contrast with the methodology used by the Centers of Medicare and Medicaid Services,<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">11,12</span></a> our endpoint was “hospital mortality” rather than mortality at 30 days. Another difference is that we did not analyze all readmissions because the Ministry of Health does not provide the complete minimum basic data set but rather the discharges by IMUs. Readmissions in other hospital departments could have been lost.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Regarding the adjustment models, there are confounding factors that could not be identified and could have a significant impact. The secondary diagnoses (comorbidities) used in the risk adjustments could correspond to conditions that are present at admission or complications that are sometimes the result of inadequate clinical management.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> Lastly, we cannot rule out that an improvement in the coding between 2007 and 2014, as well as changes in the clinical criteria for diagnosis, could have helped increase the detection of comorbidities.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0095" class="elsevierStylePara elsevierViewall">IMU discharges have grown 14% during 2007–2014, with an average of approximately 2300 IMU discharges per year.
