Osteoporosis therefore needs to be detected in these patients to prevent the onset of fractures.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Syndesmophytes, characteristic lesions of axial spondyloarthritis (axSpA), are ossifications of the external fibers of the annulus fibrosus that form intervertebral bone bridges that can extend throughout the spine, affecting the anterior longitudinal ligament and adjacent paravertebral tissue. The presence of syndesmophytes appears to affect bone mineral density (BMD) values at the lumbar level, resulting in falsely high values, which results in diagnostic limitations for dual-energy X-ray absorptiometry (DXA) (the gold standard technique for assessing BMD in clinical practice), limitations that are mostly related to the projection employed.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,4</span></a> DXA in the femoral neck has shown to be a reliable technique for assessing BMD in patients with axSpA, regardless of the presence or absence of syndesmophytes.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">5</span></a> However, some patients wear hip protheses as a complication of their disease, which hinders the BMD assessment in this location.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The trabecular bone score (TBS) was recently developed as a texture parameter to evaluate grayscale variations in pixels of DXA images of the lumbar spine.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">6</span></a> This method provides an indirect index of the trabecular bone microarchitecture, and its values do not appear to be falsified by the presence of degenerative changes or syndesmophytes at the lumbar level.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">7–11</span></a> Our study's objective was therefore to analyze the influence of syndesmophytes on lumbar TBS and BMD values for the lumbar and femoral neck and to assess the clinical and laboratory factors associated with TBS in spondyloarthritis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Method</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Population</span><p id="par0020" class="elsevierStylePara elsevierViewall">From January 2017 to June 2018, we conducted a cross-sectional study that consecutively included patients older than 18 years diagnosed with axSpA, according to the 2009 Assessment in Ankylosing Spondylitis (ASAS) criteria,<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">12</span></a> and who underwent an osteoporosis study with DXA and a radiological study of the dorsal spine and lateral lumbar region. These patients were recruited during specialized spondyloarthritis consultations at the University Hospital Virgen de la Arrixaca (Murcia, Spain). We excluded patients diagnosed with a metabolic bone disease other than osteoporosis, those who underwent a surgical procedure in the lumbar spine, those with chronic renal failure and those with a body mass index (BMI) >37<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The study protocol was approved by the ethics committee of Hospital Virgen de la Arrixaca, and the study was conducted in accordance with the principles of the Declaration of Helsinki.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Clinical–demographic variables</span><p id="par0030" class="elsevierStylePara elsevierViewall">We recorded the patients’ age, sex, BMI, arterial hypertension, diabetes mellitus, early menopause, tobacco and alcohol habits, previous frailty fracture, lumbar and femoral DXA values, use of vitamin D supplements and treatment for osteoporosis.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Disease-related variables</span><p id="par0035" class="elsevierStylePara elsevierViewall">We recorded the disease duration, disease activity (evaluated using the Bath Ankylosing Spondylitis Disease Activity Index [BASDAI] and the Ankylosing Spondylitis Disease Activity Score with C-reactive Protein [ASDAS-PCR]), the presence of syndesmophytes at the lumbar level and treatment.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Laboratory variables</span><p id="par0040" class="elsevierStylePara elsevierViewall">We analyzed the typical biochemical parameters using standardized techniques. The calcium, phosphorus and C-reactive protein (CRP) readings are expressed in mg/dL, with normal CRP values <0.05<span class="elsevierStyleHsp" style=""></span>mg/dL. The erythrocyte sedimentation rate (ESR) values are expressed in mm/h, with normal ESR values <20<span class="elsevierStyleHsp" style=""></span>mm/h. The calciotropic hormone concentration was measured by high-performance liquid chromatography for 25-OH vitamin D and by radioimmunoassay for intact parathormone (PTH). Vitamin D and PTH levels are expressed in ng/mL and pg/mL, respectively.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Bone mineral density and trabecular bone score variables</span><p id="par0045" class="elsevierStylePara elsevierViewall">BMD was measured in the lumbar spine in posterior–anterior projection (L1–L4) and in the right hip using DXA (Hologic Discovery QDR Wi S7N88240. Software version 13.5.2.1). The measures were taken by expert technicians, and calibrations were performed daily with phantoms before starting the examinations. BMD is defined as the bone mineral content divided by the explored area expressed in grams per square centimeter (g/cm<span class="elsevierStyleSup">2</span>). For the men older than 50 years and the postmenopausal women, we employed the <span class="elsevierStyleItalic">T</span>-score to classify the DXA results as normal, osteopenia and osteoporosis. For the rest of the cases, we employed the <span class="elsevierStyleItalic">Z</span>-score, considering a <span class="elsevierStyleItalic">Z</span>-score<span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>−2 as low bone mass.