According to various studies, the rate of readmission in Spain is estimated to be between 15% and 25%.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> When patients require several hospital readmissions within a period of time, we refer to this as repeated hospital admissions or multiple hospitalizations. The definition of multiple hospitalizations varies from one study to the next, such that multiple hospitalizations can be understood as the occurrence of 2 or more hospitalizations in a year or the occurrence of 2 or more hospitalizations within several years.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–5</span></a> In Spain, multiple hospitalizations can represent more than 30% of hospitalized patients in a department of internal medicine (MI).<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Patients with multiple hospitalizations are characterized by advanced age,<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5–7</span></a> having complex chronic diseases,<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7–10</span></a> having high mortality rates<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> and for requiring a considerable amount of resources. Although patients with multiple hospitalizations as a whole represent a small fraction of hospitalized patients, they generate a high proportion of hospital expenditures, to the extent that the literature frequently addresses the problem of multiple hospitalizations from the perspective of high-cost patients.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Due to progressive aging of the population and the high prevalence of chronic diseases, the healthcare approach to complex chronicity in Spain is currently a major challenge. Its importance is derived not only from the growing societal need, but also from the need to adjust and optimize financial resources directed at the public healthcare model.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> In this context, patients with multiple hospitalizations could be an indicator for an analysis of these healthcare needs, not only because they use the hospital resource more frequently (which is a high-cost resource), but also because multiple hospitalization is probably an indirect indicator of proximity to death, the time during which the largest healthcare expenditure in the life of an individual is produced.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">The primary objective of this study was to determine the differential characteristics of the admissions of patients with multiple hospitalizations in a cohort of patients treated by a department of IM. The secondary objective was to identify the factors associated with recurrent hospitalizations and especially those factors that enable us to quickly recognize patients at risk for repeated hospitalizations.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Patients and method</span><p id="par0040" class="elsevierStylePara elsevierViewall">This was a prospective study of a cohort of 613 consecutive hospitalizations that took place at the Department of IM at the Hospital Complex of Navarra A (CHN-A) between April 2 and August 10, 2011. The Department of IM of CHN-A is a department with 36 beds integrated into a tertiary hospital of 499 beds. A rehospitalized patient was defined as one who at the time of admission had been hospitalized 2 or more times in the past year, including the current admission. A patient with multiple hospitalizations was defined as one who at the time of admission had been hospitalized 3 or more times in the past year, including the current admission. Prior hospitalizations could have been in departments other than IM and for causes other than the reasons for the new hospitalization. The collection of hospitalizations was thus divided into 2 categories: admission of patients with multiple hospitalizations and admission of patients without multiple hospitalizations. The study did not include hospitalizations that were scheduled for a specific diagnostic or nonsurgical therapeutic procedure performed in less than 48<span class="elsevierStyleHsp" style=""></span>h and without complications (for example, hospitalization solely for conducting an endoscopic study or puncture/biopsy). Therefore 52 hospitalizations were excluded from the study, which represented 7.8% of the hospitalizations that occurred between the dates the sample was collected.</p><p id="par0045" class="elsevierStylePara elsevierViewall">In one part of the study, we compared episodes of multiple hospitalizations with episodes that were not multiple, and we analyzed the factors found to be related to patients having multiple hospitalizations or not. These factors included demographic characteristics (age, sex, rural or urban origin and living at home or institutionalized), the patient's clinical profile prior to hospitalization (number and type of diseases, Katz Index score,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> number of regular medications), the characteristics of the hospitalizations (duration, cause, number and type of diagnoses at discharge, social problems detected, Charlson index score<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>) and patient mortality during hospitalization. The other part of the study was a prospective analysis of the cohort of patients with and without multiple hospitalizations, which assessed the number of readmissions (in IM or another department) and the causes for readmission in the 6 months after discharge, as well as patient mortality in the 6 and 12 months following discharge.