por su alta prevalencia&#44; morbimortalidad y elevado coste<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">1</span></a>&#46; Estudios recientes estiman que la prevalencia en Espa&#241;a se sit&#250;a en torno al 13-15&#37;<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">2</span></a>&#44; observ&#225;ndose un aumento en los &#250;ltimos a&#241;os<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">3</span></a>&#46; Los datos sobre incidencia de diabetes tipo<span class="elsevierStyleHsp" style=""></span>2 &#40;DM2&#41; en nuestro entorno son escasos y muy variables&#44; describi&#233;ndose tasas de 1&#44;5 a 19 nuevos casos&#47;1&#46;000 habitantes-a&#241;o<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">4-7</span></a>&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">La progresi&#243;n desde normoglucemia a diabetes puede tardar a&#241;os&#44; mientras pueden presentarse estados intermedios donde la glucosa en sangre es superior a lo normal&#44; pero sin alcanzar valores de diabetes franca<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">8</span></a>&#46; Estos estados intermedios o prediabetes no son considerados entidades cl&#237;nicas&#44; aunque constituyen un factor de riesgo para desarrollar diabetes&#46; La Asociaci&#243;n Americana de Diabetes &#40;ADA&#41;<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">9</span></a> y la Organizaci&#243;n Mundial de Salud &#40;OMS&#41;<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">10</span></a> coinciden en definir la intolerancia a la glucosa&#44; pero difieren en los criterios de glucemia basal &#40;GB&#41; alterada y en la utilizaci&#243;n de la HbA1c para el diagn&#243;stico de prediabetes&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">El riesgo de desarrollar DM2 en las personas prediab&#233;ticas es hasta 6 veces mayor que en las normogluc&#233;micas&#46; Cada a&#241;o&#44; el 5-10&#37; de los sujetos con prediabetes desarrollar&#225;n diabetes&#44; y el 70&#37; lo har&#225;n a lo largo de su vida<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">8</span></a>&#46; Numerosos estudios han demostrado que la DM2 se puede prevenir o al menos retrasar mediante actuaciones sobre factores de riesgo ligados a los estilos de vida<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">11&#44;12</span></a>&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">El objetivo principal de este estudio es estimar la incidencia de DM2 en la poblaci&#243;n adulta de la Comunidad de Madrid &#40;CM&#41; y determinar los factores asociados a su aparici&#243;n&#59; y el secundario&#44; comparar la capacidad de los criterios de prediabetes de la ADA y la OMS para predecir la aparici&#243;n de la enfermedad&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material y m&#233;todos</span><p id="par0025" class="elsevierStylePara elsevierViewall">Estudio prospectivo poblacional procedente de la cohorte PREDIMERC<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">13</span></a>&#44; constituida en 2007&#46; Se seleccion&#243; una muestra aleatoria de 2&#46;268 personas de entre 30 y 74<span class="elsevierStyleHsp" style=""></span>a&#241;os residentes en la CM&#44; mediante un muestreo por conglomerados con estratificaci&#243;n de las zonas b&#225;sicas de salud&#44; incluyendo 60 zonas para garantizar representatividad territorial&#44; cuya tasa de respuesta fue del 56&#44;4&#37;&#46; La metodolog&#237;a se ha descrito previamente<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">13</span></a>&#46; La cohorte se constituy&#243; con 2&#46;048 personas&#44; de las que 52&#44;7&#37; eran mujeres&#44; con una media de edad de 46&#44;9<span class="elsevierStyleHsp" style=""></span>a&#241;os &#40;DE<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>12&#41;&#46; La mayor&#237;a hab&#237;an completado estudios secundarios y desarrollaban trabajos no manuales&#46; El 22&#44;7&#37; ten&#237;an valores de GB<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>100<span class="elsevierStyleHsp" style=""></span>mg&#47;dl&#44; siendo el cambio de comunidad aut&#243;noma la causa m&#225;s frecuente&#46; Tras 6&#44;4<span class="elsevierStyleHsp" style=""></span>a&#241;os de seguimiento se detectaron 44 casos incidentes de DM2&#44; lo que supuso una