It is estimated that approximately 20–25% of all hospitalized adult patients have diabetes and that the disease could be a determinant for hospital readmission.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Hospital readmissions are typically classified as early (readmissions within 30 days of the discharge) and late (readmissions 30–90 days after the discharge). The early readmission rate for internal medicine is approximately 12.4%.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">3</span></a> In studies on American Medicare patients, the readmission rate at 90 days is 34%.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">4</span></a> Hospital readmissions are typically related to disease severity, the presence of coexisting diseases, age, socioeconomic level and the delivery of suboptimal care after discharge.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">5–8</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Most studies that have analyzed hospital readmissions have focused on 3 very common diseases: heart failure, acute myocardial infarction and pneumonia.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">5,9,10</span></a> However, the determinants of hospital readmission for patients with type 2 diabetes are less known. The determinants most frequently analyzed in patients with diabetes mellitus have been the degree of glycemic control,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">11</span></a> the impact of hypoglycemia<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">12</span></a> and the presence of heart failure.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">13,14</span></a> However, readmissions could also be influenced by the degree of comorbidity, functional capacity and the type of antihyperglycemic treatment.</p><p id="par0020" class="elsevierStylePara elsevierViewall">For this reason, we conducted the present study to examine 2 aspects: the rehospitalization rate at 90 days for patients with type 2 diabetes and the determinants of rehospitalization in a broad group of patients with diabetes mellitus hospitalized in internal medicine.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design</span><p id="par0025" class="elsevierStylePara elsevierViewall">This was a retrospective cohort study. On a chosen day, we recorded the number of patients with hyperglycemia or diabetes hospitalized in internal medicine departments and how many of the patients had been hospitalized in the past 3 months.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study population</span><p id="par0030" class="elsevierStylePara elsevierViewall">The data source consisted of information from 3 cross-sectional studies conducted in 2015, 2016 and 2017 in hospitals belonging to the National Health System of Spain. Information was available from 1977 patients. These cross-sectional studies collected information during the course of a day on the size of the participating hospital, the number of patients hospitalized in the internal medicine department, the number of hospitalized patients with diabetes or hyperglycemia, the reason for the hospitalization, previous hospitalizations in the past 90 days, hospital stay up to the day of the study, demographic characteristics, comorbidity, degree of dependence, laboratory data, antihyperglycemic treatment, use of hyperglycemia-producing treatments (treatment with systemic glucocorticoids), use of enteral or parenteral nutrition, onset of hypoglycemia or extreme hyperglycemia over the course of the hospitalization and capillary blood glucose levels on the day of the study.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The patients were considered to present diabetes mellitus if their medical history showed this diagnosis or if they met the diagnostic criteria for diabetes mellitus according to the American Diabetes Association.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">15</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Response variable</span><p id="par0040" class="elsevierStylePara elsevierViewall">For the purposes of the study, the patients were classified into 2 groups: hospital readmission (if the patient had been hospitalized in the past 90 days) and no hospital readmission (if the patient had not been hospitalized in the past 90 days).</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Explanatory variables</span><p id="par0045" class="elsevierStylePara elsevierViewall">The explanatory variables included age, sex, reason for hospitalization, type of diabetes, comorbidity measured by the Charlson score,<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">16</span></a> degree of dependence as measured by the modified Rankin scale,<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">17</span></a> cognitive impairment as measured by the Global Deterioration Scale,<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">18</span></a> type of diabetes treatment prior to the hospital admission, degree of glycemic control as measured by the hemoglobin A1c level, glomerular filtration rate measured by the Chronic Kidney Disease Epidemiology Collaboration equation,<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">19</span></a> presence of hypoglycemia (glucose level <70<span class="elsevierStyleHsp" style=""></span>mg/dL) and the presence of extreme hyperglycemia (glucose level >350<span class="elsevierStyleHsp" style=""></span>mg/dL) during the hospitalization.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">The descriptive statistical analysis included measurements of frequency and percentages for categorical variables and mean and standard deviation for continuous variables. To compare the continuous variables between the patients with readmission at 90 days and those with no readmission, we used Student's <span class="elsevierStyleItalic">t</span>-test. To compare the categorical variables between the patients who were readmitted within 90 days and those who were not readmitted, we employed the chi-squared test and Fisher's test, when there were fewer than 5 observations. Two-tailed <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 were considered statistically significant.