The diagnosis of biclonal gammopathy involves determining the presence of 2 different electrophoretic bands, which can be composed of equal or different combinations of heavy and light chains. It is debatable whether the production of these paraproteins is due to the existence of a single clone of B-lymphoid cells, which subsequently diversifies into 2 independent clones after a process of antigenic selection,<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> or is instead due to the presence of 2 different clones at the start of the tumor transformation.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Although the production of paraproteins is characteristically related to the dyscrasias of plasma cells, it can be associated with other neoplasms, especially the B lineage lymphoids, and to a lesser degree with other blood components and solid organ cells. Its association with infectious diseases has also been reported, especially in relation to the hepatitis C virus (HCV) and autoimmune diseases, including Sjögren's syndrome and cryoglobulinemia.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–6</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">There are few series of patients with BG,<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–10</span></a> and to date there are none in the Spanish medical literature.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The aim of this study is to analyze the associations and characteristics of paraproteins and, in potential cases, the outcomes of patients with biclonal gammopathy studied at our center.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patients and methods</span><p id="par0050" class="elsevierStylePara elsevierViewall">We reviewed the records extracted from the file on monoclonal gammopathy at the Department of Immunology of Hospital Puerta de Hierro (Majadahonda, Spain), as well as the patients’ medical records between 1970 and 2011. Due to the study's retrospective nature, patients were accepted with a single initial reading.</p><p id="par0055" class="elsevierStylePara elsevierViewall">To analyze the patients’ serum samples, we employed the electrophoresis technique using cellulose acetate gel or agarose gel quantified by a densitometer. The concentration of circulating immunoglobulins was measured by nephelometry; the monoclonal component was not quantified.</p><p id="par0060" class="elsevierStylePara elsevierViewall">To identify the type of immunoglobulin and the Bence–Jones protein, we used the immunoelectrophoresis (Kallestad Lab, 1970–2000) or immunofixation (Sebia, 2000–2011) technique.</p><p id="par0065" class="elsevierStylePara elsevierViewall">From the medical records review, we obtained epidemiological data, such as age and sex, and information on the underlying disease and/or associated diseases, the type of paraprotein, the quantification of immunoglobulins and the treatment the patients had undergone. Lastly, we analyzed the clinical evolution and the evolution of the paraprotein of patients who underwent a second reading.</p><p id="par0070" class="elsevierStylePara elsevierViewall">We decided to group the clinical entities into 3 subgroups: biclonal gammopathy of undetermined significance (BGUS); multiple myeloma (MM), a group that included a patient with extramedullary plasmocytoma; and other malignant diseases, a group that included patients with Waldenstrom macroglobulinemia (WM), chronic lymphomas and lymphoid leukemia, other hematological neoplasms and solid tumors. Patients who were carriers of the hepatitis C virus were included in the BGUS group if this was their only diagnosis and in the section of solid organ tumors when the HCV was combined with hepatocarcinoma (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The patients were referred by the Department of Hematology or the Department of Internal Medicine, although not all attended the follow-up, as indicated in the results.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Patient characteristics</span><p id="par0080" class="elsevierStylePara elsevierViewall">Between 1970 and 2011, a total of 1626 patients with monoclonal gammopathies were studied, 47 of whom corresponded to the diagnosis of biclonal gammopathy, which represents 2.89% of the total.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The mean age at diagnosis of these 47 patients was 65.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>19.6, and 31 of them (66%) were men. There were 20 cases (42.6%) of BGUS, 9 patients (19.1%) were diagnosed with MM, and the remaining 18 (38.3%) were diagnosed with some other malignant disease. Of these 18, 4 had WM, 8 had various types of lymphoma (4 cases) or leukemia (4 cases) and 6 had solid organ tumors. The types of paraproteins found and the most common combinations are listed in <a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1 and 2</a>. The most common paraprotein combination was IgG-IgG (17, 36.2%). Some 60.6% of the light chains were κ isotype. A total of 61.7% of the patients had both paraproteins with light chains of the same type (κ–κ in 20 and λ–λ in 9).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The median patient follow-up was 2 years.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Biclonal gammopathies of undetermined significance</span><p id="par0100" class="elsevierStylePara elsevierViewall">This subgroup of 20 patients (42.6% of the total) had a mean age of 64.5<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.3 years, and 11 were <span class="elsevierStyleItalic">men</span> (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0105" class="elsevierStylePara elsevierViewall">It is worth noting that 2 of the patients had common variable immunodeficiency, one of whom also had a sarcoid-like systemic reaction (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Additionally, 3 patients (15%) had HCV infection. Of the 20 patients with BGUS, 3 had been previously diagnosed (4, 4 and 13 years earlier, respectively) with monoclonal gammopathy of undetermined significance.</p><p id="par0110" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Clinical course:</span> Fourteen patients with BGUS were followed up at our center for a median of 3 years. Three died from respiratory infections at an advanced age. There was no malignant transformation during the follow-up in any of our patients. There was no follow-up for the other 6 patients.