Its repercussion is even greater when it affects young individuals who are actively employed.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Although the disease has been classically associated with elderly patients, it can occur at any age. IBERICTUS, the largest epidemiological study on stroke conducted in Spain, estimated a stroke rate in 2006 among patients 55 years of age or younger of 9.4%,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">2</span></a> a significant incidence rate.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Stroke in young individuals (especially those younger than 45 years) has been traditionally related to uncommon conditions such as hypercoagulability states, genetic diseases and vascular malformations. In recent years, however, an increase has been observed in the incidence rate of stroke in relation to classical vascular risk factors such as smoking, arterial hypertension, dyslipidemia, obesity, diabetes<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">3–9</span></a> and psychosocial stress, a difficult to quantify condition that encompasses various interrelated elements such as occupational stress, financial problems, family conflicts and psychological disorders.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In this study, we analyzed the stroke incidence rate and characteristics of patients 55 years of age or younger who were admitted to a stroke unit of a tertiary hospital. The study sought to determine whether the onset of stroke in young patients was related to the presence of classical vascular risk factors or to psychosocial stress.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">Our hospital is a tertiary stroke centre belonging to the Madrid Stroke Network.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">10</span></a> We conducted a retrospective study based on a prospective registry of patients 55 years of age or younger with the final diagnosis of stroke who were admitted to our stroke unit in 2014. Patients with traumatic, subdural or subarachnoid hemorrhage were excluded from the study. The patients were treated according to the recommendations of the clinical practice guidelines of the Spanish Society of Neurology and of the 2010 Stroke Care Plan II.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">11–13</span></a> The study was approved by our clinical research ethics committee.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The following variables were collected according to a predefined protocol: demographic data, vascular risk factors (known or discovered during hospitalisation, such as smoking, arterial hypertension, diabetes mellitus, dyslipidemia, obesity, atrial fibrillation and emboligenic heart disease), toxic habits (alcoholism and drug consumption), presence of patent foramen oval, the taking of oral contraceptives, active oncologic disease, bleeding disorders (congenital or acquired), migraine and psychosocial stress. Psychosocial stress was defined as a state of anxiety, tension, difficulty getting to sleep or irritability related to external problems, especially family, financial and occupational. The patients were asked whether they felt stressed during the period prior to the stroke and to complete the Perceived Stress Scale (PSS) to quantify the degree of stress during the previous month.<a class="elsevierStyleCrossRefs" href="#bib0225"><span class="elsevierStyleSup">14,15</span></a> Of the various validated versions, including the original with 14 items and the 2 brief versions with 10 and 4 items, we used the latter (PSS-4) due to its greater applicability. The scale does not use cutoff points and ranges from 0 to 16 points, representing minimum and maximum stress, respectively (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">All patients were urgently assessed by the on-duty neurologist (with or without activation of the outpatient stroke code) and underwent a general and neurological examination with the National Institute of Health Stroke Scale, laboratory tests, chest radiography, electrocardiogram and cranial computed tomography. The patients who met the acute-phase treatment criteria underwent reperfusion therapy (intravenous thrombolysis, endovascular treatment or both), following the recommendations of the clinical guidelines.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">11,16</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">During hospitalisation, the etiological study was completed with neurovascular imaging tests (computed tomography angiography or neurosonology), cranial magnetic resonance imaging and cardiological tests (echocardiogram and 24-h Holter electrocardiogram). In some cases, specific tests were performed to detect coagulation, autoimmune and infectious diseases.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The stroke type was classified according to World Health Organization criteria (cerebral infarction, transient stroke, intracranial hemorrhage and cerebral venous thrombosis). We used the Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification to divide the subtypes of ischemic stroke into atherothrombotic, cardioembolic, lacunar, stroke of uncommon etiology and stroke of undetermined etiology.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> The clinical progress was determined using the modified Rankin scale (mRS),<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">18</span></a> and functional independence at 3 months (mRS 0–2) was considered good progress.</p><p id="par0050" class="elsevierStylePara elsevierViewall">For the statistical analysis, we employed the SPSS 21.0 program (for Windows). The statistical Pearson's chi-squared test and Fisher's exact test were employed to compare the categorical variables, and Student's <span class="elsevierStyleItalic">t</span>-test was used to compare the means. A <span class="elsevierStyleItalic">p</span>-value <.05 was considered significant. We conducted a subgroup analysis dividing patients into age groups (≤45 and >45 years) and by sex.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall">During 2014, 830 patients with a final diagnosis of stroke were admitted to our unit. Of these patients, 110 (13.3%) were 55 years of age or younger and were predominantly male (60.9%). Thirty-five patients (4.2%) were 45 years of age or younger, most of whom were women (19).</p><p id="par0060" class="elsevierStylePara elsevierViewall">The age range was 15–55 years, with a mean age of 46.