Despite developments in its diagnosis and treatment, HF is associated with high morbidity and mortality and creates considerable healthcare expenditures, mainly due to the costs resulting from hospital admissions.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Fluid overload has a decisive role in the etiopathogenesis of HF in the onset of clinical decompensations. However, the clinical assessment of systemic congestion is still suboptimal due to the low sensitivity of the classical examination criteria and the various commonly used additional tests, such as plain chest radiography, the laboratory measurement of natriuretic peptides and transthoracic echocardiography.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Therefore, there is a need to incorporate new diagnostic techniques that help estimate the hydration state of patients with HF simply and reproducibly, with a favorable cost-benefit relationship and broad availability. This situation is reinforced by evidence that suggests that many patients hospitalized for HF are discharged with a certain degree of systemic congestion, which could promote new decompensation episodes.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">4,5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In this context, bioelectrical impedance vector analysis (BIVA) helps assess body composition and calculate the total body water (TBW), analyzing the resistance offered by the body to the passage of alternating electrical current. TBW calculated by BIVA correlates with the central venous pressure<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">6</span></a> and has shown its clinical utility in the differential diagnosis of patients with borderline natriuretic peptide levels, providing prognostic information on short-term morbidity and mortality.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">7</span></a> Núñez et al.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a> recently demonstrated the long-term prognostic value of BIVA prior to discharge. In their study, both the dehydration (deH) and hyperhydration (hyperH) states were associated with a significantly greater risk of mortality and readmission for HF, especially among the patients with hyperH, with a linear increase in the risk for readmission for HF (hazard ratio [HR], 1.06 [1.03–1.10], <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.001 for every 1% increase in the BIVA value).<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The objective of the present study was to confirm, in a new prospective cohort, the prognostic role of assessing systemic congestion using BIVA in patients hospitalized for acute decompensated HF.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">We analyzed a prospective cohort of 133 patients hospitalized in the Department of Cardiology at our center between January and November 2014, with the diagnosis of <span class="elsevierStyleItalic">de novo</span> HF or acute decompensated HF. We excluded patients who died (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>6), who underwent valve surgery (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>13) during hospitalization, who had significant coronary artery disease and were awaiting surgical revascularization (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>5) or who had dyspnea not due to HF (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4). The final sample included 105 patients.</p><p id="par0035" class="elsevierStylePara elsevierViewall">We recorded the patients’ history, electrocardiographic and laboratory parameters (including the aminoterminal fraction of the brain natriuretic peptide [NT-proBNP]) and echocardiographic variables. All patients received the medical treatment indicated by the responsible clinician, following the recommendations in the current guidelines,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">9</span></a> which include the endovenous administration of loop diuretics.</p><p id="par0040" class="elsevierStylePara elsevierViewall">BIVA was conducted using a CardioEFG<span class="elsevierStyleSup">®</span> model (Akern Srl, Italy), with the patient supine, and through the application of electrodes to the dorsal region of the ipsilateral hand and foot, according to the manufacturer's instructions. Three measurements were taken per patient, using the mean of the measurements for the analysis. The analysis was performed after clinically stabilizing the patient and prior to hospital discharge, hiding the BIVA results from the responsible physician. The sample was divided into 3 pre-established groups according to the hydration state determined by the BIVA: hyperH (>74.3%), normal hydration [normoH] (72.7–74.3%) and deH (<72.7%).<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">10</span></a> The range of normality was determined by the device itself, according to the reference values for the general population (73.3%)<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1 standard deviation.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">11</span></a> Each BIVA is reflected in the device in a hydrograph that enables an immediate assessment of the hydration state.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In the statistical analysis, the continuous variables are expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>standard deviation, and the qualitative variables are expressed as percentages. The baseline characteristics were compared among the 3 groups using analysis of variance (ANOVA) and Kruskal–Wallis test for variables without a normal distribution. In the follow-up, total mortality, readmissions for HF and their combination were considered adverse events. The survival analysis was performed with the Kaplan–Meier method and the proportional hazard risk (HR) using the Cox model, in which all variables with <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.10 were included in the univariate analysis. Results with <span class="elsevierStyleItalic">p</span>-values<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05 were considered significant. The statistical analysis was performed using the statistical software IBM SPSS<span class="elsevierStyleSup">®</span> version 22.0.</p><p id="par0050" class="elsevierStylePara elsevierViewall">All patients signed the informed consent document, and the study was approved by our center's Clinical Research and Clinical Trials Ethics Committee.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the patients’ baseline characteristics. The sample's mean age was 69.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>13.2 years, and all patients took the drug treatment recommended in the clinical practice guidelines. Less than half of the patients (47.6%) had a normoH condition at discharge (22.8% hyperH and 29.6% deH). The groups were comparable in terms of the HF etiology, drug treatment, cardiovascular risk factors, laboratory variables and echocardiographic parameters.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">During a mean follow-up of 11 (1–18) months, a total of 37 adverse events were recorded: 29 readmissions for HF and 19 deaths. After analyzing the incidence of the combined event in the 3 groups by the Kaplan–Meier method, we observed a tendency towards a higher rate of events in the patients from the hyperH and deH groups compared with the normoH group (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). When grouping the patients from the hyperH and deH groups and comparing them with the normoH group, we confirmed a higher incidence of events during the follow-up in the hyperH group (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B). The increased risk was independent of age, the presence of atrial fibrillation and creatinine levels (Cox regression analysis: deH-hyperH group had an HR of 2.6; 95% confidence interval 1.05–6.44; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.039), as variables included in the multivariate analysis.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">The results of this study are consistent with those published by Nuñez et al.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">8</span></a> and reaffirm the potential clinical and prognostic value of BIVA in patients with HF, both in TBW monitoring (which provides better direction for diuretic therapy) and risk stratification at discharge.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The data from this validation cohort also reinforce the prognostic importance of reaching a condition of systemic normoH at discharge, such that both systemic congestion (values in the hyperH range) and excessive fluid depletion (readings in the deH range) negatively affect the patient's clinical progress. The clinical importance of this condition has been debated in the literature.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">12,13</span></a> In this context, BIVA could contribute to a more objective and reproducible definition of the optimal state of systemic hydration in HF.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Taking into account the enormous rate of hospital readmissions that HF still generates in our community (despite the creation of specialized management units<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">14</span></a>) and considering systemic congestion as the central pathophysiological axis of the acute decompensation episode, the use of a simple and available technique such as BIVA could play a role in optimizing treatment, especially when combined with the measurement of congestion biomarkers, such as natriuretic peptides and CA125. The combined use of both noninvasive techniques is promising, although larger studies are needed to define a definitive consensus on its routine use.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">15</span></a></p><p id="par0080" class="elsevierStylePara elsevierViewall">The study's limitations are its small sample size and single-center nature. Furthermore, the need to group the deH and hyperH states when comparing them with the normoH state to obtain statistical significance in the survival analysis can be considered a limitation, although the objective of demonstrating a greater risk in the incidence of events in patients outside the normoH range was achieved.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusions</span><p id="par0085" class="elsevierStylePara elsevierViewall">In the assessment of systemic hydration using BIVA prior to discharge in patients hospitalized for acute decompensated HF, the deH and hyperH states are associated with an increased risk of readmission for HF and total mortality compared with patients with normoH in the long-term follow-up.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Funding</span><p id="par0090" class="elsevierStylePara elsevierViewall">Clinical Research Grants from the <span class="elsevierStyleGrantSponsor" id="gs1">Valencian Society of Cardiology</span> (2013 and 2014 calls) were awarded to O.