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Multiple myeloma</span><p id="par0115" class="elsevierStylePara elsevierViewall">The mean age of the 9 patients (19.1%) diagnosed with MM was 62.3<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>28.12 years, 7 of whom were men (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The combination of paraproteins in this subgroup was highly heterogeneous (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0120" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Clinical course</span>: The median follow-up was 1 year. Of the 7 patients who were followed up, 5 died by the end of the follow-up due to disease progression or complications related to the disease, and 2 are currently still alive.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Other malignant diseases</span><p id="par0130" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the characteristics of the 18 patients included in this subgroup.</p><p id="par0135" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Clinical course</span>: The median follow-up was 2 years. Eleven patients died from causes directly related to their tumor disease (4 cases of solid organ tumors and 7 cases of hematologic neoplasia). Seven patients are currently still alive, 1 with WM, 1 with non-Hodgkin's lymphoma, 1 with chronic B-lymphocyte leukemia, 2 with acute B and T lymphatic leukemia and 2 with hepatocarcinoma and HCV who underwent liver transplantation.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Three of these patients only visited our center once (2 with WM and 1 with low-grade B-cell lymphoma).</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">M band evolution</span><p id="par0145" class="elsevierStylePara elsevierViewall">Of the 47 patients in our series, 16 (34%) had at least one other paraprotein reading after the diagnosis. The median time between the 2 readings was 2 years. Three of these patients (19%) maintained the 2 paraproteins (patients 12, 32 and 38).</p><p id="par0150" class="elsevierStylePara elsevierViewall">In 6 patients (38%), 1 of the monoclonal components disappeared (patients 1, 2, 6, 7, 24 and 33); 4 patients were diagnosed with BGUS and had not been treated, except for 1 who had been treated for polymyalgia rheumatica. Of the other 2 patients, 1 diagnosed with MM had been treated with chemotherapy, and the other had not been treated for WM.</p><p id="par0155" class="elsevierStylePara elsevierViewall">In 7 patients, the 2 paraproteins disappeared (44%) (patients 8, 19, 21, 28, 37, 39 and 41); 4 of the patients underwent hematopoietic progenitor cell transplantation, 1 was treated with rituximab and chemotherapy, and the other 2 were not treated for having gammopathy of undetermined significance.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The time to disappearance of 1 and 2 monoclonal components varied between 5 months and 13 years and between 6 months and 5 years, respectively.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In summary, of the 7 patients with BGUS, 6 had lost 1 or 2 paraproteins. This same occurred with the 3 patients with MM.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Discussion</span><p id="par0170" class="elsevierStylePara elsevierViewall">There are numerous series of patients with monoclonal gammopathy but not with biclonal gammopathy; only 4 series have been published to date on the latter,<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–10</span></a> none of which are from Spain. In this study, we present a retrospective series conducted in a single Spanish center, with 47 patients with BG.</p><p id="par0175" class="elsevierStylePara elsevierViewall">In our series, the frequency of biclonal gammopathy within the collection of gammopathies was 2.89%, similar to that found in the specific biclonal series published to date (1.5–4.2%)<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–10</span></a> and to others included in monoclonal gammopathy series. If immunofixation had been used from the start, given that it is a more sensitive technique, more cases would have probably been detected. A number of authors include only those patients with 2 readings at the start. Due to the retrospective nature of our study, the patients only had 1 reading.</p><p id="par0180" class="elsevierStylePara elsevierViewall">The mean age at diagnosis was more than 60 years, and the sex was predominantly male, which is consistent with that described in other gammopathy series, both monoclonal and biclonal.</p><p id="par0185" class="elsevierStylePara elsevierViewall">As with other reported series, the largest patient group with biclonal gammopathy in our cohort was the BGUS group (42.56%), followed by group with hematologic lymphoproliferative tumors (WM, leukemia and lymphomas, 25.5%), the group with MM (19.15%) and the group with solid organ tumors (12.77%) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). However, given that multiple myeloma is a specific condition and that hematologic tumors are a heterogeneous group, we consider MM as the second most frequent condition.</p><p id="par0190" class="elsevierStylePara elsevierViewall">The most common composition of Ig in our series was the IgG-IgG combination (36.2%), which is similar to the findings in the series by Nilsson et al.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> but different from those observed in the series by Kyle et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> and in the series of Riddell et al. (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> The differences in composition of the gammopathies in the 5 series (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) cannot be explained solely by epidemiological data or by associated diseases. The differences could be due to the different levels of sensitivity of the paraprotein detection technique employed, to the small number of patients or to erroneous inclusion criteria related to the aggregation or polymerization of monoclonal components.