tasa de incidencia de 3&#44;5 casos&#47;1&#46;000 personas-a&#241;o&#46; En la <a class="elsevierStyleCrossRef" href="#tbl0010">tabla 2</a> se detallan la incidencia y HR seg&#250;n las variables de exposici&#243;n estudiadas&#46; No se observaron diferencias significativas por sexo&#46; El riesgo de DM2 aument&#243; de manera significativa con la edad&#44; y se asoci&#243; con un menor nivel de formaci&#243;n&#44; pero no con la clase social&#46; La incidencia de DM2 fue mayor en personas con antecedentes familiares de diabetes&#44; obesidad&#44; obesidad abdominal&#44; hipertensi&#243;n arterial&#44; hipercolesterolemia e hipertrigliceridemia&#46; y la OMS &#40;C-Harrell<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;84&#44; p<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;234&#41;&#46;</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discusi&#243;n</span><p id="par0075" class="elsevierStylePara elsevierViewall">La informaci&#243;n sobre incidencia de DM2 en Espa&#241;a es escasa&#44; puesto que existen pocos estudios que aporten datos suficientemente representativos&#46; La estructura por edad y sexo de nuestra muestra reproduce a la poblaci&#243;n madrile&#241;a de 30 a 74<span class="elsevierStyleHsp" style=""></span>a&#241;os con amplia representatividad territorial&#44; por lo que los resultados pueden considerarse extrapolables a la poblaci&#243;n adulta de Madrid&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall">La tasa de incidencia observada en nuestro estudio fue de 3&#44;5 casos&#47;1&#46;000 personas-a&#241;o&#46; Los estudios poblacionales realizados en Espa&#241;a difieren en la metodolog&#237;a empleada y la poblaci&#243;n estudiada&#44; lo que dificulta la comparaci&#243;n de los datos&#46; V&#225;zquez et al&#46;<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">5</span></a> obtuvieron una incidencia de 8&#44;2&#47;1&#46;000 personas-a&#241;o en poblaci&#243;n &#62;<span class="elsevierStyleHsp" style=""></span>30 a&#241;os de Lejona&#59; Vald&#233;s et al&#46;<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">6</span></a>&#44; de 10&#44;8 casos&#47;1&#46;000 personas-a&#241;o en poblaci&#243;n asturiana de 30-75<span class="elsevierStyleHsp" style=""></span>a&#241;os&#44; y Soriguer et al&#46;<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">7</span></a>&#44; de 19&#44;1 casos&#47;1&#46;000 personas-a&#241;o en poblaci&#243;n malague&#241;a de 18-65<span class="elsevierStyleHsp" style=""></span>a&#241;os&#46; La incidencia menor de nuestro estudio puede deberse a los diferentes rangos de edad de la poblaci&#243;n estudiada&#44; la exclusi&#243;n de personas con HbA1c<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>6&#44;5&#37;&#44; as&#237; como al m&#233;todo empleado para diagnosticar los casos incidentes&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">Por otro lado&#44; observamos un importante incremento de la incidencia en la CM&#44; con respecto a la estimada por Zorrilla et al&#46;<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">4</span></a> a trav&#233;s de m&#233;dicos centinela en los a&#241;os noventa&#44; de 1&#44;46 por 1&#46;000 habitantes &#62;<span class="elsevierStyleHsp" style=""></span>14 a&#241;os&#46; Esta tendencia creciente es coherente con el aumento de la prevalencia observada en los estudios de base poblacional realizados en la primera d&#233;cada del siglo<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleSmallCaps">xxi</span>&#44; respecto a los de d&#233;cadas anteriores<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">3</span></a>&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Como era de esperar&#44; la incidencia de DM2 aument&#243; linealmente con la edad&#44; siendo la presencia de diabetes m&#225;s frecuente a partir de 55-60<span class="elsevierStyleHsp" style=""></span>a&#241;os<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">16&#44;17</span></a>&#46; Ha sido descrita ampliamente la relaci&#243;n inversa con el nivel de estudios<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">18</span></a>&#46; Asimismo&#44; la presencia de antecedentes familiares de diabetes constituye un importante factor de riesgo<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">19&#44;20</span></a>&#46; Meigs et