Un reto diagnóstico" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">The elderly population has a greater risk of venous thromboembolism disease (VTE), including pulmonary embolism (PE),<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">1,2</span></a> and usually has poorer outcomes due to a greater risk of adverse effects such as recurrence, bleeding and death related to pulmonary embolism event.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Aging is associated with an increase in comorbidities (e.g., heart failure, lung disease), which can hinder the diagnosis. The classical triad of symptoms of PE (sudden dyspnea, pleuritic chest pain and hemoptysis) do not always manifest completely in our patients and are not always specific to this entity, thereby resulting in a disconcerting initial evaluation of these patients for clinicians.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Small retrospective series of elderly patients (≥65 years) with confirmed PE have reported that the clinical presentation differs from that of the younger population, with syncope more prevalent in the elderly population and pleuritic pain more prevalent in the young population.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">5–7</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">One of the consequences of managing elderly patients (≥80 years) is the risk in the diagnostic delay from symptom onset to the diagnosis of a pulmonary embolism (PE). Despite the improved availability of imaging techniques such as computed tomography pulmonary angiography (CTPA), exposing this population (patients ≥80 years of age) to expensive techniques with a high risk of contrast-induced renal toxicity, in the absence of a clearly high clinical suspicion/probability of PE, is not considered an acceptable strategy, given that these patients have a high prevalence of renal failure<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">8</span></a> and a high risk of contrast-induced nephropathy.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">9</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Based on the published data, we proposed a review of the differential characteristics of our cohort of patients with PE, between the subgroup of elderly patients with PE (≥80 years) and the younger patients with PE (<80 years) to define the differential characteristics that help better manage this condition in this group age.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Materials and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">This was an observational, case–control, hospital-based study to report the incidence of PE according to age subgroup (elderly/young) in our series and detect the potential differences in clinical manifestations, comorbidity and laboratory parameters between the 2 subgroups. We conducted a comprehensive and retrospective collection by reviewing the digital medical records of all patients diagnosed with PE in the Department of Internal Medicine at our center (Hospital Virgen Macarena, Seville, Spain) between 2013 and 2016 inclusive. As inclusion criterion, we selected the patients with PE confirmed by CTPA in all cases registered during the collection period. We considered patients 80 years of age or older at the time of the thromboembolic event as elderly patients and the other patients as young patients. We defined the classic presentation triad of PE as the combination of sudden dyspnea, pleuritic pain and hemoptysis, considering those episodes that showed at least 2 of the 3 symptoms to be the typical presentation form.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The recorded variables are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">The data were processed with the SPSS statistical package v.24.0. The quantitative variables were compared with Student's <span class="elsevierStyleItalic">t</span>-test, and the qualitative variables were analyzed using the chi-squared test. A <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 was considered statistically significant.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">We included 413 patients with PE in the 4 years reviewed (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), 30% (124) of whom were elderly. The median age for the overall cohort was 72 years (interquartile range [IQR], 58–81). The elderly subgroup had a median age of 84 years (IQR, 81–87), and the young patients had a median age of 65 years (IQR, 52–73; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0001).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">There was a slight predominance of women in the overall cohort (52%), which when, analyzing by subgroup, showed a clearly defined predominance of women in the elderly group (71% vs. 44% for the elderly and young groups, respectively; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0001).</p><p id="par0055" class="elsevierStylePara elsevierViewall">In terms of the clinical presentation of PE, the classic triad (defined as the combination of dyspnea sudden, pleuritic chest pain and hemoptysis) was uncommon in our series (17 patients). The typical presentation form (defined as the presence of at least 2 of the symptoms listed above) also showed a difference between the subgroups. When we examined the symptoms individually, dyspnea was the most prevalent symptom in both subgroups, although it was predominant in the elderly group without reaching statistical significance. Chest pain was the second most common symptom and significantly more common in the younger group. The combination symptom of cough and hemoptysis predominated in the elderly group, and syncope and signs of deep vein thrombosis and edema showed no significant differences between the subgroups.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Comorbidity, measured by the Charlson index, showed a mean value of 2 points more in the elderly subgroup, which was statistically significant (4.88<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.55 vs. 2.89<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.85; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0001). This difference was based on a larger volume of heart disease and patients with chronic kidney disease (CKD) (glomerular filtration rate <50<span class="elsevierStyleHsp" style=""></span>mL/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span>). In contrast, lung disease (e.g., chronic obstructive pulmonary disease, asthma), active cancer and a history of VTE showed no significant differences between the subgroups.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Reviewing the unit where the diagnosis of PE was made, the emergency department detected the most events (69%; 285 cases), with similar percentages in the analysis by subgroup. The rest of the PE diagnoses were performed in hospitalization areas and outpatient clinics of the hospital.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The selection of the candidates for CTPA was based on the use of validated predictive scales. Thus, when reviewing our series, the simplified Wells scores showed no significant differences between subgroups.