In recent years, the Spanish population older than 65 years has grown significantly, from 14.92% in 1997 to 16.62% in 2008, and similar growth is expected for the coming years. The incidence of hip fractures in Spain in 2008 was 103.76 cases per 100,000 inhabitants, 17.77% more than that recorded in 1997, which shows an interannual growth rate of 1.5%. This growth was accompanied by an increase in patient age. The treated patients had a mean age of 78.07 years (80.13 years for women, 72.08 years for men), while in 2008, this mean age was 80.46 years (82.13 years for women; 75.71 years for men). Hip fractures are associated with a high prevalence of morbidity and mortality, with an annual mortality rate that varies between 20% and 25%.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Treatment after a fracture is long, with a varying recovery rate that requires broad support from numerous medical specialties.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">2</span></a> Hip fractures have therefore gained importance in society, both in economic and societal terms, directly affecting those who experience hip fractures and indirectly affecting the services that provide support. In Spain, the costs associated with hip fractures reach approximately 395.7 million euros,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">1</span></a> and in the United States, the costs are estimated at 9 billion dollars.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Due to the present magnitude of the problem and its predicted increase, the care models need to be protocolized, and an approach needs to be provided that is aimed at the elderly population to respond to their specific needs, both in terms of frailty and multimorbidity. There are experiences with these standardized protocols that have been shown to be beneficial in providing care for hip fractures.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">4–7</span></a> These models are based on multidisciplinary teams, in which orthopedic surgeons and internists/geriatricians share the comprehensive and individualized care of this population with special clinical needs. Thus, the role of the internal medicine specialist is growing as the result of the considerable morbidity observed in these patients. With these co-management models, these patients’ medical care is conducted according to the criteria of the trauma department, with a varying response from the medical department, typically, once complications have appeared.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The aim of this study was to demonstrate the effect of a model of co-management between trauma and internal medicine departments on elderly patients with hip fractures.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We designed a retrospective cohort study for patients older than 65 years hospitalized with hip fractures. The historical cohort (HIST) included those patients hospitalized between January 2005 and August 2006, and the co-management cohort (COFRAC) included those between January 2008 and August 2010.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We assessed the incidence of medical complications, mortality during the hospitalization and the number of emergency department visits, readmissions and mortality at 30 days after the discharge. We also analyzed the patients’ and their caregiver's demographic characteristics, the patient's clinical characteristics and that of the fracture and surgery. We performed a statistical analysis using the chi-squared test for the qualitative variables and the analysis of variance test for the quantitative variables. We employed the SPSS statistical package version 20.0.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study included 701 patients, 471 of whom formed part of the HIST cohort, while 230 formed part of the cohort COFRAC. The demographic and clinical characteristics showed differences in terms of identifying polypathology, motor deficiencies, arrhythmia other than atrial fibrillation, osteoporosis and the long-term use of drugs (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). There were no differences between the cohorts in terms of the characteristics related to the hip fractures, anesthesia or surgery (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the hospital complications, with differences in mortality, constipation, anemia and renal function control, with a greater use of blood transfusions and intravenous fluid therapy in the COFRAC cohort. There were no statistically significant differences related to the presence of delirium, although the condition was diagnosed more often in the COFRAC cohort. We identified a significant change in the management of analgesia in the COFRAC cohort, with an increased use of morphine, bisphosphonates and calcium-vitamin D supplements and a lower use of pethidine and nonsteroidal anti-inflammatory drugs. There was no difference in the length of the hospital stay between the HIST and COFRAC cohorts. In terms of the clinical impact, the COFRAC cohort had lower mortality, with no differences 30 days after the hospital discharge (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">Hip fracture is a relevant event for elderly patients and those with multimorbidity. Hip fractures are a significant cause of disability and impaired quality of life for these frail patients, who require a full year to recovery 60% of their mobility, 50% of their daily basic activities and 30% of their instrumental activities.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">8</span></a> Mortality following a hip fracture remains high (11–23% at 6 months after the injury and 22–29% at 1 year).<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">9</span></a> Co-management has been proposed to improve and optimize the outcomes for hip fractures. Co-management enables a comprehensive preoperative assessment of comorbidities, the management of polypharmacy and the early detection of complications such as delirium. Co-management focuses on reducing the time from hospitalization to surgery as much as possible.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">10</span></a> This organized care model for geriatric fractures in co-management with geriatricians has resulted in significant savings over 50 months, accompanied by an improvement in the quality of care.