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.1 years and a median of 49 years. The mean age was greater for the men (47.7 vs. 44.4 years, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05). The overall incidence rate increased exponentially with age (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">A total of 92 patients (83.6%) had ischemic strokes, 73 (66.4%) of which were cerebral infarctions and 19 (17.3%) of which were transient ischemic attacks (TIA). Of the total, 14 patients (12.7%) had an intracranial hemorrhage, and 4 (3.6%) had a cerebral venous sinus thrombosis.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Some 87.6% of the patients had at least 1 classical vascular risk factor, and 65% had 2 or more risk factors. These percentages in the 45 years or younger patient group were 71.4% and 45.7%, respectively. Smoking was the most common classical risk factor (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Most of the risk factors were more common in the over 45 year group, although only hypertension (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.01) and alcohol consumption (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03) achieved statistical significance. In the men, hypertension (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001), smoking (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.008) and diabetes (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.02) were more common than in the women, who had a higher prevalence of migraine (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.02). Many of the patients were unaware of their risk factors: 64.3% of cases of emboligenic heart disease, 51.1% of cases of dyslipidemia, 25% of cases of diabetes and 18.2% of cases of arterial hypertension were discovered during the hospitalisation.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Some 57.2% of the patients reported psychosocial stress in the stage prior to the stroke. The mean PSS score was 5.2, which was greater in the patients with stress (7.3 vs. 1.9, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05) and in the women (6.4 vs. 4.4, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05). Among the various types of stroke, there were no significant differences in the percentage of patients with stress or in the PSS scores.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The proportions for each stroke subtype are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>. The most prevalent uncommon cause for ischemic stroke was arterial dissection (7 patients, 7.6% of the ischemic strokes). The other uncommon causes were neoplasia by a procoagulant mechanism (3 cases), antiphospholipid syndrome (1 case), systemic lupus erythematosus (1 case), isolated vasculitis of the central nervous system (1 case), paradoxical embolism associated with economy class syndrome (1 case)<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">19</span></a> and a hemodynamic stroke related to prolonged syncope (1 case).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">There were 14 cases of intracranial hemorrhage, of which 12 (85.7%) occurred in men and 11 (78.6%) were hypertensive. In the other patients, the etiology was an acenocoumarol overdosing, a brain cavernoma and a pheochromocytoma. The 4 cases of cerebral venous thrombosis occurred in women; 3 were smokers and took oral contraceptives, and 2 also had primary thrombophilia. In another patient, no associated factor was found.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The mean hospital stay was 8 days (range, 1–60 days). Some 82.9% of the patients had a favourable prognosis at 3 months, 14.3% were left with severe sequela (mRS 3–5), and 3 patients (2.8%) died.</p><p id="par0095" class="elsevierStylePara elsevierViewall">A total of 17 patients (23.3% of those with cerebral infarctions) benefitted from reperfusion therapy in the acute phase: 10 (13.7%) underwent intravenous fibrinolysis, 3 (4.1%) underwent endovascular treatment and 4 (5.5%) underwent both treatments. The severity of the stroke measured with the National Institute of Health Stroke Scale was greater in the treated patients than in the untreated patients (12.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>7.5 vs. 4.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.3, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). The mean hospital stay was similar (8.6 vs. 7.9 days, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.76), and the proportion of patients with a favourable prognosis was smaller than in the patients who were treated (62.5% vs. 86.5%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.03). There were no deaths or hemorrhagic transformations among those who underwent reperfusion therapy.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">This study reveals the stroke profile of young patients who were admitted to a specialized unit in Spain. The age limit for considering that a stroke occurred in a young individual varies, and most studies set a limit between 45 and 55 years, without a specific criterion.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">3–8</span></a> In this study, we opted for 55 years to compare the proportion of strokes in young people with the data shown by the IBERICTUS study.</p><p id="par0105" class="elsevierStylePara elsevierViewall">In terms of the mean age and sex distribution, there were no differences compared with other studies with similar age intervals.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">6,8</span></a> Both in the men and women, the vascular risk factors were more common than those observed in the general population.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">20</span></a> The risk factors were more common in the men and in the over 45 year group, which would explain the predominance of men and the exponential increase in stroke observed with age. It is noteworthy that almost 3 of every 4 patients of the 45 years or younger group had some vascular risk factor, regardless of stroke etiology.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Compared with other studies, there was a greater overall prevalence of classical vascular risk factors, with higher rates of smoking, hypertension, obesity and diabetes.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">6,8</span></a> It is also worth noting the high percentage of patients whose risk factors were discovered during the hospitalisation. The high frequency of these risk factors, unexpected in the 45 years or younger group and in many cases unknown to the patients, indicated the need to implement intensive prevention strategies for the young population to prevent a first vascular event or recurrence. It has been shown that the greater the accumulation of risk factors in young adults, the greater the risk of recurrence of ischemic events.