</p><p id="par0075" class="elsevierStylePara elsevierViewall">In terms of the laboratory parameters, we found no significant differences in C-reaction protein or <span class="elsevierStyleSmallCaps">d</span>-dimer values; however, there were significant differences in the mean creatinine and troponin T (TnT) values, which were higher in the elderly group. There were also differences in the median proBNP values, although without reaching statistical significance.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Early mortality (first 30 days after the diagnosis) associated with PE in our series (13.3% of the series) showed no significant differences between subgroups. Of the 55 registered deaths, acute renal failure was only recorded in 3 patients, although the failure was not clearly attributed to contrast-induced renal toxicity. Some 41.8% (23 cases) of the cases were due to metastatic cancer, 41.8% (23 cases) were directly attributed to the PE, and 3.6% (2 cases) were attributed to hemorrhagic complications caused by anticoagulation. The remaining 9% were due to infectious complications. There were no differences between the subgroups.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0085" class="elsevierStylePara elsevierViewall">The incidence of PE in the elderly group in our cohort was very high. One of every 3 patients belonged to elderly subgroup (30%), which confirms the importance of deepening our understanding of the behavior of PE in this age group, which has not always been well studied.</p><p id="par0090" class="elsevierStylePara elsevierViewall">According to the data, the elderly and young groups were not comparable in terms of sex due to a clear and significant predominance of women in the elderly subgroup (71% vs. 44.3%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.0001), a finding mainly due, in our opinion, to women's longer life expectancy.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Our study quantified and confirmed the considerable increase in comorbidity in the elderly group compared with the young group, establishing on average an increase of up to 2 percentage points in the Charlson index in our series data, which agree with those in the consulted literature.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">4</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">Influencing the nosological entities that accompany patients during the thromboembolic event, the presence of heart disease and CKD were the comorbidities that presented a statistically significant difference in favor of the elderly subgroup. This fact is especially interesting given that dyspnea and chest pain were the most common presentation symptoms of PE in our series and is usually shared with heart failure and ischemic heart disease, which can increase the diagnostic delay for PE in older patients reported in the literature.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">10</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">It is worth noting that the prevalence of CKD in our series increased to 32.3% in the elderly group. CTPA entails a potential risk of renal toxicity and volume overload induced by the administered renal protection, although there were no significant differences between the subgroups.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The risk of cancer is known to increase with age, and there is an association between thrombosis and cancer. We would therefore expect the elderly subgroup to have more cases of active neoplasia. This assumption was rejected based on our results.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The percentages of patients diagnosed in the emergency department were very similar in the 2 subgroups, with no significant differences, which leads us to think that, for the same reason, the diagnostic delay in the elderly subgroup in our series should not be significant, although it was not expressly calculated.</p><p id="par0120" class="elsevierStylePara elsevierViewall">When reviewing our data, we see that the recommended predictive scales (simplified Wells) showed no differences between the subgroups and therefore does not help in selecting patients based on the age criterion.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In terms of the reviewed laboratory parameters, we detected no differences in the median <span class="elsevierStyleSmallCaps">d</span>-dimer levels between the subgroups, although there was a tendency toward increased <span class="elsevierStyleSmallCaps">d</span>-dimer values in the elderly subgroup. The differences in proportions according to sex and comorbidity detected in the subgroups, as well as the sample size, could have prevented statistical significance from being reached. The literature indicates that the cutoff for <span class="elsevierStyleSmallCaps">d</span>-dimer values, starting from the age of 50 years, should be stratified according to the patient's age. Previous studies have shown that ruling out PE in patients older than 80 years using typical <span class="elsevierStyleSmallCaps">d</span>-dimer levels is possible in only 5% of patients, compared with >50% for patients younger than 50 years.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">11</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The statistically significant differences in creatinine and TnT values detected in the elderly subgroup are consistent with the greater prevalence of previous heart disease and CKD observed in the elderly subgroup, which is probably due to the increased prevalence of these entities with age.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In conclusion, the difference in the median proBNP values allows us to state that this parameter could be employed to provide additional information in the diagnostic decision-making process in this elderly and polypathological patient subgroup.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0140" class="elsevierStylePara elsevierViewall">The present study shows that PE in elderly patients is a relatively common reason for hospitalization in internal medicine departments and confirms the presence of differences in clinical presentation, comorbidity and laboratory parameters between the elderly subgroup and younger patients.</p><p id="par0145" class="elsevierStylePara elsevierViewall">However, the classical presentation triad, the predictive scales and <span class="elsevierStyleSmallCaps">d</span>-dimer levels, which are the parameters employed for selecting candidates for CTPA, showed no significant differences between the subgroups, which, combined with the increased comorbidity in the elderly group, contributes to the increased difficulty in diagnosing PE in this subgroup.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The presented data, obtained from daily clinical practice, help us better understand the behavior of PE in the elderly patient cohort, which enables a more objective approach for managing this entity in this age subgroup.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span