</p><p id="par0195" class="elsevierStylePara elsevierViewall">The results of the light chain analysis are situated in the known ranges from previous publications (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), with κ chain found in 60.6%. The finding of a high frequency of light chain combinations of the same type (in 61.7%) could be explained by a common clonal origin for the 2 paraproteins in many patients with biclonal gammopathy, as suggested and demonstrated by previous evolutive studies, statistical analysis and molecular studies.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,9,11,12</span></a> In this study, however, we did not conduct molecular studies to confirm this hypothesis.</p><p id="par0200" class="elsevierStylePara elsevierViewall">In our series, as with the series by Riddell et al.,<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> no BGUS evolved to a malignant hematologic disease. However, the follow-up of these patients was heterogeneous and very limited in many patients. The risk of progression to MM was not determined.</p><p id="par0205" class="elsevierStylePara elsevierViewall">It is also noteworthy that a number of our patients had a history of diseases that likely had an immunological basis (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). We cannot conclude whether the association is causal or random. HCV infection is associated with monoclonal gammopathy in a high percentage of cases, estimated at approximately 15%.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> In our series, we found that 5 patients (10.64%) with biclonal gammopathy had chronic HCV infection. Other cases of HCV associated with biclonal gammopathy have rarely been reported.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">The main limitation of this study is that due to its retrospective nature only 2 readings for paraproteins could be obtained for 16 patients (34%), although this type of follow-up has rarely been reported in previous series.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0215" class="elsevierStylePara elsevierViewall">It is worth noting that among the patients included in the BGUS subgroup who did not undergo a specific therapy, at least 1 clonal component disappeared in 6 (30%) of the patients over the course of the evolution. This value was greater than the 15% reported in a biclonal gammopathy series<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> and one of the series by Kyle on patients with MGUS<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> in which 5% overall lost the paraprotein, 2% spontaneously and 3% after undergoing a treatment due to the combination of another disease. This fact could be due to the low concentration of paraproteins in our patients, as indicated by Kyle et al.,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> or to the sensitivity and specificity of the detection technique. Other factors could also have had an effect (such as immunosuppressive therapy, as occurred for 1 of our patients) as well as the concomitant occurrence of other diseases by mechanisms that are not so well explained.</p><p id="par0220" class="elsevierStylePara elsevierViewall">Among the patients with malignant disease, 7 of the 9 who had evolutionary information lost at least 1 monoclonal component, and of these, all but 1 (diagnosed with WM) had undergone treatment. All of the patients with MM and paraprotein follow-up had lost 1 or 2 monoclonal components. The 3 patients had undergone chemotherapy, 1 in combination with hematopoietic progenitor cell transplantation.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0225" class="elsevierStylePara elsevierViewall">Considering the low incidence of biclonal gammopathy, our study has an ample number of patients and is one of the largest series recorded in the literature and the first and only Spanish series. However, due to its retrospective character, the evolutionary data are limited. Our data suggest that BG share epidemiological characteristics with monoclonal gammopathies, and as with the latter, the clinical condition known as BGUS is the most common. Although we have not compared these data with our monoclonal gammopathy series, this is reflected in the literature. None of the patients with BGUS evolved to MM or another malignant condition, and an unexpectedly high number of the patients spontaneously lost 1 or 2 of the components. There are no known studies that have analyzed this disease in Spain. We studied the underlying diseases, characteristics of paraproteins and the evolution of a series of patients with biclonal gammopathy.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">We reviewed clonal gammopathies at the Department of Immunology of Hospital Puerta de Hierro in Madrid, between 1970 and 2011, selecting those patients with biclonal gammopathy in one reading. We collected data on the patient's epidemiology, underlying disease, associated diseases, therapies and paraprotein and immunoglobulin levels.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Of the 1626 cases of clonal gammopathies, 47 were biclonal gammopathy (2.89%). The median follow-up was 2 years. The main associated condition was biclonal gammopathies of undetermined significance (<span class="elsevierStyleItalic">BGUS</span>). The most common paraprotein combination was IgG-IgG. Upon conducting a second paraprotein reading, 81% of the patients had lost at least 1 monoclonal component. A third of the patients had not undergone treatment.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Biclonal gammopathy are fundamentally associated with biclonal gammopathies of undetermined significance. proliferation of plasma cells or their B-lymphoid progenitors, which synthesize abnormal immunoglobulins (M proteins or paraproteins). This is the first study performed in Spain that analyzes the underlying diseases, the characteristics of paraproteins and the evolution of a series of patients with biclonal gammopathy.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">What this article provides?</span><p id="par0020" class="elsevierStylePara elsevierViewall">In 47 patients with biclonal gammopathy (3% of clonal gammopathies from 1970 to 2011), the most common composition was IgG-IgG. The most frequently associated disease entity was biclonal gammopathy of undetermined significance. None of the cases evolved to a malignant disease (median follow-up of 2 years).