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">21</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">One of the novel concepts introduced in the study was the analysis of the role of psychosocial stress in patients with stroke, which affected 6 of every 10 patients. There is moderate evidence that work-related stress (excessive psychological demands, lack of social support, pressure, etc.), conflicts, crises and family stress increase the risk of cardiovascular disease.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">22</span></a> Various studies have recently observed an association between stress and cerebrovascular disease, which is independent of other risk factors.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">15,23,24</span></a> It is known that stress influences a series of biological mechanisms and promotes unhealthy habits and lifestyles such as smoking, eating poorly, physical inactivity and poor treatment adherence.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">22</span></a> There is evidence that psychological intervention programs based on treating the stress, such as cognitive behavioral therapy, can help improve the vascular risk profile and promote healthy lifestyles.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">25</span></a> We believe that working-age patients who have experienced a stroke and have a high degree of psychosocial stress could benefit from psychotherapy targeted at stress control as part of the secondary prevention program for stroke.</p><p id="par0120" class="elsevierStylePara elsevierViewall">In terms of the stroke type, the proportion of ischemic and hemorrhagic strokes was similar to that reported in the literature.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">26</span></a> Cryptogenic stroke is the most common in most stroke series in young patients, representing between a third and a half of all cases.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">8</span></a> However, the TOAST classification has strict criteria for classifying stroke events as atherothrombotic (stenosis greater than 50% of the responsible artery), which would explain why many stroke events are classified as cryptogenic, even though they occur in patients with carotid atheromatosis who have other vascular risk factors whose relationship with stroke is well documented. The proportion of ischemic stroke subtypes varies from study to study.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">27</span></a> In our series, cryptogenic stroke was followed in frequency by lacunar infarction, a finding observed in only 1 other registry,<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">28</span></a> which is explained by the high rate of arterial hypertension in the absence of arterial stenosis. Stroke of cardioembolic etiology was relatively uncommon, although it is known that occult atrial fibrillation can underlie certain cryptogenic stroke events.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">29</span></a> Despite the high prevalence of risk factors in the 45 years and younger group, only a third of the stroke events were attributed to atherothrombosis, cardioembolism or small-vessel disease, which indicates that the presence of vascular risk factors could contribute to the onset of stroke by other mechanisms.</p><p id="par0125" class="elsevierStylePara elsevierViewall">It should be emphasized that the most common cause of intracranial hemorrhaging was hypertension (78.6%), and only 3 cases were caused by other mechanisms. In general, the etiology of intracranial hemorrhaging is similar to that of the older patients, with a greater representation of arteriovenous malformations and coagulation diseases to the detriment of those related to taking anticoagulants or amyloid angiopathy.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">26</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Some 82.9% of the patients had favourable clinical progress at 3 months, a finding similar to that observed in other studies that showed functional independence rates between 78% and 94%.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">30</span></a> We should take into account that this percentage also included transient ischemic attacks and minor strokes, which have a better overall prognosis. Considering only ischemic stroke treated with reperfusion therapy (for the most part severe strokes), approximately two-thirds of the patients were independent at 3 months, a result that confirms that young patients generally have better prognoses than older patients.<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">7,31</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">The main study limitations are its retrospective character and the low specificity of the stress scale.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0140" class="elsevierStylePara elsevierViewall">In this study, the proportion of strokes in young patients was 30% higher than that of the IBERICTUS study, which indicates that the incidence rate for stroke could be increasing in this population. This increase appears to be due to a high rate of vascular risk factors in a segment of the population who are unaware of their risk profile or who do not adequately control it. Most of the vascular risk factors are modifiable, psychosocial stress included. We therefore need to promote appropriate prevention strategies to avoid vascular events. We recorded the incidence rate for all strokes and analysed demographic data, risk factors, degree of stress, stroke type and aetiology, reperfusion treatments and clinical evolution.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The study included 110 patients, the majority of whom were men (60.9%, 1.6:1 ratio). The incidence rate was 13.3% (110 of 830 strokes). Most of the patients had cardiovascular risk factors. Smoking was the most common risk factor (56.4%), followed by arterial hypertension (50%), dyslipidaemia (42.7%), obesity (33%), diabetes (18.2%) and emboligenic heart disease (12.7%). Some 64.3% of the heart disease cases and 51.1% of the dyslipidaemia cases were discovered during hospitalisation. Some 57.2% of the patients experienced psychosocial stress in the stage prior to the stroke. Some 83.6% of the stroke cases were ischaemic, 12.7% were haemorrhagic and 3.6% were venous sinus thrombosis. Of the ischaemic stroke cases, 30.4% were cryptogenic, 23.9% were lacunar, 16.3% were from uncommon causes, 15.2% were atherothrombotic and 14.1% were cardioembolic. Some 78.6% of the cerebral haemorrhage cases were hypertensive.