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">13</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">We applied TBS iNsight Clinical Data Analyzer software (version 3.1.2.0, Medimaps) on the DXA images and calculated the mean TBS in L1–L4. The TBS (expressed in arbitrary units) were divided into 3 fracture risk groups: ≥1.310, low risk; 1.230–1.310, intermediate risk; and <1.230, high risk.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Radiological evaluation</span><p id="par0055" class="elsevierStylePara elsevierViewall">To verify the presence of syndesmophytes, we assessed each space of the intervertebral disk from D12-L1 to L5-S1 using simple radiology of the lumbar spine in lateral projection. We recorded both the presence of syndesmophytes and the number of levels with syndesmophytes.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">15</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">To assess the radiological damage in sacroiliac joints, we employed the modified New York criteria.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">16</span></a> We employed the mean score for both sides in the analysis.</p><p id="par0065" class="elsevierStylePara elsevierViewall">To assess the presence of vertebral fractures, we assessed the vertebral bodies from D4 to L5 using simple radiology of the dorsal and lumbar spine in lateral projection, considering a >25% reduction in the vertebral body as a fracture.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">The descriptive analysis of the quantitative variables is presented as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation (SD) for the quantitative variables and as percentages for the qualitative variables.</p><p id="par0075" class="elsevierStylePara elsevierViewall">We employed the chi-squared test to study the relationship between qualitative variables, Student's <span class="elsevierStyleItalic">t</span>-test for independent samples to compare means between 2 groups and ANOVA for the comparison between 3 or more groups after verifying the assumptions of normality with the Shapiro–Wilk test and assumptions of homogeneity of variances with the Levene test. In the cases in which the assumptions of normality were not met, we applied nonparametric tests. To determine the possible relationship between variables, we calculated Pearson's correlation coefficient (<span class="elsevierStyleItalic">r</span>). We employed the linear regression model to determine whether the demographic variables, the disease-related variables and those related to the characteristics of the bone metabolism showed a significant effect on the TBS. A <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 was considered significant.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Results</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Clinical–demographic variables</span><p id="par0080" class="elsevierStylePara elsevierViewall">The study included 82 patients with axSpA (38 with syndesmophytes at the lumbar level and 44 without syndesmophytes). The mean age was 44.36<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11.85 years for the group without syndesmophytes and 55.7<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.27 years for the group with syndesmophytes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). Some 43.18% of the patients without syndesmophytes were women, compared with 5.26% of the patients with syndesmophytes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), and the mean number of levels affected by syndesmophytes was 3.21<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.27.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">1–6</span></a> There were no differences in disease activity measured with BASDAI and ASDAS-PCR between the two groups. The rest of the demographic and clinical characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Assessment of the trabecular bone score and bone mineral density</span><p id="par0085" class="elsevierStylePara elsevierViewall">The TBS were significantly lower for the patients with syndesmophytes than for those who had no syndesmophytes (1.345<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.14 vs. 1.412<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.112, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.006). Similarly, the BMD values in the femoral neck were lower in the group with syndesmophytes, although statistical significance was not reached (0.781<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.132 vs. 0.822<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.169<span class="elsevierStyleHsp" style=""></span>g/cm<span class="elsevierStyleSup">2</span>, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.249). However, the BMD values in the lumbar spine were significantly higher compared with the group that had no syndesmophytes (1.103<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.214 vs. 0.974<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.143, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p><p id="par0090" class="elsevierStylePara elsevierViewall">Some 2.63% of the patients with syndesmophytes presented a diagnosis of osteoporosis according to the DXA in the lumbar spine and/or femoral neck, while the diagnosis of osteoporosis increased 11.36% in the axSpA group without syndesmophytes. Considering the TBS, we found that 15.78% of the patients with axSpA and syndesmophytes had a high risk of fracture, while only 4.54% of the group without syndesmophytes had a high risk.