The presence of these ulcers is associated with greater mortality.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">1,2</span></a> In the past 20 years, the mean age of patients hospitalized in internal medicine departments has increased to 79.9 years, and their disability has steadily grown.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Various scales have been employed to assess the risk of developing pressure ulcers, including the Norton scale, which contains 5 parameters: physical state, mental state, activity, mobility and incontinence.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a> The Norton scale also assesses patients’ wellbeing and predicts the prognosis. A number of studies have shown this scale's usefulness for predicting the prognosis of elderly patients who have followed rehabilitation programs or who will undergo aortic valve transcatheter implantation.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">6,7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to determine the association between the Norton scale score and mortality in the short, medium and long term in patients hospitalized in internal medicine departments.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was a prospective, single-center cohort study of patients included in the Minimal Surgery in Patients Hospitalized in Internal Medicine (IMPIMI) study, an intervention study with 2 parallel groups designed to reduce the duration of intravenous fluid treatment and urinary catheterizations (<a class="elsevierStyleCrossRef" href="#sec0060">Appendix</a>).<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">8,9</span></a> The groups came from 2 different internal medicine rooms of the same hospital. Twice a week during the meeting of the therapeutic team of physicians and nurses, the indication for the vesical catheter was reviewed, as well as the intravenous fluid treatment and the onset of pressure ulcers and delirium in the intervention group. The intervention was a simple, general, verbal and nonspecific reminder of each patient directed at the group of physicians and nurses. Neither these meetings nor the reminders were conducted for the control group.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Procedures</span><p id="par0025" class="elsevierStylePara elsevierViewall">The study included all patients hospitalized in the Department of Internal Medicine of Hospital Royo Villanova of Zaragoza during October 2010, January 2011, May 2011 and October 2011. On admission, a nurse interviewed the patients or their relatives or companions and assessed the patients’ general health and functional capacity with the Barthel index and Norton scale. After discharge, data from the medical history and from the administrative database on hospital discharges were extracted. The following information was collected for each patient: age, sex, institutionalization, ability to perform basic daily life activities, risk of presenting pressure ulcers, major diagnostic category at discharge, duration of hospitalization and relative weight of the diagnosis-related group (DRG). We used the relative DRG weights derived from the Spanish Ministry of Health, Social Services and Equality.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">10</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">We used the Barthel index to assess basic activities of daily life, which includes 10 tasks: feeding, bathing, dressing, grooming, voiding, defecation, going to the toilet, transferring, walking and climbing up and down stairs.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">11</span></a> The range of scores is 0–100; the higher the score, the greater the individual's capacity. Thus, 0 corresponds to absolute dependence and 100 represents total autonomy for basic activities of daily life. The Barthel index prior to admission in the stable phase was recorded. The risk of developing pressure ulcers was assessed with the Norton scale (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), which includes 5 domains: physical state, mental state, activity, mobility and continence.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a> The scale ranges from 5 to 20; the lower the score, the greater the risk of pressure ulcers. There are 4 widely used cutoffs for identifying 4 risk categories: very high (<10 points), high (10–14 points), medium (15–18 points) and low (>18 points).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">A 3-year follow-up was conducted by reviewing the medical history, by telephone or by consulting the National Death Index. The follow-up was performed blind; i.e., the researchers were unaware of the Norton scale score when assessing the result.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The study was approved by the Clinical Research Ethics Committee of Aragon (PI 12/0009) and conducted according to the principles of the Declaration of Helsinki and Spanish law on the protection of personal data.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">The quantitative variables are expressed as median (interquartile range), and comparisons between groups were performed with the Kruskal–Wallis test. The qualitative variables are expressed as absolutes and percentages and were compared using the chi-squared test (applying the Yates correction) and with Fisher's exact test when necessary.</p><p id="par0050" class="elsevierStylePara elsevierViewall">To study the variables associated with mortality, we constructed a logistic regression model. The multivariate analysis included all variables with statistical significance (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.1) in the univariate analysis.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The Norton scale was calibrated with the Hosmer–Lemeshow goodness of fit test, and the discriminatory power was checked by calculating the cumulative Kaplan–Meier survival curves by quartiles of score and by calculating the area under the curve of the receiver operating characteristic (ROC) curves. The calculations were repeated in a sensitivity analysis, excluding patients with pressure ulcers. For all cases, the level of statistical significance was established for a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05. The statistical analysis was performed with the statistical package SPSS, version 22.0.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Patient characteristics</span><p id="par0060" class="elsevierStylePara elsevierViewall">There were 768 hospitalizations of 714 patients. