Survival free of the combined event at 180 days according to the combination of diameter and inspiratory collapse of the inferior vena cava.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">Systemic venous congestion is one of the underlying pathophysiological mechanisms in acute heart failure (AHF) and cardiorenal syndrome.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a> By measuring the diameter and inspiratory collapse of the inferior vena cava (IVC) by ultrasound, we can obtain an indirect calculation of the pressure in the atrium,<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">2,3</span></a> which is more reliable than other subjective methods such as estimating jugular vein engorgement during the physical examination.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">4</span></a> IVC ultrasound assessment at the patient's bedside provides information on the status of the blood volume and, thereby, the degree of systemic congestion.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Despite its usefulness, there has been little interest in the relationship between the IVC and the prognosis of patients with heart failure (HF) until very recently.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a> It has been suggested that the prognostic information provided by this ultrasound parameter in patients with chronic HF is similar to that of NT-proBNP.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to assess the usefulness of ultrasound IVC assessment as a prognostic marker in AHF, as well as its relationship with renal function and a number of systemic congestion biomarkers.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A prospective study consecutively included patients hospitalized for AHF from February 2013 to December 2014 in the University Hospital Clinic Lozano Blesa of Zaragoza. The inclusion criteria were an age greater than 18<span class="elsevierStyleHsp" style=""></span>years, a clinical diagnosis of AHF (<span class="elsevierStyleItalic">de novo</span> or exacerbated chronic HF) and aminoterminal fragment of the brain natriuretic peptide (NT-proBNP) values >300<span class="elsevierStyleHsp" style=""></span>pg/mL at admission. We excluded patients with cognitive impairment, a life expectancy shorter than 6 months, active neoplasia, advanced renal failure (glomerular filtration rate <30<span class="elsevierStyleHsp" style=""></span>mL/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span> according to the Modification of Diet of Renal Disease [MDRD-4] formula), inotropic treatment at admission or intolerance or allergies to diuretic treatment.</p><p id="par0025" class="elsevierStylePara elsevierViewall">After the admission index, all patients attended an outpatient follow-up at the specialized heart failure office, at 1, 3 and 6 months after discharge.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The primary events were mortality due to HF and the combined event of mortality and readmission for HF at 180<span class="elsevierStyleHsp" style=""></span>days. The secondary event was overall mortality. We collected demographic, laboratory and ultrasound data by reviewing the medical records.</p><p id="par0035" class="elsevierStylePara elsevierViewall">In the first 24–48<span class="elsevierStyleHsp" style=""></span>h of hospitalization, renal function was measured using urea, creatinine and cystatin serum concentrations. We measured the ion, hemoglobin, hematocrit, total protein, albumin, total cholesterol, NT-proBNP biomarker and carbohydrate antigen 125 (CA125) concentrations.</p><p id="par0040" class="elsevierStylePara elsevierViewall">During the first 24<span class="elsevierStyleHsp" style=""></span>h of hospitalization, an ultrasound was performed by an expert physician who was unaware of the patient's medical history and laboratory and radiological data. The physician used a portable V-SCAN (General Electronic Healthcare<span class="elsevierStyleSup">®</span>) device with a 1.7–3.8<span class="elsevierStyleHsp" style=""></span>MHz transducer. By performing the test in two-dimensions in the subcostal or subxiphoid plane, the physician determined the IVC diameter 1–2<span class="elsevierStyleHsp" style=""></span>cm from the right atrium and its inspiratory collapse. A diameter ≤20<span class="elsevierStyleHsp" style=""></span>mm was considered normal, as was an inspiratory collapse ≥50%.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">6</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The cohort was divided into 4 groups depending on the ultrasound results: group 1 (IVC undilated and collapse<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>50%), group 2 (IVC undilated and collapse<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>50%), group 3 (IVC dilated and collapse<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>50%) and group 4 (IVC dilated and collapse<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>50%). Furthermore, we differentiated the sample into 2 groups depending on whether the IVC was dilated or not (regardless of its collapse) and whether it was collapsed or not (regardless of whether it was dilated or not).