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The sources of information were the registries of the computerized medical history (CMH), which is an information technology tool used by the public hospitals of Navarra. The discharge diagnoses included both the primary and secondary diagnoses. The various causes for hospitalization and readmission were not exclusive, and several causes for hospitalization could occur at the same time (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The social problems detected were also not exclusive and included lack of family support, family coexistence problems, lack of social support, lack of sufficient financial resources and housing that did not meet the minimal requirements for habitability and/or functionality.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">To check the normal distribution of the variables, we performed a Kolmogorov–Smirnov test. The analysis of qualitative and ordinal variables was conducted using the chi-squared test, and the continuous quantitative variables were analyzed using the Mann–Whitney <span class="elsevierStyleItalic">U</span> test and the Kruskal–Wallis test, establishing a level of statistical significance of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05. We used binary logistic regression (forward, conditional) to adjust for statistically relevant variables (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.05) in relation to the dichotomous grouping based on multiple hospitalizations. All analyses were performed using the SPSS (version 17.0) program using the exact tests package. We used the Wilson method without continuity correction to calculate the confidence intervals of the proportions. The study was approved by the Clinical Research Ethics Committee of Navarra.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Sample characteristics</span><p id="par0060" class="elsevierStylePara elsevierViewall">The 613 hospitalizations of the sample corresponded to a population of 563 patients. During the study, 20.6% (126) of the admissions were for patients with multiple hospitalizations, and 79.4% (487) were for patients without multiple hospitalizations. For 39.3% of the hospitalizations, the patient age was <75 years, and for 60.7% the age was ≥75 years, with 25.3% of patients with multiple hospitalizations in the first group and 17.5% in the second group.</p><p id="par0065" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the main variables analyzed in the study. In the sample's hospitalizations, we observed a mean patient age of 73.6 years, a predominance of males (53.8% men) and a high number of previous diseases (mean, 9.4) and regular medications (mean, 7.6). The Katz index could only be accurately recorded in 451 hospitalizations and was abnormal in 30.4% of these hospitalizations (score ≥1). Fifty-five percent of the hospitalizations were of patients from urban communities, and 91% of the patients lived at home. The mean duration of the hospitalizations was 9.4 days. The main cause for hospitalization was the onset of a new disease (74.1%), followed by decompensation of a chronic disease (59.2%) and problems with the treatment (16.6%). The latter included problems resulting from incorrect dosing (4.1% of hospitalizations), adverse effects from the treatments (13.4%) and failure to comply with the treatment (0.8%). Only 1 case (0.2% of hospitalizations) included social problems among the causes of hospitalization, although the presence of some social problem was observed in 13.7% of the hospitalizations. The mean number of diagnoses at discharge was 5.1, and the mean score on the Charlson index was 2.8 points.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Some 34.9% (214) of the hospitalizations were followed by at least 1 new hospitalization in the first 6 months after discharge, with a mean number of readmissions in the overall sample of 0.5. The main cause for readmission was chronic disease decompensation (26.4% of all hospitalizations). During the study period (mean duration of 9.4 days for each hospitalization and 12 months of follow-up after discharge for each), 32.8% of the hospitalizations resulted in patient death (201 hospitalizations), which corresponds to 30.7% of the patients (95% confidence interval [95% CI]: 27.1–34.7).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Analysis of multiple hospitalizations</span><p id="par0075" class="elsevierStylePara elsevierViewall">From the demographic point of view, multiple hospitalizations were associated with lower age (mean age of 72.5 years and 73.9 years for patients with and without multiple hospitalizations, respectively), a predominance of males (69% of hospitalizations) and a rural origin (53.2% of hospitalizations).</p><p id="par0080" class="elsevierStylePara elsevierViewall">From the clinical point of view, patients with multiple hospitalizations were more ill and had a poorer functional state. These hospitalizations had a greater number of diagnoses at discharge and prior diseases, a greater consumption of medication and higher scores on the Charlson and Katz indices (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The main cause for admission for patients with multiple hospitalizations was chronic disease decompensation (87.3%), while for patients without multiple hospitalizations the main cause was onset of a new disease (78.4%). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> lists the most common prior diseases and diagnoses at discharge significantly associated with multiple hospitalizations. There were no differences between the admission patients with and without multiple hospitalizations in terms of the length of stay (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.868), the detection or not of social problems during hospitalization (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1.000) and inhospital patient mortality (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>.400).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">In the first 6 months after discharge, patients with multiple hospitalizations had a greater number of readmissions, with a greater proportion of readmissions caused by chronic disease decompensation (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Some 44.4% of the patients with multiple hospitalizations and 29.8% of the patients without multiple hospitalizations died in hospital during the study period. These hospitalization episodes corresponded to the death of 40.4% of the patients with multiple hospitalizations (95% CI 31.1–50.5) and 28.8% of the patients without multiple hospitalizations (95% CI 24.9–33.0), respectively.</p><p id="par0090" class="elsevierStylePara elsevierViewall">A binary logistic regression model was performed with the condition of multiple hospitalizations or nonmultiple hospitalizations as the dependent variable, which included 8 relevant predictive variables for qualification as a patient with multiple hospitalizations in the study's inclusion period (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). The variables that increased the possibility of being a patient with multiple hospitalizations were male gender, number of diseases prior to hospitalization, number of regular medicines, chronic disease decompensation as the cause of hospitalization and being a readmission patient. Those that decreased the possibility of being a patient with multiple hospitalizations were age, onset of a new disease as the cause of hospitalization and an urban origin. The model correctly classified 87.6% of the patients, with a sensitivity of 69.1%, a specificity of 92.4%, a negative predictive value of 92% and a positive predictive value of 70.2%.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">The patients in the sample had the typical characteristics observed in the departments of IM in Spain: elderly, greater proportion of male patients and a high degree of comorbidity. Furthermore, the most common diagnoses at discharge were heart failure, diabetes mellitus and chronic obstructive pulmonary disease.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18</span></a> The patients were also characterized by a poorer functional state and a higher consumption of medication than the general population (<a class="elsevierStyleCrossRefs" href="#tbl0005">Tables 1 and 2</a>).<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">From the demographic point of view and as with previous studies, we observed an association between the male sex and the condition of being a patient with multiple hospitalizations. The male sex has been identified on multiple occasions as a risk factor for readmission,<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">20,21</span></a> and in studies in which a sex had a higher proportion of multiple hospitalizations, the sex was usually male.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,11</span></a> In terms of age and in contrast with other published studies,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> our sample of patients with multiple hospitalizations were younger. Those publications in which the patients with multiple hospitalizations were younger usually studied the hospitalization of patients with profiles different from those of our study, mainly patients with psychosis or substance abuse problems.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,24</span></a> When analyzing our results, we have to consider the fact that although readmission is usually observed in older patients, it has also been postulated that advanced age, in addition to being an independent risk factor for readmission, can be a risk marker for other factors due to the fact that it is associated with chronic diseases.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Although it is undeniable that advanced age is associated with multiple diseases and functional impairment, it is likely that there is an increasingly frequent presence of a group of elderly patients with a lesser degree of complex comorbidity than that observed among the patients with multiple hospitalizations in our study. Although 60.7% of the hospitalizations in the sample were of patients older than 75 years, only 17.5% of these hospitalizations were of patients with multiple hospitalizations. Lastly, in terms of the patients’ origin, a greater percentage of the patients with multiple hospitalizations came from rural communities. Although readmitted patients and patients with multiple hospitalizations come from all areas where healthcare services are provided,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> studies on conventional readmission<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> and multiple readmission<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> have typically associated higher rates of readmission for patients from rural communities.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The clinical profile of patients with multiple hospitalizations in the sample was representative of patients with multiple, chronic and complex diseases, which are defined as patients who have 2 or more chronic diseases simultaneously. These types of patients are characterized by a special frailty and clinical susceptibility who require continuous reassessments and the successive use of various levels of healthcare.