Despite therapeutic advances, mortality and hospitalization due to HF remain high, especially in elderly patients<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5–8</span></a>. This is due to the high degree of comorbidity these patients have, which entails fragmented care, hinders clinical follow-up, and limits the implementation of treatment<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9–14</span></a>. Therefore, guidelines recommend including this profile of patients in specific care programs<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Within these specific programs, it is important to establish structured follow-up for patients with HF during the postdischarge transition, especially in the first 30 days, which is the period in which they are the most vulnerable to decompensation. National and international series estimate the one-month readmission rate due to HF to be between 25% and 35%<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17–25</span></a>. The 2019 RECALMIN report observed rates of between 23.7% and 31%<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Given this public health reality, the Spanish Society of Internal Medicine (SEMI, for its initials in Spanish), launched a care program in 2011 aimed at this type of complex chronic patients. The program is called UMIPIC (Comprehensive Management Units for Patients with HF) and is currently implemented in 34 centers across Spain<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a>. Its implementation has led to clear benefits in the reduction of HF-related admissions and emergency room visits in a group of patients followed-up on for one year with respect to the year prior to their inclusion in the program<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In order to advance the evaluation of this care model, this study’s primary aim is to analyze the HF-related readmission rate at one month and at 12 months in a group of patients attended to in the UMIPIC program compared to another group with similar characteristics that had conventional follow-up. The secondary aim was the 12-month mortality rate.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Patients and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Patients</span><p id="par0025" class="elsevierStylePara elsevierViewall">Patient data were obtained from the information included in the RICA (Heart Failure Registry) registry between March 2008 and February 2016. RICA is a multicenter, prospective, cohort registry coordinated by the SEMI’s Heart Failure and Atrial Fibrillation Working Group. The registry’s characteristics have been described previously<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28–31</span></a>. Briefly, the registry included consecutive patients hospitalized for a decompensated HF episode who were attended to by the physicians who participated in the study and had at least one year of follow-up after discharge.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Data were sent through the website <a href="https://www.registrorica.org">https://www.registrorica.org</a> and a distinction was made between those who belonged to a UMIPIC (UMIPIC group) and those who received conventional care (RICA group). Patients were evaluated on at least two occasions during the follow-up period (at one and 12 months).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The study protocol was approved by the Clinical Research Ethics Committee of the Reina Sofía University Hospital of Córdoba. Informed consent was obtained from all patients prior to their inclusion. To be included, patients had to be older than 50 years of age and have been admitted for decompensated HF according to the 2008 European Society of Cardiology criteria<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a>.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">UMIPIC program</span><p id="par0040" class="elsevierStylePara elsevierViewall">The UMIPIC program offers comprehensive continuing care to elderly, complex, chronic patients with HF<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a>. The exclusion criteria were need for close monitoring by the cardiology department (ischemic patients who require catheterization, device implantation, prosthetic valve replacement, or those with advanced functional class awaiting a heart transplant) and a lack of social or family support in the case of patients with functional and cognitive limitation.</p><p id="par0045" class="elsevierStylePara elsevierViewall">All units have nursing department staff and offer follow-up in an intensive, protocolized program based on five aspects: (1) comprehensive clinical approach with care for HF and any comorbidities; (2) continuing follow-up that includes in-person visits and telephone contact as well as hospital care during the admission; (3) formal education on self-care and support measures as well as the commitment of the patient and/or caregiver; (4) the agility and accessibility necessary in order to assist the patient in the event of unforeseen decompensation; and (5) direct communication with other specialists, such as cardiologists or nephrologists. The follow-up involves both medical and nursing consultations.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study variables</span><p id="par0050" class="elsevierStylePara elsevierViewall">Patients’ sociodemographic variables; medical history and clinical characteristics; analytical data; and electrocardiographic, echocardiographic, and treatment parameters are gathered upon inclusion<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28–31</span></a>.