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">11</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The principles of co-management for hip fractures were defined by Mendelson and Friedman<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">12</span></a> as follows: (1) most patients benefit from surgical stabilization of the fracture, because it relieves the pain, improves mobility and function, reduces blood loss and mortality and decreases the rate of institutionalization at discharge. (2) The faster these patients are operated on, the shorter the time for developing an iatrogenic disease, such as delirium, pain, pneumonia, pressure ulcer, malnutrition, urinary tract infection and thromboembolism, physical weakness, falls and additional injuries. The speed of the process increases the patients’, their family's and the staff's satisfaction and reduces costs and mortality. (3) Co-management with close communication between practitioners prevents common medical and functional complications. We need a fully coordinated, collaborative, interdisciplinary system in which information, decision making and responsibility are shared equally throughout and in which communication occurs daily. Each discipline issues written orders and notes and communicates with the patient and the team caring for the patient. This pioneering model was developed by the Geriatric Fracture Center.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">13</span></a> (4) Standardized protocols reduce unwarranted variability. The variability is therefore specific to the patient and is warranted instead of random or based on the supplier's preferences or on factors of the system itself. Standardization of material requests, protocols and nursing care plans improves quality and contains costs, because it allows for the use of less expensive generic drugs and makes it possible to achieve favorable agreements with implant providers. The length of the hospital stay is also reduced, as is the use of additional services because standardization helps prevent complications and readmissions.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">11,14,15</span></a> (5) Planning for the hospital discharge begins at admission. If the co-management is effective, the hospital stay will be shorter, and a brief period will be available to develop a comprehensive discharge plan. The needs assessment at discharge and the communication and coordination with the patient's family or with a supporting entity at admission can help avoid delays in the discharge and reduce the conditions of fear. The results of our study are consistent with these principles of co-management for hip fractures.</p><p id="par0050" class="elsevierStylePara elsevierViewall">In the COFRAC cohort, the preoperative assessment was more effective than in the HIST cohort. In the COFRAC cohort, the identification of polypathological patients, of motor deficiency, of arrhythmias other than atrial fibrillation, of osteoporosis and of the number of long-term drugs was higher. These items are valuable factors for establishing proper planning during the patient's hospitalization and discharge. Due to the considerable lack of data in the HIST cohort, we were unable to comparatively analyze variables such as height, weight, functional status, comorbidities (using the Charlson index), the history of falls and the social-familial state. All these data are important for the preoperative assessment to plan the hospital co-management and the care at discharge.</p><p id="par0055" class="elsevierStylePara elsevierViewall">This detailed preoperative evaluation provides a different co-management for the COFRAC, one that prevents hospital complications. Thus, the incidence of renal impairment was lower with the administration of a more aggressive intravenous fluid therapy, the use of nonsteroidal anti-inflammatory drugs was avoided, and the repercussion of anemia was reduced through the use of more transfusions conducted during the hospitalization. There was also a change in the prescription of analgesia, avoiding pethidine and nonsteroidal anti-inflammatory drugs in favor of morphine. Although this change in analgesia is appropriate for reducing delirium in geriatric patients, this complication was more prevalent in the COFRAC cohort. This discrepant result was due to greater accuracy in the diagnosis of delirium in the COFRAC cohort and to the underestimation of the diagnosis in the HIST cohort. This hypothesis is supported by the fact that the prevalence of delirium during hospitalization for hip fracture ranges from approximately 24% if there is co-management to 60% if there is none.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">14</span></a> The delirium will persist in a third of patients discharged to their home.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">16</span></a> Managing delirium has a significant effect on the outcomes of treatment for hip fractures, because delirium affects the patient's functional status and mobility and increases the rates of mortality and institutionalization.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Our study showed no difference in terms of the characteristics of anesthesia and surgery, including the type of anesthesia and type of surgery and the time from admission to surgery. Several studies have demonstrated the importance of early surgery, given that it reduces pain, the length of the hospital stay and the incidence of major complications.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">19–21</span></a> Although a preoperative examination conducted by an internist can theoretically identify some patients who are not medically suited and their surgery will be delayed beyond the recommended 24–48<span class="elsevierStyleHsp" style=""></span>h, early surgery is an important issue because, in general, there are no statistically significant differences between the two cohorts. In Spain, the length of stay between 1997 and 2008 showed a decreasing tendency, from 16.05 to 13.34 days.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">1</span></a> In the co-management studies, the length of stay before co-management was 9.9 days and 7.1 after co-management.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">22</span></a> This duration was similar to that of our study, except that we found no differences between the two cohorts. The explanation for this fact can be found in the fact that our standardized protocol still presented a number of areas that can be improved to optimize the planning of care.