"textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Systemic amyloidosis are a rare group of illnesses that are characterised by extracellular amyloid deposits. This amyloid is made up of a common glycosaminoglycan (GAG) nucleus and one serum amyloid P component (SAP) which acts as an acute phase reactant and is resistant to the action of the endogenous proteases; a protein characteristic of each type of amyloidosis binds to this nucleus<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>. During conformation, this protein undergoes misfolding β which makes it more insoluble and resistant to proteolysis and causes it to be deposited in the tissues<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The most common forms of systemic amyloidosis are: primary amyloidosis (AL), in which kappa or lambda light chains are deposited by monoclonal plasma cells, and which is associated with blood dyscrasias; and secondary amyloidosis, in which an acute phase reactant is deposited, and which is related to chronic inflammatory diseases, such as rheumatoid arthritis, inflammatory bowel disease, or rheumatic fever; and transthyretin amyloidosis (ATTR). More than 30 proteins responsible for causing amyloidosis have been reported in the literature, and it has been suggested that, under certain circumstances, any protein could cause amyloid deposits<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,4–6</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Transthyretin (TTR) is a protein with 127 amino acids that is responsible for transporting thyroxine and retinol, mostly produced in the liver, although 5% is produced in the cerebral choroid plexuses and in the retinal pigment epithelium. It is coded by a gene made up of 4 exons that is located in chromosome 18. This protein, for unknown reasons, tends to form amyloid fibrils that can be deposited in the tissue, causing tissue damage and cellular toxicity<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,6,7</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">TTR deposits can occur due to two mechanisms: the senile variant, which is age-related or wild-type ATTR (ATTRwt), and the hereditary variant (ATTRv). ATTRwt occurs sporadically and is associated with aging; it is more common in males over 60 years and incidence increases with age. In the case of ATTRv, it occurs as a consequence of different mutations in the TTR gene which lead to the production and deposition of a defective protein<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,7–9</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The gold standard for ATTR diagnosis continues to be histological examination; nevertheless, the most recent algorithms defend non-invasive diagnosis of ATTR with cardiac involvement. This diagnosis is performed using nuclear medicine tests such as scintigraphy with technetium-99m-3,3-diphosphono-1,2 propanodicarboxylic acid (<span class="elsevierStyleSup">99m</span>Tc-DPD), accompanied by serum and urine immunofixation testing, leaving histological examination for patients for whom a definitive diagnosis cannot be achieved using non-invasive techniques<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,10,11</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Despite advances in ATTR diagnostic techniques, and more specifically for ATTRv, diagnosis remains a clinical challenge, which makes it an underdiagnosed entity, particularly in non-endemic areas where the index of suspicion of the disease is low<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,10</span></a>.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The reason for carrying out this study is the suspicion that, given that this is an underdiagnosed entity, and we are located in a non-endemic area, there are many ATTR cases that have not been diagnosed due to lack of clinical suspicion of the entity. In this work, a series of patients with a high probability of suffering from ATTR was selected: the main objective was to detect patients with this illness, while the secondary objective was to perform a descriptive study of the principal characteristics from the different points of view of the detected patients.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0040" class="elsevierStylePara elsevierViewall">A single-center retrospective descriptive study carried out at the Lozano Blesa University Clinical Hospital (Zaragoza) with patients with suspected ATTR between September 2016 and January 2020. The initial hypothesis of the study is the existence of previously undetected subjects with ATTR.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Included subjects were patients over the age of 18, admitted to the internal medicine or cardiac wards, or assessed in out-patient clinics for this speciality, presenting with heart failure with preserved left ventricular ejection fraction (LVEF ≥ 50%) in the absence of valvular heart disease or long-standing arterial hypertension. In addition, these patients also met at least one of the following criteria: history of autonomic dysfunction (understood as the presence of characteristic symptoms such as orthostatic hypotension, syncope, sweating disorders, erectile dysfunction, and urinary incontinence), bilateral carpal tunnel syndrome of unknown cause, polyneuropathy of unknown origin or prior or current imaging tests suggestive of cardiac amyloidosis (<span class="elsevierStyleSup">99m</span>Tc-DPD scintigraphy or cardiac nuclear magnetic resonance [NMR]).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The exclusion criteria were as follows:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0055" class="elsevierStylePara elsevierViewall">Patients under the age of 18.