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The outcome variables were readmissions due to HF at one month and 12 months of follow-up and total mortality at 12 months.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">Quantitative variables were expressed as medians (interquartile range) and were compared using Student’s t test or the Wilcoxon test, depending on whether the distribution was normal or not.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Qualitative variables were expressed as frequencies and percentages and were compared by means of the chi-square test with Yates’s correction when it was considered necessary or by means of Fisher’s exact test.</p><p id="par0070" class="elsevierStylePara elsevierViewall">To adjust for differences between the two groups in regard to baseline characteristics, propensity score matching was performed by means of a Cox regression, including variables that demonstrated statistically significant differences between the two cohorts (UMIPIC and RICA). A 1:1 matching protocol without replacement was used with a caliper width of 0.2 standard deviations for the propensity score using the nearest neighbor coincidence method.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The survival analysis was conducted using Kaplan Meier curves. Comparisons of the survival curves were made by means of a log-rank test.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Finally, the number of patients that need to be treated in the UMIPIC program to avoid one HF hospitalization and one death was calculated.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Values of <span class="elsevierStyleItalic">p</span> < .05 were considered statistically significant. The statistical analysis was conducted using the SPSS program version 22 and the MatchIt R 3.2 statistical package.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Baseline characteristics</span><p id="par0090" class="elsevierStylePara elsevierViewall">A total of 2862 patients were analyzed (median age 80 years, 46% women): 809 in the UMIPIC group and 2053 in the RICA group. The demographic and clinical characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. It should be noted that three to four out of every ten patients were included following an initial episode of HF (34%–40%) and one out of three had an advanced functional class (NYHA III or IV 37%–38%), but they had high rates of independence for basic activities (Barthel 95), no cognitive decline (dementia 4%–6%), and the vast majority had good family support (93%–97%). This may explain the low rates of institutionalization despite their age and comorbidities (7%–11%).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Patients in the UMIPIC cohort were older and had higher rates of hypertension, diabetes mellitus, hyperuricemia, preserved left ventricular ejection fraction, prior HF, atrial fibrillation, kidney failure, and family support than those in the RICA group. On the other hand, more ischemic heart disease, chronic obstructive pulmonary disease, dementia, and institutionalization were observed in the RICA cohort.</p><p id="par0100" class="elsevierStylePara elsevierViewall">In regard to treatment, patients in the UMIPIC group received less digoxin, angiotensin-converting enzyme inhibitors, or angiotensin II receptor blockers and received more beta blockers and antialdosterones (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Events during follow-up</span><p id="par0105" class="elsevierStylePara elsevierViewall">In the non-propensity score matching population, a readmission rate due to HF of 2% was observed in the UMIPIC group and a rate of 5.3% was observed in the RICA group during the first month of follow-up. At 12 months, the readmission rates were 17% and 26%, respectively (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">In regard to total mortality at one year of follow-up, the rate in the UMIPIC group was 16% whereas the rate in the RICA group was 27% (<span class="elsevierStyleItalic">p</span> < .001). All differences upon comparing the non-matched groups were statistically significant (<span class="elsevierStyleItalic">p</span> < .001) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0115" class="elsevierStylePara elsevierViewall">The analysis of the propensity score matching population showed similar results in both readmissions due to HF at one month (1.9% [UMIPIC] vs. 8.4% [RICA]; <span class="elsevierStyleItalic">p</span> < .001; HR: 0.22; CI: 0.09−0.50) and at 12 months (15% [UMIPIC] vs. 30% [RICA]; <span class="elsevierStyleItalic">p</span> < .001; HR: 0.44; CI: 0.32–0.60) as well as total mortality at 12 months (17% [UMIPIC] vs. 28% [RICA]; <span class="elsevierStyleItalic">p</span> < .001; HR: 0.58; CI: 0.42–0.79) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> and <a class="elsevierStyleCrossRefs" href="#fig0010">Figs. 2 and 3</a>). The number of patients that need to be treated in the UMIPIC program to avoid one hospitalization due to HF was 11.2 and to prevent one death was 9.4.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">This study shows that implementation of the UMIPIC care program for patients with HF and high comorbidity, which is based on comprehensive continuing care by internists and nursing department staff, achieved an annual reduction of 56% in admissions due to HF and 42% in total mortality.