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Even before being admitted, patients with multiple hospitalizations had a number of previous diseases and regular medications clearly greater than that of patients without multiple hospitalizations. It is therefore not surprising that the main cause of admission for patients with multiple hospitalizations was chronic disease decompensation (87.3%), while for patients without multiple hospitalizations the main cause was onset of a new disease (78.4%). Moreover, the patients with multiple hospitalizations had a greater proportion of problems with the treatment, a larger number of diagnoses at discharge and a higher score on the Charlson index (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). The majority of the diagnoses that were significantly related to the condition of the patients with multiple hospitalizations were chronic diseases, which were also the most prevalent diseases prior to hospitalization: heart failure, diabetes mellitus, ischemic heart disease, hypertensive heart disease and chronic obstructive pulmonary disease (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). These diseases are among the most common causes for rehospitalization in general<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a> and IM in particular<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,17</span></a> and have also been identified in the literature as diseases related to multiple hospitalization.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–5,7</span></a> The most relevant diagnoses also included a mainly acute disease such as respiratory infection, which is also one of the most common primary diagnoses in IM<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,29</span></a> and is usually the cause for decompensation of the underlying chronic disease in our patients. The significant difference in the Charlson index score between the patients with multiple hospitalizations (mean, 3.86) and the patients without multiple hospitalizations (mean, 2.51) reflects the greater severity of the diseases in the former group, with a poorer prognosis for survival at 1 year, which was verified in the follow-up after discharge (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0110" class="elsevierStylePara elsevierViewall">The condition of having chronic diseases, frequently severe ones, is highly characteristic of patients with multiple hospitalizations,<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–5,7–10</span></a> such that chronic disease, along with advanced age, is the factor that is most frequently related to multiple hospitalization. However, in addition to having a higher number of diseases and greater severity of diseases, the patients with multiple hospitalizations in the sample also had a greater degree of functional limitation when compared with the patients without multiple hospitalizations, despite the fact that the patients with multiple hospitalizations were younger. We should therefore remember that functional impairment is a known predictive factor for readmission.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25,30</span></a> In the literature on multiple hospitalizations, disability is specifically included among the factors that result in patients with multiple hospitalizations requiring longer hospital stays and higher healthcare costs.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">During the first 6 months of the follow-up conducted after discharge, patients with multiple hospitalizations had more readmissions. Once again, chronic disease decompensation was the main cause for the readmission of patients with multiple hospitalizations (44.4%), which reflects the persistence of a clinical vulnerability linked to chronic diseases leading to recurrent readmissions (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Moreover, the association between multiple hospitalizations and readmission after discharge indicates another risk factor for multiple hospitalizations, which is the condition of being a readmission patient. Numerous publications have reported that being hospitalized on some prior occasion could be a risk factor <span class="elsevierStyleItalic">per se</span> for the patient being readmitted,<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28,30</span></a> a risk that increases as the number of previous hospitalizations increases.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> The condition of being a readmitted patient in many cases reflects a special vulnerability in the patient's progress, which frequently cannot be perceived through just the quantification of chronic diseases or age, given that there are patients with similar diseases and age, some of whom are readmitted and some who are not.</p><p id="par0120" class="elsevierStylePara elsevierViewall">As with previous studies,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,5</span></a> patients with multiple hospitalizations are characterized by having a high mortality rate, which is increased by advanced age and, above all, high comorbidity. The mortality of the patients with multiple hospitalizations was greater than that of the patients without multiple hospitalizations during hospitalization and in the follow-up after discharge (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Patients with multiple hospitalizations consume a high proportion of healthcare resources. This consumption is often part of a phenomenon widely described in the literature: the increase in healthcare expenditures for patients in their last year of life. This phenomenon is associated with advanced age, severe chronic diseases and functional disability.