However, citations need time (months and years) to accrue. The web, the digitalization of journals and the rise of social media have made it possible the development of article-level metrics (altmetrics or alternative metrics). These altmetrics reflect the activity in, for instance, social media platforms (e.g., Twitter, Facebook), blogs and newspapers, allowing the immediate (from the very first day) gathering of previously invisible impact of scientific research. Investigators have assessed the possible correlation between altmetrics and citation counts. A recent meta-analysis showed that there is a negligible correlation (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) between Twitter counts and citations, very weak (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.12) for blog mentions and weak (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.23) to moderate (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.51) for bookmark counts from social reference managers such as CiteULike and Mendeley, respectively.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> The objective of this study was to describe the association between altmetrics and citations counts, depending on the types of article – original investigations and opinion articles – in a selected number of highly influential medical journals.</p><p id="par0010" class="elsevierStylePara elsevierViewall">‘Altmetric Attention Score’ is the aggregator providing the most comprehensive source of social media data<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">2</span></a>: it is a composite quantitative measure of the attention an article has received. The different components of the score (from Twitter or Facebook, to newspapers or policy documents) receive different weights in the algorithm. Altmetric Attention Score tracks direct public attention, not quality.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Original investigations (i.e., research articles and systematic reviews/meta-analysis) and opinion articles (e.g., editorials, commentaries, viewpoints) published in the 2015 first 4 printed issues of 4 top-ranked general medicine journals (Lancet, JAMA, BMJ and Annals of Internal Medicine) and 1 top-ranked journal on five different medical specialties that provide Altmetric Attention Score of published articles (Lancet Infectious Diseases, JAMA Psychiatry, Gastroenterology, JAMA Pediatrics and Journal of Clinical Oncology), were included in this study.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">3</span></a> In November 2016, articles’ Altmetric Attention Score and Twitter and citation counts were retrieved from the articles’ homepage and Google Scholar, respectively. Google Scholar was chosen as the source of the citation counts because it has a higher coverage than the other two popular databases, Web of Science and Scopus. Since some articles were published on-line months before printed publication, articles got between 19 and 27 months to accrue citations, enough time to have a reasonable view of the (recent) scholarly impact of each article.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Four hundred and ten articles, 228 original investigations and 182 opinion articles, were included.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">3</span></a> For all articles the Spearman's rank correlation coefficient (<span class="elsevierStyleItalic">r</span>) was weak, <span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.38 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001); by types of article also: Original investigations (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.32; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and opinion articles (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.31; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). This was not the case between different journals (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>A). Two journals (JAMA and Lancet Infectious Diseases) got strong correlation coefficients (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.66–0.68), whereas, in the other extreme, Annals of Internal Medicine got a weak (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.37) correlation coefficient.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">All but 11 articles (97%; 5 original investigations published in Gastroenterology and 6 opinion articles published in 4 different journals) had at least one tweet. As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>B, very strong correlations (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>0.80) were found between the occurrence of, at least, one tweet and Altmetric Attention Score and with Altmetric Attention Score belonging to both original investigations and opinion articles and with articles published in general medicine journals. The range varied widely between BMJ that got the strongest correlation (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.91) and JAMA Pediatrics with the weakest, but moderate, correlation (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.45); having one tweet was weakly correlated (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.37) with citation counts. Only one journal, Lancet Infectious Diseases, showed a strong correlation (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.60) between having at least one tweet and citations count.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The visibility of research outputs comprises both the classical citations counts and a number of altmetrics that are increasingly popular among citizens and medical journals. Social media networks play a critical role in disseminating medical information among the public. Although the public is not necessarily interested in the same topics/articles than the scientific community,<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">4</span></a> data showed that among the 100 articles published in 2015 with the highest Altmetric Attention Score, 36 belonged to medical and health sciences topics.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">7</span></a> The very weak correlation between citations and social media metrics, except for Mendeley<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">1,2,8</span></a> made experts believe altmetrics should not be regarded as ‘alternative metrics’ but ‘complementary metrics’ to citations.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">2,9</span></a> This, however, was not fully observed in this analysis where Altmetric Attention Score of JAMA and Lancet Infectious Diseases were strongly correlated (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.66–0.68) with citations counts. In the opposite side was Annals of Internal Medicine with a weak correlation coefficient (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.37), similar to that described for the 50 most frequently cited emergency medicine articles (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.3)<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">10</span></a> and in solid organ transplantation (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>≤<span class="elsevierStyleHsp" style=""></span>0.23).<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">11</span></a> On the other hand, Twitter although exhibiting different impact in the journals assessed, was weakly correlated to citations counts.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A major limitation of this study is its restriction to small number of articles in a limited number of influential medical journals, which precludes the generalization of the results to other types of medical journal. In addition, this analysis did not exclude author self-citations,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">12</span></a> nor altmetrics generated by authors or publishers.</p><p id="par0040" class="elsevierStylePara elsevierViewall">This analysis showed that articles published in top-ranked medical journals had very different behaviors with respect to social media mentions and its eventual association to citations counts. Altmetric Attention Score and Twitter mentions were strongly/moderately and moderately/weakly associated to citation counts, respectively, although it widely varies depending on the journal considered. Further studies are needed including many more articles published in these 9 top-ranked medical journals to confirm these findings. Additional analyses of articles published in less influential medical journals are also warranted: their behavior could vary from that described in this study.