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">•</span><p id="par0060" class="elsevierStylePara elsevierViewall">Patients who refused to be included in the study.</p></li></ul></p><p id="par0065" class="elsevierStylePara elsevierViewall">The medical history was reviewed, and epidemiological, clinical, analytical and imaging data were collected from a database.</p><p id="par0070" class="elsevierStylePara elsevierViewall">After receiving information about the study and signing the corresponding consent form, 3 mL of peripheral blood was drawn from the patients included in the study to perform DNA extraction and thereby genetic testing using the Sanger sequencing method, sequencing the coding exonic and flanking intronic sections of exons 2, 3, and 4 of the TTR gene to confirm or rule out ATTRv diagnosis.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Genetic testing was proposed for the first-degree family members of the patients with detected pathogenic mutations.</p><p id="par0080" class="elsevierStylePara elsevierViewall">A diagnosis of amyloid cardiomyopathy was considered for those patients with positive scintigraphy (grade 2–3 Perugini) or compatible cardiac NMR.</p><p id="par0085" class="elsevierStylePara elsevierViewall">After reviewing the patients, they were classified into the following subgroups:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0090" class="elsevierStylePara elsevierViewall">No cardiac amyloidosis: negative scintigraphy scan.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0095" class="elsevierStylePara elsevierViewall">ATTR: positive scintigraphy with negative serum and urine immunofixation. If a TTR mutation existed, it was ATTRv; if it was not present, it was ATTRwt.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">•</span><p id="par0100" class="elsevierStylePara elsevierViewall">Unknown cardiac amyloidosis: patients with positive scintigraphy and positive immunofixation or positive scintigraphy for those who had not undergone immunofixation.</p></li></ul></p><p id="par0105" class="elsevierStylePara elsevierViewall">Of these patient groups, for our descriptive study, we considered the ATTRv and ATTRwt groups, as well as unknown cardiac amyloidosis.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The statistical analysis was performed using the SPSS program, version 20, using a descriptive study of the different variables collected.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In no case did the study interfere with the clinical management of the patients or the care given them by their primary physicians. The only interventions in the study were the review of the clinical history and the genetic testing of the included patients. If in any case additional testing was performed subsequent to the patient’s inclusion in the study, these were not included in the statistics of this article.</p><p id="par0120" class="elsevierStylePara elsevierViewall">This study was evaluated and approved by the Research Ethics Committee of the Autonomous Community of Aragón. In addition, all the patients were informed about the study and the volunteer nature of it and all the patients signed the informed consent form.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0125" class="elsevierStylePara elsevierViewall">A total of 190 patients were detected that met the inclusion criteria and none of the exclusion criteria. Of those, 100 patients were included, for whom it was possible to confirm or rule out disease. The other 90 patients were ruled out as it was not possible to establish nor discard an amyloidosis diagnosis with the tests carried out during regular practice because scintigraphy and/or histopathological examination was not performed as part of the aetiological study of their disease. On our side, we did not intervene in patient management.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Genetic testing was performed on 10 direct relatives of patients with pathogenic mutations, resulting in a total of 110 individuals included in the study. Despite this, said subjects are not included in the comparisons as they are part of an independent group that does not present any clinical manifestations of the disease and, therefore, are not patients affected by it (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">The subjects included in the study were aged between 20 and 94 years, with a mean age of 75.9 ± 16.2 years. The male to female ratio was 75:35 (68.2%:31.8%).</p><p id="par0140" class="elsevierStylePara elsevierViewall">A total of 3 subjects were from Romania, 5 from Ghana, one patient from Cuba, and one from Gambia. The remaining subjects were Spanish.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Regarding the tests performed on all the subjects included in the study, of the 101 individuals (one asymptomatic carrier of the disease is included since it is a patient family member with highly significant disease symptoms at a young age), cardiac scintigraphy was performed and was positive (Perugini grade 2–3) for amyloidosis in 57 of them (56.4%), inconclusive (grade 1) in 2 (1.9%), and negative in 42 (41.6%). Serum and urine immunofixation were performed in 43 patients, with alterations detected in 11 of them (25.6%). Pathological results were available for 14 patients, all from subcutaneous fat biopsies, of which two were positive for amyloid deposits, without an immunohistochemical report of the type of amyloid detected.