</p><p id="par0055" class="elsevierStylePara elsevierViewall">To perform the predictive model, we selected 66.6% of the sample (derivation cohort). We considered the hospital readmission within 90 days of the discharge as the response variable. The explanatory variables were selected among those whose value in the univariate analysis had a <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.1. The model's goodness of fit was measured with the Hosmer–Lemeshow test. The predictive model's explanatory capacity was measured with Nagelkerke's <span class="elsevierStyleItalic">R</span>-squared, and the final model's diagnostic performance was assessed by constructing the receiver operating characteristic (ROC) and calculating the area under the curve (AUC). An AUC of 1 represents a perfect diagnostic test for differentiating patients who are readmitted from those who will not need to be readmitted; an AUC of 0.5 represents a test with no diagnostic value.</p><p id="par0060" class="elsevierStylePara elsevierViewall">We confirmed the model's validity by applying the parameters at 33.3% of the remaining sample (validation cohort) and calculating the ROC curve and AUC.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The statistical analysis was performed with the statistical package SPSS, version 24.0, Armonk, NY; IBM Corp.).</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Incidence and causes of readmission</span><p id="par0070" class="elsevierStylePara elsevierViewall">During the study period, there were 704 (35.6%) readmissions in the first 90 days after the discharge out of a total of 1977 patients included in the study. The readmission rates did not significantly vary between 2015 and 2017. The hospitals with more than 500 beds showed significantly (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.02) higher readmission rates than those with fewer beds (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The main reasons for the hospital readmissions were infectious diseases (29%), cardiovascular diseases (24.4%) and respiratory diseases (14.2%) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Characteristics of the patients who were readmitted</span><p id="par0075" class="elsevierStylePara elsevierViewall">The patient characteristics that were associated in a statistically significant manner with hospital readmission at 90 days were an older age range, the male sex, a high Charlson comorbidity index, a high disability index as measured by the Rankin scale and higher cognitive impairment according to the Global Deterioration Scale. In terms of the variables related to diabetes control and the glycemic profile, the patients who were readmitted had a greater rate of hypoglycemia in the 3 previous months, were treated to a greater degree with basal insulin and rapid insulin and had greater renal impairment compared with the patients who were not readmitted. Likewise, a higher percentage of the patients who were readmitted were undergoing systemic glucocorticoid treatment compared with those who were not readmitted (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Predictive model for hospital readmission</span><p id="par0080" class="elsevierStylePara elsevierViewall">The variables that had a greater association with hospital readmission at 90 days in the multivariate analysis were the Charlson comorbidity index, the Rankin disability index, age, history of hypoglycemic episodes in the past 3 months, the use of basal insulin and glucocorticoid treatment (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). After confirming that the presence of interactions did not increase the model's precision, these interactions were not considered. The Hosmer–Lemeshow goodness of fit test showed a <span class="elsevierStyleItalic">p</span>-value of 0.190, which allows for this model to be used in making predictions. The Nagelkerke's <span class="elsevierStyleItalic">R</span>-squared value was 0.109. The AUC of the ROC was 0.676 (95% CI 0.642–0.709; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001). When the predictive model's parameters were applied to the validation cohort, the AUC was 0.661 (95% CI 0.612–0.710; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">In this study, we analyzed the frequency, main diagnoses and determinants of patients with diabetes mellitus who are readmitted to hospital within 90 days after the discharge.</p><p id="par0090" class="elsevierStylePara elsevierViewall">We have been able to confirm that the proportion of patients who were readmitted remained stable over the past 3 years, with an approximate rate of 35%. The main causes of readmission were infectious diseases, cardiovascular diseases and respiratory diseases. The diabetes-related readmissions, such as hypoglycemia, hyperosmolar coma and diabetic ketoacidosis, constituted only 2% of the causes of readmission. The multivariate analysis showed that the determinants of readmission were an age greater than 65 years, a higher index of dependence and comorbidity, the presence of hypoglycemic episodes, treatment with systemic glucocorticoids and the use of secretagogues or insulin. The multivariate model for hospital readmission had a moderate predictive capacity, with an AUC of 0.68 in the derivation cohort and 0.66 in the validation cohort. In essence, we have shown that treating diabetes with certain drugs is an independent cause of hospitalization within 90 days after the discharge.</p><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Comparison with the data from the literature</span><p id="par0095" class="elsevierStylePara elsevierViewall">There have been few publications that have examined the determinants of hospital readmission for patients with diabetes mellitus. A case-control study identified the following possible determinants of readmission at 30 days of discharge: intensification of the diabetes treatment, the presence of congestive heart failure and the number of interconsultations the patient received during their hospitalization. The analyzed determinants had a modest predictive capacity, measured by an AUC of the ROC of 0.693.