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Comparison between clinical data, inflammatory markers and disease activity in the risk groups of the trabecular bone score</span><p id="par0095" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a> show the demographic, clinical and disease-related characteristics and laboratory parameters of the patients with and without syndesmophytes, according to the TBS facture risk groups, respectively.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">The factors associated with a greater risk of fracture were longer disease duration, higher number of levels affected by syndesmophytes, higher ESR and larger BMI.</p><p id="par0105" class="elsevierStylePara elsevierViewall">There were no differences in terms of disease activity or CRP.</p><p id="par0110" class="elsevierStylePara elsevierViewall">To identify the potential risk factors associated with a low TBS, we performed a linear regression. Considering the axSpA group as a whole, including patients with and without syndesmophytes at the lumbar level, the univariate analysis showed a significant effect with ESR, the degree of sacroiliitis in simple radiology, the presence of syndesmophytes and age, while the multivariate analysis showed an association with age and femoral neck BMD. Thus, greater age, ESR levels, degree of sacroiliitis and presence of syndesmophytes, as well as lower BMD values in the femoral neck were associated with lower TBS.</p></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">Our study found lower TBS in the patients with syndesmophytes compared with those who did not have syndesmophytes.</p><p id="par0120" class="elsevierStylePara elsevierViewall">In axSpA, an increase in activity of 2 opposing bone remodeling processes are produced in the spine as a response to the inflammatory process. On one hand, there is the formation of new bone at the cortical level; on the other, there is an excessive loss of trabecular bone, which results in osteoporosis and an increased risk of vertebral fractures.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">DXA, the gold standard technique for diagnosing osteoporosis, is unable to differentiate between the trabecular and cortical bone compartment and therefore cannot show the bone microarchitecture abnormalities that occur in these patients. In addition, syndesmophytes and the ossification of longitudinal and interspinous ligaments can falsely raise DXA values.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,4</span></a> Our data confirm the results of previous studies that demonstrated that TBS does not appear to be masked by the presence of syndesmophytes, while lumbar BMD values do appear to be falsely raised.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">10,11</span></a> In fact, the patients with syndesmophytes presented a lower TBS and a higher rate of high fracture risk measured by TBS compared with those who did not have syndesmophytes. This result was expected given that these patients had greater structural damage and longer disease duration, which, in these patients, are risk factors for osteoporosis and fracture<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">18–24</span></a> and have been associated with low TBS.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">10,25</span></a> The lumbar BMD values were higher in the group with syndesmophytes, which supports the assumption that DXA is not an appropriate technique for diagnosing osteoporosis in this subgroup of patients with axSpA. The fact that syndesmophytes do not affect the TBS results but do affect lumbar BMD values could be due to the fact that TBS is determined by local variations in the gray levels and not by its absolute levels. TBS can therefore be useful for assessing bone quality and predicting fracture risk in patients with axSpA with syndesmophytes at the lumbar level.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Our study found an association between a higher BMI, ESR, disease duration and number of levels affected by syndesmophytes and having a greater risk of fracture measured by TBS. These findings are consistent with the risk factors described for fractures in patients with axSpA. In terms of BMI, there is a contradiction between the fact that a greater BMI is associated with a greater fracture risk when an association has been reported between low BMI and low BMD. However, a greater BMI could be reflecting patients with greater radiographic damage and, as a result, with greater functional disability and less physical activity, which have been linked to low BMD and fracture risk in these patients.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">18,21,23,26</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Previous studies have reported that a greater degree of sacroiliitis, radiological damage in the spine and higher ESRs and CRP levels are risk factors for low TBS in patients with axSpA,<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">10,27</span></a> which we confirmed in our study. Our study also found that the presence of syndesmophytes was a TBS risk factor.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The study's limitations include the low prevalence of osteoporosis and vertebral fractures in our population compared with that reported for this group of diseases. However, TBS was able to detect a larger percentage of patients with a high fracture risk than DXA.