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Finally, a genetic study was carried out on all individuals to detect mutations in the TTR gene which found mutations in 7 of them:<ul class="elsevierStyleList" id="lis0015"><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">•</span><p id="par0155" class="elsevierStylePara elsevierViewall">One case of ATTR amyloidosis with the Glu74Gln mutation, in a native of Romania and one of her daughters, a presymptomatic carrier of the mutation.</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">•</span><p id="par0160" class="elsevierStylePara elsevierViewall">Three cases of ATTRv with the Val142Ile mutation, one in homozygosis (native of Ghana) and 2 in heterozygosis (Spanish), and 2 presymptomatic carriers of the mutation (family members of the confirmed cases).</p></li></ul></p><p id="par0165" class="elsevierStylePara elsevierViewall">The characteristics of the patients with ATTRv can be read in an article previously published by the authors<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>.</p><p id="par0170" class="elsevierStylePara elsevierViewall">With this supplementary testing and the previously described criteria, it was possible to rule out the presence of cardiac transthyretin amyloidosis in 42 patients as well as in 3 family members of affected patients who did not have a pathogenic mutation and did not undergo further complementary tests as they were asymptomatic subjects.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Therefore, 4 patients with ATTRv were detected, in addition to 3 presymptomatic carriers of amyloidogenic mutations; 26 individuals affected by ATTRwt, 31 patients with unknown cardiac amyloidosis, and one patient with AL amyloidosis (weak uptake in <span class="elsevierStyleSup">99m</span>Tc-DPD scintigraphy, positive serum and urine immunoelectrophoresis and presence of concomitant hematologic disease).</p><p id="par0180" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the clinical characteristics, laboratory tests, and complementary tests of the patients included in the ATTRv, presymptomatic ATTRv, ATTRwt and unknown amyloidosis groups, for a total of 64 patients.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0185" class="elsevierStylePara elsevierViewall">ATTR has 2 variants depending on whether it is due to a mutation in the TTR gene (ATTRv) or if it is due to an age-associated alteration of the TTR (ATTRwt).</p><p id="par0190" class="elsevierStylePara elsevierViewall">In the case of ATTRv, more than 100 different mutations have been described, which are transmitted in an autosomal dominant manner, with the most common mutations being Val50Met and Val142Ile. In addition, the endemic distribution of this disease in certain areas such as Portugal, Japan, Sweden, or Brazil has been confirmed, but increasing cases have been reported in areas that are considered to be non-endemic<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a>.</p><p id="par0195" class="elsevierStylePara elsevierViewall">It has a general prevalence of 1/100,000 inhabitants<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>, which could be higher in endemic areas like Portugal or Sweden, with levels nearer 1/10,000<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> (Val50Met prevalence of 1/538 in northern Portugal, 4% in the north of Sweden, and prevalence of Val142Ile of 4% in the black population)<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,6,10</span></a>.</p><p id="par0200" class="elsevierStylePara elsevierViewall">However, according to the conclusions reached in the literature review, this is an underdiagnosed entity, particularly in the non-endemic areas. The data recently published by our group support this underdiagnosis idea, showing a prevalence of pathologic mutation in the TTR gene of 1/230 for the global population and 1/1269 for the European population. This gives rise to the idea that, while not all individuals develop the disease, the actual prevalence may be higher than what is currently thought<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>. In the case of individuals with TTR mutation and negative scintigraphy, from a cardiology perspective, they are considered presymptomatic carriers of the disease.</p><p id="par0205" class="elsevierStylePara elsevierViewall">The age-related ATTR variant (ATTRwt) has an estimated prevalence of 25% in series of autopsies in patients over the age of 80<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a>. In addition, a series of scintigraphy studies performed in individuals over the age of 75 with no suspicion of amyloidosis established a prevalence of grade 2–3 myocardial uptake in 3.88% of the men and 0.77% of the women<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>.</p><p id="par0210" class="elsevierStylePara elsevierViewall">The high prevalence of amyloidosis in our sample is justified by the strict patient screening undertaken to achieve detection of the largest possible number of patients with ATTR. The aim of our study was not to determine the prevalence of the disease, but rather demonstrate, with a small sample size, the existence of previously undiagnosed patients with sufficient data to suspect the presence of this entity.</p><p id="par0215" class="elsevierStylePara elsevierViewall">Through this study, we wanted to emphasise the need to include ATTR in the differential diagnosis of patients with preserved ejection fraction heart failure and, in particular, in those with signs and symptoms that are clearly related to it, the majority of which can be obtained from a thorough clinical history.