</p><p id="par0050" class="elsevierStylePara elsevierViewall">All patients who agreed to participate in the study signed an informed consent. The study was approved by the Clinical Research Ethics Committee of Aragon (CI PI13/0019) and funded by the Institute of Health Carlos III of the Ministry of Economics and Competitiveness (Research Aid Program PI12/00694).</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">The continuous variables are shown as mean and standard deviation (SD) or median and interquartile range (IQR), depending on their normality, and analyzed using the Kolmogorov–Smirnov test. Categorical variables are expressed as percentages. To compare the continuous quantitative variables, we employed Student's <span class="elsevierStyleItalic">t</span>-test and the Mann–Whitney <span class="elsevierStyleItalic">U</span> test or ANOVA for parametric and nonparametric data, respectively. To compare the qualitative variables, we employed the chi-squared test or Fisher's exact test, calculating Cramer's V index and the odds ratio (OR) as relationship measures. An alpha error probability <.05 was considered statistically significant.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Survival at 180 days was analyzed using the Kaplan–Meier (K–M) method using cumulative survival curves with the log-rank (Mantel–Cox) test. The data analysis was performed with the SPSS software package, version 22.0.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">We included 85 patients (50.6% of whom were men) with a mean age of 79.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.22<span class="elsevierStyleHsp" style=""></span>years. Some 66.2% of the patients had a preserved left ventricular ejection fraction (LVEF<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>50%). The most common HF etiologies were hypertensive (37%) and ischemic (30.9%). At baseline, 63.5% of the patients were functional class II of the New York Heart Association (NYHA), and 65% were stage C of the American Heart Association (AHA). The median hospital stay was 9<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.5<span class="elsevierStyleHsp" style=""></span>days. The population characteristics according to the ultrasound IVC parameters are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Inferior vena cava diameter and collapse</span><p id="par0070" class="elsevierStylePara elsevierViewall">In the first 24<span class="elsevierStyleHsp" style=""></span>h of hospitalization for AHF, 55.3% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>47) of the patients had a dilated IVC (>20<span class="elsevierStyleHsp" style=""></span>mm). Overall, the inspiratory collapse was >50% for 30.6% (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>26) of the patients; however, if the IVC was dilated, it only collapsed in 10.4% of cases.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The cohort was divided into 4 groups according to the presence of IVC dilation and inspiratory collapse (see Patients and methods). The frequency in descending order is as follows: group 4 (49.4%), group 1 (24.7%), group 2 (20%) and group 3 (5.9%). Those patients without sufficient inspiratory collapse (groups 2 and 4) showed higher readings of urea (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.007), creatinine (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.004), uric acid (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.008), CA125 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.005) and NT-proBNP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.009) than the other patients. By itself, IVC dilation did not imply poorer renal function, unlike when IVC dilation was associated with the absence of collapse (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Relationship between serum CA125 and inferior vena cava</span><p id="par0080" class="elsevierStylePara elsevierViewall">Taking the upper limit of normality of CA125 (35<span class="elsevierStyleHsp" style=""></span>IU/mL) as the cutoff, we found differences among the patients according to the ultrasound IVC parameters (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.029) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Those patients with CA125 levels ≥35<span class="elsevierStyleHsp" style=""></span>IU/mL had greater systemic congestion in terms of IVC repletion (64.3% IVC dilated and 80.4% uncollapsible), differing from those with CA125 <35<span class="elsevierStyleHsp" style=""></span>IU/mL (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.028 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.005, respectively). Furthermore, a significant correlation was found between IVC dilation and CA125 levels ≥35<span class="elsevierStyleHsp" style=""></span>IU/mL (Cramer's V index, 0.242; OR, 2.880; 1.102–7.527) and the lack of collapse and CA125 levels ≥35<span class="elsevierStyleHsp" style=""></span>IU/mL (Cramer's V index, 0.310; OR, 4.091; 1.486–11.262).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">In terms of dividing the patients into the previously mentioned 4 subgroups, 58.9% of those with CA125 levels ≥35<span class="elsevierStyleHsp" style=""></span>IU/mL formed part of group 4. However, only 14.3% and 5.4% of patients with high CA125 levels had an inspiratory collapse ≥50% (groups 1 and 3). Of the patients with CA125 levels <35<span class="elsevierStyleHsp" style=""></span>IU/mL, 46.3% belonged to group 1.