al&#46;<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">19</span></a> observaron que el riesgo de diabetes fue similar cuando uno de los progenitores era diab&#233;tico &#40;RR<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>3&#44;5&#41;&#44; y se duplicaba cuando lo eran ambos &#40;RR<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>6&#44;1&#41;&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">En cuanto al estado glucometab&#243;lico&#44; nuestros resultados son consistentes con las investigaciones epidemiol&#243;gicas que han demostrado que las personas con valores elevados de GB y HbA1c tienen mayor riesgo de progresar a diabetes<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">21-24</span></a>&#46; El riesgo de DM2 aument&#243; a partir de valores de GB<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>100<span class="elsevierStyleHsp" style=""></span>mg&#47;dl &#40;HR<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>4&#44;6 con GB<span class="elsevierStyleHsp" style=""></span>100-109<span class="elsevierStyleHsp" style=""></span>mg&#47;dl&#59; Respecto a la HbA1c&#44; en nuestra cohorte el estrato con valores entre 5&#44;7-6&#44;4&#37; tuvo mayor riesgo de DM2 &#40;HR<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>7&#44;2&#41;&#46; Lerner et al&#46;<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">25</span></a> observaron que el riesgo era exponencial a partir de valores de 5&#44;5&#37;&#44; duplic&#225;ndose por cada 0&#44;5&#37; de incremento de HbA1c&#46; Cheng et al&#46;<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">26</span></a> tambi&#233;n encontraron que la HbA1c fue predictiva de diabetes&#44; increment&#225;ndose el riesgo progresivamente a partir de un valor de 5&#37;&#44; pasando de 1&#44;7 a 16 para HbA1c<span class="elsevierStyleHsp" style=""></span>6&#44;0-6&#44;4&#37;&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">La obesidad es uno de los principales factores de riesgo de DM2<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">27</span></a>&#46; En nuestro estudio&#44; tanto el IMC como el PC se asociaron a DM2&#46; Es bien conocido que el IMC es un buen predictor de DM2&#44; aunque estudios recientes han otorgado al PC un papel trascendental&#46; La grasa abdominal predice mejor el desarrollo de DM2 que el IMC&#44; por lo que el PC constituye un factor de riesgo m&#225;s importante que el IMC<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">27-29</span></a>&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">Numerosos trabajos han demostrado que la realizaci&#243;n de actividad f&#237;sica reduce el riesgo de DM2<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">30</span></a>&#46; Sin embargo&#44; en nuestro estudio no se ha observado asociaci&#243;n entre la inactividad f&#237;sica y DM2&#46; Hemos estudiado tambi&#233;n el consumo de frutas y verduras como un indicador de la calidad de la dieta&#46; Aunque no est&#225; clara la asociaci&#243;n entre la ingesta de frutas y verduras con DM2<a class="elsevierStyleCrossRefs" href="#bib0365"><span class="elsevierStyleSup">31&#44;32</span></a>&#44; s&#237; est&#225; ampliamente demostrado que una dieta saludable se asocia a menor incidencia de DM2<a class="elsevierStyleCrossRefs" href="#bib0365"><span class="elsevierStyleSup">31&#44;33</span></a>&#46; En nuestro estudio se observ&#243; mayor incidencia en personas que com&#237;an &#8805;<span class="elsevierStyleHsp" style=""></span>3 raciones&#47;d&#237;a de frutas y verduras&#44; aunque no se encontr&#243; interacci&#243;n con la edad&#46; Este dato podr&#237;a explicarse porque&#44; en general&#44; las personas mayores tienen mejores h&#225;bitos alimenticios<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">17</span></a>&#46; Tambi&#233;n podr&#237;a estar relacionado con la validez y fiabilidad de los cuestionarios utilizados para medir la actividad f&#237;sica y el consumo de alimentos&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">En el modelo multivariante los factores asociados al riesgo de DM2 fueron la edad&#44; los antecedentes familiares de diabetes&#44; la GB<span class="elsevierStyleHsp" style=""></span>100-125<span class="elsevierStyleHsp" style=""></span>mg&#47;dl&#44; la HbA1c<span