</p><p id="par0220" class="elsevierStylePara elsevierViewall">At present, ATTR, and ATTRwt in particular, can no longer be considered a rare disease, but rather an underdiagnosed disease, and our data confirm this: the presence of 28.18% of individuals with cardiac amyloidosis of unknown aetiology (individuals with positive scintigraphy and positive immunofixation and those with inconclusive scintigraphy regardless of the immunofixation result), of which a large portion likely belong to the ATTR group considering the literature in this regard<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a>.</p><p id="par0225" class="elsevierStylePara elsevierViewall">In total, more than 56% of the scintigraphy scans performed on patients in this study showed cardiac uptake. This data can be misleading and in no way reflects the actual prevalence of the disease since the patient sample was highly selected to detect hidden cases and also includes patients with previous tests suggestive of cardiac amyloidosis.</p><p id="par0230" class="elsevierStylePara elsevierViewall">The aim of this study was never to calculate the prevalence of the disease, but rather to describe a specific situation at our centre which we believe can be extrapolated to other similar centres. What is certain is that, with strict screening criteria, the cost effectiveness of requested scintigraphy scans is high and allows us to detect ATTR cases in a non-invasive manner. It even allows us to detect them in non-endemic areas such as ours, as well as to request the necessary genetic studies and to detect ATTRv cases that otherwise would not have been diagnosed.</p><p id="par0235" class="elsevierStylePara elsevierViewall">The clinical manifestations of ATTRv basically depend on the gene variant that causes the disease, although varying phenotypes have been reported in patients with the same mutation depending on the geographic area and other variables. There are gene variants that basically manifest in the form of predominantly axonal sensorimotor polyneuropathy and symptoms affecting the autonomic nervous system, and others in which the presence of infiltrative cardiomyopathy with heart failure and preserved ejection fraction is dominant, although there are multiple mixed phenotypes that complicate disease diagnosis.</p><p id="par0240" class="elsevierStylePara elsevierViewall">These more typical forms can be accompanied by disorders of other organs and systems with the onset of carpal tunnel syndrome, chronic kidney disease, and vision disorders with vitreous opacities, among others. In the case of ATTRwt, mainly cardiac disorders are reported, which are largely accompanied by carpal tunnel syndrome<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8,20–22</span></a>.</p><p id="par0245" class="elsevierStylePara elsevierViewall">In our sample, in the case of ATTRv, given that the main screening and inclusion criteria was the presence of heart failure with preserved LVEF, more cardiac involvement (100%) than neurological involvement (50%) was observed. This apparently atypical presentation of the disease is thereby justified by the screening criteria<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>. In addition, the presence of 2 cases (50%) of cerebrovascular accident stands out in these patients, which correspond to ischaemic strokes, likely due to the hematologic disorders secondary to the ATTR (due to increased predisposition to presenting arrhythmia, possible atrial dysfunction due to amyloid infiltration, or even the possibility of amyloid angiopathy).</p><p id="par0250" class="elsevierStylePara elsevierViewall">On the other hand, cardiac symptoms were observed in 100% of diagnosed ATTRwt patients, 23.1% experienced neurological symptoms consisting of paraesthesia and muscle weakness, and 23.1% of patients were affected by carpal tunnel syndrome. This coincides with the data described previously in the literature<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,15,23–26</span></a>.</p><p id="par0255" class="elsevierStylePara elsevierViewall">Regarding electrocardiogram (ECG) alterations in all the types of amyloidosis detected in the study, predominant alterations include those related to heart rate (the most frequent being arrhythmia due to atrial fibrillation, with 53.1%), and the least common being the presence of the most typical amyloidosis alterations as reported in the literature, such as low voltages and pseudoinfarction pattern<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,16,22</span></a>.</p><p id="par0260" class="elsevierStylePara elsevierViewall">Echocardiographic alterations<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> occur in almost all patients diagnosed with ATTR, with the most frequent alterations being thickening of the interventricular septum with hypertrophy of the left ventricle (LV) associated with preserved LVEF. Alterations such as refraction of the myocardium and its speckled appearance appear in a low percentage of patients (26.2%, reaching 50% in ATTRv cases), as these alterations are more specific to ATTR yet less frequent.</p><p id="par0265" class="elsevierStylePara elsevierViewall">As previously stated, the gold standard for amyloidosis diagnosis continues to be tissue biopsy with amyloid detection, however this is not very worthwhile in the majority of cases, particularly in terms of ATTRwt. In our study, this procedure was performed in 11 patients with positive scintigraphy, of which only 18.2% had positive results.