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Heart failure mortality</span><p id="par0090" class="elsevierStylePara elsevierViewall">Seventeen patients (20.2%) had died at 180<span class="elsevierStyleHsp" style=""></span>days of follow-up; 14 (80.3%) due to HF and 3 for other reasons (1 from stroke and 2 from septic shock). Ten (58.8%) of the 17 deaths occurred in the first month after discharge, and 8 (47%) occurring during the index hospitalization. Eleven of the 14 (78.6%) patients who died from HF had IVC dilation at admission, and in 85.7% of the cases, the collapse was <50%. In terms of the distribution according to the ultrasound patterns, 12 (70.6%) of the deaths belonged to group 4 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.112). There were no significant differences in overall mortality in terms of IVC dilation (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.156) or collapse (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.480).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Combined event of mortality and heart failure readmission</span><p id="par0095" class="elsevierStylePara elsevierViewall">Thirty-three patients (38.8%) had a combined event during the 180<span class="elsevierStyleHsp" style=""></span>days of follow-up. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the clinical, laboratory tests and hemodynamic characteristics based on whether the combined events occurred. In the univariate analysis, NT-proBNP levels (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.012), urea levels (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.02) and IVC inspiratory collapse <50% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.047) were associated with a poorer prognosis. Those patients with a lack of collapse and CA125 levels >35<span class="elsevierStyleHsp" style=""></span>IU/mL experienced events more frequently (46.7% vs. 29.7%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.04).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">After subdividing the patients into 4 groups according to the ultrasound results, it was shown that 57.6% of those with the combined event at 6 months belonged to group 4. The combined-event rates in each group were as follows: group 1, 19%; group 2, 47.1%; group 3, 40%; and group 4, 45.2% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.197). An IVC collapse <50% at admission was related to a greater likelihood of readmission or death due to HF at 180 days (45.8% if the collapse was <50% vs. 23.1% if the collapse was ≥50%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.048). In terms of dilation, there were no significant differences in the combined variable at 180<span class="elsevierStyleHsp" style=""></span>days (44.7% if IVC was dilated vs. 31.6% if IVC was undilated; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.218).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Survival analysis</span><p id="par0105" class="elsevierStylePara elsevierViewall">Shorter survival free of the combined event was found in those patients with IVC inspiratory collapse <50% (log-rank, 0.047) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>a). However, the time to readmission or death was not statistically shorter than in the isolated presence of IVC dilation (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>b) or overall after subdividing the ultrasound patterns into 4 groups. The paired analysis on the strata did show differences in terms of survival between group 1 on the one hand and groups 2 and 4 on the other (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>c).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">The results of this study confirm the importance of ultrasound assessment of the IVC at the bedside of patients with AHF,<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">7,8</span></a> as supplementary information to that obtained from the case history and physical examination. With this simple, safe, fast, repeatable and painless technique, we could determine that more than 50% of the patients with AHF showed signs of venous congestion through a dilated IVC. In a higher percentage, the inspiratory collapse index was less than 50%, which has important prognostic consequences. The ultrasound congestion data are correlated with the concentrations of a number of systemic congestion biomarkers, such as CA125. Based on our results, those patients without IVC inspiratory collapse more frequently present renal function impairment (higher urea, creatinine and cystatin values) and a poorer prognosis, which supports the role of systemic congestion in renal function impairment during HF exacerbation episodes.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">1,9–11</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">It has recently been suggested that venous congestion is one of the mechanisms that promote renal dysfunction in patients with AHF.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">10,12</span></a> Reduced renal flow due to reduced cardiac output is not the only pathophysiological determinant of renal impairment in AHF. Splanchnic congestion, with the subsequent increase in intraabdominal pressure, which causes renal compression and increased hydrostatic pressure in the Bowman capsule, is also a pathophysiological determinant.