class="elsevierStyleHsp" style=""></span>5&#44;7-6&#44;4&#37; y el PC<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>102<span class="elsevierStyleHsp" style=""></span>cm en hombres y &#8805;<span class="elsevierStyleHsp" style=""></span>80<span class="elsevierStyleHsp" style=""></span>cm en mujeres&#44; que globalmente predijeron el 85&#37; de la probabilidad de aparici&#243;n de DM2 &#40;C-Harrell<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;85&#41;&#46; El bajo n&#250;mero de casos incidentes podr&#237;a haber influido en la asociaci&#243;n de determinadas variables con la aparici&#243;n de DM2&#44; dado que limit&#243; el n&#250;mero de variables en la construcci&#243;n del modelo m&#225;ximo de partida&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">No hemos encontrado diferencias en la capacidad predictiva de los estados prediab&#233;ticos seg&#250;n la ADA y la OMS&#46; Las diferencias entre la ADA y la OMS para definir prediabetes han generado discusi&#243;n en numerosos estudios&#46; La aplicaci&#243;n de los criterios de la ADA conlleva un incremento significativo de las personas prediab&#233;ticas<a class="elsevierStyleCrossRefs" href="#bib0380"><span class="elsevierStyleSup">34-37</span></a>&#46; En este sentido&#44; Vald&#233;s et al&#46;<a class="elsevierStyleCrossRef" href="#bib0385"><span class="elsevierStyleSup">35</span></a> comprobaron que la inclusi&#243;n de GB<span class="elsevierStyleHsp" style=""></span>100-109<span class="elsevierStyleHsp" style=""></span>mg&#47;dl como prediabetes aumentaba la sensibilidad &#40;de 43&#44;2 a 75&#37;&#41; para predecir diabetes&#44; y la combinaci&#243;n de GB y HbA1c<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>5&#44;5&#37; obtuvo un excelente valor predictivo de diabetes &#40;AUC<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#44;88&#41;<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">38</span></a>&#46; Estos resultados fueron similares a los observados por Heianza et al&#46;<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">23</span></a> en la poblaci&#243;n japonesa&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">El seguimiento de los pacientes se realiz&#243; a trav&#233;s de la HCE de atenci&#243;n primaria&#44; cuya utilizaci&#243;n en estudios epidemiol&#243;gicos es cada vez m&#225;s frecuente<a class="elsevierStyleCrossRef" href="#bib0405"><span class="elsevierStyleSup">39</span></a>&#46; En un estudio previo evaluamos la validez y la concordancia de la diabetes registrada en la HCE de atenci&#243;n primaria en comparaci&#243;n con los datos del estudio PREDIMERC&#44; siendo la sensibilidad&#44; la especificidad y la concordancia de la HCE para detectar diabetes conocida o diagnosticada del 83&#44;5&#37;&#44; 98&#44;1&#37; y 0&#44;79&#44; respectivamente<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">40</span></a>&#46; De Burgos et al&#46;<a class="elsevierStyleCrossRef" href="#bib0415"><span class="elsevierStyleSup">41</span></a> demostraron la validez de los diagn&#243;sticos registrados de diabetes en la HCE para estudios epidemiol&#243;gicos&#44; hallando que el 99&#44;5&#37; de los casos cumpl&#237;an con los criterios de diagn&#243;sticos de diabetes&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">Hay que considerar un posible sesgo de selecci&#243;n con relaci&#243;n a la tasa de respuesta del estudio basal &#40;56&#44;4&#37;&#41;&#44; aunque es similar a otros estudios poblacionales de examen de salud que incluyen muestras biol&#243;gicas<a class="elsevierStyleCrossRef" href="#bib0420"><span class="elsevierStyleSup">42</span></a>&#46; Adem&#225;s&#44; AP-Madrid &#250;nicamente incluye a usuarios del sistema p&#250;blico&#44; por lo que&#44; considerando que la cobertura sanitaria p&#250;blica en Espa&#241;a es superior al 95&#37;&#44; es de esperar que la subestimaci&#243;n sea peque&#241;a&#46;</p><p id="par0130" class="elsevierStylePara elsevierViewall">En conclusi&#243;n&#44; este estudio aporta una estimaci&#243;n valiosa de la incidencia de DM2 y los factores asociados en la CM&#44; siendo el primer estudio prospectivo poblacional que se efect&#250;a en la regi&#243;n&#46; Los resultados ponen de manifiesto la importancia del estado glucometab&#243;lico como factor predictivo de DM2&#44; Resumen
Objetivos

