However, this possibility could be especially important, given that attendance in emergency departments in Spain is much higher than that in other countries.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">9</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to assess, in patients diagnosed with pulmonary tuberculosis, the frequency of emergency department visits during the 6 months prior to the diagnosis and the reasons for the visits. We also sought to determine whether the reasons were potentially related to this disease, the degree of suspicion in the emergency department and the factors associated with this suspicion, so as to suggest strategies for identifying these cases.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and method</span><p id="par0020" class="elsevierStylePara elsevierViewall">We conducted a retrospective study of all cases of pulmonary tuberculosis in adults (older than 14 years), with microbiological confirmation, in a health department (urban and coastal area with 164,000 inhabitants) based on the centralized registration of the Department of Epidemiology of the Public Health Center, during a 7-year period (from January 2011 to December 2017). The study was approved by the local clinical research ethics committee.</p><p id="par0025" class="elsevierStylePara elsevierViewall">For each case of tuberculosis, we reviewed the medical history to determine whether the patient had been admitted to the general emergency department in the 6 months prior to the diagnosis. If the patient had been treated in the emergency department, we reviewed the date, number of visits during that period, age, sex, country of origin, social-family situation (with data from the public health center), reason for the visit, history (tobacco use, alcoholism, human immunodeficiency virus infection, known contact with tuberculosis), symptoms, radiography findings, diagnostic suspicion and adopted measures. We considered that the visit could potentially be related to the tuberculosis (or its suspicion) when the patient presented any of the following findings: general symptoms of fever, asthenia and weight loss or respiratory symptoms of cough with expectoration and hemoptysis. We also considered this possibility if the chest radiography revealed any impairment not present in previous radiographs.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We conducted a descriptive study of the findings and an analysis of the differences between the group in which the condition was suspected and the group in which it was not suspected. The data are presented as numbers and percentages or mean and standard deviation, unless otherwise noted. We employed the chi-squared test for small samples<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">10</span></a> to compare the categorical variables and the Mann–Whitney <span class="elsevierStyleItalic">U</span> test to compare the continuous variables. Values of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 were considered statistically significant.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">During the study period, 54 adult patients were diagnosed with microbiologically confirmed pulmonary tuberculosis (mean annual incidence rate of 4.7 cases per 100,000 inhabitants). Of the 54 included patients, 38 (70%) had been treated for any reason in the emergency department in the 6 months prior to the diagnosis and were included in the study.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Of these 38 patients, 6 (16%) were treated for processes that were considered unrelated to pulmonary tuberculosis. The 6 patients consisted of 1 woman and 5 men, with a mean age (±SD) of 56<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17<span class="elsevierStyleHsp" style=""></span>years (range, 35–82), all of whom were Spanish and half of whom had associated social problems (lack of a permanent home, reclusive or user of illegal drugs). The mean time from the last visit prior to diagnosis to the tuberculosis diagnosis was 70<span class="elsevierStyleHsp" style=""></span>days (range, 1–132; median, 108 days). In these 6 cases, the reasons for the visit were (one diagnosis for each patient): contusion, acute urine retention, urinary tract infection, sore throat, proctorrhagia and inguinal hernia. Two of the patients underwent chest radiography, which showed no abnormalities. These 6 patients made 9 emergency department visits. The characteristics of the patients with a diagnosis of tuberculosis and their care are listed in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">For 32 patients, the symptoms during the visit or the radiological findings were considered related to the tuberculosis, according to the listed definitions. For 12 (38%) of these patients, the possibility of pulmonary tuberculosis was suspected in the emergency department (this possibility was recorded in the medical history, or a mycobacterial study was requested). These patients had visited the emergency department a total of 14 times (range, 1–2). The diagnoses at admission to or discharge from the emergency department for these patients were tuberculosis in 7 cases, pneumonia in 4 and hemoptysis in 1. Of the latter 5 patients, 2 were admitted with pneumonia and 1 with hemoptysis. The other 2 patients with pneumonia were readmitted to the Infectious Diseases office. In these 5 cases, the diagnosis was reached within one week of the emergency department visit.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In the remaining 20 patients (62%), the diagnosis was not suspected. These patients had visited 1–9 times in the 6 months prior to the diagnosis, although not always (when there was more than one visit) for reasons of infection or respiratory issues. Pneumonia was the most often established diagnosis.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The patients for whom tuberculosis was suspected had various characteristics that differentiated them from the other patients, such as a higher rate of general symptoms (83% vs. 45%, respectively) and respiratory symptoms (100% vs. 65%, respectively) and a higher rate of cavitation in the radiography. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> lists the differences between the two patient groups.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">The main results of this study are that, in our setting, a very high percentage of patients with pulmonary tuberculosis visited the emergency department in the months prior to the diagnosis and that this diagnostic possibility was not contemplated in many of the patients. These findings are important because suspecting pulmonary tuberculosis could have prompted an earlier start of therapy and stopped a possible chain of transmission. Moreover, precautionary measures could have been taken to avoid infecting emergency department personnel. Healthcare workers are known to be at significant risk of tuberculous infection, especially in departments with potentially greater contact with this microorganism<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">11,12</span></a> and where the need for prioritizing an early diagnosis has been indicated.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">11</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">In our study, 70% of the patients visited the emergency department for various reasons, compared with less than 50% of patients in series from English-speaking countries in studies similar to ours.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">3–8</span></a> This fact is probably not due to our patients’ symptom severity but rather to the fact that hospital emergency department visits in Spain are more frequent than in other countries.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">9</span></a> We analyzed a 6-month period prior to the diagnosis because it includes the potential variability in the duration of tuberculosis symptoms.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">1,2</span></a> However, other studies have analyzed periods between 3 and 12<span class="elsevierStyleHsp" style=""></span>months.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">3–8</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">In our series, the possibility of pulmonary tuberculosis was not suspected in more than half of the patients who showed symptoms that could already be related to this disease. This finding is not exceptional, given that series from other countries have reported a lack of diagnostic suspicion in many cases<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">3–8</span></a> and have concluded that although the diagnosis is more likely for patients with typical symptoms,<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a> it is difficult to propose criteria for detecting this disease due to the heterogeneity of the clinical presentations.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">This diagnostic difficulty can be attributed to 2 main factors: first, this disease can present with a broad clinical and radiological spectrum<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">13</span></a>; second, the process has a low incidence in our setting. In our area, the incidence rate of pulmonary tuberculosis was 4.7 cases per 100,000 inhabitants, a much lower rate than that of Spain as a whole (8.5) and that of the Community of Valencia (6.9).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">1</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">In a systematic review of the diagnostic delay for tuberculosis in the general population, the main factors associated with this delay were HIV infection, chronic respiratory symptoms, advanced age, the female sex, alcoholism and abuse of other toxins.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">2</span></a> In our series, none of these data were associated with a lower diagnostic suspicion of tuberculosis; however, a series in the United States reported that patients with social marginalization problems or who used illegal drugs had a lower probability of establishing the tuberculosis diagnosis in the emergency department, despite having respiratory symptoms.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">Our experience shows that, as with other countries, many patients with tuberculosis are admitted to the emergency department without the diagnosis being suspected, with the risk that entails. These data demonstrate the need to increase the diagnostic suspicion of tuberculosis even in areas with a low prevalence. The small number of patients who constituted the study population in our series limits our ability to identify characteristics that differentiate patients in whom tuberculosis is suspected. Pulmonary tuberculosis was suspected in the emergency department for 12 (38%) of these patients and not suspected in the remaining 20 patients (62%).