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">10,11,13</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">The baseline patient characteristics regarding sex, age, ejection fraction, HF etiology and hospital stay showed no significant differences in terms of the ultrasound IVC parameters. Clinical signs such as jugular venous distention and hepatomegaly are related to greater congestion shown through the IVC examination. Occasionally, the clinical examination by itself does not help in accurately estimating the degree of systemic congestion during AHF,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">14</span></a> a characteristic for which the ultrasound visualization of the IVC can help.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Among the accepted markers of congestion,<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">15–17</span></a> we analyzed the relationship between CA125 and venous congestion estimated through the IVC. When CA125 levels were within normal limits (0–35<span class="elsevierStyleHsp" style=""></span>IU/mL), the IVC showed no abnormalities in size or collapse and vice-versa. Furthermore, if the CA125 concentrations exceeded the normal threshold, less than 20% of the patients had a good inspiratory collapse of the IVC. In our study, we also observed that NT-proBNP concentrations were higher in the patients with uncollapsible IVC (whether dilated or not). Pellicore et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a> suggested that the ultrasound examination of the IVC provided similar information to that of natriuretic peptides, as a marker of cardiac dysfunction.</p><p id="par0130" class="elsevierStylePara elsevierViewall">In terms of the prognosis, the absence of inspiratory collapse of the IVC at admission, as a reflection of systemic congestion, was associated with an increased likelihood of death and readmission due to HF at 180<span class="elsevierStyleHsp" style=""></span>days. Similar studies have reinforced the relationship between congestion and the prognosis.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">18</span></a> In prognostic terms, the lack of IVC collapse appears to be more significant than the degree of dilation. Regardless of the IVC diameter, patients with a poorer prognosis were those with an inadequate collapse (groups 2 and 4), showing higher rates of the combined event at 180<span class="elsevierStyleHsp" style=""></span>days.</p><p id="par0135" class="elsevierStylePara elsevierViewall">For all of these reasons, we believe that this simple assessment of venous congestion is useful for patients with AHF. The repeated use of this technique during hospitalization could serve as a guide for monitoring fluid overload, as has been postulated for various patient profiles.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">8,19,20</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">The main limitations of this study are its small sample size and its performance in a single hospital center, which could compromise its external validity. Furthermore, the ultrasound results are based on a portable device with certain limitations for making specific measurements. Due to its characteristics, the V-SCAN device helps perform estimates, more than exact measurements. This fact could explain the results that indicate that the degree of venous collapse is more sensitive and is more related to the prognosis than the IVC diameter in absolute terms. Lastly, all patients were treated with intravenous diuretics at admission, but the intensity of this treatment depended on the physician responsible for the patient, who was blind to the ultrasound examination, which could have influenced the results of the initial ultrasound.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusions</span><p id="par0145" class="elsevierStylePara elsevierViewall">Ultrasound assessment of the IVC in the acute decompensation phase of HF is a simple and useful tool that helps to quickly and noninvasively stratify the prognosis of these patients in the first moments of medical care. The endpoints were mortality due to HF and the combined event of mortality and readmission for HF at 180 days.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Some 24.7% of the patients had an undilated IVC and ≥50% collapse (group 1); 20% had an undilated IVC and <50% collapse (group 2), 5.9% had a dilated IVC and ≥50% collapse (group 3); and 49.4% had a dilated IVC and <50% collapse (group 4). The lack of inspiratory collapse but not IVC dilation was related to higher concentrations of urea (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.007), creatinine (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.004), uric acid (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.008), NT-proBNP (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.009) and CA125 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.005). Las variables de valoración fueron la mortalidad por IC y el evento combinado de mortalidad y reingreso por IC a los 180<span class="elsevierStyleHsp" style=""></span>días.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">El 24,7% de los pacientes presentó una VCI no dilatada y colapso ≥50% (grupo 1); el 20% VCI no dilatada y colapso <50% (grupo 2); el 5,9% VCI dilatada y colapso ≥50% (grupo 3); el 49,4% VCI dilatada y colapso <50% (grupo 4).