</p><p id="par0125" class="elsevierStylePara elsevierViewall">It is important to note that the magnitude of the benefit obtained in the UMIPIC program is much higher than that of other programs evaluated to date. In this regard, various meta-analyses and studies published show the reduction in readmissions due to HF or mortality at one year does not exceed 20% for each of the variables<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,14,17–20,33,34</span></a>.</p><p id="par0130" class="elsevierStylePara elsevierViewall">In our country, results on the implementation of another care program have recently been reported and show a reduction in the all-cause readmission rate and HF readmission rate at 30 days of 8% and 14%, respectively<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">35</span></a>. It should be taken into account that the benefits provided by the UMIPIC program are in a group of patients that is self-sufficient or with social and family support and therefore may not be applicable to the entire population with HF.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The reduction in readmissions due to HF in patients followed-up on in the UMIPIC program was already evident in the first 30 days following discharge, which is the most vulnerable period in terms of readmissions. Other studies that have been published very recently have analyzed the implementation of a program in this transition period following discharge, but they have not demonstrated any benefit<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">36,37</span></a>.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The results of this study confirm the benefits of the UMIPIC program observed in a previous study that compared the incidence of events in a patient sample before and after their inclusion in the program<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>. It should be noted that the UMIPIC program has not entailed an increase in medical staff in the departments where it has been implemented, but has entailed an increase in nursing staff, which have been part-time in the majority of cases.</p><p id="par0145" class="elsevierStylePara elsevierViewall">In our opinion, patients with HF who most benefit from a structured care program are those who meet the characteristics of patients included in the UMIPIC program. Unlike those selected in studies published to date<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,17,18,32</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">33,35,36</span></a>, UMIPIC patients are older and have greater comorbidity and complexity.</p><p id="par0150" class="elsevierStylePara elsevierViewall">In addition to nursing department staff, another element which is clearly different in the UMIPIC program is the involvement of internists. Their multifaceted nature and holistic vision allows them to take on the responsibility of attending to patients’ comorbidities in a comprehensive manner, consulting with other specialists in a planned, direct way when they deem it appropriate.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Another relevant aspect of the UMIPIC program is its usefulness in the total population of patients with HF regardless of whether the left ventricular ejection fraction is reduced, mid-range, or preserved. Although there is no treatment that modifies the prognosis of HF with preserved left ventricular ejection fraction, integrated care of comorbidities surely contributes to a reduction in events among these patients.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Taking into account that the RICA registry control population—which the UMIPIC program group was compared to—had pre-established follow-up that was probably closer monitoring than that of patients attended to in a conventional manner, the benefit of the UMIPIC program is foreseeably even greater than what has been observed in this study.</p><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Limitations of the study</span><p id="par0165" class="elsevierStylePara elsevierViewall">This study has the limitations inherent to its observational nature, given that it is not possible to guarantee an absence of bias in patient selection and the interventions performed despite using a propensity score matching method to select the samples of the two cohorts evaluated.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Another limitation is the lack of information on emergency room visits in the RICA registry, which probably would have shown an additional benefit of the UMIPIC program. Data on quality of life or the magnitude of interventions performed (number of consultations, telephone visits, educational sessions, etc.) were not available either. Finally, neither the presence of differences in in-hospital mortality prior to the start of the study nor the percentage of losses to follow-up in both cohorts were not studied.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Nevertheless, the sample analyzed is large and the results are relevant. We evaluated HF readmissions during 12 months of follow-up and total mortality after episodes of HF hospitalization. UMIPIC patients were older with higher rates of comorbidity and preserved ejection fraction than the RICA group.