</p><p id="par0270" class="elsevierStylePara elsevierViewall">Currently, new diagnostic algorithms, at least for cases with cardiac involvement, are based on non-invasive diagnostic tests: serum and urine immunofixation, imaging tests with echocardiogram, cardiac NMR and, above all else, the use of bone scintigraphy chiefly performed with bisphosphonates is worthy of note, and subsequent use of genetic testing in the case of detecting amyloidosis due to TTR<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,21,22,25,26</span></a>.</p><p id="par0275" class="elsevierStylePara elsevierViewall">In the study of our patients, <span class="elsevierStyleSup">99</span>Tc-DPD scintigraphy was performed in 91.8% (100% of patients diagnosed with amyloidosis) while serum and urine immunofixation was performed in only 39.1%, this being necessary criteria to be able to establish a diagnosis of ATTR without biopsy.</p><p id="par0280" class="elsevierStylePara elsevierViewall">The lack of immunofixation testing in many of the included patients represents a significant study limitation, as the coexistence of patients with ATTR and AL has been reported<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a>. This test would allow us to clearly determine the type of amyloidosis these patients have, and therefore they would not need to be included in the unknown amyloidosis subgroup. Amyloidosis could have been included in the initial diagnosis of these patients, without fully completing its etiological study. This results in a failure to diagnose numerous patients who could benefit from more targeted treatments for this disease, avoiding other treatments that we know to be harmful.</p><p id="par0285" class="elsevierStylePara elsevierViewall">Despite the fact that our environment is not an endemic area for ATTRv, this study illustrates the presence of various positive cases for the disease. We must include ATTR in the differential diagnosis of patients with preserved LVEF heart failure and emphasise the need to perform genetic testing on all patients with ATTR given that it is the only way to differentiate between the inherited and wild type forms, with the implications this represents.</p><p id="par0290" class="elsevierStylePara elsevierViewall">The main limitation of our study is the small sample size. This is due to the decision to require 2 suspicion criteria for patient inclusion, thereby increasing the likelihood of recruiting patients with ATTR. It is obvious that meeting one of the criteria for including amyloidosis in the differential diagnosis would be sufficient, but the fundamental aim of the study was not to establish the prevalence of the disease in our population, but rather to demonstrate that there are cases of ATTR, and more specifically ATTRv (and to carry out a descriptive study of those individuals) in a non-endemic population. The use of such a selective sample allowed us to find cases while studying very few patients.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0295" class="elsevierStylePara elsevierViewall">Amyloidosis, and more specifically ATTR in its two variants, is a disease that must be taken into account in the differential diagnosis of patients with heart failure with preserved LVEF, especially if associated with carpal tunnel syndrome or neurological manifestations. It can affect all organs and systems, the most frequent being neurological and cardiac involvement. This study aims to detect possible ATTR cases and carry out a descriptive study of them.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">Descriptive single-centre study carried out in a tertiary hospital, which included patients with suspected ATTR between September 2016 and January 2020.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">A total of 190 suspected ATTR patients were detected. The study includes 100 of these patients, as well as 10 relatives of patients in whom ATTR was detected in its genetic variant (ATTRv). In total, ATTRv was detected in 7 individuals (3 with a presymptomatic mutation of the disease), 16 patients with age-related ATTR and 31 individuals with unknown cardiac amyloidosis with the tests performed, which confirms the presence of this disease in non-endemic areas.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">ATTR is a disease that must be taken into account in the differential diagnosis of patients with heart failure with preserved LVEF, especially if associated with neurological symptoms.</p></span>"

Puede afectar a todos los órganos y sistemas, siendo la más frecuente la afectación neurológica y cardíaca. El objetivo de este estudio es detectar posibles casos de ATTR y realizar un estudio descriptivo de los mismos.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Material y métodos</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Estudio descriptivo unicéntrico realizado en un hospital de tercer nivel en el que se incluyen pacientes con sospecha de ATTR entre septiembre de 2016 y enero de 2020.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">Se detectan 190 pacientes sospechosos de ATTR. En el estudio se incluyen 100 de ellos, así como 10 familiares de pacientes en los que se detecta ATTR en su variante genética. En total, se detecta ATTR variante genética en 7 individuos (3 con mutación presintomática de la enfermedad), 16 pacientes con ATTR asociada a la edad y 31 individuos con amiloidosis cardíaca no filiada con las pruebas realizadas, lo que confirma la presencia de esta enfermedad en áreas no endémicas.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">La ATTR es una enfermedad que se ha de tener en cuenta en el diagnóstico diferencial de pacientes que presentan insuficiencia cardíaca con FEVI preservada, principalmente si se asocia a síntomas neurológicos.</p></span>"
