This technique helps assess various elements of the ischemic cascade&#44; such as angina pectoris&#44; electrocardiographic changes and segmental contractility abnormalities of the left ventricle wall&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">There is little information on the possible association between the exaggerated hypertensive response to exercise &#40;EHRE&#41; and the onset of myocardial ischemia&#46; It has been speculated that the intense increase in blood pressure &#40;BP&#41; during exercise and the subsequent increase in myocardial oxygen consumption could trigger subendocardial ischemia&#44; even in the absence of significant coronary stenosis&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">1</span></a> However&#44; this possible association is controversial and has not been investigated through echocardiography on the treadmill and at peak exercise&#46;<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">1&#8211;5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to analyze the association between EHRE and the onset of angina&#44; ischemic electrocardiographic changes and segmental contractility abnormalities in patients with known or suspected coronary artery disease who underwent treadmill exercise stress echocardiography&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Patients</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was an observational retrospective study on a database of 12&#44;215 patients&#44; belonging to the reference healthcare area of a tertiary hospital&#44; who underwent stress echocardiography between March 1&#44; 1995 and June 28&#44; 2013&#46; We excluded 1218 patients from the study for using beta-blockers in the 48<span class="elsevierStyleHsp" style=""></span>h prior to the test<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">6</span></a> and 950 for whom the BP did not increase above its baseline value during the exercise&#46;<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">7&#44;8</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Clinical variables</span><p id="par0025" class="elsevierStylePara elsevierViewall">The presence of diabetes mellitus&#44; arterial hypertension &#40;AHT&#41; and hypercholesterolemia was based on the personal history and on taking specific drugs&#46; A prior history of ischemic heart disease was defined as a history of myocardial infarction&#44; coronary revascularization or angiographic documentation of significant coronary stenosis&#46; We established the following chest pain groups&#58; typical angina&#44; probable or atypical angina and chest pain of nonischemic origin&#44; according to the Diamond scale&#46;<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">9</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">In terms of baseline electrocardiographic data&#44; we recorded the rhythm and those findings that could hinder the interpretation of the test &#40;left bundle-branch block&#44; pacemaker rhythm&#44; left ventricular hypertrophy with overload pattern&#44; digitalis impregnation&#44; preexcitation syndrome or other repolarization disorders&#41;&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Stress test</span><p id="par0035" class="elsevierStylePara elsevierViewall">All patients underwent treadmill exercise stress echocardiography&#46; We obtained data on the heart rate &#40;HR&#41;&#44; BP &#40;using the auscultatory method&#41; and 12-lead electrocardiogram &#40;ECG&#41;&#44; at baseline and at each stage of the exercise protocol&#44; as is performed in conventional stress testing&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">The patients were encouraged to perform a treadmill exercise protocol until one of the following objectives was achieved&#58; physical exhaustion&#44; intense angina&#44; severe arrhythmia with hemodynamic compromise&#44; intense arterial hypertension &#40;AHT&#41; &#40;systolic BP<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>240<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleHsp" style=""></span>Hg or diastolic BP<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>110<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleHsp" style=""></span>Hg&#41; or a hypotensive response to the exercise &#40;reduction of systolic BP<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>20<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleHsp" style=""></span>Hg above baseline&#41;&#46; EHRE was defined&#44; in absolute terms&#44; as a systolic BP response &#8805;220<span class="elsevierStyleHsp" style=""></span>mm Hg at peak exercise&#46; Electrocardiographic ischemia was defined as the onset&#44; during the test&#44; of a change in horizontal&#47;descending ST-segment elevation &#8805;1<span class="elsevierStyleHsp" style=""></span>mm to 80<span class="elsevierStyleHsp" style=""></span>ms of the J point in 1 or more leads&#46; For the analysis of the electrocardiographic ischemia&#44; we excluded 2354 patients for having an uninterpretable baseline ECG&#46; A submaximum test was defined as the inability to reach 85&#37; of the theoretical maximum HR&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Exercise echocardiography</span><p id="par0045" class="elsevierStylePara elsevierViewall">We performed a two-dimensional echocardiogram in the standard apical and parasternal projections at baseline&#44; at peak exercise and immediately after the exercise&#46;<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">10</span></a> The echocardiographic analysis was performed using a digital &#8220;quad-screen&#8221; format&#46; We assessed the segmental motility following the 16-segment model of the left ventricle&#46; Each segment was assessed according to a 4-point scale with the following values&#58; normal motility<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>1&#44; hypokinesia<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>2&#44; akinesia<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>3 and dyskinesia<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>4&#46; We calculated the segmental motility index &#40;SMI&#41; at rest and during exercise as the sum of the scores divided by the total number of segments&#46; Furthermore&#44; we calculated the difference between the SMI in exercise and at rest &#40;&#916;SMI&#41;&#46; Echocardiographic ischemia during exercise was defined as the presence of new onset segmental contractility abnormalities or the worsening of previously