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Hip fractures can lead to significantly unfavorable consequences, with 26% of patients dying at 1 year of the hip fracture.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">23</span></a> Hospital mortality is low, with a rate of 4.71–5.85%, according to the official statistics of Spain. In clinical trials, co-management represents a reduction of approximately 5% in hospital mortality rates,<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">24</span></a> with a mortality rate of 2.5–9.7% prior to surgery and 1.5–4.5% after the surgery.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">3,7,14</span></a> Our results agree with the published data: in the HIST cohort, the mortality was 4.6%, as are the rates in Spain, and in the COFRAC cohort, the mortality was reduced by almost 3.3%. However, our study showed no differences in mortality at 30 days after the discharge, which indicated that planning is required in other areas of care. Not delaying the surgery and reducing hospital mortality, along with not extending the hospital stay, are elements that support the implementation of co-management.</p><p id="par0070" class="elsevierStylePara elsevierViewall">In our study, readmission at 30 days after the hip fracture was very low compared with previous studies (<1% vs. 10–15%), and there were no differences due to the intervention.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">7,14,25</span></a> A number of cultural factors are probably involved in this finding. In Spanish culture, there is considerable acceptance of care from the family for the elderly and for dependent patients, especially by mothers and firstborn daughters. There have been no studies that have investigated the causes that encourage the women in a family to take charge of this task, but there are likely various psychoeducational factors, learned social role factors and ancestral family dynamic factors that are involved. Another factor that can play an important role is that Spanish healthcare does not have many long-stay hospitals, and therefore families are the basis of care for patients.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">26</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The functional status at discharge was similar in the 2 study cohorts, with 10% of the patients bedridden all day and 50% bedridden half of the day and using medical walkers. These data are similar to those of published studies on co-management in hip fractures, with a loss of mobility in 60% and 50% of patients at months 6 and 12 from discharge, respectively.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">3</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">Osteoporosis and vitamin D deficiency are highly prevalent in the elderly, and these conditions represent high risk factors for presenting hip fractures and for a high severity of that fracture.<a class="elsevierStyleCrossRefs" href="#bib0280"><span class="elsevierStyleSup">27,28</span></a> However, only 10% of elderly patients hospitalized for hip fractures were undergoing the appropriate treatment for these processes, and the majority were not taking their medication properly.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">29</span></a> This fact is also evident in our study. In the HIST cohort, the prior diagnosis of osteoporosis and the prescription of bisphosphonates, calcium and vitamin D supplements at discharge were lower than in the COFRAC cohort. This is another interesting aspect of the importance of the preoperative assessment, because it enables optimal treatment of osteoporosis.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Our study has a number of significant limitations. Given its retrospective nature, the study lacked information on the HIST cohort, which prevented us from analyzing a number of variables, specifically, the patients’ functional status and actual prevalence of delirium. We did not analyze the mortality at 1 year from discharge, and we cannot therefore compare the long-term results of our intervention with those of other co-management protocols. The reason for this fact is that we initially only wanted to analyze the impact of our protocol in the hospital. The short-term results show lower mortality in the COFRAC cohort, but without statistical differences.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">7</span></a> Della Rocca reported a similar tendency at 6 and 12 months, but also without statistical differences. This finding likely shows that hip fractures in elderly and frail patients are not only a bone fracture. Hip fractures in frail patients result in a rupture in their biologic equilibrium. In a significant percentage of patients, this equilibrium cannot be recovered, which results in a poor prognosis. We need standard multidisciplinary discharge protocols in which responsibility is shared among primary care, orthopedic surgeons, internists, physical medicine and rehabilitation. In this context, telemedicine and home monitoring can have an important role.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0090" class="elsevierStylePara elsevierViewall">The protocol of co-management between the internal medicine and trauma departments developed for patients hospitalized for hip fractures enable a better preoperative assessment, with better handling of complications and reduced short-term mortality. Our objective is to analyze clinical impact of a co-management care model between orthopedic surgery and internal medicine departments for elderly patients admitted with hip fracture in a tertiary referral hospital.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Retrospective cohort study of patients older than 65 years old admitted with hip fracture between January 2005 and August 2006 (HIST cohort) without a co-management care model, and between January 2008 and August 2010 (COFRAC cohort) with a co-manEdadment care model. Analysis of demographic, clinical and surgery characteristics, complications incidence and mortality and re-admissions at 30 days was made.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A total of 701 patients were included (471 HIST, 230 COFRAC).