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the flow diagram for patient inclusion. The Norton scale was not recorded for 90 (12.6%) of these patients, who were therefore excluded from the study. There were no differences in the demographic, functional and clinical characteristics between the included and excluded patients (data not shown).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Ultimately, 624 patients were included, consisting of 301 (48.2%) men and 323 (51.8%) women, with a median age of 79 (17) years. The median Norton score at admission was 16 (7). The patients’ baseline characteristics by risk category, according to the Norton scale, are listed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. The patients with lower scores were older, more often women and institutionalized and had a lower functional capacity as measured with the Barthel index. These patients also had more respiratory, renal and urinary tract diseases but had a lower rate of circulatory, gastrointestinal, hepatic, bile duct and pancreatic diseases. The length of stay and the relative DRG weight were greater for the patients with a higher Norton score (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Mortality</span><p id="par0070" class="elsevierStylePara elsevierViewall">A 36-month follow-up was conducted for the patients, and there were no losses to the follow-up. During hospitalization, 74 (11.9%) patients died, 176 (28.2%) died at 6 months, 212 (34.0%) died at 1 year, and 296 (47.4%) died at 3 years (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows the cumulative Kaplan–Meier survival curves. The higher the risk on the Norton scale, the greater the mortality.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the variables that were related to mortality. Age and the Norton score were independently associated with mortality for all evaluated periods. The miscellaneous category of disease was associated with lower mortality during hospitalization, and gastrointestinal and nervous system diseases were associated with lower long-term mortality.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The calibration of the Norton scale was appropriate in the Hosmer–Lemeshow goodness of fit test (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.602, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.328, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.137 and <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.157 for mortality during hospitalization, at 6, 12 months and 3 years, respectively). <a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a> shows the ROC curves of the Norton scale for predicting mortality. The area under the curve was >0.7 for the short, medium and long term. The data analysis excluding patients with pressure ulcers did not change the results (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">The study's main finding was the usefulness of the Norton scale for predicting the short, medium and long-term prognosis of patients hospitalized in internal medicine departments.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The Norton scale was developed to predict the risk of developing pressure ulcers, associating lower scores with greater risk. The scale is a simple tool that has shown good interobserver reliability.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">12</span></a> A number of studies have already reported the association between this scale and the onset of falls and the outcome of rehabilitation programs for patients with hip fractures, arthroplasty or stroke.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">13–16</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">The Modified Early Warning Scale has been employed to predict mortality during hospitalization in emergency departments. This tool includes 5 parameters: systolic blood pressure, heart rate, respiratory rate, temperature and level of consciousness.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">17</span></a> Another scale introduced to assess the risk of hospital mortality is the Multidimensional Prognostic Index.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">18,19</span></a> This index includes 6 scales: the Katz index, the Lawton-Brody index, the Pfeiffer questionnaire, the Cumulative Illness Rating Scale, the Mini Nutritional Assessment and the Exton-Smith scale. The index requires 45–60<span class="elsevierStyleHsp" style=""></span>min to calculate the score. The Norton scale, however, can be calculated within approximately 1–2<span class="elsevierStyleHsp" style=""></span>min and can be applied by physicians and nurses.</p><p id="par0100" class="elsevierStylePara elsevierViewall">There are also several scales for mortality following hospital discharge. The Walter scale includes 6 variables: sex, dependence for daily life activities, congestive heart failure, cancer, creatinine and albumin.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">20,21</span></a> In its original study, the scale showed good discriminatory power, with areas under the curve of 0.75 and 0.79 in the referral and validation cohorts, respectively, although other subsequent studies have shown lower values.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">20,21</span></a> The Multidimensional Prognostic Index has also been used to predict mortality after discharge,<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">22</span></a> with a similar discriminatory power as that obtained in the present study.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The usefulness of the Norton scale for predicting mortality at 1 year has been previously reported for patients who are still in rehabilitation<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">6</span></a> and for those who undergo aortic valve transcatheter implantation.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">7</span></a> The association between the Norton scale and a longer hospitalization, more complications and hospital mortality has also been reported for patients hospitalized in internal medicine departments.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">23</span></a> However, no relationship has been observed with mortality at 1 year. In the present study, the Norton scale also had good discriminatory power for predicting mortality at 3 years. To our knowledge, there have been no published studies that have assessed the Norton scale's predictive power beyond a year.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The primary limitation of this study is its single-center character.
