Children are an especially vulnerable group whose health and care are listed as objectives in the new Sustainable Development Goals.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">2</span></a> Within the framework of international cooperation, projects are being conducted in training, direct healthcare and pediatric surgical campaigns, but these have high costs and isolated effects on health.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a> The almost complete lack of African specialists and the scarce pediatric training of African general practitioners is the main handicap.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Asynchronous or deferred telemedicine through online scientific platforms is an innovative alternative in healthcare cooperation,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">4</span></a> thanks to permanent online bidirectionality between Africa and northern countries that does not require projects in the field.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">5</span></a> The unstoppable development of information and communication technology and globalized Internet access have improved in recent years, with the most remote areas of the world and those without resources benefitting the most from health projects.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">A few years ago, a Spanish nongovernmental development organization started an asynchronous telemedicine program in Africa that enables diagnostic collaboration, training and resource optimization to improve African pediatric health.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">7</span></a> The organization's secondary objective was the evacuation to Spain of the more complex pediatric patients for treatment, with subsequent follow-up after their return.</p><p id="par0020" class="elsevierStylePara elsevierViewall">This program's objective is to improve pediatric health, through the training and online collaboration of African-Spanish practitioners and to reduce the morbidity and mortality of complex diseases with no therapeutic options.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The Recover Hospitals for Africa Foundation<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">8</span></a> is a Spanish nongovernmental development organization whose activity focuses on improving the healthcare and developing the capabilities of African hospitals. The “Health 2.0: Connected African-Spanish Practitioners” (<span class="elsevierStyleItalic">Salud 2.0: profesionales de África-España conectados</span>)<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">9</span></a> program began in 2013 and has a scientific platform that is accessed online with a personal password, connecting African practitioners with Spanish volunteer specialists online. There are currently 23 active groups divided by specialties. Within the pediatrics group, African general practitioners upload pediatric cases in a format of individual clinical data, with the option of incorporating diagnostic tests (laboratory tests and/or radiological tests) and a space for African-Spanish discussion comments, in multiple languages. There is currently an ongoing collaboration with 18 hospitals in Cameroon, one in Burkina Faso and one in Congo and sporadic collaboration with Equatorial Guinea, Guinea-Bissau and Ethiopia.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Since its launch in 2013, more than 100 clinical pediatric cases have been uploaded, with more than 70 diagnostic tests, which have generated more than 250 discussion comments between practitioners on both continents. The incorporated cases are acute (hospitalization or consultation) and prevalent cases of severe traumatological, urological and cardiac diseases, among others. Some of these cases are treatable thanks to this program, and others include diseases that have no therapeutic options in Africa. Spanish volunteer specialists, in collaboration with healthcare funding institutions and private institutions in Spain, objectively assess the most severe cases. Depending on age, progression, therapeutic feasibility and the actual possibility for improvement, waiting lists are created for the patients’ possible evacuation to and treatment in Spain. The processing of the patient's and companion's travel permits, the transfer to Spain, treatment, recovery and return to Africa are supported by the Recover Foundation. Welcoming institutions and accompanying volunteers also collaborate in the process.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The main problems are those of facilities and technical means, connectivity in remote areas far from cities and the high routine workloads for practitioners and volunteers in the exchange of information. This program is sustainable thanks to local and private funding and to the selfless work of Spanish volunteers and healthcare institutions.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Thanks to the Africa-Spain telemedicine platform, the evacuation program (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) has transferred 29 children for treatment in Spain since the program's launch (of the more than 100 cases treated in collaboration). Of these, 34.5% were boys<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a> and 65.5% were girls, between the ages of 1 and 14 years. The most prevalent diagnosis was heart disease (34.1%), followed by severe traumatological processes (20.7%). The most common treatment was cardiac valve replacement surgery (24.1%) and gastrointestinal surgery (17.2%). Severe gastrointestinal and urological diseases and congenital heart disease affected the younger children, and cardiac surgeries were the most often performed operation.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">An annual follow-up after their return to Africa was achieved for 73.9% of the treated children. The most typical situations were disease persistence but with increased quality of life (44.8%), definitive healing (13.8%) and asymptomatic conditions with good functional grades (10.3%).</p><p id="par0050" class="elsevierStylePara elsevierViewall">Improving pediatric health in Africa requires resources and the continuing education of general practitioners, given the almost complete lack of trained pediatricians. African healthcare systems usually focus on prenatal and postnatal aspects and on preventing and treating acute infectious diseases, among others.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">10</span></a> In general, they avoid more complex cases, such as severe intestinal, trauma and cardiac cases, which require greater therapeutic rigor and are associated with disability and loss of quality of life. These conditions are precisely the most often identified and treated thanks to this telemedicine program, which match the enormous benefits reported in other telemedicine projects conducted in sub-Saharan Africa.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">11,12</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The continuing education of African health professionals is essential. Telemedicine provides guidance for their own needs,<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">13</span></a> without obliging them to perform volunteer projects, and adapts to their availability at any time and place thanks to the use of information and communication technology. This program has generated more than 100 cases thanks to the collaboration between practitioners in both continents and has optimized existing resources by adapting to the international recommendations for managing diseases.</p><p id="par0060" class="elsevierStylePara elsevierViewall">In short, a greater impact on health is provided by a feasible system in areas without fast Internet connections, enabling the assessment of cases and receiving treatment in specialties as complex as cardiac and gastrointestinal surgery without the need for clinical assessment projects in the field.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The asynchronous telemedicine projects between Africa and Spain are effective in pediatric health, although improvements are needed in the electronic platform and in overcoming the problems of connectivity. Diagnostic collaboration, resource optimization and the adaptation of international protocols are made possible thanks to a new model of online specialist volunteerism.
