IE has a high mortality, and the therapeutic recommendations are based mainly on expert opinion.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a> Although there are recommendations from the European Society of Cardiology (ESC), approved by the International Society of Chemotherapy for Infection and Cancer (ISC) and the European Society of Clinical Microbiology and Infectious Diseases (ESCMID), the antimicrobial regimens employed by the various specialists involved in treating the disease are heterogeneous. The aim of this study was to determine whether adherence to the antibiotic treatment recommended by the ESC guidelines is related or not to a reduction in the mortality and in-hospital complications of the disease in a university hospital center.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">Between January 1, 2005 and March 30, 2014, all cases of IE consecutively diagnosed in the University Hospital Nuestra Señora de la Candelaria (Tenerife) were included. Data were systematically and prospectively collected and entered into an anonymous database. The hospital's reference population is approximately 600,000 inhabitants, living in Tenerife, La Gomera and El Hierro. For all patients, the disease severity was assessed using the Simplified Acute Physiologic Score-II (SAPS II),<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">2</span></a> and the comorbidity was measured using the Charlson index.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">3</span></a> The surgical risk was calculated using the EuroSCORE II scale.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a> We defined acute renal failure according to the RIFLE criteria.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">5</span></a> For the diagnosis of cerebrovascular complications or peripheral embolism, a confirmation imaging test was required.</p><p id="par0015" class="elsevierStylePara elsevierViewall">A cardiologist with experience in managing the disease, who was blind to the patients’ personal information and the physicians who had treated them, checked whether the antibiotic treatment employed (once the germ and antibiogram had been identified) complied or not with the current ESC recommendations (published in 2004 and 2009) at the time of diagnosis.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">1,6</span></a> We excluded those cases in which the microorganism was not identified or whose antibiotic treatment was not included in the guidelines.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The follow-up period for detecting events was the hospitalization period, and events to compare among the treatment groups were death, the development of heart or renal failure, the onset of embolisms and valve annulus involvement.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The study was approved by the center's ethics committee and respected the current regulations on the use of patient data.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0030" class="elsevierStylePara elsevierViewall">We employed the Kolmogorov–Smirnov test to determine whether the continuous variables followed a normal distribution. These variables are presented as median (interquartile range) or as mean (standard deviation) as appropriate. The categorical data are shown as absolute numbers (percentages).</p><p id="par0035" class="elsevierStylePara elsevierViewall">To compare the differences among the patient groups, we employed the chi-squared test for categorical variables and the Mann–Whitney <span class="elsevierStyleItalic">U</span> test or Student's <span class="elsevierStyleItalic">t</span>-test for continuous variables.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The propensity score was calculated using binary logistic regression, using the definitive treatment as the dependent variable. As covariates, we used age, SAPS II, prosthetic involvement, annular complications, infection by <span class="elsevierStyleItalic">Staphylococcus aureus</span>, the development of heart or kidney failure and the need for surgery. After obtaining the probability or propensity score, we applied the pairing method to balance the distributions of the two groups and homogenize them. To this end, we decided on a pairing ratio of 1:2. Furthermore, we applied the nearest neighbor pairing criterion. All statistical analyses were performed with the SPSS program (version 21) and PS Matching software.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">During the study period, 162 cases of IE were diagnosed. Some 95.6% of the cases met the modified Duke criteria<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">7</span></a> for definitive IE. In 31 of the cases, the microorganism was not identified (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>15), death occurred before getting the results of the blood cultures or the causal agent was not covered in the guidelines (<span class="elsevierStyleItalic">Pseudomonas</span> spp., <span class="elsevierStyleItalic">Enterobacter cloacae</span>, <span class="elsevierStyleItalic">Escherichia coli</span>, <span class="elsevierStyleItalic">Bacillus</span> spp. and <span class="elsevierStyleItalic">Lactobacillus</span> spp.).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Ultimately, the study group included 131 patients. Of these, the treatment administered to 85 (64.9%) patients followed the ESC recommendations. In the remaining 46 patients, the reasons for not following the recommendations were the use of daptomycin (14 cases), vancomycin in the absence of resistance (nine cases), teicoplanin (five cases) and other antibiotics such as levofloxacin, imipenem and meropenem. In 37 cases (20 of them in the treatment group that adhered to the guidelines), the decision was made to modify the antimicrobial employed during the course of the disease due to the results of the antibiogram, mainly because of persistent fever (17 cases) or a suspected adverse reaction (five cases).</p><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the baseline characteristics of the two treatment groups. There were no differences in terms of the degree of comorbidity, location or severity of the infection, surgical treatment or risk estimated by EuroSCORE II. There were a higher proportion of cases of <span class="elsevierStyleItalic">Streptococcus viridans</span> and a lower proportion of <span class="elsevierStyleItalic">Staphylococcus aureus</span> in the treatment group that adhered to the guidelines, although the differences were not statistically significant.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> lists the complications during the follow-up. After applying the pairing based on the propensity score, the hospital mortality was 29.2% when the treatment adhered to the guidelines and 28.2% when it did not (OR, 1.048; 95% CI 0.442–2.484; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.916). Sepsis was the most common cause of death in both groups (11 [42.3%] and seven patients [46.7%]). The proportion of recurrence during the study period was similar in the two groups (two patients in each group).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">In the analyzed patient series, mortality, in-hospital complications and the need for surgery were not more frequent among the patients with IE who were applied alternative antibiotic therapy regimens to those proposed by the ESC. Nevertheless, the treatment was adjusted according to the antibiogram, and the patients were treated by personnel with experience in this disease. These aspects could justify the lack of differences in the results between the two patient groups, a fact that should be emphasized given that the improper use of antibiotics has been associated with an increased probability of hospital death due to EI.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> In the treatment group that did not adhere to the guidelines, <span class="elsevierStyleItalic">Staphylococcus aureus</span> was predominant, and the use of antibiotics such as daptomycin, teicoplanin and vancomycin (in the absence of allergies or resistance) was common. There are several reasons for employing antibiotherapy different from the recommended ones. Firstly, the indication for antibiotic treatment in IE is based on a low-grade evidence.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">1</span></a> There are alternative treatments, which are likely as effective when used correctly and according to the microorganism's resistance pattern. Additionally, changes in antibiotherapy based on the patient's progression are common in clinical practice. All of this creates a tendency toward variability in the final selection of the antibiotic employed.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Despite the apparent lack of differences in the results of the patients treated outside the guidelines’ recommendations, we should consider that the follow-up period was brief, and there might be other adverse effects that are more difficult to detect but no less significant. The use of recently incorporated antibiotics can represent a greater financial cost when compared with less expensive and equally effective treatments, such as those recommended by scientific societies, such that the final net clinical benefit might not differ but the cost does. This issue requires a pharmacoeconomic analysis in order to provide an answer, which is beyond the objectives of our study. Lastly, we should consider the possible “ecological” impact caused by the emergence of multiresistant microorganisms, as a possible adverse effect from not adhering to the guidelines. In fact, the emergence of resistances has necessitated the implementation of national consensus protocolization programs on the use of antibiotics,<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a> an especially pressing problem in critical care units, which require the involvement of all medical personnel.<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">10,11</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The main limitations of our study are the small number of cases, the single-center origin and the inherent limitations of a retrospective analysis, which we have attempted to compensate for with the use of the propensity score.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Our results should not be interpreted in favor of the use of alternative antibiotics to those recommended in the guidelines. Quite the opposite, following the recommended regimens in these guidelines is as effective as the other regimens based on recently incorporated antibiotics, but their financial and microbiological cost is probably lower.