existing contractility abnormalities at rest&#44; except for the move from akinesia to dyskinesia<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">11</span></a> and isolated hypokinesia of the inferobasal segment&#46;<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">12&#44;13</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">The categorical variables are expressed as percentages&#44; personal history of coronary artery disease&#44; type of chest pain&#44; atrial fibrillation&#44; baseline left ventricular ejection fraction&#44; baseline BP&#44; HR at peak exercise and functional capacity &#40;METs&#41;&#46; The statistical analysis was performed using the SPSS version 20&#46;0 package &#40;SPSS Inc&#46;&#44; Chicago&#44; IL&#44; USA&#41;&#46;</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0060" class="elsevierStylePara elsevierViewall">We assessed 10&#44;047 patients with a mean age of 62<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>12 years&#44; 60&#46;8&#37; of whom were men&#46; A total of 402 patients &#40;4&#37;&#41; developed an EHRE &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#41;&#46; There were no significant differences in terms of age and sex between those with an EHRE and those without&#46; The patients with an EHRE showed a higher prevalence of diabetes and AHT and a lower frequency of a personal history of coronary artery disease&#46; There were no significant differences regarding the consumption of antihypertensive drugs&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The patients who developed an EHRE had higher baseline systolic BPs and left ventricular ejection fractions &#40;at baseline and at peak exercise&#41;&#44; while their functional capacity was lower&#46; The probability of resting segmental motility abnormalities&#44; echocardiographic ischemia and angina during the exercise was lower in the patients with an EHRE&#46; There were no significant differences regarding the baseline HR or the probability of a submaximum test&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">The prevalence of angina during the exercise was significantly higher in the patients without an EHRE &#40;13&#46;8&#37;&#41; and 21&#37; and 19&#46;1&#37; developed ischemic electrocardiographic changes &#40;<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>&#46;33&#41; &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46; The predictors of an EHRE in the logistic regression analysis are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In the multivariate analysis&#44; the onset of an EHRE was associated with a lower probability of exercise-induced chest pain and echocardiographic ischemia&#46; However&#44; we observed no significant relationship between an EHRE and the electrocardiographic abnormalities of ischemia&#46;</p><p id="par0085" class="elsevierStylePara elsevierViewall">Exercise-induced AHT is often reported as a cause of false positives in the exercise test&#46; A relationship has been reported between an EHRE and the onset of ST-segment depression&#44; which has been attributed to increased myocardial oxygen consumption&#46;<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">1</span></a> In fact&#44; young and healthy individuals can develop ST-segment depression and ventricular contraction abnormalities during sudden and vigorous physical exercise&#46;<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">14</span></a> Moreover&#44; it is possible that an EHRE promotes the development of ventricular hypertrophy&#44; with the consequent reduction in coronary reserve&#46;<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">15</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">However&#44; as with this study&#44; other studies have also observed a relationship between an EHRE and ST-segment changes&#46;<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">2&#44;3</span></a> These studies also had angiographic data that demonstrated a lower prevalence of coronary artery disease in patients with an EHRE&#46;<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">2&#44;3</span></a> Moreover&#44; Kane et al&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">2</span></a> observed no significant relationship between an EHRE and the presence of myocardial perfusion defects&#46; However&#44; Smelley et al&#46;<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">16</span></a> concluded that an EHRE is associated with a high prevalence of transient ischemic dilation in single-photon emission computed tomography images of myocardial perfusion in patients with no other perfusion defect&#46; The authors linked this result with possible EHRE-induced global subendocardial ischemia&#46; Similarly&#44; Ha et al&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">4</span></a> reported that an excessive BP increase during exercise is associated with the onset or worsening of contractility abnormalities&#44; which can be seen in the absence of significant coronary stenosis verified angiographically&#46; However&#44; Jurrens et al&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">5</span></a> found that an EHRE is not related to false positives in exercise echocardiography&#44; because most patients with an EHRE and positive echocardiography showed truly significant coronary artery stenosis&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">This study has the limitations inherent in observational retrospective studies whose sample comes from a single center&#46; These results are only applicable to patients who undergo treadmill stress tests for a clinical indication and cannot be extrapolated to other patient subgroups&#44; such as those who are asymptomatic&#46; Despite excluding patients treated with beta-blockers in the 48<span class="elsevierStyleHsp" style=""></span>h prior to the test&#44; a residual effect of these drugs cannot be ruled out&#46; Lastly&#44; we did not analyze the coronary angiography findings for the patients for whom it was considered necessary&#46; That analysis would have entailed a significant selection bias&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">In conclusion&#44; the exaggerated increase in systolic BP during exercise could be a marker for ruling out coronary ischemia&#46; In this study&#44; an EHRE was negatively and independently associated with the onset of chest pain and echocardiographic ischemia in patients with known or suspected coronary artery disease&#46;