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">20</span></a> Collins et al. assessed another model for early readmission at 30 days of discharge for patients with type 2 diabetes.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">21</span></a> The significant variables in the model were the patient's age, male sex, the number of previous visits to the emergency department and the hospital stay. The model showed a robust predictive capacity, with an AUC of 0.82. However, the model's predictive capacity was not verified in an independent patient cohort. Other models of hospital readmission specifically for patients with diabetes mellitus included the measurement of glycated hemoglobin. Regardless of the glycated hemoglobin value, the mere fact of having this information was significantly associated with a reduction in readmission rates, although this measurement was only requested in 18% of the episodes.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">11</span></a> Having the glycated hemoglobin result enables adjustments in the treatment during the hospitalization and at discharge. In previous studies conducted by our group, the proportion of patients with recent glycated hemoglobin values was typically 50–60%.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">22</span></a> In our analysis, the glycated hemoglobin reading did not add discriminative power to the predictive model.</p><p id="par0100" class="elsevierStylePara elsevierViewall">The predictive models that assess the readmission of patients older than 65 years in the United States show modest degrees of accuracy. A study conducted in California showed that the payment system defined by the type of chronic disease and degree of disability was the most robust determinant of a model that had a predictive value measured by an AUC curve of 0.71.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">23</span></a> In Canada, the LACE index has been defined as a good tool for predicting the expected risk of readmission and mortality at 30 days. The components of the LACE index are the length of the hospital stay, the emergency nature of the hospitalization, the Charlson comorbidity score and the number of previous emergency department visits.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">24</span></a> The LACE score ranges from 0 (2.0% of expected risk) to 19 points (43.7% of expected risk). A score greater than 10 is considered discriminative for predicting hospital readmission. In the original publication, the AUC of the LACE index was 0.684. This index has since been applied to other patient cohorts from other countries and has shown a highly variable discriminative capacity, with an AUC of the ROC between 0.57 and 0.71.<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">25–28</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">Among our study's limitations, it is worth noting that the model had a relatively modest predictive capacity. However, the AUC of our model was similar to the more commonly used predictive models, such as LACE. The predictive model included clinical factors but did not consider societal or logistical factors. The societal and logistical factors classically associated with hospital readmission have been hospitalizations in the previous 6–12 months,<a class="elsevierStyleCrossRefs" href="#bib0325"><span class="elsevierStyleSup">29,30</span></a> being black,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">7</span></a> low health knowledge,<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">31</span></a> living alone or with no family or social support<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">32</span></a> and belonging to a low socioeconomic level.<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">33,34</span></a> Unfortunately, the social factors are rarely modifiable for preventing future readmissions. An additional limitation was that our model only analyzed hospital readmission of patients with diabetes at 90 days of discharge. A considerable majority of predictive models analyze the percentage of readmissions at 30 days of discharge; however, in health department research studies, other time limits are used to measured readmissions, such as 90 days and 1 year from the discharge. In our case, we chose a limit of 90 days to have a period that offered a sufficient number of outcomes to develop the predictive derivation model and predictive validation model.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The usefulness of our study is that it enables clinicians to identify a subgroup of patients who need special care to prevent hospital readmission. In this frail population, identified by the predictive model, the implementation of integrated programs that include educational aspects for the patient and main caregiver and the establishment of glycemic control objectives adapted to the patient's quality of life and life expectancy can help reduce readmissions. In other areas, the use of integrated programs has helped reduce the readmission rate of patients with diabetes and amputation due to peripheral arterial disease.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">35</span></a> Other programs that have been effective in reducing readmissions have focused on providing medical-surgical units with nursing staff trained in diabetes to improve the care of patients with hyperglycemia during their hospital stay and improve the patient's transition at discharge.<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">36</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">In conclusion, our predictive model helps identify the frail population with diabetes admitted to internal medicine departments. This population requires additional strategies to reduce the possibility of hospital readmission in the subsequent 90 days. These strategies should entail reduced dosages of secretagogues or insulin and controlling comorbidity, with a check-up preferably in consultations.</p></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Funding</span><p id="par0120" class="elsevierStylePara elsevierViewall">This study was implemented with assistance from Novo-Nordisk laboratories.