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">The logistic regression model used in this study showed that there are 2 major factors when predicting whether a hospitalized patient will be a patient with multiple hospitalizations or not. The most important factor is a previous hospitalization (<span class="elsevierStyleItalic">i.e.</span>, being a readmitted patient), a factor that has already been included in readmission prediction models in previous studies.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> The other important factor is the patient's chronic disease burden, which was reflected in the model by the number of diseases previous to hospitalization, the number of medications, chronic disease decompensation as the cause for hospitalization and the onset of a new disease as the cause for hospitalization. No specific disease was more significant than the others as a predictor of multiple hospitalizations. The accumulation of chronic diseases and problems in the same patient was more relevant than the fact of having a specific disease.</p><p id="par0130" class="elsevierStylePara elsevierViewall">A patient with multiple hospitalizations represents a paradigm of a high-cost patient for hospital departments.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> This study showed that patients with multiple hospitalizations present a profile of patients with multiple, chronic and complex diseases. These patients are therefore included among the target populations for new management strategies for chronically ill patients and those with multiple diseases, strategies that are being promoted by scientific societies and ministries of health in Spain.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Our study provides new data on the characteristics of patients with multiple hospitalizations, which can facilitate the planning of resources directed to chronically ill patients with complex healthcare needs.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The limitations of this study include those related to the content and quality of the information, which are the result of working with a secondary database such as the CMH, as well as those resulting from the diagnostic typology of patients with multiple, chronic and complex diseases, which precluded the breakdown of diagnoses into primary and secondary. Nevertheless, the CMH contains fairly reliable data on patients, which enables a good approximation in the analysis of hospitalized patient populations. The data available in the CMH on the patients’ socioeconomic and cultural characteristics is more limited than the clinical and demographic data. The value of the study results on social problems is therefore more limited.</p><p id="par0140" class="elsevierStylePara elsevierViewall">In conclusion, the patients with multiple hospitalizations included in the sample of patients at the Department of IM of CHN-A are characterized (when compared with patients without multiple hospitalizations) by greater healthcare complexity. This was evident not only by their much greater use of healthcare resources but also by their greater association with chronic disease decompensation, greater consumption of medication, higher functional impairment in daily life activities and higher mortality rates. In this study, we analyzed the factors associated with the multiple hospitalizations of a cohort of patients treated at a department of internal medicine.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">A total of 613 consecutive hospitalizations were analyzed. A multiple-hospitalization patient was defined as one who at the time of admission had been hospitalized 3 or more times in the past year. We analyzed the relationship between demographic, clinical and societal factors on one hand and having been hospitalized on multiple occasions on the other. We also analyzed readmissions in the 6 months after discharge, as well as mortality during the hospitalization and in the 6 and 12 months after discharge.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">When compared with patients who have not been hospitalized on multiple occasions, multiple-hospitalization patients are more likely to be male, younger and to have greater comorbidity, greater consumption of medicines and higher Katz Index scores. The diseases that were most obviously associated with multiple hospitalizations were heart failure, diabetes mellitus and chronic obstructive pulmonary disease. Además, se analizó el reingreso en los 6 meses siguientes al alta así como el fallecimiento en el ingreso y en los 6 y 12 meses siguientes al alta.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Los multiingresadores se caracterizaron frente a los no multiingresadores por ser de sexo masculino, ser más jóvenes y presentar mayor comorbilidad, mayor consumo de medicaciones y mayor puntuación en el índice de Katz. La principal causa de ingreso de los multiingresadores fue la «descompensación de una enfermedad crónica» (87,3%). Las enfermedades que se asociaron de forma más destacada con el multiingreso fueron la insuficiencia cardiaca, la diabetes mellitus y la enfermedad pulmonar obstructiva crónica. En los 6 primeros meses tras el alta los multiingresadores presentaron más reingresos. These patients were mostly men from rural communities who had several associated chronic diseases (heart failure, diabetes mellitus and chronic pulmonary disease). They were admitted to hospital for decompensation of the disease and had a higher Charlson Index.