Estimar la incidencia y los factores de riesgo de diabetes tipo2 en la población adulta de Madrid (España) y comparar los modelos predictivos de diabetes tipo2 basados en los criterios de prediabetes de la American Diabetes Association (ADA) y la Organización Mundial de la Salud (OMS).

Material y métodos

Estudio prospectivo de una cohorte poblacional formada por 2048 individuos de entre 30 y 74años sin diabetes. Al inicio del estudio se realizó una encuesta epidemiológica y se midió la glucemia basal, la HbA1c, el índice de masa corporal y el perímetro de la cintura. Se realizó un seguimiento de 6,4años. Los casos nuevos de diabetes tipo2 se identificaron a través de la historia clínica electrónica de atención primaria.

Resultados

La incidencia de diabetes tipo2 fue 3,5 casos/1.000 personas-año. En el análisis multivariante las variables que se asociaron con la aparición de diabetes tipo2 fueron la edad, los antecedentes familiares de diabetes, la glucemia basal (100-125mg/dl), la HbA1c (5,7-6,4%) y el perímetro de la cintura (≥94cm en hombres y ≥80cm en mujeres). De estas, las más significativamente asociadas fueron la glucemia basal y la HbA1c. Los criterios de la ADA y la OMS para definir prediabetes tuvieron la misma capacidad predictiva.

Conclusión

La incidencia de diabetes tipo2 estimada en Madrid fue inferior a la encontrada en otros estudios poblacionales, siendo el estado glucometabólico el principal factor asociado a la progresión a diabetes tipo2. No se han detectado diferencias entre la prediabetes definida por la ADA y la OMS para predecir la aparición de la enfermedad.

Objectives

Determine the incidence and risk factors of type2 diabetes in the adult population of Madrid (Spain) and compare the predictive models of type2 diabetes based on the prediabetes criteria of the American Diabetes Association (ADA) and the World Health Organisation (WHO).

Material and methods

A prospective study was conducted on a population cohort composed of 2048 individuals between 30 and 74years of age with no diabetes. At the start of the study, an epidemiological survey was performed, and baseline glycaemia, HbA1c, body mass index and waist circumference were measured. A follow-up of 6.4years was conducted. New cases of type2 diabetes were identified using the electronic primary care medical history.

Results

The incidence of type2 diabetes was 3.5 cases/1000 person-years. In the multivariate analysis, the variables that were associated with the onset of type2 diabetes were age, family history of diabetes, baseline glycaemia (100-125mg/dL), HbA1c (5.7-6.4%) and waist circumference (≥94cm for men and ≥80cm for women). Of these, the most significantly associated variables were baseline glycaemia and HbA1c. The ADA and WHO criteria for defining prediabetes had the same predictive capacity.

Conclusion

The incidence of type2 diabetes measured in Madrid was lower than that found in other population studies, with the glucometabolic state the main factor associated with progression to type2 diabetes. There were no differences between the prediabetes defined by the ADA and the WHO for predicting the onset of the disease.
